    Smoothening and Segmentation of ECG Signals Using Total Variation Denoising –Minimization-Majorization and Bottom-Up Approach

    AbstractAn ECG Signal records electrical activity of heart. It includes information on heart's rhythm and is useful for diagnosis of heart related diseases. It encounters with various artifacts during acquisition and transmission. The unwanted signals/noises present in ECG signals disturb the clinical information present in it. This paper tries to reduce unwanted signals through Majorization-Minorization approach to optimize total variation in the signals. The denoised signal is then segmented using bottom up approach. The results show significant improvement in signal to noise ratio and successful segmentation of sections of ECG signals

The Shwasa Kriya is one of the main functions of PranaVata. Pranavaha Srotas means the channel, which carries the external air into the body to sustain the life. Thus the Pranavaha Srotas is most important for maintaining life and longevity. Hridaya and Mahasrotas are the Mula Sthana of Pranavaha Srotas. The diseases like Shwasaand Hikka have their origin from the Mulasthana of Srotas. The Mulasthana of Srotas is important area for proper functioning of Srotas. Hence the treatment of any disease should also include intervention for the betterment of Srotas. It implies the necessity of a thorough knowledge of Srotas, its Mulasthana, its normal functioning etc. In this respect here an effort is made to thoroughly understand the Pranavaha Srotas, its Mula Sthana and Shawashan Prakriya etc

    Benign Mucous Membrane Pemphigoid (MMP) is a vesiculobullous (VB) lesion that affects oral and nasal mucosa, oropharynx, conjunctiva, and genital mucosa. It is considered an autoimmune disease. Autoantibodies are produced due to several external factors or genetic factors. It creates many complications, such as defects in vision, voice alteration (hoarseness), airway obstruction, and dyspareunia. The subepithelial blisters form due to these antibodies. Treatment modalities differ from topical steroids to systemic steroid therapy based on the nature and severity of clinical symptoms

    Functional outcome of mobile bearing total knee arthroplasty a minimum of 5 years follow up study

    AIMS: To evaluate clinical, functional and radiological outcome following mobile bearing TKR performed in Department of Orthopaedics Christian Medical College, Vellore, for a follow up period of 5 years or more. OBJECTIVES: To assess post op infection, To calculate duration of stay in hospital, To assess BMI, To assess need for Blood transfusion, To assess range of movement, To assess posterior condyle offset, To assess joint line, To assess alignment, To assess pain, To assess functional outcome with knee society score. METHODS: Retrospective observational study. Patients who had undergone mobile bearing TKR between 2001 and August 2009 were recruited into the study after informed consent. They were informed by telephone and post, to come for follow up in Ortho OPD. Their current clinical and functional scoring was calculated using the Knee Society Score. Plain Radiograph of knees, AP & lateral views were taken at the follow up visit and radiological assessment of the joint was done .Their pre op American Knee society score was obtained from the Inpatient chart of their admission for the operation. Other variables like Age of the patient, sex, occupation, duration of hospitalisation were also analysed during the follow up visit. RESULTS AND CONCLUSION: Age increase showed a decrease in Knee Society Score and Functional Score, but no statistical significance was found. BMI was associated with decreased Knee Society Score and Functional score, but it was not statistically significant. BMI and mobility had shown poorly significant negative correlation. There was considerable improvement in the American Knee Society Score and functional score post operatively. No correlation between posterior condyle offset and knee range of movement or knee society score was found. There was significant elevation of the joint line post operatively. Very few patients developed erythema, redness and resolved after treatment with intra venous antibiotics

    A study of dengue fever in pregnancy and its maternal and fetal prognosis

    Background: Dengue is a vector borne viral disease. Female Anopheles mosquito is the vector for the disease. Recently, there is an increase in the incidence of dengue fever in adult population in South Asian countries. With increasing rate of adult dengue fever victims, the number of infected pregnant women has also been increased. Dengue, during pregnancy may be associated with various complications, including abortions, preterm delivery, maternal mortality, low birth weight, neonatal admissions and fetal anomalies. Timely intervention can improve the maternal as well as fetal outcome. This study was aimed to assess the clinical profile, maternal and fetal outcome of dengue fever during pregnancy.Methods: The study was carried out on 25 pregnant females diagnosed and serologically confirmed to have dengue fever and were admitted in Umaid hospital, associated to Dr. S. N. Medical College Jodhpur, Rajasthan, India. Patients were included irrespective of the period of gestation of contracting the disease. Serological testing for dengue virus specific antigen and antibody was done for the diagnosis of dengue fever. The World Health Organization (WHO) classification and case definitions 2009 were used to categorize the dengue patients. A predesigned proforma was used to collect data related to maternal and fetal consequences both during pregnancy and at birth, as well as the effect on the newborn. Informed and written consent was taken from all those who participated in the study.Results: Thrombocytopenia (platelet count <1.5lakh/mm3) was found in 22 (88%) patients out of which 6 (24%) of them had platelet count below 20,000 cells/mm3 and 3(12%) patients required platelet transfusion. Other complications observed were spontaneous abortions (4%); preterm birth (16%), oligohydramnios (8%) and antepartum hemorrhage (4%). One patient was admitted to Intensive Care Unit. Fetal distress and meconium stained amniotic fluid was observed in 16% and 12% patients respectively. Adverse fetal outcome was observed in form of low birth weight, prematurity. 8% of the babies required NICU admission and 4% were Intra Uterine Fetal Death (IUFD).Conclusions: Maternal infection with the dengue virus during antenatal period represents a real risk of premature birth. Early onset or late onset in pregnancy appeared to have a bad prognosis.  A high index of clinical suspicion is essential in any pregnant woman with fever during the epidemic. The treatment of dengue in pregnancy is mainly conservative as in non-pregnant adults. In case of high risk cases early referral to well-equipped health centres where technical, transfusion and intensive care facilities are available may prove lifesaving

    Analysing the consumer preference of fluid milk in province no. 2 of Nepal

    Information is an asset for any industry. Some information such as the consumer preference is hidden deep in the mind of the consumer which is difficult to access. Studies have revealed that the consumer preferences can be measured effectively and their research may provide a deeper understanding of the choices that consumers make when deciding to select an offer against another. Milk is one of the major components of diet for the people around the globe. The demand for milk and other dairy products is generally income elastic. The marketing of fluid milk is not similar as compared to other consumer-based goods. The demand for milk and milk products depend considerably on the consumption pattern, food habits, geographical region, urbanization and life style. The study was conducted to analyse the consumer preference of fluid milk in Province no. 2 of Nepal. Rautahat and Saptari districts from Province no. 2 were selected for the study. The total sample size of 180 household was selected for study but data from 159 households was only taken for consideration. Consumer preference was analysed using tabular and percentage analysis. Garret’s ranking technique was adopted to analyse the reason for preference of fluid milk by household consumer. From the study it was clear that almost all the households irrespective of the income and other socio-economic factors, preferred fluid milk. Nutritive value was found to be the most important reason for preference of fluid milk. The other reason for preference of fluid milk were taste, quality, availability, price and satisfaction. The consumption of fluid milk was found to be dependent over several socio- economic factors such as education, income, gender etc. The differences in consumption behaviour of the consumers provide an important inference to marketing and promotion strategies of dairy/ food products. Different promotion strategies based on different consumption determinants are perhaps necessary for effective marketing in a specific area

    The Prevalence Onychomycosis in North Western Region of Rajasthan

    NTRODUCTION: Onychomycosis is a fungal infection of nail caused by dermatophytes, yeast and mould. Onchomycosis infection continues to spread worldwide and found to persist everywhere. Onchomycosis infection prevalence is continuously increasing and the possible fungal pathogens are also increasing. All nail diseases are not fungal in origin hence laboratory investigations are needed to differentiate accurately between fungal infections and other conditions.AIM: to find out mycological profile &amp; morphological identification of fungal agent in onchomycosis infection and their prevalence in a part of Rajasthan.MATERIALS AND METHOD: A total 50 samples of nail clipings were investigated in department of microbiology by using 20% KOH and culture tubes of Sabouraud’s Dextrose Agar (SDA) which were mixed with chloramphenicol and cycloheximide.RESULT: A total of 50 samples (18 male and 32 females) were tested in period of one year (January to December 2016). Among them 28(56%) samples showed fungal growth, the predominat fungual pathogen in present study were Tricophyton spp. 12 (42.86%) followed by yeasts 09 (32.14%) then molds 07(25%). Fungi commonly presented in the middle age, between 31-40 years of age, due to trauma at the work site and in women, due to their wet work.CONCLUSION: This study highlighted that Dermatophyte T.Rubrum as the main fungal pathogen which caused onychomycosis in the study region. As there are several fungi which cause the infection so it is necessary to perform culture for appropriate treatment So, it is imperative to diagnose it properly by using microbiological techniques and to treat it properly. For proper management of onychomycosis, diagnosis and accurate treatment play a key role in better outcome
