    A pyrene-armed hexahomotrioxacalix[3]arene as a multi-sensor via synergistic and demetallation effects

    A new pyrene-armed hexahomotrioxacalix[3]arene L has been synthesized, which exhibits a pronounced fluorescence enhancement response toward CuĀ²āŗ ions via a ZnĀ²āŗ or CdĀ²āŗ triggered synergistic effect. Additionally, the LĀ·CuĀ²āŗ+ complex can subsequently serve as a sensor for Fā» via anion-induced demetallation. The fluorescence responses by the input of CuĀ²āŗ, ZnĀ²āŗ/CdĀ²āŗ and Fā» can be constructed as a combinational logic gate which mimics a set of molecular traffic signals

    A novel Doppler echocardiographic index integrating left and right ventricular function is superior to conventional indices for predicting adverse outcome of acute myocardial infarction

    The ratio of peak diastolic early velocity (E) of left ventricular (LV) inflow to peak diastolic longitudinal velocity (eā€™) of the mitral annulus (E/eā€™) is thought to reflect LV filling pressure, and tricuspid annulus velocity at systole (sā€™-T) is thought to reflect right ventricular function. However, it has not been reported on significance of the combined use of E/eā€™ and (sā€™-T) to predict outcome of acute myocardial infarction (AMI). Over 21 months, beginning in January 2007, we enrolled 65 AMI patients who were measured hemodynamic and echocardiographic parameters by Swan-Ganz (SG) catheterization just after reperfusion therapy and echocardiography immediately after reperfusion therapy. Cardiac index (CI) and pulmonary capillary wedge pressure (PCWP) via SG catheter were measured, and routine echocardiographic indices, including E, eā€™, E/eā€™, and (sā€™-T) were determined. In addition, we defined the functional integrated bi-myocardial tissue Doppler (FIT) index as (sā€™-T)Ć·E/eā€™. The relationships between CI, PCWP, and echocardiographic indices were investigated, including FIT index. Moreover, we investigated whether FIT index could predict adverse cardiac events. FIT index was significantly associated with not only CI but also PCWP. In the Cox proportional hazards model, FIT index<1.0 was a significant predictor for adverse outcome of AMI after adjustment for age, Killip class, history of previous coronary revascularization, location of culprit lesion, and LV ejection fraction. The novel index defined as (sā€™-T)Ć·E/eā€™ could be quite useful predictor of prognosis in AMI

    Measurements of Primary and Atmospheric Cosmic-Ray Spectra with the BESS-TeV Spectrometer

    Primary and atmospheric cosmic-ray spectra were precisely measured with the BESS-TeV spectrometer. The spectrometer was upgraded from BESS-98 to achieve seven times higher resolution in momentum measurement. We report absolute fluxes of primary protons and helium nuclei in the energy ranges, 1-540 GeV and 1-250 GeV/n, respectively, and absolute flux of atmospheric muons in the momentum range 0.6-400 GeV/c.Comment: 26 pages, 9 figures, 3 tables, Submitted to Phys.     The cosmic-ray proton, helium, and muon spectra at small atmospheric depths of 4.5 -- 28 g/cm^2 were precisely measured during the slow descending period of the BESS-2001 balloon flight. The variation of atmospheric secondary particle fluxes as a function of atmospheric depth provides fundamental information to study hadronic interactions of the primary cosmic rays with the atmosphere.Comment: 21 pages, 11 figures, 4 table

    Precise Measurements of Atmospheric Muon Fluxes with the BESS Spectrometer

    The vertical absolute fluxes of atmospheric muons and muon charge ratio have been measured precisely at different geomagnetic locations by using the BESS spectrometer. The observations had been performed at sea level (30 m above sea level) in Tsukuba, Japan, and at 360 m above sea level in Lynn Lake, Canada. The vertical cutoff rigidities in Tsukuba (36.2 N, 140.1 E) and in Lynn Lake (56.5 N, 101.0 W) are 11.4 GV and 0.4 GV, respectively. We have obtained vertical fluxes of positive and negative muons in a momentum range from 0.6 to 20 GeV/c with systematic errors less than 3 % in both measurements. By comparing the data collected at two different geomagnetic latitudes, we have seen an effect of cutoff rigidity. The dependence on the atmospheric pressure and temperature, and the solar modulation effect have been also clearly observed. We also clearly observed the decrease of charge ratio of muons at low momentum side with at higher cutoff rigidity region.Comment: 35 pages, 9 figures. Submitted to Astroparticle Physic

    Big insulin-like growth factor 2-producing multiple solitary fibrous tumors treated with debulking surgery: A case report

    BackgroundNon-islet cell tumor hypoglycemia (NICTH) is a rare paraneoplastic syndrome caused by a tumor-producing high molecular weight form of insulin-like growth factor 2 (IGF2) known as big IGF2. The only curative treatment for this condition is surgical resection of the responsible tumors. However, this may not be feasible in cases with multiple metastases at diagnosis of NICTH, and no standard treatment strategy for multiple tumors has been established. The effects of pharmacological therapies including somatostatin analogs are often inefficient and remain difficult to predict.Case descriptionA 68-year-old man was admitted to our hospital due to impaired consciousness and severe hypoglycemia. His medical history included diagnosis of a left temporal solitary fibrous tumor (SFT) at the age of 48 years, after which local recurrent and metastatic tumors were repeatedly resected. Four years before admission, multiple intraabdominal and subcutaneous tumors were detected and, being asymptomatic, were managed conservatively. Laboratory exam on admission demonstrated hypoglycemia accompanied with low serum insulin and IGF1 levels. Computed tomography (CT) scan revealed multiple intraabdominal and subcutaneous tumors increasing in size. Serum big IGF2 was detected on immunoblot analysis, and he was diagnosed as NICTH. In addition, tumor uptake was observed on 68Ga-labelled 1,4,7,10-tetraazacyclododecane-N,Nā€™,Nā€™ā€™,Nā€™ā€™ā€™-tetraacetic acid-d-Phe1-Tyr3-octreotide positron emission tomography/CT (DOTATOC-PET/CT). Since larger tumor is more suspicious about responsible producibility of big IGF2, we planned to resect large ones preferentially and reduce the amounts of residual tumors. Debulking surgery was performed by removing eleven intraabdominal tumors; the hypoglycemia was then completely corrected. Histological analyses revealed the resected tumors to be metastases of SFT having somatostatin receptor 2 expression. In immunoblot analysis, the resected tumors were found to be positive for big IGF2; serum big IGF2 was undetectable after surgery.ConclusionWe present a case of NICTH with multiple metastatic SFTs. We strategically performed debulking surgery, which led to remission of hypoglycemia. This result demonstrates a pioneering practical solution for NICTH cases with multiple tumors. In addition, in cases of SFTs presenting with NICTH, positivity of DOTATOC-PET/CT as well as single-dose administration of octreotide may be predictive of the efficacy of somatostatin-based therapy

    Index markers of chronic fatigue syndrome with dysfunction of TCA and urea cycles

    Chronic fatigue syndrome (CFS) is a persistent and unexplained pathological state characterized by exertional and severely debilitating fatigue, with/without infectious or neuropsychiatric symptoms, lasting at least 6 consecutive months. Its pathogenesis remains incompletely understood. Here, we performed comprehensive metabolomic analyses of 133 plasma samples obtained from CFS patients and healthy controls to establish an objective diagnosis of CFS. CFS patients exhibited significant differences in intermediate metabolite concentrations in the tricarboxylic acid (TCA) and urea cycles. The combination of ornithine/citrulline and pyruvate/isocitrate ratios discriminated CFS patients from healthy controls, yielding area under the receiver operating characteristic curve values of 0.801 (95% confidential interval [CI]: 0.711ā€“0.890, Pā€‰<ā€‰0.0001) and 0.750 (95% CI: 0.584ā€“0.916, Pā€‰=ā€‰0.0069) for training (nā€‰=ā€‰93) and validation (nā€‰=ā€‰40) datasets, respectively. These findings provide compelling evidence that a clinical diagnostic tool could be developed for CFS based on the ratios of metabolites in plasma

    Efficacy of prosultiamine treatment in patients with human T lymphotropic virus type I-associated myelopathy/tropical spastic paraparesis: results from an open-label clinical trial

    Background: Human T lymphotropic virus type I (HTLV-I)-associated myelopathy/tropical spastic paraparesis (HAM/TSP) is a chronic myelopathy characterized by motor dysfunction of the lower extremities and urinary disturbance. Immunomodulatory treatments are the main strategy for HAM/TSP, but several issues are associated with long-term treatment. We conducted a clinical trial with prosultiamine (which has apoptotic activity against HTLV-I-infected cells) as a novel therapy in HAM/TSP patients. Methods: We enrolled 24 HAM/TSP patients in this open-label clinical trial. Prosultiamine (300 mg, orally) was administered once daily for 12 weeks. We monitored changes in the motor function of the lower extremities and urinary function as well as copy numbers of the HTLV-I provirus in peripheral blood mononuclear cells (PBMCs). Results: Improvement in the motor function of the lower extremities based on a reduction in spasticity (for example, decrease in time required for walking and descending a flight of stairs) was observed. In an urodynamic study (UDS), bladder capacity and detrusor pressure and then maximum flow rate increased significantly. Detrusor overactivity and detrusor-sphincter dyssynergia improved in 68.8% and 45.5% of patients observed at pretreatment, respectively. Improvement in UDS corresponded with improvements in the score of nocturia-quality of life questionnaire. HTLV-I proviral copy numbers in PBMCs decreased significantly (approximately 15.4%) compared with pretreatment levels.Conclusions: These data suggest that prosultiamine can safely improve motor dysfunction of the lower extremities and urinary disturbance as well as reduce HTLV-I provirus levels in peripheral blood.
