    ā€œI wish Iā€™d told themā€: a qualitative study examining the unmet psychosexual needs of prostate cancer patients during follow-up after treatment

    <b>Objective</b> To gain insight into patients' experiences of follow-up care after treatment for prostate cancer and identify unmet psychosexual needs.<p></p> <b>Methods</b> Semi-structured interviews were conducted with a purposive sample of 35 patients aged 59-82 from three UK regions. Partners were included in 18 interviews. Data were analyzed using constant comparison. <p></p> <b>Results</b> (1) Psychosexual problems gained importance over time, (2) men felt they were rarely invited to discuss psychosexual side effects within follow-up appointments and lack of rapport with health care professionals made it difficult to raise problems themselves, (3) problems were sometimes concealed or accepted and professionals' attempts to explore potential difficulties were resisted by some, and (4) older patients were too embarrassed to raise psychosexual concerns as they felt they would be considered 'too old' to be worried about the loss of sexual function.<p></p> <b>Conclusion</b> Men with prostate cancer, even the very elderly, have psychosexual issues for variable times after diagnosis. These are not currently always addressed at the appropriate time for the patient.Practice implications Assessments of psychosexual problems should take place throughout the follow-up period, and not only at the time of initial treatment. Further research examining greater willingness or reluctance to engage with psychosexual interventions may be particularly helpful in designing future intervention

    Technical problems of design and flight qualification of the proposed classes of surface insulation materials and leading edge materials were reviewed. A screening test plan, a preliminary design data test plan and a design data test plan were outlined. This program defined the apparent critical differences between the surface insulators and the leading edge materials, structuring specialized screening test plans for each of these two classes of materials. Unique testing techniques were shown to be important in evaluating the structural interaction aspects of the surface insulators and a separate task was defined to validate the test plan. In addition, a compilation was made of available information on proposed material (including metallic TPS), previous shuttle programs, pertinent test procedures, and other national programs of merit.     The Intrinsic Size of Sagittarius A* from 0.35 cm to 6 cm

    We present new high-resolution observations of Sagittarius A* at wavelengths of 17.4 to 23.8 cm with the Very Large Array in A configuration with the Pie Town Very Long Baseline Array antenna. We use the measured sizes to calibrate the interstellar scattering law and find that the major axis size of the scattering law is smaller by ~6% than previous estimates. Using the new scattering law, we are able to determine the intrinsic size of Sgr A* at wavelengths from 0.35 cm to 6 cm using existing results from the VLBA. The new law increases the intrinsic size at 0.7 cm by ~20% and <5% at 0.35 cm. The intrinsic size is 13^{+7}_{-3} Schwarzschild radii at 0.35 cm and is proportional to lambda^gamma, where gamma is in the range 1.3 to 1.7.Comment: ApJL, in pres

    Oxygen Moment Formation and Canting in Li2CuO2

    The possibilities of oxygen moment formation and canting in the quasi-1D cuprate Li2CuO2 are investigated using single crystal neutron diffraction at 2 K. The observed magnetic intensities could not be explained without the inclusion of a large ordered oxygen moment of 0.11(1) Bohr magnetons. Least-squares refinement of the magnetic structure of Li2CuO2 in combination with a spin-density Patterson analysis shows that the magnetization densities of the Cu and O atoms are highly aspherical, forming quasi-1D ribbons of localised Cu and O moments. Magnetic structure refinements and low-field magnetization measurements both suggest that the magnetic structure of Li2CuO2 at 2 K may be canted.     Modification of erythrocyte Na+/Li+ countertransport kinetics by two types of thiol group

    AbstractErythrocyte Naāˆ’/Li+ countertransport activity is decreased by reagents that react with thiol groups. An understanding of the role of these groups in control of Na+/Li+ countertransport may help to explain its association with disease states. The effect of thiol reactive agents on the kinetic parameters of Na+/Li+ countertransport has not previously been described. In choline medium, N-ethylmaleimide (NEM) and iodoacetamide (IAamide) cause a rapid decrease of about 40% in Km for external sodium (Km(So)) that is complete in 10 s with a much smaller change in Vmax and an increase in the Vmax/Km ratio. In Na medium, NEM and IAamide both cause a rapid decrease in Km(So) and Vmax. With NEM the partial reduction in Vmax is complete in 100 s although the NEM is sufficient to reduce Vmax up to 15 min. With IAamide the decrease in Vmax is initially slower but it continues apparently towards complete inhibition. These results indicate at least two types of thiol group controlling Na+/Li+ countertransport kinetics. The type 1 thiol reaction is Na independent and causes an increase in the apparent rate constant for Na association with the unloaded carrier so that Vmax/Km rises and Km(So) decreases. The type 2 thiol reaction is facilitated by Na at the outside ion-binding site and causes a decrease in Vmax, possibly by total blockage of carriers with IAamide but by a different mechanism with NEM such as reduced turnover rate

    Patient safety in community dementia services: what can we learn from the experiences of caregivers and healthcare professionals?

    Objectives This study aims to explore how patient safety in community dementia services is understood by caregivers, and healthcare professionals. Methods Cross-sectional analysis of guided one-to-one interviews with 10 caregivers, and 10 healthcare professionals. Results Caregivers and healthcare professionals identified a range of issues including medication errors, mis-communication between professionals, unclear service pathways and the effects of stress on caregiversā€™ behaviour. Caregivers and professionals differed in their attitudes to balancing safety with patient autonomy and who is responsible for managing safety. Conclusions This article helps to define the nature of safety issues in the context of community care for people with dementia. In contrast to hospital medicine, where the ideal treatment world is safe with all risks managed or minimised, in dementia some risks are actively taken in the interests of promoting autonomy. Caregiversā€™ views differ from those of health professionals but both parties see potential for collaborative working to manage risk in this context, balancing the promotion of autonomy with the minimisation of potential harm
