    Comparison of Trunk Activity during Gait Initiation and Walking in Humans

    To understand the role of trunk muscles in maintenance of dynamic postural equilibrium we investigate trunk movements during gait initiation and walking, performing trunk kinematics analysis, Erector spinae muscle (ES) recordings and dynamic analysis. ES muscle expressed a metachronal descending pattern of activity during walking and gait initiation. In the frontal and horizontal planes, lateroflexion and rotation occur before in the upper trunk and after in the lower trunk. Comparison of ES muscle EMGs and trunk kinematics showed that trunk muscle activity precedes corresponding kinematics activity, indicating that the ES drive trunk movement during locomotion and thereby allowing a better pelvis mobilization. EMG data showed that ES activity anticipates propulsive phases in walking with a repetitive pattern, suggesting a programmed control by a central pattern generator. Our findings also suggest that the programs for gait initiation and walking overlap with the latter beginning before the first has ended

    Time-Related Changes in Detrusor Overactivity in Awake Rats with Spinal Cord Injury Observed by Simultaneous Registrations of Intravesical and Intraabdominal Pressures

    PurposeTo compare the physical characteristics of detrusor overactivity (DO) induced by intravesical infusion of saline in awake, sham rats and rats with chronic spinal cord injury (SCI), by simultaneous registrations of intravesical and intraabdominal pressures.MethodsMale Sprague-Dawley rats, normal or with a spinal vascular clip at the level of Th9, were investigated cystometrically 1 and 4 weeks after SCI. Intravesical pressure (IVP) and intraabdominal pressure (IAP) were recorded simultaneously to evaluate true DO. During the filling phase, the event of IVP rises, defined as increments that exceeded 2 cmH2O from baseline, were determined as DO according to the absence of simultaneous changes in IAP.ResultsAll SCI rats exhibited DO during the filling phase, which was not shown in sham rats. The frequency and pressure of DO had a tendency to decrease with time. The DO frequency of SCI rats after 4 weeks (0.9±0.2 min-1) was decreased compared with that after 1 week (2.1±0.4 min-1; P<0.05). The DO pressure of SCI rats after 4 weeks (8.4±1.9 cmH2O) was decreased compared with that after 1 week (11.6±2.9 cmH2O; P>0.05).ConclusionsCystometric studies in awake male SCI rats showed some significant changes in bladder function after SCI. All SCI rats exhibited DO during the filling phase, and showed different physical characteristics of DO over the course of time. The neurological basis of these time-related changes remains poorly understood, but may provide important prognostic information about long-term urological management in SCI patients

    Real-life study to assess effectiveness and safety of eculizumab in patients with neuromyelitis optica spectrum disorders in France: protocol for ECUP4, an observational study

    BackgroundEculizumab, a humanized monoclonal antibody targeting the C5 complement protein, has been approved for the treatment of neuromyelitis optica spectrum disorders (NMOSD) in adult patients who are anti-aquaporin-4 (AQP4) antibody positive (Ab+). The aim of this study is to evaluate the long-term effectiveness and safety of eculizumab in French adults with NMOSD and to describe patients' characteristics, disability, and quality of life using data collected in a real-world setting.MethodsThis is the protocol for ECUP4, an ongoing prospective, observational, non-comparative, multicenter study conducted in 32 reference centers in France. Eligible patients must also be enrolled in NOMADMUS, a nested cohort of the French national multiple sclerosis registry (OFSEP). The primary endpoint is the annualized relapse rate. Secondary endpoints include the long-term safety of eculizumab, as well as patients' characteristics, treatment outcomes, disability, pain, visual acuity, and quality of life. Visits and treatments follow routine clinical practice. The case report forms (CRF) comprise data recorded in the context of the NOMADMUS cohort, collected during routine visits. The inclusion period is planned for 3 years, with no limitation on the number of patients enrolled. The maximum follow-up duration will be 5.5 years.ConclusionThe efficacy and safety of eculizumab in patients with AQP4+ NMOSD have been demonstrated in randomized clinical trials that showed a significant reduction in the risk of relapse, with a safety profile consistent with other indications. This study will provide clinical and patient-reported evidence of the benefits of eculizumab, using data from a real-world setting in France.Trial registration numberThis study is registered at the French public repertory Health data Hub, N° F20211228123801.     Pulsed Radiofrequency Lesioning of the Axillary and Suprascapular Nerve in Calcific Tendinitis

    The patient was a 45-year-female who presented with pain at right shoulder and right upper arm. The patient suffered from right shoulder and arm pain for 3 years and had pain management which was performed using medication and conservative management after she had been diagnosed with calcific tendinitis. However, substantial pain relief was not consistently achieved, and recurrence of pain was reported. Therefore, we performed right axillary nerve and suprascapular nerve block through pulsed radiofrequency. Two months after the procedure, the shoulder pain gradually subsided with the size reduction of the calcified nodule and she needed no more pain management

    Sistematización del proceso participativo para la creación de la política de innovación en la UNED

    Las universidades, como organizaciones sociales requieren transitar en un proceso de transformación que les permita repensar su quehacer y, alcanzar una mayor cercanía a las comunidades educativas, por medio del trabajo en sus áreas sustantivas. La innovación, se constituye en este contexto, en un acicate para la educación superior; que invoca cambio y transformación profunda en lo que hace y en las formas, procesos y herramientas que emplea para lograrlo. Esta concepción de innovación va más allá de lo que se considera en estos días y en diferentes contextos, reducida al uso intensivo de las tecnologías de la información y comunicación. La necesidad de sistematizar y generar conocimiento que permitiera comprender y fortalecer las capacidades innovadoras en la UNED, propició que un grupo interdisciplinario, conformado por decisión abierta e individual de sus integrantes y, con representación de las distintas Vicerrectorías de la universidad, se organizara y conformara el Nodo de Innovación. Este grupo asumió la tarea de investigar el quehacer innovador de la institución y el estudio de distintos marcos de referencia teórico-conceptuales sobre la innovación y la organización para ofrecer a las autoridades universitarias y la comunidad educativa en general, un diseño de sistema de innovación particular orientado a la UNED; sustentado en la aprobación de una política institucional y su respectivo plan de implementación. Esta ponencia explica la sistematización diseñada especialmente por el Nodo de Innovación, en la definición de esa propuesta de la política de innovación como sistema y que desde el mes de julio del 2017, se encuentra en discusión dentro de dos comisiones del Consejo Universitario, con la intención de ser llevada y aprobada al seno del plenario de ese órgano político colegiado
