    Gut Microbiota Composition of Biliary Atresia Patients Before Kasai Portoenterostomy Associates With Long-term Outcome

    BACKGROUND AND AIMS: Biliary atresia (BA) is a cholestatic, fibro-obliterative cholangiopathy of unknown etiology. BA is primarily treated by a surgical approach, i.e. the Kasai portoenterostomy (KPE), to obtain clearance of jaundice (COJ). The gut microbiota (GM) composition has been associated with the course of several cholestatic liver diseases. It is largely unknown, however, whether GM composition associates with the outcome of KPE. We compared the GM composition of BA patients and controls and assessed if GM composition before KPE was related to COJ after KPE. METHODS: We compared feces of term born BA patients before KPE and controls (patients undergoing inguinal hernia repair) by 16S rRNA sequencing. Composition and alpha diversity of the GM were compared between BA and controls before KPE and after KPE, between patients with COJ vs. without COJ (total serum bilirubinā€Š<ā€Šorā€Šā‰„ā€Š20ā€ŠĪ¼mol/Lā€Š<ā€Š6mo post-KPE). RESULTS: Alpha diversity was comparable between BA (nā€Š=ā€Š12, age 1.6[1.3-1.8]mo) and controls (nā€Š=ā€Š6, age 2.0[1.4-2.1]mo; Pā€Š=ā€Š0.22). Compared to controls, BA patients had lower abundances of Bifidobacteriaceae (Ī²=-1.98, Pā€Š<ā€Š0.001) and Lachnospiraceae (Ī²=-1.84, Pā€Š=ā€Š0.007), and higher abundances of Streptococcus (Ī²=1.13, Pā€Š=ā€Š0.003). The alpha diversity prior to KPE correlated negatively with COJ (Rā€Š=ā€Š-0.63, Pā€Š=ā€Š0.03). Lower alpha diversity pre-KPE was associated with COJ [+] (Ī²logitā€Š=ā€Š-0.64, Pā€Š=ā€Š0.04). We observed greater abundances of genus Acinetobacter (Ī²=1.27, Pā€Š=ā€Š0.03) and family Clostridiaceae (Ī²=1.45, Pā€Š=ā€Š0.03) and lower abundances of the family Enterobacteriaceae, (genera Klebsiella (Ī²=-1.21, Pā€Š=ā€Š0.01), Salmonella (Ī²=-1.57, Pā€Š=ā€Š0.02)) in COJ [+] vs. COJ [-]. CONCLUSIONS: The gut microbiota of biliary atresia patients prior to Kasai portoenterostomy associates with outcome, clearance of jaundice, suggestive of predictive and mechanistic roles of the gut microbiota composition in bile homeostasis

    Prognosis of Biliary Atresia After 2-year Survival With Native Liver: A Nationwide Cohort Analysis

    OBJECTIVES: To determine the prognosis of patients with biliary atresia (BA) after two years of native liver survival (NLS) and to identify prognostic factors for continued NLS after two years of age. METHODS: We retrospectively analyzed perioperative, laboratory and outcome parameters of all BA patients in The Netherlands between January 1987-June 2015 with NLS of at least two years. We compared parameters between patients who continued to have their native liver (NLS+) to those who did not, either by transplant or death (NLS-). RESULTS: We included 100 patients. Upon a median follow-up of 16.4 years, NLS ended in 37% by liver transplantation (LTx) and in 6% by (pre-transplant) mortality. NLS rates at 5, 10, 15, 18 years of age were 89, 72, 60, 54%, respectively. Corresponding overall survival rates were 98, 90, 87, 87%, respectively. Six months post-Kasai, NLS+ patients had higher clearance of jaundice (COJ) rate, significantly lower total and direct serum bilirubin, aspartate-aminotransferase and alkaline phosphatase levels, compared with NLS- patients (each Pā€Š<ā€Š.05). Cox regression could only assess a significant effect of COJ on continued NLS. Main indications for LTx after the age of two were irreversible jaundice and portal hypertension. CONCLUSIONS: Eighty-seven percent of patients with two-year NLS reach adult age, and more than 50% with their native liver. However, a pre-transplant mortality of 6% exists among patients who reach the age of two years with their native livers. Early life parameters, other than COJ, did not have a significant effect on continued NLS after two years of age

    A Higher Incidence of Isolated Biliary Atresia in Rural Areas: Results from an Epidemiological Study in The Netherlands

    OBJECTIVES: Environmental factors may be involved in the pathogenesis of biliary atresia (BA). This epidemiological study aimed to analyse the relationships between the incidence of BA, the incidence of confirmed viral or bacterial infections and population density, as well as geographical and temporal clustering of BA in the Netherlands. STUDY DESIGN: Correlations between the monthly incidence of BA and the number of confirmed infections were assessed. BA incidence per province was calculated and compared to the province with highest population density. Birthplaces were classified as rural or urban. Temporal clustering of month of birth and month of conception were analysed. We performed analyses for isolated BA (IBA) and syndromic BA (SBA) separately. Chi, logistic regression and Walter & Elwood test were used. RESULTS: 262 IBA and 49 SBA patients, born between 1987-2018, were included. IBA incidence correlated to the number of confirmed infections of e.g. C. Trachomatis (Rā€Š=ā€Š0.14;Pā€Š=ā€Š0.02) and Adenovirus (Rā€Š=ā€Š0.22;Pā€Š=ā€Š0.005). We observed a higher incidence of IBA (0.75/10,000;ORā€Š=ā€Š1.86;Pā€Š=ā€Š0.04) and SBA (0.27/10,000;ORā€Š=ā€Š6.91;Pā€Š=ā€Š0.001) in Groningen and a higher incidence of SBA in Gelderland (0.13/10,000;ORā€Š=ā€Š3.35;Pā€Š=ā€Š0.03). IBA incidence was 68% higher in rural (0.67/10,000) vs. urban areas (0.40/10,000) (Pā€Š=ā€Š0.02). The estimated month of conception of patients with SBA clustered in November (85% increase compared to average SBA incidence [0.09/10,000;Pā€Š=ā€Š0.04]). CONCLUSIONS: IBA incidence correlated weakly with national confirmed infections. IBA and SBA incidence varied geographically in the Netherlands. IBA incidence was higher in rural than in urban areas, which may be explained decreased exposure to pathogens. Our results provide support for a role of environmental factors in the pathogenesis of IBA

    Genotype correlates with the natural history of severe bile salt export pump deficiency

    Background & aims: Mutations in ABCB11 can cause deficiency of the bile salt export pump (BSEP), leading to cholestasis and end-stage liver disease. Owing to the rarity of the disease, the associations between genotype and natural history, or outcomes following surgical biliary diversion (SBD), remain elusive. We aimed to determine these associations by assembling the largest genetically defined cohort of patients with severe BSEP deficiency to date. Methods: This multicentre, retrospective cohort study included 264 patients with homozygous or compound heterozygous pathological ABCB11 mutations. Patients were categorized according to genotypic severity (BSEP1, BSEP2, BSEP3). The predicted residual BSEP transport function decreased with each category. Results: Genotype severity was strongly associated with native liver survival (NLS, BSEP1 median 20.4 years; BSEP2, 7.0 years; BSEP3, 3.5 years; p <0.001). At 15 years of age, the proportion of patients with hepatocellular carcinoma was 4% in BSEP1, 7% in BSEP2 and 34% in BSEP3 (p = 0.001). SBD was associated with significantly increased NLS (hazard ratio 0.50; 95% CI 0.27-0.94: p = 0.03) in BSEP1 and BSEP2. A serum bile acid concentration below 102 Ī¼mol/L or a decrease of at least 75%, each shortly after SBD, reliably predicted NLS of ā‰„15 years following SBD (each p <0.001). Conclusions: The genotype of severe BSEP deficiency strongly predicts long-term NLS, the risk of developing hepatocellular carcinoma, and the chance that SBD will increase NLS. Serum bile acid parameters shortly after SBD can predict long-term NLS. Lay summary: This study presents data from the largest genetically defined cohort of patients with severe bile salt export pump deficiency to date. The genotype of patients with severe bile salt export pump deficiency is associated with clinical outcomes and the success of therapeutic interventions. Therefore, genotypic data should be used to guide personalized clinical care throughout childhood and adulthood in patients with this disease

    BACKGROUND AND AIMS: Mutations in ATPase phospholipid transporting 8B1 (ATP8B1) can lead to familial intrahepatic cholestasis type 1 (FIC1) deficiency, or progressive familial intrahepatic cholestasis type 1. The rarity of FIC1 deficiency has largely prevented a detailed analysis of its natural history, effects of predicted protein truncating mutations (PPTMs), and possible associations of serum bile acid (sBA) concentrations and surgical biliary diversion (SBD) with long-term outcome. We aimed to provide insights by using the largest genetically defined cohort of patients with FIC1 deficiency to date. APPROACH AND RESULTS: This multicenter, combined retrospective and prospective study included 130 patients with compound heterozygous or homozygous predicted pathogenic ATP8B1 variants. Patients were categorized according to the number of PPTMs (i.e., splice site, frameshift due to deletion or insertion, nonsense, duplication), FIC1-A (n = 67; no PPTMs), FIC1-B (n = 29; one PPTM), or FIC1-C (n = 34; two PPTMs). Survival analysis showed an overall native liver survival (NLS) of 44% at age 18 years. NLS was comparable among FIC1-A, FIC1-B, and FIC1-C (% NLS at age 10 years: 67%, 41%, and 59%, respectively; P = 0.12), despite FIC1-C undergoing SBD less often (% SBD at age 10 years: 65%, 57%, and 45%, respectively; P = 0.03). sBAs at presentation were negatively associated with NLS (NLS at age 10 years, sBAs = 194 mu mol/L: 15%; P = 0.03). SBD decreased sBAs (230 [125-282] to 74 [11-177] mu mol/L; P = 0.005). SBD (HR 0.55, 95% CI 0.28-1.03, P = 0.06) and post-SBD sBA concentrations < 65 mu mol/L (P = 0.05) tended to be associated with improved NLS. CONCLUSIONS: Less than half of patients with FIC1 deficiency reach adulthood with native liver. The number of PPTMs did not associate with the natural history or prognosis of FIC1 deficiency. sBA concentrations at initial presentation and after SBD provide limited prognostic information on long-term NLS
