Early Pollen Sensitization in Children Is Dependent upon Regional Aeroallergen Exposure
Introduction. Aeroallergen sensitization occurs at an earlier age than previously noted. The purpose of this paper was to identify which pollens cause early sensitization in young children presenting with rhinitis symptoms. Methods. This paper was a retrospective analysis of skin test results from 2- to 8-year-old patients presenting with a history consistent with allergic rhinitis. Patients were tested to aeroallergens common to the Great Basin along with a histamine and saline control. Pollen counts were obtained from a Reno, NV-certified counting station. Results. 123 children less than 8 years of age were identified. Over 50% of these children were sensitized to at least one aeroallergen. Chemopodaciae, timothy, alfalfa, black walnut, olive, mountain cedar and willow were predominating sensitizing aeroallergens of the Great Basin Region. Pollen counts were notable for a early spring peak for the tree season, grass season in May and weed season in August. Pollen levels continued to November at low levels. Discussion. Aeroallergens causing early sensitization differed from those which had predominately been reported in other regions of the United States. Pediatric allergists should consider performing a local review of sensitizing aeroallergens in their region to assist with identification and management of allergic rhinitis in their youngest patients. Strengthening the global system of protected areas post-2020: A perspective from the IUCN World Commission on Protected Areas
Protected areas are the cornerstones of biodiversity conservation and have never been more relevant than at the present time when the world is facing both a biodiversity and a climate change crisis. The International Union for Conservation of Nature (IUCN) World Commission on Protected Areas (WCPA) has been helping to set global standards and best practice guidelines in protected area planning and management for 60 years. Following this guidance, many countries have made significant progress toward their Aichi Target 11 commitments under the Convention on Biological Diversity (CBD). The global community will be coming together at the 15th Conference of the Parties of the CBD to set new biodiversity conservation targets for the next decade, as milestones to 2050 and a vision of âa world living in harmony with nature.â This paper lays out the WCPA perspective on priorities for supporting effective protected and conserved areas for the post-2020 era
An objective and subjective comparison of Clear Care versus various multipurpose solutions
In this study funded by Ciba Vision, Ciba\u27s Clear Care one-step hydrogen peroxide solution was objectively and subjectively compared to various multipurpose solutions. Twenty-three soft contact lens wearers with various habitual multipurpose solutions (Optifree=5, Complete=6, Renu=ll, Solocare=l) were assigned to either their own solution or the Clear Care solution for disinfection and storage of their lenses for two-week phases. A seven day wash-out period without lens wear was completed before each phase to eliminate any corneal staining. At the initial, one-week, and two-week visits, subjects were asked to complete a subjective questionnaire, visual acuity with contact lenses was recorded, and a detailed slit lamp examination of external ocular health was performed. The depth, area and type of any corneal staining was determined by fluorescein staining and documented using a five region template. Clear Care was statistically found (p\u3c0.001) to provide better overall comfort and vision, and reduced symptoms of dryness to a greater extent than various multipurpose solutions. The depth of corneal staining was consistently found to be superficial and Clear Care was found to have a smaller average area and lesser average type of corneal staining after a two-week period as compared to multipurpose solutions. This study showed that Clear Care is an excellent alternative to multipurpose solutions
The first simultaneous pancreas, renal transplant in a patient with HIV in Australia
We report the first case of a simultaneous pancreas and renal transplantation, in Australia, in a 45 year old male with long standing human immunodeficiency virus infection, type 1 diabetes mellitus and diabetic nephropathy requiring haemodialysis. This patient experienced previous virological failure and subsequent resistance to most nucleoside and non-nucleoside reverse transcriptase inhibitors. However, using novel combinations of anti-retroviral agents, along with careful monitoring, successful outcomes were achieved during the peri and post transplantation period, with excellent pancreas and renal graft function at one year

Increased inflammatory response in aged mice is associated with age-related zinc deficiency and zinc transporter dysregulation
Aging is a complex process associated with physiological changes in numerous organ systems. In particular, aging of the immune system is characterized by progressive dysregulation of immune responses, resulting in increased susceptibility to infectious diseases, impaired vaccination efficacy, as well as systemic low grade chronic inflammation. Increasing evidence suggest that intracellular zinc homeostasis, regulated by zinc transporter expression, is critically involved in the signaling and activation of immune cells. We hypothesize that epigenetic alterations and nutritional deficits associated with aging may lead to zinc transporter dysregulation, resulting in decreases in cellular zinc levels and enhanced inflammation with age. The goal of this study was to examine the contribution of age-related zinc deficiency and zinc transporter dysregulation on the inflammatory response in immune cells. The effects of zinc deficiency and age on the induction of inflammatory responses were determined using an in vitro cell culture system as well as an aged mouse model. We showed that zinc deficiency, particularly the reduction in intracellular zinc in immune cells, was associated with increased inflammation with age. Furthermore, reduced Zip 6 expression enhanced proinflammatory response, and age-specific Zip 6 dysregulation correlated with an increase in Zip 6 promoter methylation. Furthermore, restoring zinc status via dietary supplementation reduced aged-associated inflammation. Our data suggested that age-related epigenetic dysregulation in zinc transporter expression may influence cellular zinc levels and contribute to increased susceptibility to inflammation with age.This is the authors' peer-reviewed accepted manuscript.
