    A rare case of severe gastroenteritis caused by Aeromonas hydrophila after colectomy in a patient with anti-Hu syndrome: a case report

    BACKGROUND Aeromonas hydrophila is a gram-negative facultative anaerobic coccobacillus, which is an environmental opportunistic pathogen. A. hydrophila are involved in several infectious diseases such as gastroenteritis, septicemia and wound infections. However, gastroenteritis caused by Aeromonas spp. are rare and the clinical relevance of Aeromonas species in stool specimens is still under debate. CASE PRESENTATION Our case concerns a 32-year-old woman who presented at hospital with a worsening watery diarrhea and fever requiring intensive care. A cholera-like illness was diagnosed. The patient had a past history of an anti-Hu syndrome with a myenteric ganglionitis. A molecular multiplex RT-PCR (QIAstat-Dx Gastrointestinal Panel, QIAGEN) covering a broad spectrum of diverse gastrointestinal pathogens performed directly from the stool was negative but the stool culture revealed growth of A. hydrophila. Further investigations of the A. hydrophila strain in cell cultures revealed the presence of a cytotoxic enterotoxin. CONCLUSIONS Although A. hydrophila rarely causes gastroenteritis, Aeromonas spp. should be considered as a causative agent of severe gastroenteritis with a cholera-like presentation. This case highlights the need to perform culture methods from stool samples when PCR-based methods are negative and gastrointestinal infection is suspected

Die Voraussetzungen für die Teilnahme an Hochschulbildung reduzierten sich auf die Existenz eines Internetanschlusses. Bald aber stellten die MOOC-Anbieter fest, dass Flexibilisierung und tiefe Kosten nicht genügten, um Hochschulbildung zu konkurrenzieren. Daher rich(te)ten sich MOOC-Anbieter neu aus – auf Weiterbildung und damit Personen mit abgeschlossener Hochschulbildung. Auch deshalb sind MOOCs bisher, gemessen am Ziel der Disruption traditioneller Hochschulbildung, als Misserfolg zu werten

    Evaluation of commercial slides for detection of immunoglobulin G against Bartonella henselae by indirect immunofluorescence

    Four commercial slides were compared with in-house slides for the detection of immunoglobulin G (IgG) againstBartonella henselae in 58 healthy persons from a rural region by an indirect immunofluorescence assay. MRL-BA slides (MRL Diagnostics, USA) and Virion slides (Virion, Switzerland) with agar-derivedBartonella henselae showed IgG titers of ≥ 1∶256 in 44.8% and 51.7%, respectively, whereas Bion slides (Bios, Germany), MRL-Vero slides (MRL Diagnostics), and in-house slides with cell-associatedBartonella henselae showed such titers in 3.4%, 5.1% and 3.4%, respectively. The MRL-Vero slides (Bartonella IgG substrate slides, MRL Diagnostics) were further evaluated with 26 patients with cat scratch disease, 20 patients with lymphadenopathy not due to cat scratch disease, 100 blood donors from an urban area, and 120 blood donors from a mixed urban/rural area. In our mixed urban/rural population the IgG titer of 1∶256 had a sensitivity of 84.6% and a specificity of 93.4% for the serodiagnosis of cat scratch disease. Seroprevalence was higher in blood donors from the mixed area (50.8%) than from the urban area (37%). MRL-Vero slides were considered useful for the serodiagnosis of cat scratch disease by indirect immunofluorescence and have replaced our in-house system.     Purpose: The SAMBA 2 BB audio processor for the BONEBRIDGE bone conduction implant features a new automatic listening environment detection to focus on target speech and to reduce interfering speech and background noises. The aim of this study was to evaluate the audiological benefit of the SAMBA 2 BB (AP2) and to compare it with its predecessor SAMBA BB (AP1). Methods: Prospective within-subject comparison study. We compared the aided sound field hearing thresholds, speech understanding in quiet (Freiburg monosyllables), and speech understanding in noise (Oldenburg sentence test) with the AP1 and AP2. Each audio processor was worn for 2 weeks before assessment and seven users with single-sided sensorineural deafness (SSD) participated in the study. For speech understanding in noise, two complex noise scenarios with multiple noise sources including single talker interfering speech were used. The first scenario included speech presented from the front (S0NMIX), while in the second scenario speech was presented from the side of the implanted ear (SIPSINMIX). In addition, subjective evaluation using the SSQ12, APSQ, and the BBSS questionnaires was performed. Results: We found improved speech understanding in quiet with the AP2 compared to the AP1 aided condition (on average + 17%, p = 0.007). In both noise scenarios, the AP2 lead to improved speech reception thresholds by 1.2 dB (S0NMIX, p = 0.032) and 2.1 dB (SIPSINMIX, p = 0.048) compared to the AP1. The questionnaires revealed no statistically significant differences, except an improved APSQ usability score with the AP2. Conclusion: Clinicians can expect that patients with SSD will benefit from the SAMBA 2 BB by improved speech understanding in both quiet and in complex noise scenarios, when compared to the older SAMBA BB.     Risk of decompression illness among 230 divers in relation to the presence and size of patent foramen ovale

    Background The risk of developing decompression illness (DCI) in divers with a patent foramen ovale (PFO) has not been directly determined so far; neither has it been assessed in relation to the PFO's size. Methods In 230 scuba divers (age 39±8 years), contrast trans-oesophageal echocardiography (TEE) was performed for the detection and size grading (0-3) of PFO. Prior to TEE, the study individuals answered a detailed questionnaire about their health status and about their diving habits and accidents. For inclusion into the study, ⩾200 dives and strict adherence to decompression tables were required. Results Sixty-three divers (27%) had a PFO. Overall, the absolute risk of suffering a DCI event was 2.5 per 104 dives. There were 18 divers (29%) with, and 10 divers (6%) without, PFO who had experienced ⩾1 major DCI events \batchmode \documentclass[fleqn,10pt,legalpaper]{article} \usepackage{amssymb} \usepackage{amsfonts} \usepackage{amsmath} \pagestyle{empty} \begin{document} (P=0.016)(P=0.016) \end{document}. In the group with PFO, the incidence per 104 dives of a major DCI, a DCI lasting longer than 24 h and of being treated in a decompression chamber amounted to 5.1 (median 0, interquartile range [IQR] 0-10.0), 1.9 (median 0, IQR 0-4.0) and 3.6 (median 0, IQR 0-9.8), respectively and was 4.8-12.9-fold higher than in the group without PFO \batchmode \documentclass[fleqn,10pt,legalpaper]{article} \usepackage{amssymb} \usepackage{amsfonts} \usepackage{amsmath} \pagestyle{empty} \begin{document} (P<0.001)(P{<}0.001) \end{document}. The risk of suffering a major DCI, of a DCI lasting longer than 24 h and of being treated by recompression increased with rising PFO size. Conclusion The presence of a PFO is related to a low absolute risk of suffering five major DCI events per 104 dives, the odds of which is five times as high as in divers without PFO.     Falcon Takes Off - A Hardware Implementation of the Falcon Signature Scheme

    Falcon is one out of three post-quantum signature schemes which have been selected for standardization by NIST in July 2022. To the best of our knowledge, Falcon is the only selected algorithm that does not yet have a publicly reported hardware description that performs signing or key generation. The reason might be that the Falcon signature and key generation algorithms do not fit well in hardware due to the use of floating-point numbers and recursive functions. This publication describes the first hardware implementation for Falcon signing and key generation. To overcome the complexity of the Falcon algorithms, High-Level Synthesis (HLS) was preferred over a hardware description language like Verilog or VHDL. Our HLS code is based on the C reference implementation available at NIST. We describe the required modifications in order to be compliant with HLS, such as rewriting recursive functions into iterative versions. The hardware core at security level 5 requires 45,223 LUTs, 41,370 FFs, 182 DSPs, and 37 BRAMs to calculate one signature in 8.7 ms on a Zynq UltraScale+ FPGA. Security level 5 key generation takes 320.3 ms and requires 100,649 LUTs, 91,029 FFs, 1,215 DSPs, and 69 BRAMs

    Automated liver segmental volume ratio quantification on non-contrast T1-Vibe Dixon liver MRI using deep learning.

    PURPOSE To evaluate the effectiveness of automated liver segmental volume quantification and calculation of the liver segmental volume ratio (LSVR) on a non-contrast T1-vibe Dixon liver MRI sequence using a deep learning segmentation pipeline. METHOD A dataset of 200 liver MRI with a non-contrast 3 mm T1-vibe Dixon sequence was manually labeledslice-by-sliceby an expert for Couinaud liver segments, while portal and hepatic veins were labeled separately. A convolutional neural networkwas trainedusing 170 liver MRI for training and 30 for evaluation. Liver segmental volumes without liver vessels were retrieved and LSVR was calculated as the liver segmental volumes I-III divided by the liver segmental volumes IV-VIII. LSVR was compared with the expert manual LSVR calculation and the LSVR calculated on CT scans in 30 patients with CT and MRI within 6 months. RESULTS Theconvolutional neural networkclassified the Couinaud segments I-VIII with an average Dice score of 0.770 ± 0.03, ranging between 0.726 ± 0.13 (segment IVb) and 0.810 ± 0.09 (segment V). The calculated mean LSVR with liver MRI unseen by the model was 0.32 ± 0.14, as compared with manually quantified LSVR of 0.33 ± 0.15, resulting in a mean absolute error (MAE) of 0.02. A comparable LSVR of 0.35 ± 0.14 with a MAE of 0.04 resulted with the LSRV retrieved from the CT scans. The automated LSVR showed significant correlation with the manual MRI LSVR (Spearman r = 0.97, p < 0.001) and CT LSVR (Spearman r = 0.95, p < 0.001). CONCLUSIONS A convolutional neural network allowed for accurate automated liver segmental volume quantification and calculation of LSVR based on a non-contrast T1-vibe Dixon sequence

    Patches of Bare Ground as a Staple Commodity for Declining Ground-Foraging Insectivorous Farmland Birds

    Conceived to combat widescale biodiversity erosion in farmland, agri-environment schemes have largely failed to deliver their promises despite massive financial support. While several common species have shown to react positively to existing measures, rare species have continued to decline in most European countries. Of particular concern is the status of insectivorous farmland birds that forage on the ground. We modelled the foraging habitat preferences of four declining insectivorous bird species (hoopoe, wryneck, woodlark, common redstart) inhabiting fruit tree plantations, orchards and vineyards. All species preferred foraging in habitat mosaics consisting of patches of grass and bare ground, with an optimal, species-specific bare ground coverage of 30–70% at the foraging patch scale. In the study areas, birds thrived in intensively cultivated farmland where such ground vegetation mosaics existed. Not promoted by conventional agri-environment schemes until now, patches of bare ground should be implemented throughout grassland in order to prevent further decline of insectivorous farmland birds

    Geo-referenced archive databases on mountain organisms are very promising tools for achieving a better understanding of mountain biodiversity and predicting its changes. The Global Mountain Biodiversity Assessment (GMBA) of DIVERSITAS, in cooperation with the Global Biodiversity Information Facility, encourages a global effort to mine biodiversity databases on mountain organisms. The wide range of climatic conditions and topographies across the world's mountains offers an unparalleled opportunity for developing and testing biodiversity theory. The power of openly accessible, interconnected electronic databases for scientific biodiversity research, which by far exceeds the original intent of archiving for mainly taxonomic purposes, has been illustrated. There is an urgent need to increase the amount and quality of geo-referenced data on mountain biodiversity provided online, in order to meet the challenges of global change in mountains
