    Incidental findings detected on preoperative CT imaging obtained for robotic-assisted joint replacements: clinical importance and the effect on the scheduled arthroplasty

    OBJECTIVE: To determine the type and frequency of incidental findings detected on preoperative computed tomography (CT) imaging obtained for robotic-assisted joint replacements and their effect on the planned arthroplasty. MATERIALS AND METHODS: All preoperative CT examinations performed for a robotic-assisted knee or total hip arthroplasty were obtained. This resulted in 1432 examinations performed between September 2016 and February 2020 at our institution. These examinations were initially interpreted by 1 of 9 fellowship-trained musculoskeletal radiologists. Using a diagnosis search, the examination reports were then reviewed to catalog all incidental findings and further classify as significant or non-significant findings. Demographic information was obtained. In those with significant findings, a chart review was performed to record the relevant workup, outcomes, and if the planned arthroplasty was affected. RESULTS: Incidental findings were diagnosed in 740 (51.7%) patients. Of those with incidental findings, 41 (5.5%) were considered significant. A significant finding was more likely to be detected in males (P = 0.007) and on the hip protocol CT (P = 0.014). In 8 patients, these diagnoses resulted in either delay or cancelation of the arthroplasty. A planned total hip arthroplasty was more likely to be altered as compared to a knee arthroplasty (P = 0.018). CONCLUSION: Incidental findings are commonly detected by radiologists on preoperative CT imaging obtained for robotic-assisted joint replacement. Several were valuable findings and resulted in a delay or even cancelation of the planned arthroplasty after the detection of critical diagnoses, which if not identified may have resulted in devastating outcomes

    The Lorentz force in atmospheres of CP stars: Īø\theta Aurigae

    Several dynamical processes may induce considerable electric currents in the atmospheres of magnetic chemically peculiar (CP) stars. The Lorentz force, which results from the interaction between the magnetic field and the induced currents, modifies the atmospheric structure and induces characteristic rotational variability of the hydrogen Balmer lines. To study this phenomena we have initiated a systematic spectroscopic survey of the Balmer lines variation in magnetic CP stars. In this paper we continue presentation of results of the program focusing on the high-resolution spectral observations of A0p star \aur (HD 40312). We have detected a significant variability of the HĪ±\alpha, HĪ²\beta, and HĪ³\gamma spectral lines during full rotation cycle of the star. This variability is interpreted in the framework of the model atmosphere analysis, which accounts for the Lorentz force effects. Both the inward and outward directed Lorentz forces are considered under the assumption of the axisymmetric dipole or dipole+quadrupole magnetic field configurations. We demonstrate that only the model with the outward directed Lorentz force in the dipole+quadrupole configuration is able to reproduce the observed hydrogen line variation. These results present new strong evidences for the presence of non-zero global electric currents in the atmosphere of an early-type magnetic star.Comment: 10 figure

    Four aspects of self-image close to death at home

    Living close to death means an inevitable confrontation with one's own existential limitation. In this article, we argue that everyday life close to death embodies an identity work in progress. We used a narrative approach and a holistic-content reading to analyze 12 interviews conducted with three persons close to death. By illuminating the unique stories and identifying patterns among the participantsā€™ narratives, we found four themes exemplifying important aspects of the identity work related to everyday life close to death. Two of the themes, named ā€œInside and outside of meā€ and ā€œSearching for togetherness,ā€ represented the core of the self-image and were framed by the other themes, ā€œMy place in spaceā€ and ā€œMy death and my time.ā€ Our findings elucidate the way the individual stories moved between the past, the present, and the future. This study challenges the idea that everyday life close to impending death primarily means limitations. The findings show that the search for meaning, new knowledge, and community can form a part of a conscious and ongoing identity work close to death

    Single-component plasma of photoelectrons ATRAP Collaboration

    Abstract Ten-nanosecond pulses of photoelectrons liberated by intense UV laser pulses from a thin gold layer are captured into a single-component plasma that is ideally suited to cool antiprotons (p) for antihydrogen (H) production. Up to a billion electrons are accumulated using a series of laser pulses, more than are needed for efficientp cooling in the large traps now being used for loadingp forH production. The method is demonstrated within an enclosed vacuum space that is entirely at 4 K, and is thus compatible with the exceptional cryogenic vacuum that is desirable for the long-term storage of antihydrogen. The pitfalls of other electron accumulation methods are entirely avoided, including the particle heating and declining efficiency of field emission point loading, and the heat load and contamination of thermionic emission methods.     How Treatment Partners Help: Social Analysis of an African Adherence Support Intervention

    Treatment partnering is an adherence intervention developed in sub-Saharan Africa. This paper describes the additional social functions that treatment partners serve and shows how these functions contribute to health and survival for patients with HIV/AIDS. Ninety-eight minimally structured interviews were conducted with twenty pairs of adult HIV/AIDS patients (NĀ =Ā 20) and treatment partners (NĀ =Ā 20) treated at a public HIV-care setting in Tanzania. Four social functions were identified using inductive, category construction and interpretive methods of analysis: (1) encouraging disclosure; (2) combating stigma; (3) restoring hope; and (4) reducing social difference. These functions work to restore social connections and reverse the isolating effects of HIV/AIDS, strengthening access to essential community safety nets. Besides encouraging ARV adherence, treatment partners contribute to the social health of patients. Social health as well as HIV treatment success is essential to survival for persons living with HIV/AIDS in sub-Saharan Africa
