    Living with Alzheimer’s Disease: A Study of Adult Day Health Services in Massachusetts

    The role of adult day health care (ADHC) is gaining increased attention as the nation prepares for the large cohort of baby boomers entering their later years. Many boomers are aging with physical and cognitive impairments, including Alzheimer\u27s disease and related dementias. Projections indicate that Massachusetts, along with the nation as a whole, is experiencing an increasing rate of older persons as baby boomers enter late-life. The Commonwealth can expect that persons with Alzheimer\u27s disease and their care partners will need community-based services that are specifically designed for adults with cognitive impairments. However, a report by the Robert Wood Johnson Foundation found that there is a serious lack of adult day care services for the state\u27s elderly population. The 2003 report found that Massachusetts is only meeting 62% of needs for adults with physical and cognitive impairments, and at least 78 more programs are needed in the state. Yet, programs in Massachusetts continue to close. The specific objectives of the study were to: (1) describe existing practices of adult day health care services in Massachusetts for persons living with Alzheimer\u27s disease and related dementias, (2) explore programs that are specifically designed for participants who are in late-stage dementia, (3) address challenges that adult day health care services are now encountering, and (4) envision new paradigms for meeting the needs of persons with early-stage and early-onset dementia

    Stepwise multimodality imaging assists in atrial myxoma diagnosis and management

    The case of a 67-year-old man who presented for elective gastroenterology procedures and was in atrial fibrillation is discussed. Transthoracic echocardiography revealed a large atrial mass. Preoperative coronary angiography revealed a heavily vascularized mass. Use of cardiac magnetic resonance identified the cardiac mass as likely an atrial myxoma.

    Climate modulates internal wave activity in the Northern South China Sea

The definitive version was published in Geophysical Research Letters 42 (2015): 831–838, doi:10.1002/2014GL062522.Internal waves (IWs) generated in the Luzon Strait propagate into the Northern South China Sea (NSCS), enhancing biological productivity and affecting coral reefs by modulating nutrient concentrations and temperature. Here we use a state-of-the-art ocean data assimilation system to reconstruct water column stratification in the Luzon Strait as a proxy for IW activity in the NSCS and diagnose mechanisms for its variability. Interannual variability of stratification is driven by intrusions of the Kuroshio Current into the Luzon Strait and freshwater fluxes associated with the El Niño–Southern Oscillation. Warming in the upper 100 m of the ocean caused a trend of increasing IW activity since 1900, consistent with global climate model experiments that show stratification in the Luzon Strait increases in response to radiative forcing.     Persistence of virus-specific immune responses in the central nervous system of mice after West Nile virus infection

    <p>Abstract</p> <p>Background</p> <p>West Nile virus (WNV) persists in humans and several animal models. We previously demonstrated that WNV persists in the central nervous system (CNS) of mice for up to 6 months post-inoculation. We hypothesized that the CNS immune response is ineffective in clearing the virus.</p> <p>Results</p> <p>Immunocompetent, adult mice were inoculated subcutaneously with WNV, and the CNS immune response was examined at 1, 2, 4, 8, 12 and 16 weeks post-inoculation (wpi). Characterization of lymphocyte phenotypes in the CNS revealed elevation of CD19<sup>+ </sup>B cells for 4 wpi, CD138 plasma cells at 12 wpi, and CD4<sup>+ </sup>and CD8<sup>+ </sup>T cells for at least 12 wpi. T cells recruited to the brain were activated, and regulatory T cells (Tregs) were present for at least 12 wpi. WNV-specific antibody secreting cells were detected in the brain from 2 to 16 wpi, and virus-specific CD8<sup>+ </sup>T cells directed against an immunodominant WNV epitope were detected in the brain from 1 to 16 wpi. Furthermore, these WNV-specific immune responses occurred in mice with and without acute clinical disease.</p> <p>Conclusions</p> <p>Virus-specific immune cells persist in the CNS of mice after WNV infection for up to 16 wpi.</p

    Enhanced interlayer neutral excitons and trions in trilayer van der Waals heterostructures

    Vertically stacked van der Waals heterostructures constitute a promising platform for providing tailored band alignment with enhanced excitonic systems. Here we report observations of neutral and charged interlayer excitons in trilayer WSe2-MoSe2-WSe2 van der Waals heterostructures and their dynamics. The addition of a WSe2 layer in the trilayer leads to significantly higher photoluminescence quantum yields and tunable spectral resonance compared to its bilayer heterostructures at cryogenic temperatures. The observed enhancement in the photoluminescence quantum yield is due to significantly larger electron-hole overlap and higher light absorbance in the trilayer heterostructure, supported via first-principle pseudopotential calculations based on spin-polarized density functional theory. We further uncover the temperature- and power-dependence, as well as time-resolved photoluminescence of the trilayer heterostructure interlayer neutral excitons and trions.     Mass coral mortality under local amplification of 2°C ocean warming

The definitive version was published in Scientific Reports 7 (2017): 44586, doi:10.1038/srep44586.A 2°C increase in global temperature above pre-industrial levels is considered a reasonable target for avoiding the most devastating impacts of anthropogenic climate change. In June 2015, sea surface temperature (SST) of the South China Sea (SCS) increased by 2 °C in response to the developing Pacific El Niño. On its own, this moderate, short-lived warming was unlikely to cause widespread damage to coral reefs in the region, and the coral reef “Bleaching Alert” alarm was not raised. However, on Dongsha Atoll, in the northern SCS, unusually weak winds created low-flow conditions that amplified the 2°C basin-scale anomaly. Water temperatures on the reef flat, normally indistinguishable from open-ocean SST, exceeded 6°C above normal summertime levels. Mass coral bleaching quickly ensued, killing 40% of the resident coral community in an event unprecedented in at least the past 40 years.     Massive Clusters in the Inner Regions of NGC 1365: Cluster Formation and Gas Dynamics in Galactic Bars

    Cluster formation and gas dynamics in the central regions of barred galaxies are not well understood. This paper reviews the environment of three 10^7 Msun clusters near the inner Lindblad resonance of the barred spiral NGC 1365. The morphology, mass, and flow of HI and CO gas in the spiral and barred regions are examined for evidence of the location and mechanism of cluster formation. The accretion rate is compared with the star formation rate to infer the lifetime of the starburst. The gas appears to move from inside corotation in the spiral region to looping filaments in the interbar region at a rate of ~6 Msun/yr before impacting the bar dustlane somewhere along its length. The gas in this dustlane moves inward, growing in flux as a result of the accretion to ~40 Msun/yr near the ILR. This inner rate exceeds the current nuclear star formation rate by a factor of 4, suggesting continued buildup of nuclear mass for another ~0.5 Gyr. The bar may be only 1-2 Gyr old. Extrapolating the bar flow back in time, we infer that the clusters formed in the bar dustlane outside the central dust ring at a position where an interbar filament currently impacts the lane. The ram pressure from this impact is comparable to the pressure in the bar dustlane, and both are comparable to the pressure in the massive clusters. Impact triggering is suggested. The isothermal assumption in numerical simulations seems inappropriate for the rare fraction parts of spiral and bar gas flows. The clusters have enough lower-mass counterparts to suggest they are part of a normal power law mass distribution. Gas trapping in the most massive clusters could explain their [NeII] emission, which is not evident from the lower-mass clusters nearby.Comment: 21 pages, 4 figures, accepted by ApJ, 702, September 2009, revision adds reference
