Ethische dilemma’s bij eigen regie: oproep tot reflectie
De decentralisaties in zorg en welzijn moeten bijdragen aan een transitie van verzorgingsstaat naar participatiesamenleving. Van burgers wordt verwacht dat zij eigen regie voeren. Ten eerste door langer voor zichzelf te zorgen en een beroep te doen op hun eigen omgeving als ze hulp nodig hebben. Ten tweede door, als er
wel professionele ondersteuning nodig is, ook daarin een actieve rol te vervullen. Eigen regie wordt vooral positief gelabeld door beleidsmakers: het levert zorggebruikers meer autonomie op, laat de zorg beter aansluiten bij de vraag en bespaart ook nog eens kosten. Eigen regie is daarmee een hoerawoord waar je met goed fatsoen niet tegen kan zijn. In deze bijdrage laten we echter zien dat het stimuleren van eigen regie zorgverleners confronteert met ethische dilemma’s. Reflectie op deze dilemma’s is van groot belang om op die manier recht te doen aan verschillende waarden in de zorg
The complexity of shaping self-management in daily practice
_Background and context:_ Many countries are giving patients a more active role in health care, on both the individual and collective level. This paper focuses on one aspect of the participation agenda on the individual level: self-management. The paper explores self-management in practice, including the implications of the difficulties encountered.
_Objective:_ to gain insight into the complexity of self-management practice. This is crucial for developing both self-management interventions and the participation policy agenda.
_Methods:_ qualitative semi-structured interviews with experts (n=6) and patients with a chronic condition (n=20).
_Results:_ in terms of level of involvement and type of activity, shaping self-management in practice depends on personal and social dynamics, patients’ ideas of the good life, and their interactions with care professionals. Clashes can arise when patients and professionals hold differing ideas, based on different values, about the level and type of patient involvement.
_Discussion:_ The discussion on self-management should account for the fact that how we define self-management is very much a normative issue
Doelgericht deelnemen Achtergrondstudie naar burgerparicipatie bij het toezicht
Sinds de jaren tachtig van de vorige eeuw is er groeiende aandacht voor burgerparticipatie in
de gezondheidszorg. Participatie vindt niet alleen op het individuele niveau plaats, waar
burgers kunnen kiezen en actief kunnen participeren in beslissingen over hun eigen zorg,
maar ook op het collectieve niveau. Patiënten worden gevraagd om het
patientenperspectief in te brengen in besluitvorming over bijvoorbeeld richtlijnontwikkeling,
onderzoeksagenda’s, overheidsbeleid en kwaliteitsbeleid in ziekenhuizen. Ook bij de IGZ
staat burgerparticipatie op de kaart. Er is echter nog weinig bekend over participatie bij
toezicht. Om meer inzicht te krijgen in deze materie is door het iBMG
Decentralisatie en Democratische Vertegenwoordiging:een pilot-onderzoek naar vertegenwoordigingsclaims op het lokale niveau
In dit rapport presenteren we de resultaten die zijn voortgekomen uit ons pilot-onderzoek naar
democratische vertegenwoordiging in het sociale domein in twee gemeenten. We merkten in
het onderzoek hoezeer alle betrokkenen er groot belang aan hechten om de omvangrijke
decentralisaties van jeugdzorg, maatschappelijke ondersteuning en arbeidsparticipatie
democratisch te verankeren. We merkten ook dat het vaak een worsteling is (geweest) om
participatie en vertegenwoordiging in het gemeentelijke beleid invulling te geven in de praktijk.
Dit rapport biedt niet alleen inzicht in die worsteling, maar is ook een verkenning hoe diverse
vormen van vertegenwoordig
Does structural form matter?:A comparative analysis of pooled procurement mechanisms for health commodities
IntroductionPooled procurement can be seen as a collaboration initiative of buyers. Such mechanisms have received increased attention during the Covid-19 pandemic to improve access to affordable and quality-assured health commodities. The structural form of pooled procurement mechanisms ranges from a third-party organization that procures on behalf of its buyers to a buyer’s owned mechanism in which buyers operate more collaboratively. However, little is known about how these types of pooled procurement mechanisms differ in terms of characteristics, implementation and developmental process. To fill this gap, we compared four pooled procurement mechanisms. Two buyer’s owned mechanisms: the Organisation of the Eastern Caribbean States (OECS) and the Pacific Island Countries (PIC). And two third-party mechanisms: the Global Drug Facility (GDF) and the Asthma Drug Facility (ADF).MethodsFor this qualitative study, we used a multiple case-study design. The cases were purposefully selected, based on a most-similar case study design. We used the Pooled Procurement Guidance to collect data on individual cases and compared our findings between the case studies. For our analysis, we drew upon peer-reviewed academic articles, grey literature documents and 9 semi-structured interviews with procurement experts.ResultsBuyers within a buyer’s owned mechanisms differ in procurement systems, financing structures, product needs and regulatory and legal frameworks. Therefore, buyers within such mechanisms require relative alignment on motivations, goals and operations of the mechanism. Our study showed that buyers’ relative homogeneity of characteristics and their perceived urgency of the problems was particularly relevant for achieving that alignment.Third-party organization mechanisms require less alignment and consensus-building between buyers. To participate, buyers need to align with the operations of the third-party organization, instead of other buyers. Elements that were essential for the successful implementation and operation of such mechanisms included the procurement secretariat’s ability to create local and global awareness around the problem, to induce political will to act upon the problem, to mobilize sufficient funding and to attract qualified staff.ConclusionTo successfully sustain pooled procurement mechanisms over time, key actors should drive the mechanism through continuous and reflexive work on stakeholder engagement, mobilization of funding and alignment of interests and needs
Pectobacterium carotovorum subsp carotovorum can cause potato blackleg in temperate climates
It is well established that the pectinolytic bacteria Pectobacterium atrosepticum (Pca) and Dickeya spp. are causal organisms of blackleg in potato. In temperate climates, the role of Pectobacterium carotovorum subsp. carotovorum (Pcc) in potato blackleg, however, is unclear. In different western and central European countries plants are frequently found with blackleg from which only Pcc can be isolated, but not Pca or Dickeya spp. Nevertheless, tubers vacuum-infiltrated with Pcc strains have so far never yielded blackleg-diseased plants in field experiments in temperate climates. In this study, it is shown that potato tubers, vacuum-infiltrated with a subgroup of Pcc strains isolated in Europe, and planted in two different soil types, can result in up to 50% blackleg diseased plants
