    The National Military Strategy (2004), the Capstone Concept for Joint Operations (2005), and the Quadrennial Defense Review (2006) specifically highlight a new focus by the Department of Defense (DoD) on knowledge in operations as opposed to traditional weapons platforms. As such, each of the military services have put into place KM programs to varying degrees. According to Stankosky’s (2005) four pillars of KM framework, managing an organization’s knowledge assets can be most effectively accomplished by addressing four key elements--leadership, organization, technology, and learning—the “learning” pillar including KM education. Given that research on KM education is sparse (Ruth et al, 2000) and that organizations that do not address KM education are more likely to fail with KM efforts (Koenig, 2004), this multiple-case study provides a first look at KM education across the DoD. The preliminary results indicate that nature and importance of programs vary across the services, and, despite some leadership support, the resources needed to execute them are not always available

    Knowledge and attitudes of men to prostate cancer

    Objective: To ascertain the current level of understanding about prostate cancer (PCa), including treatment options and potential side effects of treatment, among older men. Design and Setting: Questionnaires administered by general practitioners (GPs) in 5 general practices in the Perth metropolitan and regional areas of Western Australia. Participants: Convenience sample of men aged 40-80 years (n=503) with or without prostate cancer presenting for routine consultations. Main outcome measures: Knowledge and attitudes of men to prostate cancer Results: Eighty percent of men did not know the function of the prostate and 48% failed to identify PCa as the most common internal cancer in men. Thirty-five percent had no knowledge of the treatments for PCa and 53% had no knowledge of the side effects of treatments. Asked how they would arrive at a decision about treatment, 70% stated they would ask the GP/specialist for all their options and then decide themselves. Conclusion: This study confirms a deficit in knowledge of the disease among men in the at risk age group. Lack of knowledge encompassed areas which could delay diagnosis and hence treatment. Overall the population preferred some GP/specialist involvement in treatment decision making

    A general formalism is developed to construct a Markov chain model that converges to a one-dimensional map in the infinite population limit. Stochastic fluctuations are therefore internal to the system and not externally specified. For finite populations an approximate Gaussian scheme is devised to describe the stochastic fluctuations in the non-chaotic regime. More generally, the stochastic dynamics can be captured using a stochastic difference equation, derived through an approximation to the Markov chain.     Effect of supervised aerobic exercise rehabilitation on physical fitness and quality-of-life in survivors of critical illness: an exploratory minimized controlled trial (PIX study)

    BACKGROUND: Evidence is limited for the effectiveness of interventions for survivors of critical illness after hospital discharge. We explored the effect of an 8-week hospital-based exercise-training programme on physical fitness and quality-of-life. METHODS: In a parallel-group minimized controlled trial, patients were recruited before hospital discharge or in the intensive care follow-up clinic and enrolled 8–16 weeks after discharge. Each week, the intervention comprised two sessions of physiotherapist-led cycle ergometer exercise (30 min, moderate intensity) plus one equivalent unsupervised exercise session. The control group received usual care. The primary outcomes were the anaerobic threshold (in ml O(2) kg(−1) min(−1)) and physical function and mental health (SF-36 questionnaire v.2), measured at Weeks 9 (primary time point) and 26. Outcome assessors were blinded to group assignment. RESULTS: Thirty patients were allocated to the control and 29 to the intervention. For the anaerobic threshold outcome at Week 9, data were available for 17 control vs 13 intervention participants. There was a small benefit (vs control) for the anaerobic threshold of 1.8 (95% confidence interval, 0.4–3.2) ml O(2) kg(−1) min(−1). This advantage was not sustained at Week 26. There was evidence for a possible beneficial effect of the intervention on self-reported physical function at Week 9 (3.4; −1.4 to 8.2 units) and on mental health at Week 26 (4.4; −2.4 to 11.2 units). These potential benefits should be examined robustly in any subsequent definitive trial. CONCLUSIONS: The intervention appeared to accelerate the natural recovery process and seems feasible, but the fitness benefit was only short term.     Variations in Duschinsky rotations in m-fluorotoluene and m-chlorotoluene during excitation and ionization

    We investigate Duschinsky rotation/mixing between three vibrations for both m-fluorotoluene (mFT) and m-chlorotoluene (mClT), during electronic excitation and ionization. In the case of mFT, we investigate both the S1 → S0 electronic transition and the D0+ ← S1 ionization, by two-dimensional laser-induced fluorescence (2D-LIF) and zero-electron-kinetic energy (ZEKE) spectroscopy, respectively; for mClT, only the D0+ ← S1 ionization was investigated, by ZEKE spectroscopy. The Duschinsky mixings are different in the two molecules, owing to shifts in vibrational wavenumber and variations in the form of the fundamental vibrations between the different electronic states. There is a very unusual behavior for two of the mFT vibrations, where apparently different conclusions for the identity of two S1 vibrations arise from the 2D-LIF and ZEKE spectra. We compare the experimental observations to the calculated Duschinsky matrices, finding that these successfully pick up the key geometric changes associated with each electronic transition and so are successful in qualitatively explaining the vibrational activity in the spectra. Experimental values for a number of vibrations across the S0, S1, and D0+ states are reported and found to compare well to those calculated. Assignments are made for the observed vibration-torsion (“vibtor”) bands, and the effect of vibrational motion on the torsional potential is briefly discussed

    Ex-vivo HRMAS of adult brain tumours: metabolite quantification and assignment of tumour biomarkers

    Background: High-resolution magic angle spinning (HRMAS) NMR spectroscopy allows detailed metabolic analysis of whole biopsy samples for investigating tumour biology and tumour classification. Accurate biochemical assignment of small molecule metabolites that are “NMR visible” will improve our interpretation of HRMAS data and the translation of NMR tumour biomarkers to in-vivo studies. Results: 1D and 2D 1H HRMAS NMR was used to determine that 29 small molecule metabolites, along with 8 macromolecule signals, account for the majority of the HRMAS spectrum of the main types of brain tumour(astrocytoma grade II, grade III gliomas, glioblastomas, metastases, meningiomas and also lymphomas). Differences in concentration of 20 of these metabolites were statistically significant between these brain tumour types. During the course of an extended 2D data acquisition the HRMAS technique itself affects sample analysis: glycine, glutathione and glycerophosphocholine all showed small concentration changes; analysis of the sample after HRMAS indicated structural damage that may affect subsequent histopathological analysis. Conclusions: A number of small molecule metabolites have been identified as potential biomarkers of tumour type that may enable development of more selective in-vivo 1H NMR acquisition methods for diagnosis and prognosis of brain tumours

    Late Holocene Rupture History of the Alpine Fault in South Westland, New Zealand

    Abstract Strata and fault relationships revealed in five trenches excavated across the recent trace of the Alpine fault at the Haast, Okuru, and Turnbull Rivers, South Westland, New Zealand, record the three most recent surface-faulting events. Using back-stripping techniques to remove the three faulting events and the sedimentary units associated with the faulting restores the cross-sections to gravel-bed floodplains at the Haast and Okuru Rivers, at about A.D. 750. Horizontal and vertical offsets of stream channels and terrace risers reveal characteristic displacements of about 8–9 m dextral and up to 1 m vertical per event. Cumulative dextral displacement is 25 ± 3 m in the past three events. The most recent surface-rupture event was probably in A.D. 1717, and the next prior events were about A.D. 1230 ± 50 and about A.D.750 ± 50. The timing of these events is consistent with past large-great earth- quakes on the southern section of the Alpine fault inferred from off-fault data, but there are fewer events identified in trenches. Our three-event dataset indicates the aver- age surface-rupture recurrence interval for the South Westland section of the fault is about 480 years, much longer than the current elapsed time of 295 years. Therefore, the Alpine fault in South Westland may not be close to rupture as is often speculated
