Childhood traumatic infarction causing left ventricular aneurysm: Diagnosis by two-dimensional echocardiography
After being struck by an automobile, a 9 year old boy developed transient right bundle branch block followed by electrocardiographic changes of inferior wall myocardial infarction and an increase of serum cardiac enzymes. Two-dimensional echocardiography demonstrated a prominent septal aneurysm. Subsequent cardiac catheterization confirmed septal aneurysm and demon- strated an additional inferior aneurysm. Six weeks after the accident, resection of the inferior aneurysm was performed. At 1 year follow-up study, residual septal aneurysm and cardiomegaly are present, but the boy is asymptomatic
Development and psychometric evaluation of ICIQ-PadPROM: A quality of life questionnaire to assess the treatment effect of absorbent continence products

Aim: To describe the development and evaluation of the ICIQ-PadPROM, the first self-reported quality of life questionnaire to assess the treatment effect of absorbent continence products, a new addition to the set of ICIQ modules providing international standardized assessment of lower pelvic dysfunction. Methods: Developed in four phases, question items semi-structured interviews were conducted with pad using men (n = 19), women (n = 6), with secondary analysis undertaken on transcripts (n = 15 women) from a previous study. Validity of a draft 67 item questionnaire was tested through cognitive debriefing interviews (n = 34) and postal survey (n = 239). Reliability was evaluated by 65 users with a 3 week interval between completions. Expert opinion and factor analysis were used to reduce the final questionnaire to 17 scored and 3 unscored items. Results: The questionnaire comprises four scored domains: Pad design and Physical Effects (seven items), Psychological Effects (four items), Social Effects and Pad Leakage (three items) and Burden of Pad Use (three items), plus three unscored items. Levels of missing data ranged from 0 to 7.6%, with fair-to-moderate agreement. The Cronbach's alpha coefficient for all question items was 0.91 and factor analysis was undertaken to reduce redundancy. Conclusion: Existing incontinence-related outcome tools measure change in symptoms and quality of life impact. The ICIQ PadPROM questionnaire is the first to measure the impact of absorbent continence products on quality of life in the absence of any change in symptoms and will help policy-makers, clinicians, industry, and researchers to evaluate different products designs and materials for different patient populations
Heterogeneity of preferences for the benefits of environmental stewardship: a latent-class approach
Since 2005, Environmental Stewardship (ES) has been the principal agri-environment scheme for England and is the key instrument for the delivery of increased environmental benefits from agricultural landscapes. The main objective of this study is to investigate whether or not individuals' preferences for the environmental benefits associated with ES vary depending on types of landscapes within which these benefits are delivered. A latent class model is applied to data obtained from a choice experiment survey of over 1000 respondents sampled across England. The results suggest that individuals have heterogenous preferences for the benefits of ES, though different segments of the population with more homogenous preferences can be identified. In particular, higher levels of benefit are often associated with the operation of ES in landscapes close to where respondents live. This leads to the suggestion that, in order to maximise the benefits of ES, its implementation could take this result into account by encouraging greater uptake from farmers whose land is closer to large populations

Deep venous thrombosis and pulmonary embolism : Prediction, prevention and treatment*
The manifestations of thromboembolism are highly variable and frequently obscure. Clinical diagnosis of these entities is difficult, and the majority of cases will not be diagnosed by the means presently available to us. Nevertheless, a heightened awareness of the vagaries of this disease can result in considerable improvement in diagnostic accuracy. Prompt diagnosis on the basis of the slightest clinical suspicion, followed by immediate and adequate anticoagulant therapy, should result in a measureable decrease in thromboembolic complications. However, if one hopes to achieve a profound reduction in incidence of thromboembolism, the only approach presently available is prophylactic rather than therapeutic. The use of measures to increase venous return in patients who are immobilized in bed, plus the institution of prophylactic anticoagulant therapy in a selected group of patients with a high predisposition to the development of thromboembolic diseases, should result in a significant reduction in disability and mortality from this theoretically preventable disease.Peer Reviewedhttp://deepblue.lib.umich.edu/bitstream/2027.42/32447/1/0000529.pd
