Analysis of Bcl6 in a genetically tractable human B cell line
B cell lymphoma 6 (Bcl6) is a proto-oncogene functioning as a transcriptional
repressor at the germinal centre (GC) stage of B cell development, regulating
proliferation, differentiation and survival. Bcl6 is commonly dysregulated in GCderived
lymphomas and hence a promising therapeutic target. Although a
number of agents targeting Bcl6 are being developed, there is currently no
good model available for functional analysis and hence drug target validation.
To address this issue, I have established a conditional BCL6 knockout cell line
for in-depth functional analyses of Bcl6. I have used homologous recombination
to disrupt one BCL6 allele in the human Burkittâs lymphoma cell line DG75 with
a zeocin resistance cassette and have shown this line to be capable of
supporting tetracycline (tet)-regulated Bcl6 expression from an integrated
expression construct. The zeocin cassette used to disrupt the first BCL6 allele
was subsequently deleted by Cre recombination. This allowed me to reuse the
same targeting construct to disrupt the second BCL6allele. I subsequently set
out to characterise the resulting conditional knockout cell line and found that
depletion of Bcl6 leads to less proliferation, characterised by a prolonged
G0/G1cell cycle phase. I went on to further characterise the effect of Bcl6
depletion in my system by cDNA microarray analysis, which revealed
interesting insights into functions of Bcl6 such as the modulation of B cell
receptor signalling and calcium signalling. The system I have developed is the
first precisely regulated genetic model for studying Bcl6 function and will be of
great benefit for research aiming to develop Bcl6 targeted drug therapy
Effects of Pay for Performance on Performance Outcomes in Pennsylvaniaâs Medicaid Managed Care Program
Background and Purpose: A goal in healthcare is to reduce costs while improving quality of care, and population health. Pay for Performance (P4P) is one intervention being utilized by Medicaid programs to achieve this goal. While P4P has become a favorite among policymakers, its effectiveness in improving health outcomes is still under investigation. We assessed the role of P4P to improve provider performance by evaluating Pennsylvaniaâs Medicaid Managed Care (MMC) Program.
Methods: We conducted a retrospective longitudinal analysis of providers within the HealthChoices MMC Program in Pennsylvania. We examined annual provider performance outcomes for seven providers across six measures collected between 2012 and 2016. Performance outcomes were compared to inflation-adjusted incentive payouts across providers.
Results/Outcomes: Between 2012 and 2016 controlling high blood pressure, comprehensive diabetes monitoring, prenatal care in the first trimester and annual dental visits improved between 1.2 to 6.5 percent. Performance on frequency of prenatal care and adolescent well care visits declined by 1.4 and 2.7 percent respectively. While controlling for inflation, benchmark and improvement payouts increased over time except for adolescent well care visit payouts. In 2016, payout as a percent of annual revenue ranged from 0.3 to 1.44 percent.
Conclusions: Regardless of pay-outs, the P4P initiative tracks performance measures which may be increasing provider awareness and therefore driving improvements. In addition to individual metrics, opportunities exist to consider bundled measures to see if focus on disease-approach rather than individual procedures is a more effective means to drive quality improvement and cost containment.
Clinical and Social Relevance: Pay for Performance may still be a viable solution for improving accessibility, affordability, and quality for vulnerable populations. Knowing definitively should be made a priority to best suit the health needs of Pennsylvaniaâs Medicaid Population

Not so Dangerous After All? Venom Composition and Potency of the Pholcid (Daddy Long-Leg) Spider Physocyclus mexicanus
Pholcid spiders (Araneae: Pholcidae), officially "cellar spiders" but popularly known as "daddy long-legs," are renown for the potential of deadly toxic venom, even though venom composition and potency has never formally been studied. Here we detail the venom composition of male Physocyclus mexicanus using proteomic analyses and venom-gland transcriptomes ("venomics"). We also analyze the venom's potency on insects, and assemble available evidence regarding mammalian toxicity. The majority of the venom (51% of tryptic polypeptides and 62% of unique tryptic peptides) consists of proteins homologous to known venom toxins including enzymes (astacin metalloproteases, serine proteases and metalloendopeptidases, particularly neprilysins) and venom peptide neurotoxins. We identify 17 new groups of peptides (U1-17-PHTX) most of which are homologs of known venom peptides and are predicted to have an inhibitor cysteine knot fold; of these, 13 are confirmed in the proteome. Neprilysins (M13 peptidases), and astacins (M12 peptidases) are the most abundant venom proteins, respectively representing 15 and 11% of the individual proteins and 32 and 20% of the tryptic peptides detected in crude venom. Comparative evidence suggests that the neprilysin gene family is expressed in venoms across a range of spider taxa, but has undergone an expansion in the venoms of pholcids and may play a central functional role in these spiders. Bioassays of crude venoms on crickets resulted in an effective paralytic dose of 3.9 mu g/g, which is comparable to that of crude venoms of Plectreurys tristis and other Synspermiata taxa. However, crickets exhibit flaccid paralysis and regions of darkening that are not observed after P. tristis envenomation. Documented bites on humans make clear that while these spiders can bite, the typical result is a mild sting with no long-lasting effects. Another Shipment of Six Short-Period Giant Planets from TESS
We present the discovery and characterization of six short-period, transiting
giant planets from NASA's Transiting Exoplanet Survey Satellite (TESS) --
TOI-1811 (TIC 376524552), TOI-2025 (TIC 394050135), TOI-2145 (TIC 88992642),
TOI-2152 (TIC 395393265), TOI-2154 (TIC 428787891), & TOI-2497 (TIC 97568467).
All six planets orbit bright host stars (8.9 <G< 11.8, 7.7 <K< 10.1). Using a
combination of time-series photometric and spectroscopic follow-up observations
from the TESS Follow-up Observing Program (TFOP) Working Group, we have
determined that the planets are Jovian-sized (R = 1.00-1.45 R),
have masses ranging from 0.92 to 5.35 M, and orbit F, G, and K stars
(4753 T 7360 K). We detect a significant orbital eccentricity
for the three longest-period systems in our sample: TOI-2025 b (P = 8.872 days,
= ), TOI-2145 b (P = 10.261 days, =
), and TOI-2497 b (P = 10.656 days, =
). TOI-2145 b and TOI-2497 b both orbit subgiant host
stars (3.8 g 4.0), but these planets show no sign of inflation
despite very high levels of irradiation. The lack of inflation may be explained
by the high mass of the planets; M (TOI-2145
b) and M (TOI-2497 b). These six new discoveries
contribute to the larger community effort to use {\it TESS} to create a
magnitude-complete, self-consistent sample of giant planets with

Association of respiratory symptoms and lung function with occupation in the multinational Burden of Obstructive Lung Disease (BOLD) study
Background
Chronic obstructive pulmonary disease has been associated with exposures in the workplace. We aimed to assess the association of respiratory symptoms and lung function with occupation in the Burden of Obstructive Lung Disease study.
Methods
We analysed cross-sectional data from 28â823 adults (â„40â
years) in 34 countries. We considered 11 occupations and grouped them by likelihood of exposure to organic dusts, inorganic dusts and fumes. The association of chronic cough, chronic phlegm, wheeze, dyspnoea, forced vital capacity (FVC) and forced expiratory volume in 1â
s (FEV1)/FVC with occupation was assessed, per study site, using multivariable regression. These estimates were then meta-analysed. Sensitivity analyses explored differences between sexes and gross national income.
Results
Overall, working in settings with potentially high exposure to dusts or fumes was associated with respiratory symptoms but not lung function differences. The most common occupation was farming. Compared to people not working in any of the 11 considered occupations, those who were farmers for â„20â
years were more likely to have chronic cough (OR 1.52, 95% CI 1.19â1.94), wheeze (OR 1.37, 95% CI 1.16â1.63) and dyspnoea (OR 1.83, 95% CI 1.53â2.20), but not lower FVC (ÎČ=0.02â
L, 95% CI â0.02â0.06â
L) or lower FEV1/FVC (ÎČ=0.04%, 95% CI â0.49â0.58%). Some findings differed by sex and gross national income.
Conclusion
At a population level, the occupational exposures considered in this study do not appear to be major determinants of differences in lung function, although they are associated with more respiratory symptoms. Because not all work settings were included in this study, respiratory surveillance should still be encouraged among high-risk dusty and fume job workers, especially in low- and middle-income countries.publishedVersio

Cohort Profile: Burden of Obstructive Lung Disease (BOLD) study
The Burden of Obstructive Lung Disease (BOLD) study was established to assess the prevalence of chronic airflow obstruction, a key characteristic of chronic obstructive pulmonary disease, and its risk factors in adults (â„40 years) from general populations across the world.
The baseline study was conducted between 2003 and 2016, in 41 sites across Africa, Asia, Europe, North America, the Caribbean and Oceania, and collected high-quality pre- and post-bronchodilator spirometry from 28â828 participants.
The follow-up study was conducted between 2019 and 2021, in 18 sites across Africa, Asia, Europe and the Caribbean. At baseline, there were in these sites 12â502 participants with high-quality spirometry. A total of 6452 were followed up, with 5936 completing the study core questionnaire. Of these, 4044 also provided high-quality pre- and post-bronchodilator spirometry.
On both occasions, the core questionnaire covered information on respiratory symptoms, doctor diagnoses, health care use, medication use and ealth status, as well as potential risk factors. Information on occupation, environmental exposures and diet was also collected
Analysis of Bcl6 in a genetically tractable human B cell line
B cell lymphoma 6 (Bcl6) is a proto-oncogene functioning as a transcriptional repressor at the germinal centre (GC) stage of B cell development, regulating proliferation, differentiation and survival. Bcl6 is commonly dysregulated in GCderived lymphomas and hence a promising therapeutic target. Although a number of agents targeting Bcl6 are being developed, there is currently no good model available for functional analysis and hence drug target validation. To address this issue, I have established a conditional BCL6 knockout cell line for in-depth functional analyses of Bcl6. I have used homologous recombination to disrupt one BCL6 allele in the human Burkittâs lymphoma cell line DG75 with a zeocin resistance cassette and have shown this line to be capable of supporting tetracycline (tet)-regulated Bcl6 expression from an integrated expression construct. The zeocin cassette used to disrupt the first BCL6 allele was subsequently deleted by Cre recombination. This allowed me to reuse the same targeting construct to disrupt the second BCL6allele. I subsequently set out to characterise the resulting conditional knockout cell line and found that depletion of Bcl6 leads to less proliferation, characterised by a prolonged G0/G1cell cycle phase. I went on to further characterise the effect of Bcl6 depletion in my system by cDNA microarray analysis, which revealed interesting insights into functions of Bcl6 such as the modulation of B cell receptor signalling and calcium signalling. Weather whiplash events in Europe and North Atlantic assessed as continental-scale atmospheric regime shifts
Abstract The term âweather whiplashâ describes abrupt transitions from one persistent weather regime to another substantially different one, such as from a frigid cold spell to anomalous warmth. Weather whiplash events (WWEs) are often highly disruptive to agriculture, ecosystems, infrastructure, and human activities. While no consistent definition exists, we identify WWEs based on substantial shifts in the continental-scale, upper-atmosphere circulation. As first demonstrated in our earlier study focused on the NE Pacific/North American region, a WWE is detected when one persistent, large-scale pattern in 500 hPa height anomalies shifts to another distinctly different one. Patterns are identified using self-organizing maps (SOMs), which create a matrix of representative patterns in the data. In the present study, we apply this approach to identify WWEs in the North Atlantic/European sector. We analyze the occurrence of WWEs originating with long-duration events (four or more days) in each pattern. A WWE is detected when the pattern two days following a long-duration event is substantially different, measured with distance thresholds internal to the matrix. Changes in WWE frequency, past and future, are assessed objectively based on reanalysis output and climate model simulations. We find that future changes under RCP 8.5 forcing exhibit distinct trends, especially in summer months, while those based on reanalysis are less clear. Patterns featuring positive height anomalies in high latitudes are projected to produce more WWEs in the future, while patterns exhibiting negative anomalies produce fewer. Shifts in temperature and precipitation extremes associated with these WWEs are diagnosed
