    A Rare Case of Bilateral Maxillary Paramolars between 1st and 2nd Molars

    Paramolars are supernumerary teeth usually found buccally or palatally near the molars. Very few articles are found in the literature about bilateral maxillary paramolars. Most of the cases, the paramolars are situated between the second and third molars, while in very few cases, as in our case it was found between first and second molars.     Obstetric violence: a health system study

    Background: Disrespect and abuse during facility-based childbirth is a global problem with differing driving factors in different contexts. Obstetric Violence (OV) refers to professional deficiencies in maternity care. The objective of this study was to assess knowledge and attitudes towards OV in a cohort of members of the obstetric healthcare team at a tertiary care, teaching hospital in Western India (Mumbai).Methods: A questionnaire-based study involving 80 participants comprising of trainee doctors and faculty, nursing students, staff and teachers from the department of Obstetrics and Gynaecology and hospital administration.Results: 57.97% of participants had heard the term ‘Obstetric violence’ earlier. 75.36% reported verbal abuse as a form of OV, others being physical abuse, non-consented care, discriminatory care, abandonment/neglect or refusal of care and imposition of interventions without scientific basis. 53% and 89.8% of the participants did not consider routine episiotomies and artificial rupture of membrane respectively as forms of OV. 84.06% and 59.4% of participants considered instrumental delivery without consent and Caesarean section citing safety/convenience respectively as forms of OV. 82.6% participants endorsed the need of birth companion. Improving the number and training of healthcare providers and better institutional policies on respectful maternity care were suggested as solutions.Conclusions: Majority of the participants had witnessed some form of OV. The need for practical training of healthcare personnel and better infrastructure in the healthcare system were emphasised, but there appeared to be a lack of consciousness of the paternalistic mindset and approach to women in labour. Soft skills training of healthcare providers with emphasis on key ethical principles like autonomy, respect and dignity is crucial to address the issue of OV.

    Background: Malignant tumours in the nose and paranasal sinuses are challenging problems in the head and neck tumours. They represent the area of greatest histological diversity in the body with every tissue type represented. Clear cell odontogenic carcinoma (CCOC) is a rare neoplasm of the jaws and was first described by two separate groups of researchers, Hansen et al., and Waldron et al., in 1985Case presentation: We report case of a male with right maxillary tumour. The patient presented with painful swelling over right side of face with mucopurulent nasal discharge on applying pressure over the swelling. The histopathology of specimen taken during total maxillectomy confirmed the diagnosis.Conclusion: In most cases of maxillary sinus tumours, the medial spread into nasal cavity is seen primarily followed by the superior spread through the infra-orbital canal or the inferior spread ,as a result of which nasal blockage ,epistaxis, mass or hyposmia are the most common presenting symptoms along with dental symptoms. It is a diagnostic challenge and clinicians should be aware of similar association as it may result in delayed treatment with fatal consequences. Also, Clear cell odontogenic carcinoma is a rare malignant odontogenic neoplasm with benign looking histology. The acknowledgment of this rare tumour and its distinction from other clear cell neoplasms is crucial in establishing the appropriate therapeutic plan

    Trigonometric Parallaxes of Massive Star Forming Regions: III. G59.7+0.1 and W 51 IRS2

    We report trigonometric parallaxes for G59.7+0.1 and W 51 IRS2, corresponding to distances of 2.16^{+0.10}_{-0.09} kpc and 5.1^{+2.9}_{-1.4} kpc, respectively. The distance to G59.7+0.1 is smaller than its near kinematic distance and places it between the Carina-Sagittarius and Perseus spiral arms, probably in the Local (Orion) spur. The distance to W 51 IRS2, while subject to significant uncertainty, is close to its kinematic distance and places it near the tangent point of the Carina-Sagittarius arm. It also agrees well with a recent estimate based on O-type star spectro/photometry. Combining the distances and proper motions with observed radial velocities gives the full space motions of the star forming regions. We find modest deviations of 5 to 10 km/s from circular Galactic orbits for these sources, both counter to Galactic rotation and toward the Galactic center.Comment: 16 pages, 6 figures; to appear in the Astrophysical Journa

    Simulation for Cybersecurity: State of the Art and Future Directions

    In this article, we provide an introduction to simulation for cybersecurity and focus on three themes: (1) an overview of the cybersecurity domain; (2) a summary of notable simulation research efforts for cybersecurity; and (3) a proposed way forward on how simulations could broaden cybersecurity efforts. The overview of cybersecurity provides readers with a foundational perspective of cybersecurity in the light of targets, threats, and preventive measures. The simulation research section details the current role that simulation plays in cybersecurity, which mainly falls on representative environment building; test, evaluate, and explore; training and exercises; risk analysis and assessment; and humans in cybersecurity research. The proposed way forward section posits that the advancement of collecting and accessing sociotechnological data to inform models, the creation of new theoretical constructs, and the integration and improvement of behavioral models are needed to advance cybersecurity efforts

    Clostridium perfringens Epsilon Toxin Compromises the Blood-Brain Barrier in a Humanized Zebrafish Model

Available on open access from Elsevier vis the DOI in this recordClostridium perfringens epsilon toxin (ETX) is hypothesized to mediate blood-brain barrier (BBB) permeability by binding to the myelin and lymphocyte protein (MAL) on the luminal surface of endothelial cells (ECs). However, the kinetics of this interaction and a general understanding of ETX's behavior in a live organism have yet to be appreciated. Here we investigate ETX binding and BBB breakdown in living Danio rerio (zebrafish). Wild-type zebrafish ECs do not bind ETX. When zebrafish ECs are engineered to express human MAL (hMAL), proETX binding occurs in a time-dependent manner. Injection of activated toxin in hMAL zebrafish initiates BBB leakage, hMAL downregulation, blood vessel stenosis, perivascular edema, and blood stasis. We propose a kinetic model of MAL-dependent ETX binding and neurovascular pathology. By generating a humanized zebrafish BBB model, this study contributes to our understanding of ETX-induced BBB permeability and strengthens the proposal that MAL is the ETX receptor.National Institute of Neurological Disorders and StrokeWilma S. and Laurence A.
