    New Methods for the Objective Measurement of Bra Fit

    The fit of a bra is critical, not only from a comfort and support perspective but it can significantly impact on the health and well-being of the wearer (Chan et al. 2001). Bra fitting is particularly important for adolescent females as it can affect the size, shape, and composition of their bones and breasts during this growth period (Strasburger 2006). Wood et al. (2008) found that 80% of young women, aged 18-26, that they investigated, wore the wrong sized bras. Bra fitting is still an inherently subjective process. It relies on the experience of a professional bra fitter to assess the fit against a checklist of fitting criteria. More accurate fitting could be achieved if the process was made more objective. This paper presents the results of a study to objectively measure the fit of crop-top bras. Crop top bras are currently very popular because they are highly extensible, body hugging garments that are comfortable to wear, imperceptible under clothing and protective. Furthermore they have no supporting wires to cause excessive skin pressure which may inhibit body growth (Ashby 2005). However a critical feature of these bras is that they can cause breast deformation which detracts from their other beneficial features. This study specifically addresses these effects of breast deformation and defines new, objectively measurable parameters to quantify it, through a series of wearer trials and the analysis of 3D body scans conducted for a range of commercial crop top bras and newly developed bras with increased cup volumes designed to reduce the effects of breast deformation

    Medical management of cerebral edema in large hemispheric infarcts

    Acute ischemic stroke confers a high burden of morbidity and mortality globally. Occlusion of large vessels of the anterior circulation, namely the intracranial carotid artery and middle cerebral artery, can result in large hemispheric stroke in ~8% of these patients. Edema from stroke can result in a cascade effect leading to local compression of capillary perfusion, increased stroke burden, elevated intracranial pressure, herniation and death. Mortality from large hemispheric stroke is generally high and surgical intervention may reduce mortality and improve good outcomes in select patients. For those patients who are not eligible candidates for surgical decompression either due timing, medical co-morbidities, or patient and family preferences, the mainstay of medical management for cerebral edema is hyperosmolar therapy. Other neuroprotectants for cerebral edema such as glibenclamide are under investigation. This review will discuss current guidelines and evidence for medical management of cerebral edema in large hemispheric stroke as well as discuss important neuromonitoring and critical care management targeted at reducing morbidity and mortality for these patients

There is much to be learned from the pandemic's first year data that will likely remain applicable to future epidemics and possible pandemics. With only minor variants in virus strain, countries across the globe have suffered roughly the same pandemic by first glance, yet few locations exhibit the same patterns of viral spread, growth, and control as the state of Hawai'i.     During mouse embryogenesis, progenitors within the liver known as hepatoblasts give rise to adult hepatocytes and cholangiocytes. Hepatoblasts, which are specified at E8.5-E9.0, have been regarded as a homogeneous progenitor population that initiate differentiation from E13.5. Recently, scRNA-seq analysis has identified sub-populations of transcriptionally distinct hepatoblasts at E11.5. Here, we show that hepatoblasts are not only transcriptionally but also functionally heterogeneous, and that a subpopulation of E9.5-E10.0 hepatoblasts exhibit a previously unidentified early commitment to cholangiocyte fate. Importantly, we also identify a subpopulation constituting 2% of E9.5-E10.0 hepatoblasts that express the adult stem cell marker Lgr5, and generate both hepatocyte and cholangiocyte progeny that persist for the lifespan of the mouse. Combining lineage tracing and scRNA-seq, we show that Lgr5 marks E9.5-E10.0 bipotent liver progenitors residing at the apex of a hepatoblast hierarchy. Furthermore, isolated Lgr5+ hepatoblasts can be clonally expanded in vitro into embryonic liver organoids, which can commit to either hepatocyte or cholangiocyte fates.     Phytoplankton community changes in Kuantan Port (Malaysia), with emphasis on the paralytic-shellfish toxin-producing dinoflagellate Alexandrium tamiyavanichii

    The Kuantan Port (Pahang, Malaysia, South China Sea) is a multi-cargo port located on the east coast of Peninsular Malaysia. The port has served as an important seaway to major ports in Asia-Pacific regions. In November 2013 and August 2014, two incidents of paralytic shellfish poisoning (PSP) have been consecutively reported in the Port. In this study, a field investigation was undertaken in the Port from April 2015 to May 2016 as an effort to continuously monitor the occurrence of HAB species following the PSP episodes in the year 2013–2014. Phytoplankton and hydrographic samples were collected for quantitative and qualitative assessments in a monthly interval. To precisely quantify the PSP-toxins producing species Alexandrium tamiyavanichii, a real-time quantitative PCR (qPCR) assay was applied to detect the motile cells and cysts. The results revealed the presence of A. tamiyavanichii but with extremely low cell abundances (<0.1% of the total abundances). The species was found co-existed with other Alexandrium species. Alexandrium abundance was associated with salinity and nitrogen to phosphorus ratios but negatively correlated with PO4-P and NH4-N as revealed in the canonical correspondence analysis. Low cell abundances of diarrhetic-shellfish toxins producing dinoflagellates (Dinophysis spp.) and fish-killing species (Prorocentrum sigmoides, Akashiwo sanguinea, Noctiluca scintillans, Chattonella spp.) were also encountered in the port. The results of this study would provide useful baseline information for the assessment and management of ballast water in Malaysian ports and its territorial waters

    Prospective Study on Retinal Nerve Fibre Layer Thickness Changes in Isolated Unilateral Retrobulbar Optic Neuritis

    Purpose. To investigate the retinal nerve fibre layer (RNFL) thickness after unilateral acute optic neuritis using optical coherence tomography (OCT). Patients and Methods. This prospective cohort study recruited consecutive patients with a first episode of isolated, unilateral acute optic neuritis. RNFL thickness and visual acuity (VA) of the attack and normal fellow eye were measured at presentation and 3 months in both the treatment and nontreatment groups. Results. 11 subjects received systemic steroids and 9 were treated conservatively. The baseline RNFL thickness was similar in the attack and fellow eye (P≥0.4). At 3 months, the attack eye had a thinner temporal (P=0.02) and average (P=0.05) RNFL compared to the fellow eye. At 3 months, the attack eye had significant RNFL thinning in the 4 quadrants and average thickness (P≤0.0002) compared to baseline. The RNFL thickness between the treatment and nontreatment groups was similar at baseline and 3 months (P≥0.1). Treatment offered better VA at 3 months (0.1 ± 0.2 versus 0.3 ± 0.2 LogMAR, P=0.04). Conclusion. Generalized RNFL thinning occurred at 3 months after a first episode of acute optic neuritis most significantly in the temporal quadrant and average thickness. Visual improvement with treatment was independent of RNFL thickness

    Beliefs about the Use of Instagram: An Exploratory Study

    Given the rapid technological advancement, social media and communication devices continue to grow exponentially. Hence, social networking has become a major communication method in contemporary society. As a mobile application, Instagram has been widely used for personal reasons as well as business activities. Due to the lack of literature in understanding why people use Instagram in emerging markets, the present study serves as groundwork to explore the subject matter in Malaysia. The theory of reasoned action (TRA) was adapted so as to provide theoretical base to explain specific beliefs about Instagram. This provides better understanding of attitude and usage behavior towards Instagram. Qualitative method was adopted by means of personal interview in order to elicit belief factors about Instagram. Thirty-four interviews were conducted on the basis of data replicability, and all transcriptions were subsequently analyzed using content analysis. Results indicate that behavioral beliefs about Instagram are composed of five factors, and they are labeled as personal gratification, features usefulness, socializing role, product information and entertainment. Normative beliefs, in turn, are made up by six factors, and they are labeled as siblings, relatives, close friends/peers, friends in general, Facebook friends, and application reviewers. This exploratory study contributes a fundamental knowledge about Instagram and social networking communication with a theoretical stance. From managerial standpoint, it also elucidates the effect of communication manner and people from the same age-group on adopting the product

    Guidelines for peer support in high‐risk organizations: An international consensus study using the delphi method

    Despite widespread adoption of peer‐support programs in organizations around the world whose employees are at high risk of exposure to potentially traumatic incidents, little consensus exists regarding even the most basic concepts and procedures for these programs. In this article, consensus refers to a group decision‐making process that seeks not only agreement from most participants, but also resolution of minority objections. The aim of the current study was to develop evidence‐informed peer‐support guidelines for use in high‐risk organizations, designed to enhance consistency around goals and procedures and provide the foundation for a systematic approach to evaluation. From 17 countries, 92 clinicians, researchers, and peer‐support practitioners took part in a 3‐round web‐based Delphi process rating the importance of statements generated from the existing literature. Consensus was achieved for 62 of 77 (81%) statements. Based upon these, 8 key recommendations were developed covering the following areas: (a) goals of peer support, (b) selection of peer supporters, (c) training and accreditation, (d) role of mental health professionals, (e) role of peer supporters, (f) access to peer supporters, (g) looking after peer supporters, and (h) program evaluation. This international consensus may be used as a starting point for the design and implementation of future peer‐support programs in high‐risk organizations

    Hong Kong Chinese school children with elevated urine melamine levels: A prospective follow up study

    <p>Abstract</p> <p>Background</p> <p>In 2008, the outbreak of kidney stones in children fed by melamine-tainted milk products in Mainland China has caused major public concern of food safety. We identified Hong Kong school children with elevated urine melamine level from a community-based school survey in 2007-08 and reviewed their clinical status in 2009.</p> <p>Methods</p> <p>In 2007-08, 2119 school children participated in a primary and secondary school survey in Hong Kong using a cluster sampling method. Urine aliquots from 502 subjects were assayed for melamine level. High urine melamine level was defined as urine melamine/creatinine ratio >7.1 μg/mmol. Subjects with high urine melamine level were invited for clinical evaluation in 2009 including urinalysis and ultrasound imaging of the urinary system.</p> <p>Results</p> <p>The age range of this subcohort was 6 - 20 years with 67% girls (335 female and 167 male subjects). The spot urine melamine/creatinine ratio of the 502 urine aliquots ranged from undetectable to 1467 μg/mmol (median 0.8 μg/mmol). Of these, 213 subjects had undetectable level (42%). We invited 47 (9%) subjects with high urine melamine level for re-evaluation and one subject declined. The median duration of follow-up was 23.5 months (interquartile range: 19.8 - 30.6 months). None of the 46 subjects (28% boys, mean age 13.9 ± 2.9 years) had any abnormality detected on ultrasound study of the urinary system. All subjects had stable renal function with a median urine albumin-creatinine ratio of 0.70 mg/mmol (interquartile range: 0.00 - 2.55 mg/mmol).</p> <p>Conclusions</p> <p>Hong Kong Chinese school children with high urine melamine levels appeared to have benign clinical course in the short term although a long term follow-up study is advisable in those with persistently high urine melamine level.</p
