    The prognostic value of different glucose abnormalities in patients with acute myocardial infarction treated invasively

    BACKGROUND: Diabetes (DM) deteriorates the prognosis in patients with coronary heart disease. However, the prognostic value of different glucose abnormalities (GA) other than DM in subjects with acute myocardial infarction (AMI) treated invasively remains unclear. AIMS: To assess the incidence and impact of GA on clinical outcomes in AMI patients treated with percutaneous coronary intervention (PCI). METHODS: A single-center, prospective registry encompassed 2733 consecutive AMI subjects treated with PCI. In all in-hospital survivors (n = 2527, 92.5%) without the history of DM diagnosed before or during index hospitalization standard oral glucose tolerance test (OGTT) was performed during stable condition before hospital discharge and interpreted according to WHO criteria. The mean follow-up period was 37.5 months. RESULTS: The incidence of GA was as follows: impaired fasting glycaemia - IFG (n = 376, 15%); impaired glucose tolerance - IGT (n = 560, 22%); DM (n = 425, 17%); new onset DM (n = 384, 15%); and normal glucose tolerance – NGT (n = 782, 31%). During the long-term follow-up, death rate events for previously known DM, new onset DM and IGT were significantly more frequent than those for IFG and NGT (12.3; 9.6 and 9.4 vs. 5.6 and 6.4%, respectively, P < 0.05). The strongest and common independent predictors of death in GA patients were glomerular filtration rate < 60 ml/min/1,73 m^2 (HR 2.0 and 2.8) and left ventricle ejection fraction < 35% (HR 2.5 and 1.8, all P < 0.05) respectively. CONCLUSIONS: Glucose abnormalities are very common in AMI patients. DM, new onset DM and IGT increase remote mortality. Impaired glucose tolerance bears similar long-term prognosis as diabetes

In order to check this, we asked over one thousand two hundred young Poles to evaluate their identification with the transhumanistic theory of education. The results are quite surprising: its show that they disagree with the assumptions of this theory, while they rather agree with the postulates of more traditional (and no technology-based) concepts of education

    Hematopoietic stem cell mobilization with the reversible CXCR4 receptor inhibitor plerixafor (AMD3100)—Polish compassionate use experience

    Recent developments in the field of targeted therapy have led to the discovery of a new drug, plerixafor, that is a specific inhibitor of the CXCR4 receptor. Plerixafor acts in concert with granulocyte colony-stimulating factor (G-CSF) to increase the number of stem cells circulating in the peripheral blood (PB). Therefore, it has been applied in the field of hematopoietic stem cell mobilization. We analyzed retrospectively data regarding stem cell mobilization with plerixafor in a cohort of 61 patients suffering from multiple myeloma (N = 23), non-Hodgkin’s lymphoma (N = 20), or Hodgkin’s lymphoma (N = 18). At least one previous mobilization attempt had failed in 83.6% of these patients, whereas 16.4% were predicted to be poor mobilizers. The median number of CD34+ cells in the PB after the first administration of plerixafor was 22/μL (range of 0–121). In total, 85.2% of the patients proceeded to cell collection, and a median of two (range of 0–4) aphereses were performed. A minimum of 2.0 × 106 CD34+ cells per kilogram of the patient’s body weight (cells/kg b.w.) was collected from 65.6% of patients, and the median number of cells collected was 2.67 × 106 CD34+ cells/kg b.w. (0–8.0). Of the patients, 55.7% had already undergone autologous stem cell transplantation, and the median time to neutrophil and platelet reconstitution was 12 and 14 days, respectively. Cases of late graft failure were not observed. We identified the diagnosis of non-Hodgkin’s lymphoma and previous radiotherapy as independent factors that contributed to failure of mobilization. The current report demonstrates the satisfactory efficacy of plerixafor plus G-CSF for stem cell mobilization in heavily pre-treated poor or predicted poor mobilizers

    Communication skills learning during medical studies in Poland: opinions of final-year medical students

    INTRODUCTION: Despite the increasing emphasis on the medical curriculum, students’ empathy and communicationskills decline with time. As students’ attitudes and experiences may influence the effectivenessof communication training and their future communication style, this study aimed to evaluate the opinionsof last-year medical students on communication skills learning.MATERIAL AND METHODS: Following the fully mixed concurrent equal status design methodology, we invitedsixth-year medical students of our university to fill a paper questionnaire with closed and open questions ontheir perspectives on communication skills learning.RESULTS: Questionnaires were completed by a representative sample of 166 students. The majority of themrecognized the importance and benefits of communication training, and the need to increase its significancein the medical curriculum. They noticed that students with low communication skills lack motivationfor improvement. Students also underlined the significance of the hidden curriculum, reporting a contrastbetween communication classes and the behavior of some physicians. Moreover, students less positively inclinedtowards communication training doubted its effectiveness and importance, suggesting that it shouldbe offered for volunteers.CONCLUSIONS: In order to meet the expectations of students with positive opinions and overcome the lackof motivation or engagement of negatively inclined students, more emphasis should be placed on communicationtraining. Changes in communication training should include increasing its practical character andintensity, emphasizing its importance and benefits, and paying more attention to hidden curriculum aspects.Revisions to the medical school application system should be considered to underline the importanceof interpersonal competencies already at the admission stage

    Changes in the Concentrations of Proangiogenic Cytokines in Human Brain Glioma and Acute Lymphoblastic Leukemia

    Acute lymphoblastic leukemia (ALL) and glioma are some of the most common malignancies, with ALL most often affecting children and glioma affecting adult men. Proangiogenic cytokines and growth factors play an important role in the development of both of these tumors. Glioma is characterized by an extremely extensive network of blood vessels, which continues to expand mainly in the process of neoangiogenesis, the direct inducers of which are cytokines from the family of vascular endothelial growth factors, i.e., vascular endothelial growth factor (VEGF-A) and its receptor vascular endothelial growth factor receptor 2 (VEGF-R2), as well as a cytokine from the fibroblast growth factor family, fibroblast growth factor 2 (FGF-2 or bFGF). Growth factors are known primarily for their involvement in the progression and development of solid tumors, but there is evidence that local bone marrow angiogenesis and increased blood vessel density are also present in hematological malignancies, including leukemias. The aim of this study was to examine changes in the concentrations of VEGF-A, VEGF-R2, and FGF-2 (with a molecular weight of 17 kDa) in a group of patients divided into specific grades of malignancy (glioma) and a control group; changes of VEGF-A and FGF-2 concentrations in childhood acute lymphoblastic leukemia and a control group; and to determine correlations between the individual proteins as well as the influence of the patient’s age, diet, and other conditions that may place the patient in the risk group. During the statistical analysis, significant differences in concentrations were found between the patient and control groups in samples from people with diagnosed glioma and from children with acute lymphoblastic leukemia, but in general, there are no significant differences in the concentrations of VEGF-A, VEGF-R2, and FGF-2 between different grades of glioma malignancy. Among individuals treated for glioma, there was no significant impact from the patient’s gender and age, consumption of food from plastic packaging, frequency of eating vegetables and fruit, smoking of tobacco products, the intensity of physical exercise, or the general condition of the body (Karnofsky score) on the concentrations of the determined cytokines and receptor. The listed factors do not bring about an actual increase in the risk of developing brain glioma

    Looking for Razors and Needles in a Haystack: Multifaceted Analysis of Suicidal Declarations on Social Media—A Pragmalinguistic Approach

    In this paper, we study language used by suicidal users on Reddit social media platform. To do that, we firstly collect a large-scale dataset of Reddit posts and annotate it with highly trained and expert annotators under a rigorous annotation scheme. Next, we perform a multifaceted analysis of the dataset, including: (1) the analysis of user activity before and after posting a suicidal message, and (2) a pragmalinguistic study on the vocabulary used by suicidal users. In the second part of the analysis, we apply LIWC, a dictionary-based toolset widely used in psychology and linguistic research, which provides a wide range of linguistic category annotations on text. However, since raw LIWC scores are not sufficiently reliable, or informative, we propose a procedure to decrease the possibility of unreliable and misleading LIWC scores leading to misleading conclusions by analyzing not each category separately, but in pairs with other categories. The analysis of the results supported the validity of the proposed approach by revealing a number of valuable information on the vocabulary used by suicidal users and helped to pin-point false predictors. For example, we were able to specify that death-related words, typically associated with suicidal posts in the majority of the literature, become false predictors, when they co-occur with apostrophes, even in high-risk subreddits. On the other hand, the category-pair based disambiguation helped to specify that death becomes a predictor only when co-occurring with future-focused language, informal language, discrepancy, or 1st person pronouns. The promising applicability of the approach was additionally analyzed for its limitations, where we found out that although LIWC is a useful and easily applicable tool, the lack of any contextual processing makes it unsuitable for application in psychological and linguistic studies. We conclude that disadvantages of LIWC can be easily overcome by creating a number of high-performance AI-based classifiers trained for annotation of similar categories as LIWC, which we plan to pursue in future work
