Septic arthritis of the hip in adults with sickle cell anemia

Although the presence of osteonecrotic bone is known to make joints more prone to infection, acute septic joint in hip osteonecrosis has not frequently been reported in adults with sickle cell disease. The clinical features at the time of admission, imaging findings suggesting the diagnosis, modes of treatment and sequelae of septic arthritis of twenty-four hip joints with osteonecrosis in patients with sickle cell disease were studied retrospectively over a 25-years period. This study evaluated also the complications, the efficiency and the risk of total hip arthroplasty in these patients. Most patients were in the third decade of life. Staphylococcus and Gram negative infection predominated. Treatment was first conservative but most of the patients needed surgery to treat infection and sequelae related to infection. A total hip arthroplasty was performed later in twenty joints. No deaths were observed, but complications occurred. Twenty of the patients in our study underwent delayed total hip arthroplasties following repeated aspirations of the joint and intravenous antibiotics. With an experienced surgical and medical team and multidisciplinary management of these patients undergoing total hip arthroplasty after hip infection, our rate of complications was acceptable

Rice (Oryza sativa L.) is one of the most important cereals, which provides 20% of the world’s food energy. However, its productivity is poorly assessed especially in the global South. Here, we provide a first study to perform a deep learning-based approach for instantaneously estimating rice yield using RGB images. During ripening stage and at harvest, over 22,000 digital images were captured vertically downwards over the rice canopy from a distance of 0.8 to 0.9m at 4,820 harvesting plots having the yield of 0.1 to 16.1 t ha-1 across six countries in Africa and Japan. A convolutional neural network (CNN) applied to these data at harvest predicted 68% variation in yield with a relative root mean square error (rRMSE) of 0.22. Even when the resolution of images was reduced (from 0.2 to 3.2cm pixel-1 of ground sampling distance), the model could predict 57% variation in yield, implying that this approach can be scaled by use of unmanned aerial vehicles. Our work offers low-cost, hands-on, and rapid approach for high throughput phenotyping, and can lead to impact assessment of productivity-enhancing interventions, detection of fields where these are needed to sustainably increase crop production

"AIの目"によるイネ収穫量の簡単・迅速推定. 京都大学プレスリリース. 2023-07-21.Rice (Oryza sativa L.) is one of the most important cereals, which provides 20% of the world’s food energy. However, its productivity is poorly assessed especially in the global South. Here, we provide a first study to perform a deep-learning-based approach for instantaneously estimating rice yield using red-green-blue images. During ripening stage and at harvest, over 22, 000 digital images were captured vertically downward over the rice canopy from a distance of 0.8 to 0.9 m at 4, 820 harvesting plots having the yield of 0.1 to 16.1 t·ha⁻¹ across 6 countries in Africa and Japan. A convolutional neural network applied to these data at harvest predicted 68% variation in yield with a relative root mean square error of 0.22. The developed model successfully detected genotypic difference and impact of agronomic interventions on yield in the independent dataset. The model also demonstrated robustness against the images acquired at different shooting angles up to 30° from right angle, diverse light environments, and shooting date during late ripening stage. Even when the resolution of images was reduced (from 0.2 to 3.2 cm·pixel−1 of ground sampling distance), the model could predict 57% variation in yield, implying that this approach can be scaled by the use of unmanned aerial vehicles. Our work offers low-cost, hands-on, and rapid approach for high-throughput phenotyping and can lead to impact assessment of productivity-enhancing interventions, detection of fields where these are needed to sustainably increase crop production, and yield forecast at several weeks before harvesting

Nationwide retrospective observational study of idiopathic dendriform pulmonary ossification : clinical features with a progressive phenotype

Background: Diffuse pulmonary ossification is a specific lung condition that is accompanied by underlying diseases. However, idiopathic dendriform pulmonary ossification (IDPO) is extremely rare, and the clinical features remain unclear. In this study, we aimed to report the clinical characteristics of IDPO. Methods: We conducted a nationwide survey of patients with IDPO from 2017 to 2019 in Japan and evaluated the clinical, radiological, and histopathological findings of patients diagnosed with IDPO. Results: Twenty-two cases of IDPO were identified. Most subjects (82%) were male, aged 22-56 years (mean (SD), 37.9 (9.1)) at diagnosis. Nearly 80% of the subjects were asymptomatic, and the condition was discovered during a medical check-up. However, 36% of the subjects showed a decline in forced vital capacity (%FVC) predicted <80% at diagnosis. The typical radiological features of high-resolution CT (HRCT) are calcified branching structures that are predominantly distributed in the lower lung fields without any other conspicuous finding. Histopathological analysis also showed dendriform ossified lesions from the intraluminal areas to interstitial areas. Notably, during the follow-up period of 20 years, disease progression was found in 88% on HRCT and more than 50% on pulmonary function tests (FVC and/or forced expiratory volume in 1s). Two cases with rapid decline of 10% /year in %FVC predicted were observed.)) at diagnosis. Nearly 80% of the subjects were asymptomatic, and the condition was discovered during a medical check-up. Further research and long-term (>20 years) follow-up are required to clarify the pathogenesis and clinical findings in IDPO

    Tissue bioengineering in orthopedics

The use of cells for the purpose of orthopedic tissue engineering started more than 300 years ago. The first attempt of bone grafting was reported in 1668 by the Dutch surgeon Job-Van Meek'ren. In 1867, Ollier performed a series of experiments using transplanted periosteum and concluded that transplanted periosteum and bone remained alive and formed new bone. The osteogenic potential of transplanted bone marrow was later documented by Goujon in 1869, then by Macewen in 1881. Efforts of Albee and Phemister highlighted further the utility of bone transplantation for the healing of fractures and bone defects. The techniques for autografting pioneered by these individuals remained largely unchanged until today. Advances in understanding of the biology of osteogenic cells, the availability of many highly purified peptide growth factors, and the capacity to create highly specialized implantable materials have launched an explosion of new advances in bone grafting and bone regeneration, all under the banner of tissue engineering. This new field is rapidly expanding the armamentarium of orthopedic surgeons in every setting in which bone healing is required. Composites of cells and matrices are at the core of this revolutio

Numerical Analysis of Kidney Stone Fragmentation by Short Pulse Impingement

    Get PDF
In this work, numerical analyses are performed to study the behavior of stresses generated inside a kidney stone by direct pulse impingement during extracorporeal shock wave lithotripsy (ESWL), which leads to fragmentation of the stone. LS-DYNA, an explicit Finite Element code for non-linear dynamic analysis is employed to model the problems. Effects of pulse duration and acoustic property of the stone on stress field evolution inside the stone are studied for the pulse duration of 0.5 to 5.0μs and two acoustic impedances of actual kidney stones. The use of double shock wave sources to fragment the kidney stone is also considered in this paper. The effectiveness of this method for the kidney stone fragmentation is confirmed. Finally, the numerical analysis for the stone fragmentation is well compared with experimental results to confirm that the numerical analyses in this work provide reasonable results.・rights:日本機械学会・rights:本文データは学協会の許諾に基づきCiNiiから複製したものである・relation:isVersionOf:http://ci.nii.ac.jp/naid/110004820527
