    TEST OF PHYSIOLOGICAL PERFORMANCE: RATIONALE AND FEASIBILITY

    Rigorous clinical evaluation of the physiological performance is currently performed with complex and long procedures which need expensive technology and skilled operators. In a wide range of situations (frail patients, daily clinical practice, etc.), these approaches are difficult to be applied and simpler tests, with a lack of scientific background, are mandatory. To avoid these problems, we propose a test (test of physiological performance (TOPP)) to evaluate the physiological behavior of a subject, in a really easy and safe clinical setting, measuring only the heart rate. The subject is submitted to an active standing-up test and then two submaximal exercises (with a low power load) on a cycle-ergometer. The heart rate modifications due to each submaximal step are analyzed by exponential interpolation to calculate the ascending and descending time constants and evaluate the way each subject adapts his heart rate to work. The standard deviation of the RR for each stationary phase (warm-up, load, recovery) was calculated as an index of short-term variability. Then a standard Fourier analysis of the stationary periods of the standing-up procedures allows to quickly and easily evaluate the autonomic nervous activation. We tested the protocol on five healthy subjects to verify the feasibility and the acceptance of the procedure. The five subjects demonstrated a good tolerance of the entire procedure. The standing-up showed a behavior of the autonomic system consistent with the physiology (with an increase in sympathetic activation in the passage to standing position). The analysis of the two submaximal steps highlights how younger and trained subjects present lower heart rates (both in the ascending phase and in the recovery) with a quicker adaptation ability (smaller time constants) consistent with what is expected. The short-term variability of heart rate is greater in young and trained subjects, thus confirming how the sympatho-vagal balance, in these subjects, is more dynamic. The proposed test is well tolerated by the subjects and the results, albeit in a small cohort of healthy volunteers, are consistent with what is expected from physiology and is already present in the literature. Our work aims to be a proposal with a feasibility check of a method for evaluating performance. The work to be done for the clinical validation of the TOPP is still long, but we are aware that it can give important results and that the TOPP can become an effective tool for the assessment of the physiological performance even of fragile subjects

    Film cooling adiabatic effectiveness measurements of pressure side trailing edge cooling configurations

    Nowadays total inlet temperature of gas turbine is far above the permissible metal temperature; as a consequence, advanced cooling techniques must be applied to protect from thermal stresses, oxidation and corrosion the components located in the high pressure stages, such as the blade trailing edge. A suitable design of the cooling system for the trailing edge has to cope with geometric constraints and aerodynamic demands; state-of-the-art of cooling concepts often use film cooling on blade pressure side: the air taken from last compressor stages is ejected through discrete holes or slots to provide a cold layer between hot mainstream and the blade surface. With the goal of ensuring a satisfactory lifetime of blades, the design of efficient trailing edge film cooling schemes and, moreover, the possibility to check carefully their behavior, are hence necessary to guarantee an appropriate metal temperature distribution. For this purpose an experimental survey was carried out to investigate the film covering performance of different pressure side trailing edge cooling systems for turbine blades. The experimental test section consists of a scaled-up trailing edge model installed in an open loop suction type test rig. Measurements of adiabatic effectiveness distributions were carried out on three trailing edge cooling system configurations. The baseline geometry is composed by inclined slots separated by elongated pedestals; the second geometry shares the same cutback configuration, with an additional row of circular film cooling holes located upstream; the third model is equipped with three rows of in-line film cooling holes. Experiments have been performed at nearly ambient conditions imposing several blowing ratio values and using carbon dioxide as coolant in order to reproduce a density ratio close to the engine conditions (DR=1.52). To extend the validity of the survey a comparison between adiabatic effectiveness measurements and a prediction by correlative approach was performed to compare the experimental results with 1D methodologies

    Selective Laser Sintering versus Multi Jet Fusion: A Comprehensive Comparison Study Based on the Properties of Glass Beads-Reinforced Polyamide 12

    Selective laser sintering (SLS) and multi jet fusion (MJF) are the most widespreadpowder bed fusion additive manufacturing techniques for fabricating polymericparts since they offer great designflexibility, productivity, and geometricalaccuracy. However, these technologies differ in the thermal energy source usedto melt the powders as well as the innovative use of printing agents featured inthe latter one to promote material consolidation and to avoid thermal bleeding atthe part contours. The use of a single powder made of glass beads-reinforcedpolyamide 12 (PA12/GB) for the fabrication of MJF and SLS samples makespossible a systematic comparison of the printed parts properties. A thoughtfulanalysis of the microstructure and mechanical properties of the samples revealsdifferences and peculiarities between the two technologies. SLS exhibits lowerporosity and higher mechanical performances when the parts are printed alongthe build plane thanks to the powerful heating ensured by the laser. In contrast,MJF samples show higher mechanical isotropy with greatflexural and tensilebehavior for vertically oriented parts. The role of glass beads in the materialbehavior is defined by their mechanical properties, meaning higher rigidity andlower strength compared to neat PA12, and fracture mechanism

    Mesenchymal stromal cells (MSCs) are the leading cell candidates in the field of regenerative medicine. These cells have also been successfully used to improve skeletal muscle repair/regeneration; however, the mechanisms responsible for their beneficial effects remain to be clarified. On this basis, in the present study, we evaluated in a co-culture system, the ability of bone-marrow MSCs to influence C2C12 myoblast behavior and analyzed the cross-talk between the two cell types at the cellular and molecular level. We found that myoblast proliferation was greatly enhanced in the co-culture as judged by time lapse videomicroscopy, cyclin A expression and EdU incorporation. Moreover, myoblasts immunomagnetically separated from MSCs after co-culture expressed higher mRNA and protein levels of Notch-1, a key determinant of myoblast activation and proliferation, as compared with the single culture. Notch-1 intracellular domain and nuclear localization of Hes-1, a Notch-1 target gene, were also increased in the co-culture. Interestingly, the myoblastic response was mainly dependent on the paracrine release of vascular endothelial growth factor (VEGF) by MSCs. Indeed, the addition of MSC-derived conditioned medium (CM) to C2C12 cells yielded similar results as those observed in the co-culture and increased the phosphorylation and expression levels of VEGFR. The treatment with the selective pharmacological VEGFR inhibitor, KRN633, resulted in a marked attenuation of the receptor activation and concomitantly inhibited the effects of MSC-CM on C2C12 cell growth and Notch-1 signaling. In conclusion, this study provides novel evidence for a role of MSCs in stimulating myoblast cell proliferation and suggests that the functional interaction between the two cell types may be exploited for the development of new and more efficient cell-based skeletal muscle repair strategies

    Surfactant lung delivery with LISA and InSurE in adult rabbits with respiratory distress

    Background In preterm infants, InSurE (Intubation–Surfactant–Extubation) and LISA (less invasive surfactant administration) techniques allow for exogenous surfactant administration while reducing lung injury associated with mechanical ventilation. We compared the acute pulmonary response and lung deposition of surfactant by LISA and InSurE in surfactant-depleted adult rabbits. Methods Twenty-six spontaneously breathing surfactant-depleted adult rabbits (6–7 weeks old) with moderate RDS and managed with nasal continuous positive airway pressure were randomized to 3 groups: (1) 200 mg/kg of surfactant by InSurE; (2) 200 mg/kg of surfactant by LISA; (3) no surfactant treatment (Control). Gas exchange and lung mechanics were monitored for 180 min. After that, surfactant lung deposition and distribution were evaluated monitoring disaturated-phosphatidylcholine (DSPC) and surfactant protein C (SP-C), respectively. Results No signs of recovery were found in the untreated animals. After InSurE, oxygenation improved more rapidly compared to LISA. However, at 180’ LISA and InSurE showed comparable outcomes in terms of gas exchange, ventilation parameters, and lung mechanics. Neither DSPC in the alveolar pool nor SP-C signal distributions in a frontal lung section were significantly different between InSurE and LISA groups. Conclusions In an acute setting, LISA demonstrated efficacy and surfactant lung delivery similar to that of InSurE in surfactant-depleted adult rabbits. Impact Although LISA technique is gaining popularity, there are still several questions to address. This is the first study comparing LISA and InSurE in terms of gas exchange, ventilation parameters, and lung mechanics as well as surfactant deposition and distribution. In our animal study, three hours post-treatment, LISA method seems to be as effective as InSurE and showed similar surfactant lung delivery. Our findings provide some clarifications on a fair comparison between LISA and InSurE techniques, particularly in terms of surfactant delivery. They should reassure some of the concerns raised by the clinical community on LISA adoption in neonatal units

    Phase transitions in biological membranes

    Native membranes of biological cells display melting transitions of their lipids at a temperature of 10-20 degrees below body temperature. Such transitions can be observed in various bacterial cells, in nerves, in cancer cells, but also in lung surfactant. It seems as if the presence of transitions slightly below physiological temperature is a generic property of most cells. They are important because they influence many physical properties of the membranes. At the transition temperature, membranes display a larger permeability that is accompanied by ion-channel-like phenomena even in the complete absence of proteins. Membranes are softer, which implies that phenomena such as endocytosis and exocytosis are facilitated. Mechanical signal propagation phenomena related to nerve pulses are strongly enhanced. The position of transitions can be affected by changes in temperature, pressure, pH and salt concentration or by the presence of anesthetics. Thus, even at physiological temperature, these transitions are of relevance. There position and thereby the physical properties of the membrane can be controlled by changes in the intensive thermodynamic variables. Here, we review some of the experimental findings and the thermodynamics that describes the control of the membrane function.Comment: 23 pages, 15 figure
