    Findings from Year Two of the External Evaluation of the Healthy & Active Communities Initiative

    The Missouri Foundation for Health has funded 33 projects under its Healthy & Active Communities (H&AC) Initiative in two-year funding cycles. A set of 15 projects was funded beginning in 2005 while a set of 18 was funded starting in 2006. This report is the second of three annual reports that assess the extent to which the H&AC Initiative is achieving its objectives. The report builds upon the findings described in “Findings from Year One of the External Evaluation of the Healthy & Active Communities Initiative.” The Missouri Foundation for Health contracted with the Institute of Public Policy, Truman School of Public Affairs at the University of Missouri to provide an evaluation of the success of the Initiative as a whole. This focus differs from the typical evaluation where evaluators are assessing and reporting on the success of individual funded projects. Instead, the evaluation looked across the funded projects to identify common factors of success. Continuing from the framework established in 2006, the evaluation team worked from a socio-ecological model. This model assumes that complex prevention programs such as the H&AC projects must use a multi-faceted approach in order to change behavior on individual, organizational and community levels simultaneously. To evaluate programs with multiple approaches such as these, the evaluators determined that cluster evaluation, a strategy developed by the Kellogg Foundation, could be used to identify successful features of the Initiative as a whole. This method enables the evaluators to identify successful program and community conditions that transcend the individual projects

    Teaching advocacy communication to pediatric residents: the efficacy of applied improvisational theater (AIT) as an instructional tool

    In today’s communication landscape, the public often turn to the Internet and social media instead of their physician for health information. To remain relevant and respected amidst the wealth of health information available online, physicians need to offer something the Internet cannot fully emulate: empathetic imagination and an ability to instantaneously tailor messages to reach and teach worried and often confused audiences effectively. We developed an instructional communication module for pediatric residents that used applied improvisational theater to help residents develop complex and dynamic communication skills. The module included opportunities to develop empathy, practice audience analysis, distill messages to key points, and apply these skills in media and community contexts. Attendees completed surveys regarding their perceptions of curricular structure, efficacy, and utility. Preliminary results indicate gains in communication confidence and skills. This type of instructional communication and training module encourages healthcare practitioners to position themselves as trusted experts and partners in helping clients make meaning of health information, thus empowering a new generation of pediatricians to bridge communication gaps created by new technologies and increased access to multiple information sources

    Those Who Teach Can Do: Skill Development Through Undergraduate Teaching

    Upon entering the workforce or progressing into a graduate program, undergraduate students are expected to possess a number of both technical and psychological skills, as well as transferable skills, often termed “soft skills” (Appleby, 2003). In addition to coursework, experience through institutional research or undergraduate teaching may provide the necessary opportunities for development of transferable skills. Research presently supports the idea that undergraduate teaching assistants report the acquisition of professional skills through interactions with students and professors (Fingerson & Culley, 2001). These interactions and responsibilities may create similar effects, in terms of professional skill development, to those expected from performing undergraduate research, such as confident communication and leadership (Shalk et al., 2009). Through work as an undergraduate teaching assistant, it is thought that students will develop key knowledge, skills, and characteristics (KSC’s) identified as essential by potential employers through the U.S. Department of Labor’s Occupational Information Network (Appleby, 2019). The aim of the present study is to measure gains in teaching assistants’ perceptions of their transferable skills and professional abilities. The researchers have constructed a survey intended to assess students’ perceived competencies and expectations of competencies following completion of the teaching assistant experience. Over the course of three semesters the researchers have surveyed students enrolled in an undergraduate teaching assistant practicum. Students receive course credit and, in addition to gaining professional experience, engage in reflection intended to encourage self-evaluation and integration of professional abilities and goals. The constructed survey is based on the work of Appleby (2007; 2019) and it measures perceived competency in the following domains: integrity, attention to detail, dependability, initiative, flexibility, persistence, communication, goal setting, analytical thinking, self-control, stress tolerance and cooperation. Individual difference measures were also captured, for example, Need for Cognition (Cacioppo et al., 1984) and Brief Resilience Scale (Smith et al., 2008). The researchers also created additional questions to assess the impact of the global pandemic. We hypothesize that this scale will effectively capture gains in perceptions of transferable skills and professional abilities following a TA practicum course. We also hypothesize that individual differences, such as resilience and need for cognition may be correlated. This survey has potential for use with diverse groups of teaching assistants in various departments and educational institutions

    Microarray-based DNA methylation profiling: technology and applications

    This work is dedicated to the development of a technology for unbiased, high-throughput DNA methylation profiling of large genomic regions. In this method, unmethylated and methylated DNA fractions are enriched using a series of treatments with methylation sensitive restriction enzymes, and interrogated on microarrays. We have investigated various aspects of the technology including its replicability, informativeness, sensitivity and optimal PCR conditions using microarrays containing oligonucleotides representing 100 kb of genomic DNA derived from the chromosome 22 COMT region in addition to 12 192 element CpG island microarrays. Several new aspects of methylation profiling are provided, including the parallel identification of confounding effects of DNA sequence variation, the description of the principles of microarray design for epigenomic studies and the optimal choice of methylation sensitive restriction enzymes. We also demonstrate the advantages of using the unmethylated DNA fraction versus the methylated one, which substantially improve the chances of detecting DNA methylation differences. We applied this methodology for fine-mapping of methylation patterns of chromosomes 21 and 22 in eight individuals using tiling microarrays consisting of over 340 000 oligonucleotide probe pairs. The principles developed in this work will help to make epigenetic profiling of the entire human genome a routine procedure

    No pain, no gain? The effects of pain-promoting neuropeptides and neurotrophins on fracture healing

    Neuropeptides and neurotrophins are key regulators of peripheral nociceptive nerves and contribute to the induction, sensitization, and maintenance of pain. It is now known that these peptides also regulate non-neuronal tissues, including bone. Here, we review the effects of numerous neuropeptides and neurotrophins on fracture healing. The neuropeptides calcitonin-gene related peptide (CGRP), substance P (SP), vasoactive intestinal peptide (VIP), and pituitary adenylate cyclase-activating peptide (PACAP) have varying effects on osteoclastic and osteoblastic activity. Ultimately, CGRP and SP both accelerate fracture healing, while VIP and PACAP seem to negatively impact healing. Unlike the aforementioned neuropeptides, the neurotrophins nerve growth factor (NGF) and brain-derived neurotrophic factor (BDNF) have more uniform effects. Both factors upregulate osteoblastic activity, osteoclastic activity, and, in vivo, stimulate osteogenesis to promote fracture healing. Future research will need to clarify the exact mechanism by which the neuropeptides and neurotrophins influence fracture healing. Specifically, understanding the optimal expression patterns for these proteins in the fracture healing process may lead to therapies that can maximize their bone-healing capabilities and minimize their pain-promoting effects. Finally, further examination of protein-sequestering antibodies and/or small molecule agonists and antagonists may lead to new therapies that can decrease the rate of delayed union/nonunion outcomes and fracture-associated pain

    RNA helicase signaling is critical for type I interferon production and protection against rift valley fever virus during mucosal challenge

    Rift Valley fever virus (RVFV) is an emerging RNA virus with devastating economic and social consequences. Clinically, RVFV induces a gamut of symptoms ranging from febrile illness to retinitis, hepatic necrosis, hemorrhagic fever, and death. It is known that type I interferon (IFN) responses can be protective against severe pathology; however, it is unknown which innate immune receptor pathways are crucial for mounting this response. Using both in vitro assays and in vivo mucosal mouse challenge, we demonstrate here that RNA helicases are critical for IFN production by immune cells and that signaling through the helicase adaptor molecule MAVS (mitochondrial antiviral signaling) is protective against mortality and more subtle pathology during RVFV infection. In addition, we demonstrate that Toll-like-receptor-mediated signaling is not involved in IFN production, further emphasizing the importance of the RNA cellular helicases in type I IFN responses to RVFV

    A Distinct DNA Methylation Shift in a Subset of Glioma CpG Island Methylator Phenotypes during Tumor Recurrence

    Glioma diagnosis is based on histomorphology and grading; however, such classification does not have predictive clinical outcome after glioblastomas have developed. To date, no bona fide biomarkers that significantly translate into a survival benefit to glioblastoma patients have been identified. We previously reported that the IDH mutant G-CIMP-high subtype would be a predecessor to the G-CIMP-low subtype. Here, we performed a comprehensive DNA methylation longitudinal analysis of diffuse gliomas from 77 patients (200 tumors) to enlighten the epigenome-based malignant transformation of initially lower-grade gliomas. Intra-subtype heterogeneity among G-CIMP-high primary tumors allowed us to identify predictive biomarkers for assessing the risk of malignant recurrence at early stages of disease. G-CIMP-low recurrence appeared in 9.5% of all gliomas, and these resembled IDH-wild-type primary glioblastoma. G-CIMP-low recurrence can be characterized by distinct epigenetic changes at candidate functional tissue enhancers with AP-1/SOX binding elements, mesenchymal stem cell-like epigenomic phenotype, and genomic instability. Molecular abnormalities of longitudinal G-CIMP offer possibilities to defy glioblastoma progression
