Сон тесно связан с особой работой мозга, активно задействована память, особенно зрительные воспоминания, поэтому сны могу быть осознаны и истолкованы человеком. Ученый отмечает, что сновидения, бессознательны и всегда связаны прошлым и со способностями ассоциации и воображения, которые естественны для любого человека. В теории Фрейда сны имеют кроме естественных, физиологических причин еще и психические. Заслуги Рамона и Кахаля состоит также в изучении проблем внушения и гипноза с точки зрения практической медицины

The Elimination of Low-Molecular-Weight Proteins in Patients with Isolated Acute Renal Failure

Objective: to study the effectiveness of some types of semipermeable dialysis membranes and replacement therapy techniques in patients with isolated acute renal failure (iARF). Subjects and methods: Eighty-nine patients aged 24 to 67 years, who received intensive and replacement/maintenance therapy, were examined. The patients were divided into 3 groups in accordance with their condition rated by the APACHE III scale, from the used dialysis membranes and renal replacement/maintenance therapy options. Results. By varying the permeability of a membrane, its area and the volume of convection, we can control the rate of substance elimination, which is similar to that of test markers having a molecular weight of 100 to 15000 Da. Conclusion. Adequate replacement therapy for iARF is possible only when high-flux, high-permeability dia-lyzers are applied. The indices of hemodialysis/hemodiafiltration adequacy in terms of urea cannot be determinants in patients with iARF. The achievement of elimination of low-molecular-weight proteins — markers of uremic intoxication to 30—35% and/or an increase in effective albumin concentrations as a summary marker of toxicity by 16—20% is of much more importance. Discussion of works of art in the lessons for the discipline "Cultural studies" in medical University

The article analyzes the texts of fiction from the point of view of the personal characteristics of the doctor and his social role in society. It is proposed to discuss these works in the classroom in the course of "Cultural Studies" in medical school, which is one of the ways to implement interdisciplinary connections of the discipline with "Psychology", "Sociology", "Philosophy" and "Bioethics". Being also one of the teaching methods, it helps to attract students' attention to the studied subject, expand the horizons of future doctors and the formation of their moral qualities, General cultural and professional competencies.В статье проводится анализ текстов художественной литературы с точки зрения личностных характеристик врача и его социальной роли в обществе. Galactic Rotation Parameters from Data on Open Star Clusters

Currently available data on the field of velocities Vr, Vl, Vb for open star clusters are used to perform a kinematic analysis of various samples that differ by heliocentric distance, age, and membership in individual structures (the Orion, Carina--Sagittarius, and Perseus arms). Based on 375 clusters located within 5 kpc of the Sun with ages up to 1 Gyr, we have determined the Galactic rotation parameters Wo =-26.0+-0.3 km/s/kpc, W'o = 4.18+-0.17 km/s/kpc^2, W''o=-0.45+-0.06 km/s/kpc^3, the system contraction parameter K = -2.4+-0.1 km/s/kpc, and the parameters of the kinematic center Ro =7.4+-0.3 kpc and lo = 0+-1 degrees. The Galactocentric distance Ro in the model used has been found to depend significantly on the sample age. Thus, for example, it is 9.5+-0.7 kpc and 5.6+-0.3 kpc for the samples of young (50 Myr) clusters, respectively. Our study of the kinematics of young open star clusters in various spiral arms has shown that the kinematic parameters are similar to the parameters obtained from the entire sample for the Carina-Sagittarius and Perseus arms and differ significantly from them for the Orion arm. The contraction effect is shown to be typical of star clusters with various ages. It is most pronounced for clusters with a mean age of 100 Myr, with the contraction velocity being Kr = -4.3+-1.0 km/s.Comment: 14 pages, 4 figures, 2 table

МАНЕВР РЕКРУТМЕНТА В УСЛОВИЯХ ОБЩЕЙ АНЕСТЕЗИИ ПРИ АБДОМИНАЛЬНЫХ ОПЕРАЦИЯХ

The article reviews the using recruitment maneuver (RM) under general anesthesia in abdominal surgery in the patients with initially intact lungs. Goal: to detect the most optimal ways of RM, required temporary characteristics of RM and groups of patients in whom RM will be especially useful. Conclusion. Using RM together with other elements of protective artificial pulmonary ventilation during general anesthesia with high risk of respiratory complications is an effective and safe tool allowing prevention of respiratory complications. Stepwise RM allows preventing unfavorable hemodynamic incidents while performing RM in the patients with high anesthetic risk. В обзоре рассматривается применение рекрутмент-маневра (РМ) в условиях общей анестезии при абдоминальных операциях у пациентов с исходно не поврежденными легкими. Цель: выявление оптимальных способов выполнения РМ, определение необходимых временных характеристик РМ, а также групп пациентов, применение РМ у которых будет особенно полезно. Заключение. Применение РМ в совокупности с другими элементами протективной искусственной вентиляции легких во время общей анестезии у пациентов с высоким риском респираторных осложнений является эффективным и безопасным механизмом, позволяющим предупредить развитие респираторных осложнений. У пациентов с высоким анестезиологическим риском пошаговый способ выполнения РМ позволяет предотвратить появление неблагоприятных гемодинамических инцидентов во время выполнения РМ.

Элиминация низкомолекулярных белков у больных с изолированной острой почечной недостаточностью

Материал и методы. Обследовано 89 больных в возрасте от 24 до 67 лет, получавших интенсивную и заместительную/поддерживающую терапию. Пациенты разделены на три группы, исходя из тяжести состояния, оцененной по шкале APACHE III, по использованным диализным мембранам и методам заместительной/поддерживающей почечной терапии. Результаты. Варьируя проницаемость мембраны, ее площадь и объем конвекции, мы можем управлять скоростью элиминации веществ со сходной, с исследованными маркерами, молекулярной массой от 100 до 15000 Да. Заключение. Полноценное обеспечение заместительной терапии иОПН возможно только с использованием высокопоточных, высокопроницаемых диализаторов. Индексы адекватности ГД/ГДФ по мочевине не могут являться определяющими у больных с иОПН. Значительно более важным является достижение элиминации низкомолекулярных белков-маркеров уремической интоксикации на уровне до 30—35% и/или увеличение ЭКА, как суммарного маркера токсичности на 16—20%.
