Echocardiographic evaluation from a different perspective in asthmatic patients Introduction: It is known that there is a complex interaction between asthma and cardiovascular physiology. Some investigations on echocardiography and electrocardiography (ECG) in asthmatic patients have revealed many findings such as pulmonary hypertension (PHT) and arrhythmia. In this study, we aimed to perform tissue Doppler imaging (TDIE) and conventional echocardiographic (CEI) assessment with many indexes of arrhythmia on electrocardiography (ECG) in asthmatic patients. Materials and Methods: A total of 89 patients, 63 females (70.8%) and 26 males (29.2%), were included in this study. Patients were divided into three groups, and then each group was separated in two groups as mild-moderate and severe asthma. Results: There was no difference among groups with respect to age, sex and anthropometric data. There was no difference between the groups with respect to indexes of arrhythmia on ECG (p> 0.05). Mitral annular plane systolic excursion (MAPSE), tricuspid annular alane systolic excursion (TAPSE) and both ventricular diastolic velocities on CEI were similar between the groups, except for left ventricular A wave velocity which was higher in severe asthmatic patients (p 0.05).Conclusion: Based on these results, it can be suggested that LA mechanical functions and intra-atrial LA electromechanical durations were impaired in severe asthmatic patients

Obstructive sleep apnea syndrome (OSAS) is characterized with interruption of sleep with apnea attacks and is associated with various diseases such as metabolic syndrome. GDF-15 (Growth differentiation factor 15) is a cytokine belonging to the transforming growth factor - β superfamily and it has been found to be associated with chronic heart failure, acute pulmonary embolism, and acute attacks of chronic obstructive pulmonary disease (COPD). It was aimed to evaluate the relationship between disease severity and mean GDF-15 in patients diagnosed with OSAS. Our study was carried out with 75 patients with OSAS. GDF-15 levels were checked by in the morning fasting blood test. Among the 75 patients, 27 constituted the mild, 22 constituted the moderate and 26 constituted the severe OSAS groups. The mean ages of the patients were not significantly different between groups (53.17 ±10, 54.61±10.2 and 53.5±11.6 respectively; p>0,05). GDF-15 levels were not significantly different between groups separately (p>0.05). There was only a weak positive relationship between GDF-15 and NREM and REM (p<0.05). In our study, it was revealed that there was no significant relationship between the OSAS severity groups and GDF-15. Our determination of a positive relationship with NREM and REM may have been related to reduction of minute ventilation experienced in OSAS-diagnosed patients, tachycardic fluctuations, their more severe nature and increased right ventricular pressure. Consequently, our current knowledge indicates that GDF-15 is not much guiding in the prediction and monitoring of OSAS severity. © 2022 Ondokuz Mayis Universitesi. All rights reserved

İntroduction: Although breast cancer is a type of cancer that is mainly seen in advanced ages, its incidence is increasing in early ages in developing countries. While advanced age may create limitations in treatment due to the person's functional capacity and low life expectancy, treatments may be exaggerated at a young age due to the long-life expectancy and expectations from life. In this study, we aimed to examine possible differences in approach in two different age groups diagnosed with breast cancer. Materials and Methods: The retrospective study included 123 patients over the age of 70 or under 35 who applied to our hospital's breast surgery outpatient clinic between 2016 and 2021 and were diagnosed with breast cancer. The patients' complaints at the outpatient clinic, the histopathological features of the tumor, and the treatments applied were compared. Results: 64 (52%) of the patients included in the study were over 70 years old, and 59 (47%) were under 35 years old. Patients in both groups were applied to the outpatient clinic with a palpable mass. Histopathologically, invasive ductal cancer constituted the majority in both groups. While the elderly patients were lower grade and Estrogen receptor-positive, the younger patients were high grade, Estrogen receptor negative, and mostly triple negative. Conclusions: Regardless of age, breast cancer is a cancer type that can have better results with early diagnosis. While making the treatment decision the decision should be made according to the characteristics of the tumor, comorbidity, and life expectancy, regardless of the patient's age

MITF-MIR211 axis is a novel autophagy amplifier system during cellular stress

Macroautophagy (autophagy) is an evolutionarily conserved recycling and stress response mechanism. Active at basal levels in eukaryotes, autophagy is upregulated under stress providing cells with building blocks such as amino acids. A lysosome-integrated sensor system composed of RRAG GTPases and MTOR complex 1 (MTORC1) regulates lysosome biogenesis and autophagy in response to amino acid availability. Stress-mediated inhibition of MTORC1 results in the dephosphorylation and nuclear translocation of the TFE/MITF family of transcriptional factors, and triggers an autophagy- and lysosomal-related gene transcription program. The role of family members TFEB and TFE3 have been studied in detail, but the importance of MITF proteins in autophagy regulation is not clear so far. Here we introduce for the first time a specific role for MITF in autophagy control that involves upregulation of MIR211. We show that, under stress conditions including starvation and MTOR inhibition, a MITF-MIR211 axis constitutes a novel feed-forward loop that controls autophagic activity in cells. Direct targeting of the MTORC2 component RICTOR by MIR211 led to the inhibition of the MTORC1 pathway, further stimulating MITF translocation to the nucleus and completing an autophagy amplification loop. In line with a ubiquitous function, MITF and MIR211 were co-expressed in all tested cell lines and human tissues, and the effects on autophagy were observed in a cell-type independent manner. Thus, our study provides direct evidence that MITF has rate-limiting and specific functions in autophagy regulation. Collectively, the MITF-MIR211 axis constitutes a novel and universal autophagy amplification system that sustains autophagic activity under stress conditions. No sponso

PURPOSEThe clinical management of high-risk lesions using image-guided biopsy is challenging. This study aimed to evaluate the rates at which such lesions were upgraded to malignancy and identify possible predictive factors for upgrading high-risk lesions.METHODSThis retrospective multicenter analysis included 1.343 patients diagnosed with high-risk lesions using an image-guided core needle or vacuum-assisted biopsy (VAB). Only patients managed using an excisional biopsy or with at least one year of documented radiological follow-up were included. For each, the Breast Imaging Reporting and Data System (BI-RADS) category, number of samples, needle thickness, and lesion size were correlated with malignancy upgrade rates in different histologic subtypes. Pearson’s chi-squared test, the Fisher–Freeman–Halton test, and Fisher’s exact test were used for the statistical analyses.RESULTSThe overall upgrade rate was 20.6%, with the highest rates in the subtypes of intraductal papilloma (IP) with atypia (44.7%; 55/123), followed by atypical ductal hyperplasia (ADH) (38.4%; 144/375), lobular neoplasia (LN) (12.7%; 7/55), papilloma without atypia (9.4%; 58/611), flat epithelial atypia (FEA) (8.7%; 10/114), and radial scars (RSs) (4.6%; 3/65). There was a significant relationship between the upgrade rate and BI-RADS category, number of samples, and lesion size Lesion size was the most predictive factor for an upgrade in all subtypes.CONCLUSIONADH and atypical IP showed considerable upgrade rates to malignancy, requiring surgical excision. The LN, IP without atypia, pure FEA, and RS subtypes showed lower malignancy rates when the BI-RADS category was lower and in smaller lesions that had been adequately sampled using VAB. After being discussed in a multidisciplinary meeting, these cases could be managed with follow-up instead of excision
