    Speckle-tracking strain assessment of left ventricular dysfunction in synthetic cannabinoid and heroin users

    Objective: There is growing evidence regarding the numerous adverse effects of synthetic cannabinoids (SCBs) on the cardiovascular system; however, no studies have shown the cardiovascular effects of opioids using strain echocardiography. This study examines the cardiac structure and function using echocardiographic strain imaging in heroin and synthetic cannabinoid users. Methods: This double-blind study included patients who were admitted or referred to a rehabilitation center for heroin (n=31) and synthetic cannabinoid users (n=30). Heroin users and synthetic cannabinoid users were compared with healthy volunteers (n=32) using two-dimensional (2D) speckle-tracking (ST) echocardiography. Results: No differences were found in the baseline characteristics and 2D echocardiography values. The mean global longitudinal strain value was -20.5%+/- 2.4% for SCB users, -22.3%+/- 2.4% for opioid users, and -22.5%+/- 2.2% for healthy volunteers (p=0.024). The mean apical 2-chamber (AP2C) L-strain values were -20.1%+/- 3.1%, -22.4%+/- 3.0%, and -22.3%+/- 2.8% for SCB users, opioid users, and healthy volunteers, respectively (p=0.032). The mean apical 4-chamber (AP4C) L-strain values were -20.7%+/- 2.5% for SCB users, -23.2%+/- 3.2% for opioid users, and -23.8%+/- 3.1% for healthy volunteers (p<0.001). Conclusion: SCBs are potential causes of subclinical left ventricular dysfunction

The dihedral angle between the benzene and thio­phene rings is 62.26 (7)°. The mol­ecular conformation is stabilized by an intra­molecular O—H⋯O hydrogen bond, generating an S(6) ring.     Cervical Vertebral Anomalies in Patients With Transverse Maxillary Deficiency

    Objective:The literature indicates a correlation between vertebral morphologic anomalies and some orthodontic malocclusions. The aim of this study was to examine the occurrence of the types of cervical vertebral anomalies (CVAs) in subjects with transverse maxillary deficiency (TMD) and to compare this with the occurrence of CVA in a control group without TMD.Materials and Method:A sample of 47 Turkish patients (17 boys, 30 girls) with TMD and another sample of 47 Turkish patients (15 boys, 32 girls) with adequate maxillary transversal dimensions was studied. A visual assessment of the cervical column was made using lateral cephalometric films. Characteristics of the cervical column were classified according to Sandham as fusions and posterior arch deficiencies of C1 (PADs). Clinically normal appearance was determined when the first 4 vertebrae could be traced as separate entities with all anatomic components present. Differences in occurrence of CVA between the groups and between genders were assessed by v2 independence test.Results:In the TMD group, 10,6% of the subjects had fusion in the cervical column, and 14,9% had PAD. In the control group, 17% of the subjects had fusion of the cervical column, and 8,5% had PAD. The occurrence of PAD was significantly increased in the TMD group (p,0.05), while no significant difference was found between the groups for occurrence of fusion (p.0.05). No statistically significant gender differences were found in the occurrence of CVA in the TMD group and the control group (p.0.05).Conclusion:No difference was found in the occurrence of fusion between the subjects with TMD and the subjects without TMD, and the occurrence of PAD was significantly increased in subjects with TMD

    Association Between Angiographic Complications and Clinical Outcomes Among Patients With Acute Coronary Syndrome Undergoing Percutaneous Coronary Intervention An EARLY ACS (Early Glycoprotein IIb/IIIa Inhibition in Non–ST-Segment Elevation Acute Coronary Syndrome) Angiographic Substudy

    ObjectivesThe goal of this analysis was to determine the association between intraprocedural complications and clinical outcomes among patients with high-risk non–ST-segment elevation acute coronary syndrome (NSTEACS) undergoing percutaneous coronary intervention (PCI).BackgroundAmong patients undergoing PCI for NSTEACS, the relationship between intraprocedural complications and clinical outcomes, independent of epicardial and myocardial perfusion, has not been well characterized.MethodsThe EARLY ACS (Early Glycoprotein IIb/IIIa Inhibition in Non–ST-Segment Elevation Acute Coronary Syndrome) trial enrolled 9,406 patients with high-risk NSTEACS undergoing an early invasive strategy. Of these, 1,452 underwent angiographic assessment in an independent core laboratory and did not have a myocardial infarction (MI) between enrollment and angiography. We assessed the relationship between abrupt closure, loss of side branch(es), distal embolization, and no-reflow phenomenon and 30-day clinical outcomes in these patients.ResultsOf the patients, 166 (11.4%) experienced an intraprocedural complication. Baseline clinical characteristics were similar between patients who did and did not have complications. The 30-day composite of death or MI was significantly higher among patients with an intraprocedural complication (28.3% vs. 7.8%, odds ratio [OR]: 4.68, 95% confidence interval [CI]: 3.2 to 7.0, p < 0.001). Individually, both mortality (3.0% vs. 0.9%, OR: 3.60, 95% CI: 1.2 to 10.5, p = 0.019) and MI (27.1% vs. 7.4%, OR: 4.66, 95% CI: 3.1 to 7.0, p < 0.001) were significantly increased. After adjusting for differences in post-PCI epicardial and myocardial perfusion, the association with 30-day death or MI remained significant.ConclusionsAmong high-risk NSTEACS patients undergoing an invasive strategy, the incidence of intraprocedural complications is high, and the occurrence of these complications is associated with worse clinical outcomes independent of epicardial and myocardial perfusion.(Early Glycoprotein IIb/IIIa Inhibition in Patients With Non–ST-segment Elevation Acute Coronary Syndrome [EARLY ACS]; NCT00089895

    In this paper, an attempt is made to model the total power requirements of a full scale Reflective Intelligent Surface (RIS) with switches(microfluidics based and CMOS based) operating at 140 GHz frequencies.
