    Motivations, Capability Handicaps and Firm Responses in the Early Phase of Internationalization from Emerging Economies: A study in the Indian Pharmaceutical Industry

    This paper identifies and analyses the motivations, capability handicaps and responses of a sample of Indian pharmaceutical firms in the early phase of internationalization. It distinguishes between the experiences of two types of internationalisers –initial internationalisers and later internationalisers - in the industry. It argues that the initial internationalisers face several discontinuities vis-a-vis the experience of meeting the needs of domestic market. They need to cultivate new capabilities by leveraging on whatever is available within the firms and the external environment. Their capability to cultivate depends on their internal processes to absorb the new experiences. The later internationalisers do not experience these handicaps. They can benefit from the industry experience and congregate capabilities to move faster. Their capability to congregate depends on the initial endowments of the founders. Based on its findings, the paper outlines scope for further research in capability building for internationalization in the context of emerging economies.

    An interventional study on functional outcome of combined anterior cruciate ligament and anterolateral ligament reconstruction

    Background: The anterolateral ligament (ALL) is an important structure for rotational stability of knee joint after anterior cruciate ligament (ACL) rupture. Outcome of combined ACL and ALL reconstruction will change view of high demanding ACL tear cases.Methods: A hospital based prospective interventional study was done in Department of Orthopaedics, SMS Medical College, Jaipur to find the functional outcome of combined anterior cruciate ligament and anterolateral ligament reconstruction. A total of 45 patients underwent ACL and ALL reconstruction. Indications for a combined procedure were associated grade 3 pivot shifts, high level of sporting activity and pivoting sports. Patients were assessed pre- and post-operatively with objective and subjective International Knee Documentation Committee (IKDC) score, Lysholm score, and Tegner activity scale.Results: The mean follow up time was around 5 months. Two patients lost to follow up. Leaving 45 patients for final evaluation. At the last follow-up, all patients had full range of motion. The Lysholm, subjective IKDC, and objective IKDC scores were significantly improved (all p<0.0001). Pre-operatively, 38 patients had a grade 3 pivot shifts and 7 had a grade 2 according to the IKDC criteria. Post-operatively, 42 patients had a negative pivot shift (grade 0), and 3 patients were grade 1 (p<0.0001).Conclusions: This study demonstrates that a combined reconstruction can be an effective procedure without specific complications at a minimum follow-up of 6 months. Longer follow up is required to know any long term complications and functional outcome

    Eosinophilia refers to peripheral blood absolute eosinophil count above the ULN, normal range of AEC is 0.05-0.5× 109/l (1-6%). Hyper eosinophilia refers to AEC above 1.5×109/l. Hypereosinophilia can affect multiple organs and can cause cardiomyopathy, gastroenteritis, cutaneous lesions, pneumonitis, and neuritis. In addition, some patients develop thromboembolic complications. We are presenting a case who presented to us with thromboembolic complication later diagnosed as hypereosinophilia with Bone marrow showing myeloid associated eosinophilia (Primary eosinophilia)

    Gravitational baryogenesis of cosmological constant dominated universe

    In this paper, we have studied the gravitational baryogenesis of isotropic and homogeneous universe in the frame-work of general relativity. We investigate an exact and new solution of Einstein's field equations for FRW metric. Our solution represents a transitioning model of the universe which was expanding in decelerated mode and it transit in accelerated mode after dominance of cosmological constant Λ\Lambda. We observe that gravitational baryogenesis occurs in the derived universe and derived baryon entropy ratio is in good agreement with its observational value.Comment: 8 Pages, 4 Figures, Accepted in Gravitation and Cosmolog

    Enhanced Spam Detection System for Twitter Social Networking Platform

    Twitter social site is one of the most popular Online Social Networking Site (OSN) used by popular people such as Ministers, businessman, large companies, actors to share their information. In this site, around 500 million of tweets are posted monthly by the total 313 million Twitter active users. The widespread of Twitter has drawn the interest of spammers. These malicious actors exploit the platform for various nefarious purposes, including monitoring authentic users, disseminating harmful software, and promoting their agendas through URLs embedded in tweets. They engage in tactics like secret following and unfollowing legitimate users, all with the intent of gathering sensitive information.To resolve this problem, a secure spam detection based on machine learning approach is designed. The designed used stop word removal, word to vector model to refined and dimensionally reduced the data. To enhance the quality of the data Cosine similarity is also been applied to measure the similarity score among the tweets and based upon that Artificial Neural Network (ANN) is trained. Later on, it is used to test the efficiency by examining the performance parameters in terms of precision, recall and F-measure. Also, the comparative analysis has been performed to present the efficiency of the work. The average precision, recall and F measure of proposed spam detection model of 0.9252, 0.6107 and 0.734 are obtained

    A CROSS-SECTIONAL STUDY OF TYPE 2 DIABETES MELLITUS PREVALENCE AMONG THE INDIAN RURAL POPULACE IN BIHAR.

    Background: Diabetes is difficult to manage in resource-poor countries like India, where many people are diagnosed late and have complications. This study seeks to increase diabetes research in rural India. The main goal of this study is to ascertain the incidence of Type 2 diabetes mellitus among rural populations in Bihar, India, who are 25 years of age and older. Methods: During the study, health camps were held and went door-to-door to determine rural diabetes prevalence. Fasting capillary blood glucose was tested with a glucose meter. All adults then consumed 75 g of glucose dissolved in 200 milliliters of water in five minutes. Two hours after intake, post-load capillary blood glucose was measured. Diabetes was confirmed by collecting blood samples in a fluorine vacutainer, with fasting plasma glucose levels ≄126 mg/dl and/or 2-hour postprandial glucose levels ≄200 mg/dl as diagnostic criteria. Results: According to the survey, 7.03% of rural residents had Type 2 diabetes; females had a greater prevalence of the disease (56%) than males (44%). Diabetes affected 28% of participants over the age of 70, compared to 11% of those in the 25–39 age range. Furthermore, 34.77% of diabetics were newly diagnosed cases, while 8.04% of participants had a prediabetic diagnosis. Conclusions: This study shows an elevated incidence of Type 2 diabetes in rural Bihar, India. Rural areas need comprehensive early diagnosis, care, and diabetes prevention education to meet this growing health burden. Recommendation: Early detection, management, and preventative education are crucial in rural western Uttar Pradesh, where Type 2 diabetes is common. Community-based awareness initiatives and accessible healthcare can lessen diabetes complications in the region

    Scrub typhus in North India: clinical spectrum, laboratory parameters and treatment outcome in tertiary care teaching hospital

    Background: Scrub typhus is an acute febrile illness causing serious complications leading to significant mortality especially if there is delay in diagnosis and treatment. It is caused by Orentia tsutsugamushi a gram negative bacterium and transmitted by the bite of the trombiculid mite (chigger). This study was undertaken to document the clinical manifestations, laboratory parameters and treatment outcomes of scrub typhus cases.Methods: This retrospective study was done in a tertiary care teaching hospital which included 40 confirmed cases of scrub typhus. The diagnosis was confirmed by positive IgM ELISA. Clinical spectrum and manifestations, laboratory parameters and course in hospital with outcomes were evaluated. Factors associated with complications and mortality were analyzed.Results: The mean age of the patients was 40 ±15 yrs with almost equal proportion of males and females (47.5 vs 52.5%). The most common presenting symptoms were Fever (100%), shortness of breath (40%), altered sensorium (22.5%), nausea/vomiting (10%), and diarrhea (7.5%). Mean duration of fever before presentation to hospital was 11.1±4.9 days. Eschar was seen in 15% of patients. Common laboratory abnormalities documented was thrombocytopenia (85%), elevated transaminases (57%) leukocytosis (45%), and leucopenia (15%). About 37.5% of patients developed multiple organ dysfunction syndrome (MODS) with case fatality rate was 10%. Acute renal failure, acute hepatitis, need of ventilator support and CNS dysfunction was higher among patient with MODS.Conclusions: Scrub typhus patients can have a wide range of manifestation ranging from mild illness to serious and life threatening complications like acute respiratory distress syndrome, acute renal failure, and acute hepatitis and CNS dysfunction. High index of suspicion with early recognition and treatment is key for good outcome. Use of empirical doxycycline may be lifesaving
