    Detection of hepatitis B virus covalently closed circular DNA in the plasma of Iranian HBeAg-negative patients with chronic hepatitis B

    Background: Covalently closed circular DNA (cccDNA) of hepatitis B virus (HBV) is a marker of HBV replication in the liver of patients infected with HBV. Objectives: This study aimed to investigate the association between the presence of cccDNA in the plasma samples of Iranian treatment-naive patients with chronic hepatitis B infection and HBV viral load and HBsAg levels. Patients and Methods: From April 2012 to May 2015, 106 treatment-naive patients with chronic hepatitis B infection were enrolled in this cross-sectional study. The HBsAg titer was measured by the Roche HBsAg II assay on the Cobas e411 system, and HBV DNA quantitation was performed using the COBAS TaqMan 48 kit. Real-time polymerase chain reaction was performed for the detection of HBV cccDNA. Results: The mean (SD) age of the patients was 41.1 ± 12.4 years (range, 20 - 62 years). From a total of 106 study participants, 67 (63.2) were males. The HBV cccDNA was detected in plasma specimens in 19 (17.9) out of the total 106 patients, and a significant relationship was found between the presence of cccDNA in plasma sample of males (23.9) and females (7.7) (P = 0.039). Also, a significant correlation was found between the presence of cccDNA in plasma sample of the patients and HBV viral load level (P < 0.0001) and HBsAg titer (P = 0.0043). Conclusions: This study showed that cccDNA can be detected in the plasma specimen of 17.9 of Iranian treatment-naive patients with chronic hepatitis B infection. Therefore, designing prospective studies focusing on the detection of cccDNA in these patients would provide more information. © 2015, Kowsar Corp

    The effects of maternal and paternal body mass index on live birth rate after intracytoplasmic sperm injection cycles

    Background: We designed the present study to evaluate the simultaneous effect of obesity in couples on in vitro fertilization/intracytoplasmic sperm injection (IVF/ICSI) outcomes. Materials and Methods: In this cross-sectional study, performed at Royan Institute between January 2013 and January 2014, we evaluated the recorded data of all patients during this time period. The study population was limited to couples who underwent ICSI or IVF/ICSI cycles with autologous oocytes and fresh embryo transfers. We recorded the heights and weights of both genders and divided them into groups according to body mass index (BMI). Multilevel logistic regression analysis was used to determine the odds ratio for live births following ICSI or IVF/ICSI. Results: In total, 990 couples underwent IVF/ICSI cycles during the study period. Among the ovulatory women, a significant difference existed between the BMI groups. There was a 60 decrease 95% confidence interval (CI): 0.11-0.83 in the odds of a live birth among overweight subjects and 84% (95% CI: 0.02-0.99) decrease among obese subjects. Among the anovulatory women, the association between the BMI and live births presented no clear tendencies. We did not observe any significant relationship between male BMI and live birth rate. The results demonstrated no significant association between the couples� BMI and live birth rate. Conclusion: Based on the present findings, increased female BMI independently and negatively influenced birth rates after ICSI. However, increased male BMI had no impact on live births after ICSI, either alone or combined with increased female BMI. © 2019, Royan Institute (ACECR). All rights reserved

    Effect of illness perception improvement on risk factors of coronary artery disease

    Background: Illness perception can affect health-related behaviors and disease outcomes. Objectives: To determine the effect of an educational intervention of improving illness perception on some modifiable risk factors of coronary artery disease. Methods: In this clinical trial, 100 patients undergoing coronary angiography that met the inclusion criteria were divided randomly into two intervention and control groups. In the intervention group, three educational sessions were conducted individually, while usual care was conducted for the control group. Measurements on fasting blood glucose, cholesterol, triglyceride, blood pressure, body mass index, and smoking status were gathered at baseline, immediately and six months after the intervention in both groups. Data were analyzed using the SPSS. Results: The mean of systolic blood pressure (p<0.005), fasting triglycerides (p<0.005), and fasting blood glucose (p<0.005) were significantly different before and after the intervention between the two groups. Conclusion: Improvement illness perception through educational intervention can affect risk factors of coronary artery disease

    Diagnostic accuracy of body mass index and fasting glucose for the prediction of gestational diabetes mellitus after assisted reproductive technology

    Background: The aim of the present study was to determine the maternal pre-pregnancy body mass index (BMI), first-trimester fasting blood sugar (FBS), and the combination of (BMI+FBS) cut-points for at-risk pregnant women conceived by assisted reproductive technology (ART) to better predict the risk of developing gestational diabetes mellitus (GDM) in infertile women. Materials and Methods: In this nested case-control study, 270 singleton pregnant women consisted of 135 (GDM) and 135 (non-GDM) who conceived using ART were assessed. The diagnosis of GDM was confirmed by a one-step glucose tolerance test (O-GTT) using 75 g oral glucose. BMI was classified base on World Health Organization (WHO) criteria. The relationship between BMI, FBS, and BMI+FBS with the risk of GDM development was determined by logistic regression and adjusted for confounding factors. Receiver operating characteristic (ROC) curve analysis was performed to assess the value of BMI, FBS, and BMI+FBS for the prediction of GDM. Results: The GDM group had significantly higher age, BMI, family history of diabetes, and history of polycystic ovary syndrome in comparison with the non-GDM group (P&lt;0.05). Overweight and obese women had 3.27, and 5.14 folds increase in the odds of developing GDM, respectively. There was a 17 increase in the risk of developing GDM with each 1 mg/dl increase in fasting glucose level. The cut points for FBS 84.5 mg/dl (72.9 sensitivity, 74.4 specificity), BMI 25.4 kg/m 2 (68.9 sensitivity, 62.8 specificity), and BMI+FBS 111.2 (70.7 sensitivity, 80.6 specificity) was determined. Conclusion: The early screening and high-quality prenatal care should be recommended upon the co-occurrence of high FBS (�84.5 mg/dl) in the first-trimester of the pregnancy and the BMI (�25.4 kg/m2) in pre-pregnancy period in women undergone ART.     A comparative study of long interspersed element-1 protein immunoreactivity in cutaneous malignancies

    Background: Skin cancer is the most common cancer worldwide and commonly classified into malignant melanoma (MM) and Nonmelanoma skin cancers (NMSCs), which mainly include basal cell carcinoma (BCC) and squamous cell carcinoma (SCC). The extent to which Long Interspersed Element-1 (LINE-1, L1) ORF1p is expressed in cutaneous malignancies remains to be evaluated. This study aimed to assess LINE-1 ORF1p immunoreactivity in various skin cancer subtypes. Method: The expression level of LINE-1 ORF1p was evaluated in 95 skin cancer specimens comprising 36 (37.9) BCC, 28 (29.5) SCC, and 31 (32.6) melanoma using the tissue microarray (TMA) technique. Then the association between expression of LINE-1 encoded protein and clinicopathological parameters was analyzed. Results: We showed that LINE-1 ORF1p expression level was substantially higher in BCC and SCC patients compared with melanoma samples (p 0.05). Conclusions: According to our observation, LINE-1 ORF1p immunoreactivity in various skin tumor subtypes extends previous studies of LINE-1 expression in different cancers. LINE-1ORF1p overexpression in NMSCs compared with MM can be considered with caution as a tumor-specific antigen for NMSCs. © 2020 The Author(s)

    Type-2 fuzzy linear systems

    Fuzzy Linear Systems (FLSs) are used in practical situations where some of the systems parameters or variables are uncertain. To date, investigations conducted on FLSs are restricted to those in which the uncertainty is assumed to be modeled by Type-1 Fuzzy Sets (T1FSs). However, there are many situations where considering the uncertainty as T1FSs may not be possible due to different interpretations of experts about the uncertainty. Moreover, solutions of FLSs are T1FSs which do not provide any information about a measure of the dispersion of uncertainty around the T1FSs. Therefore, in this research a model of uncertain linear equations system called a type-2 fuzzy linear system is presented to overcome the shortcomings. The uncertainty is represented by a special class of type-2 fuzzy sets – triangular perfect quasi type-2 fuzzy numbers. Additionally, conditions for the existence of a unique type–2 fuzzy solution to the linear system are derived. A definition of a type-2 fuzzy solution is also given. The applicability of the proposed model is illustrated using examples in the pulp and paper industry, and electrical engineering

    Outcomes of chronic total occlusion percutaneous coronary intervention from the RAIAN (RAjaie - Iran) registry

    Objective: While most of the evidence in CTO interventions emerge from Western and Japanese studies, few data have been published up today from the Middle East. Objective of this study was to evaluate technical success rates and clinical outcomes of an Iranian population undergoing CTO PCI in a tertiary referral hospital. Moreover, we sought to evaluate the efficacy of our CTO teaching program. Methods: This is a retrospective single-center cohort study including 790 patients who underwent CTO PCI performed by operators with different volumes of CTOs PCI performed per year. According to PCI result, all patients have been divided into successful (n&nbsp;=&nbsp;555, 70.3&nbsp;%) and unsuccessful (n&nbsp;=&nbsp;235, 29.7&nbsp;%) groups. Study endpoints were Major Adverse Cardiovascular Events and Health Status Improvement evaluated using the Seattle Angina Questionnaire at one year. Results: A global success rate of 70&nbsp;% for antegrade and 80&nbsp;% for retrograde approach was shown despite the lack of some CTO-dedicated devices. During the enrollment period, the success rate increased significantly among operators with a lower number of CTO procedures per year. One-year MACE rate was similar in both successful and unsuccessful groups (13.5&nbsp;% in successful and 10.6&nbsp;% in unsuccessful group, p&nbsp;=&nbsp;0.173). One year patients' health status improved significantly only in successful group. Conclusions: No significant differences of in-hospital and one-year MACE were found between the successful and unsuccessful groups. Angina symptoms and quality of life significantly improved after successful CTO PCI. The RAIAN registry confirmed the importance of operator expertise for CTO PCI success

    A simple genetic algorithm for calibration of stochastic rock discontinuity networks

    Este artículo propone un método para llevar a cabo la calibración de las familias de discontinuidades en macizos rocosos. We present a novel approach for calibration of stochastic discontinuity network parameters based on genetic algorithms (GAs). To validate the approach, examples of application of the method to cases with known parameters of the original Poisson discontinuity network are presented. Parameters of the model are encoded as chromosomes using a binary representation, and such chromosomes evolve as successive generations of a randomly generated initial population, subjected to GA operations of selection, crossover and mutation. Such back-calculated parameters are employed to make assessments about the inference capabilities of the model using different objective functions with different probabilities of crossover and mutation. Results show that the predictive capabilities of GAs significantly depend on the type of objective function considered; and they also show that the calibration capabilities of the genetic algorithm can be acceptable for practical engineering applications, since in most cases they can be expected to provide parameter estimates with relatively small errors for those parameters of the network (such as intensity and mean size of discontinuities) that have the strongest influence on many engineering applications
