G-genotyping of rotaviruses in stool samples in Salento, Italy
Rotaviruses are the most common agents of diarrhoeal illnessin infants and young children. Gastroenteritis caused byrotaviruses is also more likely to be associated with severe dehydration compared to other viral gastroenteritis.We determined the G-genotype of rotaviruses circulating in theSalento. During 2004, 144 stool samples were collected fromsubjects with a positive screening test and stored at -20 °C untilconfirmation could take place using molecular biology techniques. A reverse transcription-polymerase chain reaction (RTPCR) assay for the amplification of gene VP7 of rotavirus wasdeveloped and used; G-genotype was determined by sequencing and phylogenetic analysis. A total of 101 stool samples were detected positive by RT-PCRand 35 were sequence analyzed and classified into G1, G2, G4,G9 and G12 type. The overall relative incidence of G-types inSalento is different to that of other developed countries. Themost prevalent genotype was G2 while genotype G1 was present at low levels. It is also interesting to note the presence ofG9 rotavirus, which is now recognized as the fifth globallyimportant rotavirus genotype.In this study, therefore, we demonstrate the usefulness of a simple method for correctly determining the G genotypes circulating in a geographic region
Atherosclerosis is associated with a higher risk of hepatic steatosis in HIV-infected patients
Introduction. Cardiovascular Diseases (CD) have emerged as a leading cause of morbidity and mortality in HIV population. Some studies have reported higher carotid Intima Media Thickness (c-IMT), a measure of subclinical atherosclerosis (AT), in this cohort of patients.Methods. Here, we evaluate the role of Hepatic Steatosis (HS) as likely marker for AT in 128 HIV-infected patients without hepatitis C infection. c-IMT has been detected non-invasively by carotid ultrasonography to assess the progression of AT. HS has been evaluated using a process based on vibration-controlled transient elastography (Fibroscan) by a novel ultrasonic controlled attenuation parameter (CAP). The cut-off value for defining the presence of significant HS was CAP > 259 dBm-1. Results. AT has been detected in 26 patients (20.3%), whereas steatosis of grade 2 (S2) in 31 (24.2%). The variables statistically related to AT were age, obesity, diabetes, hypertension and S2. In the multivariate analysis, AT was only associated (p < 0.001) with age and S2. The optimal cut-off value indicated by ROC curve for predicting AT was CAP > 250 dB/m-1.Discussion. Our results highlight the presence of AT in HIV-infected persons and its association with fatty liver disease; therefore, HS assessment in HIV population results crucial to predict AT and CD
Transbulbar B-Mode Sonography in Multiple Sclerosis: Clinical and Biological Relevance
Abstract Optic nerve sheath diameter quantification by transbulbar B-mode sonography is a recently validated technique, but its clinical relevance in relapse-free multiple sclerosis patients remains unexplored. In an open-label, comparative, cross-sectional study, we aimed to assess possible differences between patients and healthy controls in terms of optic nerve sheath diameter and its correlation with clinical/paraclinical parameters in this disease. Sixty unselected relapse-free patients and 35 matched healthy controls underwent transbulbar B-mode sonography. Patients underwent routine neurologic examination, brain magnetic resonance imaging and visual evoked potential tests. The mean optic nerve sheath diameter 3 and 5 mm from the eyeball was 22–25% lower in patients than controls and correlated with the Expanded Disability Status Scale (r = −0.34, p = 0.048, and r = −0.32, p = 0.042, respectively). We suggest that optic nerve sheath diameter quantified by transbulbar B-mode sonography should be included in routine assessment of the disease as an extension of the neurologic examination
In vitro diagnosis of sepsis: a review

The limited knowledge of the pathophysiology in association with the lack of in vitro diagnostic methods for the certain and quick determination of the causative microbiological agents and their antibiotic resistance means the condition is still critical and of high impact in health care. The current gold standard method to detect the sepsis-causing pathogens, which is based on blood culture, is still insufficiently sensitive and slow. The new culture-independent molecular biology-based techniques can lead to the identification of a broad range of microorganisms and resistance markers within a few hours and with high sensitivity and specificity; nevertheless, limitations of, for example, the polymerase chain reaction-based methods still hamper their application in the clinical routine. This review summarizes the in vitro diagnostic methods and their approach in the clinical diagnosis of the bloodstream infections, and explores their advantages and disadvantages at the current state of the art. A quick analysis of the future prospective in multiplex technologies for microbiological diagnosis of sepsis is also provided. Caesarean Section in the World: a new ecological approach
Introduction. This study aimed to estimate the most recent caesarean section rates in the world and examine the association between these rates and old and new indicators of health care.
Methods. Authors analyzed the Caesarean Section (CS) rates, also in geo-economic and economic groups, and correlated them to maternal and neonatal mortality, to births attended by skilled health personnel and to births among adolescents. Analysis of covariance
and piecewise regressions were used for the statistical analysis.
Results. In 47.2% of the countries, the CS rate exceeded 15%. Countries of Latin America and the Caribbean along with Europe, North America and Oceania had the highest values. The analysis showed an inverse association between CS rates and Maternal Mortality (MMR) and Neonatal Mortality (NMR) for all geographical areas except for Europe. The greatest association was observed in lower-middle-income countries. In developing countries only 50% of cases, occur in medical facilities and only half of these are seen by medical, nursing and obstetrical staff. Age of the mother appears to influence the outcome and choice of delivery type. Countries where an high ABR rate is present have low CS use.
Conclusions. To best evaluate the consequences of the increasing rate of CS, it would be useful to identify the most sensitive outcome indicators
Using the cytokinesis-block micronucleus cytome assay to evaluate Chromosomal DNA damage in chronic renal patients undergoing Bicarbonate Haemodialysis and Haemodiafiltration
Introduction. Chronic Renal Failure (CRF) patients are considered to show genomic instability and are associated with a high risk of both cardiovascular diseases and cancer. We explored DNA damage due to two dialysis treatments in 20 patients undergoing bicarbonate haemodialysis, 20 undergoing haemodiafiltration and 40 healthy subjects.Methods. The cytokinesis-block micronucleus (MN) assay was performed on peripheral blood lymphocytes to evaluate genetic damage.Results. A higher frequency of MN in the dialysis groups compared with controls was found. The results do not show a relationship between genetic instability and the type, frequency and duration of haemodialysis. The average BD and HDF treatment time was respectively 3.8±6.3 and 3.7±3.9 yrs. CAT and scintigraphy was independently correlated with high levels of MN.Discussion. Overall, the frequency of MN in CRF patients undergoing dialysis therapy was observed to be higher. Further studies need to be performed on a larger number of patients and for a longer period
Characterization of microplastics in water bottled in different packaging by Raman spectroscopy
Raman spectroscopy was applied to quantitatively and qualitatively characterize microplastics (MPs) in bottled water packaged in three different materials: polyethylene terephthalate (PET), recycled PET (rPET) and glass. The results showed a low mean concentration of MPs in all samples, with higher values in glass bottles (8.65 +/- 5.39 p L-1) than in PET (5.09 +/- 3.28 p L-1) or rPET (3.33 +/- 1.34 p L-1) bottles. Through the use of a method capable of detecting smaller particles, MPs of 20-100 mu m were dominant and fragments were the most abundant particle shape. PET was the prevalent polymer in PET bottles underlying the possible contribution of packaging in MP contamination, while polyethylene (PE) with additives prevailed in rPET and glass bottles, suggesting a contribution from the cap. A standardized protocol would allow comparable data to be obtained and allow an objective assessment of exposure, in view of plans to monitor contaminants of emerging concern (including MPs) under recent European legislation.MPs were detected by Raman spectroscopy in bottled water from different packaging
Spatial distribution of fungal microflora in the sediment of a brackish lake (Lake Alimini Grande, Italy) used for fish production and bathing
A study of the distribution of fungal microflora was conducted on the sediment of Lake Alimini Grande in order to contrib- ute to the evaluation of the ecosystem characteristics that can effect the process of decomposition. The isolation and identification of fungal species and ergosterol analysis were performed on sediment samples taken from 33 monitorin stations in autumn and winter .Altogether, 24 strains belong- ing to 8 genera were isolated. Trichoderma spp (41.6%) and Aspergillus spp (20.8%) were the dominant genera: in par- ticular, Trichoderma was present near the Traugnano marsh, whereas Aspergillus was isolated in the area of connection to the sea
