    Managing the Energy Trilemma in the Philippines

    Background The transition to an energy mix with lower carbon emissions is hampered by the existence of the so-called Energy Trilemma. The primary consequence is a trade-off between various objectives of energy policy, e.g., equity and sustainability. This conflict can lead to policy gridlock if policymakers are unable to prioritize the goals. This paper proposes a framework and methodology to manage the trilemma by applying methods related to multi-criteria decision-making in order to assign weights to the various components of the trilemma. Results Following the International Energy Agency (IEA), an expanded concept of energy security is adopted and translates to a version of the trilemma different from that of the World Energy Council. This study takes into account autarky, price, supply, and carbon emissions. The values of these variables are generated by a software called PLEXOS and are incorporated in a welfare function. Trade-offs and complementarities among the four variables are taken into account by the equations in the PLEXOS model. Meanwhile, weights for each of the components of the trilemma are obtained using the Analytical Hierarchy Process. The experts interviewed for this exercise are considered hypothetical heads of the Philippine Department of Energy (DOE). Conclusion Two scenarios were compared: a market-based simulation and one where a carbon-tax was imposed. As expected, the carbon-tax leads to a fall in the level of carbon emissions but a rise in the cost of electricity. Because the demand for electricity has a higher price elasticity among lower income classes, the carbon-tax will worsen equity. Attempting to resolve the conflict among the goals of energy policy is difficult leading to a possible gridlock. Policy options can, however, be ranked using the values generated by the welfare function. The ranking clearly depends on the preference or priorities of the hypothetical head of the DOE but at least a decision could be reached. In this manner, trade-offs are measured and the trilemma can be managed even if it is not resolved

    A review of implant provision for hypodontia patients within a Scottish referral centre

    Background: Implant treatment to replace congenitally missing teeth often involves multidisciplinary input in a secondary care environment. High quality patient care requires an in-depth knowledge of treatment requirements. Aim: This service review aimed to determine treatment needs, efficiency of service and outcomes achieved in hypodontia patients. It also aimed to determine any specific difficulties encountered in service provision, and suggest methods to overcome these. Methods: Hypodontia patients in the Unit of Periodontics of the Scottish referral centre under consideration, who had implant placement and fixed restoration, or review completed over a 31 month period, were included. A standardised data collection form was developed and completed with reference to the patient's clinical record. Information was collected with regard to: the indication for implant treatment and its extent; the need for, complexity and duration of orthodontic treatment; the need for bone grafting and the techniques employed and indicators of implant success. Conclusion: Implant survival and success rates were high for those patients reviewed. Incidence of biological complications compared very favourably with the literature

    Sanitation challenges in Dar es salaam: The potential of Simplified Sewerage Systems

    In the context of growing urbanization, sanitation in many cities is an acute crisis with severe social and environmental consequences. The Sustainable Development Goal (SDG) of sanitation for all by 2030 is increasingly elusive. Municipalities have been experimenting with a range of lower-cost sanitation solutions. Simplified Sewerage Systems (SSS) have emerged in different cities as one response, but with mixed results. This paper evaluates an SSS project in an informal settlement in Dar es Salaam, Tanzania. Drawing on a combination of a survey and focus group discussions, the paper examines the social and economic impacts of the SSS and identifies a set of key concerns for future urban sanitation interventions: affordability, maintenance and governance. We conclude by considering the implications for future research and practice on urban sanitation, including the limits of technology-based approaches and the need to focus on the diverse needs of residents in place

    A multi-method approach for the integrative assessment of soil functions: Application on a coastal mountainous site of the Philippines

    The projected increase of the world\u27s population and the sustainability challenges the agricultural sector is facing, call for the enhancement of multi-functionality in agriculture in order to simultaneously provide food while meeting environmental targets. Here, we use the Functional Land Management (FLM) framework to assess the supply of and the demand for soil functions to inform agri-environmental policy for Udalo, a mountainous site in the Philippines. As many emerging communities in developing nations, Udalo is on the cusp of rapid development due to the construction of a major road increasing its accessibility and attractiveness for land investment. We assessed the supply of four soil functions in relation to six land-use types and four slope categories. The function “productivity” was assessed by interviews with 128 farmers, “habitat for biodiversity” by a vegetation survey, and “soil conservation” and “water conservation” via a literature review. The demand for functions was first assessed from the “top-down” policy perspective via interviews and reviews of policy targets, then complemented by integrating the local “bottom-up” demands for functions. These were assessed by applying a Q methodology, providing insights in the prioritisation of functions from the perspective of 22 local actors. Maps of supply and demands were generated for each function: supply maps by overlaying land use and slope category, top-down demand maps from administrative zoning/land-use plans, and bottom-up demand maps from local actors designation of geomorphological areas. Our results revealed contrasting demands for functions, as well as a heterogeneous spatial distribution of supply and demands. Discrepancies emerged (i) between supply and demand, (ii) between bottom-up (local) demands and the top-down (policy driven) demand, and (iii) among local actors perspectives. Our study indicates that discrepancies are not necessarily conflicting, but can uncover pathways for defining compromises, representing attainable policy entry points. Not one single development model can meet the needs of every stakeholder; however, a combination of land uses and management strategies can meet divergent interests and allow for optimisation of functions. This integrative approach of FLM provides a socially embedded biophysical analysis and is a valuable tool for the design of customized land-use and agri-environmental policies

    Assisted evolution enables HIV-1 to overcome a high trim5α-imposed genetic barrier to rhesus macaque tropism

    Diversification of antiretroviral factors during host evolution has erected formidable barriers to cross-species retrovirus transmission. This phenomenon likely protects humans from infection by many modern retroviruses, but it has also impaired the development of primate models of HIV-1 infection. Indeed, rhesus macaques are resistant to HIV-1, in part due to restriction imposed by the TRIM5α protein (rhTRIM5α). Initially, we attempted to derive rhTRIM5α-resistant HIV-1 strains using two strategies. First, HIV-1 was passaged in engineered human cells expressing rhTRIM5α. Second, a library of randomly mutagenized capsid protein (CA) sequences was screened for mutations that reduced rhTRIM5α sensitivity. Both approaches identified several individual mutations in CA that reduced rhTRIM5α sensitivity. However, neither approach yielded mutants that were fully resistant, perhaps because the locations of the mutations suggested that TRIM5α recognizes multiple determinants on the capsid surface. Moreover, even though additive effects of various CA mutations on HIV-1 resistance to rhTRIM5α were observed, combinations that gave full resistance were highly detrimental to fitness. Therefore, we employed an 'assisted evolution' approach in which individual CA mutations that reduced rhTRIM5α sensitivity without fitness penalties were randomly assorted in a library of viral clones containing synthetic CA sequences. Subsequent passage of the viral library in rhTRIM5α-expressing cells resulted in the selection of individual viral species that were fully fit and resistant to rhTRIM5α. These viruses encoded combinations of five mutations in CA that conferred complete or near complete resistance to the disruptive effects of rhTRIM5α on incoming viral cores, by abolishing recognition of the viral capsid. Importantly, HIV-1 variants encoding these CA substitutions and SIVmac239 Vif replicated efficiently in primary rhesus macaque lymphocytes. These findings demonstrate that rhTRIM5α is difficult to but not impossible to evade, and doing so should facilitate the development of primate models of HIV-1 infection

    NUMERICAL SOLUTION OF TWO-FLUID ELECTROOSMOTIC FLOW

    ABSTRACT Two-fluid flows in microchannel are often found in biological analysis, such as during ion exchange or solvent extraction from one phase to another. In this article, a numerical scheme is presented to describe a two-fluid flow in microchannel with electroosmotic (EO) effects. In this twofluid system, the interfacial viscous force of a high EO mobility fluid drags a low EO mobility fluid; the high EO mobility fluid is driven by electroosmosis. We particularly analyze the electric double layer (EDL) regions close to the wall and the interface in the high EO mobility fluid. As the governing equation of the electrical potential is singularly perturbed, finer meshes are adopted to capture these EDL regions. In simulation, the interface between the two fluids evolves along the flow direction as the flow develops. Level set method is used to capture the interface implicitly. A localized mass preservation scheme is used to ensure mass conservation. A finite-volume method is used to solve the coupled electric potential equation, level set equations and Navier-Stokes equation. The validity of the numerical scheme is evaluated by comparing its predictions with the results of the analytical solutions in the fully developed regions.     Focused molecular analysis of small cell lung cancer: feasibility in routine clinical practice

    © 2015 Abdelraouf et al.Background: There is an urgent need to identify molecular signatures in small cell lung cancer (SCLC) that may select patients who are likely to respond to molecularly targeted therapies. In this study, we investigate the feasibility of undertaking focused molecular analyses on routine diagnostic biopsies in patients with SCLC. Methods: A series of histopathologically confirmed formalin-fixed, paraffin-embedded SCLC specimens were analysed for epidermal growth factor receptors (EGFR), KRAS, NRAS and BRAF mutations, ALK gene rearrangements and MET amplification. EGFR and KRAS mutation testing was evaluated using real time polymerase chain reaction (RT-PCR cobas®), BRAF and NRAS mutations using multiplex PCR and capillary electrophoresis-single strand conformation analysis, and ALK and MET aberrations with fluorescent in situ hybridization. All genetic aberrations detected were validated independently. Results: A total of 105 patients diagnosed with SCLC between July 1990 and September 2006 were included. 60 (57 %) patients had suitable tumour tissue for molecular testing. 25 patients were successfully evaluated for all six pre-defined molecular aberrations. Eleven patients failed all molecular analysis. No mutations in EGFR, KRAS and NRAS were detected, and no ALK gene rearrangements or MET gene amplifications were identified. A V600E substitution in BRAF was detected in a Caucasian male smoker diagnosed with SCLC with squamoid and glandular features. Conclusion: The paucity of patients with sufficient tumour tissue, quality of DNA extracted and low frequency of aberrations detected indicate that alternative molecular characterisation approaches are necessary, such as the use of circulating plasma DNA in patients with SCLC

    Novel Escape Mutants Suggest an Extensive TRIM5α Binding Site Spanning the Entire Outer Surface of the Murine Leukemia Virus Capsid Protein

    After entry into target cells, retroviruses encounter the host restriction factors such as Fv1 and TRIM5α. While it is clear that these factors target retrovirus capsid proteins (CA), recognition remains poorly defined in the absence of structural information. To better understand the binding interaction between TRIM5α and CA, we selected a panel of novel N-tropic murine leukaemia virus (N-MLV) escape mutants by a serial passage of replication competent N-MLV in rhesus macaque TRIM5α (rhTRIM5α)-positive cells using a small percentage of unrestricted cells to allow multiple rounds of virus replication. The newly identified mutations, many of which involve changes in charge, are distributed over the outer ‘top’ surface of N-MLV CA, including the N-terminal β-hairpin, and map up to 29 Ao apart. Biological characterisation with a number of restriction factors revealed that only one of the new mutations affects restriction by human TRIM5α, indicating significant differences in the binding interaction between N-MLV and the two TRIM5αs, whereas three of the mutations result in dual sensitivity to Fv1n and Fv1b. Structural studies of two mutants show that no major changes in the overall CA conformation are associated with escape from restriction. We conclude that interactions involving much, if not all, of the surface of CA are vital for TRIM5α binding

    Value of implantable loop recorders in patients with structural or electrical heart disease

    Purpose: In patients with structural heart disease (SHD) or inherited primary arrhythmia syndrome (IPAS), the occurrence of unexplained syncope or palpitations can be worrisome as they are at increased risk of sudden cardiac death. An implantable loop recorder (ILR) can be a useful diagnostic tool. Our purpose was to compare the diagnostic yield, arrhythmia mechanism, and management in patients with SHD, patients with IPAS, and those without heart disease. Methods: Retrospective single-center study in consecutive patients who underwent an ILR implantation. Results: Between March 2013 and December 2016, a total of 94 patients received an ILR (SHD, n = 20; IPAS, n = 14; no SHD/IPAS, n = 60). The type of symptoms at the time of implantation was similar between groups. During a median follow-up of 10 months, 45% had an ILR-guided diagnosis. Patients with IPAS had a lower diagnostic yield (14%) in comparison to the other groups (no SHD/IPAS 47%, P = 0.03; SHD 60%, P = 0.01, respectively). Furthermore, patients with SHD had a higher incidence of nonsustained VT in comparison to patients without SHD/IPAS (30 versus 3%, P < 0.01). ILR-guided therapy was comparable between groups. In the SHD group, a high proportion (10%) received an implantable cardioverter-defibrillator; however, this was not statistically significantly higher than the other groups (no SHD/IPAS 3%, IPAS 0%, P = 0.08). Conclusions: In comparison to patients without heart disease, the diagnostic yield of an ILR was lower in patients with IPAS and the prevalence of ILR-diagnosed nonsustained VT was higher in patients with SHD
