    Screening for Social Determinants of Health in Children with Attention-Deficit/Hyperactivity Disorder

    Purpose/Background Increased social determinants of health (SDoH) are associated with poorer health outcomes and decreased treatment compliance. Addressing SDoH can be associated with increased treatment compliance, although currently there is no standard for completing SDoH screening. The WE CARE screening tool is a validated screen that asks about 6 SDoH and whether the family is interested in assistance if the screen is positive. The purpose of this project was to implement the use of the WE CARE screening tool for patients with attention deficit hyperactivity disorder (ADHD) at follow-up visits. Methods From October 17, 2022 through November 22, 2022, charts were reviewed for patients scheduled with the participating nurse practitioner and physicians who met inclusion criteria. The WE CARE screening tool was administered and reviewed by the provider, then a referral to social work could be initiated if needed. Results Following chart review, 23 patients met eligibility criteria, and of these 12 (52%) completed a WE CARE form. Needs were identified on 2 of the completed forms (17 %), and no referrals were made to social work. Implications for Nursing Practice During the implementation period of this quality improvement project, only about half of eligible families had a screening form completed, either because the parent refused or because the physician or nurse practitioner forgot to give the form to the family. Based on the results of this initial implementation phase, more information is needed from the participating healthcare providers about whether they did not feel comfortable using the screen, thought the screen was helpful, or if there were other factors impacting the rate of form completion. It may also be helpful to try to use this screening tool with new patients first instead of follow-ups. The results of the current project do not yet support a change in practice by implementing this tool

    Differential expression analysis for sequence count data

    *Motivation:* High-throughput nucleotide sequencing provides quantitative readouts in assays for RNA expression (RNA-Seq), protein-DNA binding (ChIP-Seq) or cell counting (barcode sequencing). Statistical inference of differential signal in such data requires estimation of their variability throughout the dynamic range. When the number of replicates is small, error modelling is needed to achieve statistical power.

*Results:* We propose an error model that uses the negative binomial distribution, with variance and mean linked by local regression, to model the null distribution of the count data. The method controls type-I error and provides good detection power.

    Application of Statistical Shape Modeling to Predict Clinical Metric of Femoral Head Coverage in Patients with Developmental Dysplasia

    Developmental dysplasia of the hip (DDH) is described as under-coverage of the femoral head by the acetabulum, resulting in mechanical instability. Though DDH is often diagnosed using plain film radiographs, these images cannot adequately capture 3D joint coverage. Herein, we applied a 3D statistical shape model (SSM) to the femur and hemi-pelvis of patients with DDH to objectively measure shape variation and evaluated whether SSM outputs could predict measurements of joint coverage. The femur and hemi-pelvis were semi-automatically segmented from CT images (83 hips from 47 females with DDH). Surfaces of each hip were reconstructed from segmentations, aligned, and input into a multi-domain SSM (shapeworks.sci.utah.edu). Correspondence particles were automatically placed over the bone surfaces and a subset on the femoral head and acetabulum were isolated for a joint-specific model. Modes of shape variation were determined with principal component analysis (PCA). A sparse model of PCA modes predicting coverage was determined using linear regression with Lasso regularization. Coverage measurements ranged from 27.3% to 39.4%. Eight and 13 modes were selected for the full bone and joint-specific models, respectively. These modes represented 6.1% and 39.6% of the overall shape variation for full bone and joint-specific models with mean prediction errors of 0.9% and 0.6% coverage, respectively (Figure 1). Selected modes represented the depth of the acetabulum and oblateness of the femoral head, aligning well with the clinical description of DDH. In addition, the full bone model captured morphological and pose-related differences potentially related to altered muscle paths or differences in femoral torsion

    Combination of Statistical Shape Modeling and Statistical Parametric Mapping to Quantify Cartilage Contact Mechanics in Hip Dysplasia

    Finite element models can predict subject-specific chondrolabral stresses and help to elucidate the effect of under-coverage and incongruency of the hip joint in patients with dysplasia. However, complex stress patterns are difficult to generalize and evaluate statistically. With an established correspondence across shapes from statistical shape modeling (SSM), statistical parametric mapping (SPM) allows for evaluation of local variability while preserving model subject-specificity. Herein, we evaluated the combined application of SSM and SPM to compare cartilage contact stress between control subjects and patients with dysplasia. Previously published hip joint contact stresses were mapped onto chondrolabral surface meshes and incorporated into an SSM. Principal component analysis (PCA) quantified shape variation. Contact stress values from heel-strike of stair ascent (AH), stair descent (DH), and level walking (WH) and mid-stance of level walking (WM) were evaluated. Using SPM, regions of significant contact stress variation were identified based on test statistics from general linear statistical models and corrected for multiple comparisons using Gaussian random fields. Shape differences of the femoral and acetabular cartilage with labrum were captured by two and one PCA mode, respectively. Contact stress differences were observed in anterosuperior regions of the femoral cartilage for AH and DH and of the acetabular cartilage for DH and WM (Figure 1). The SPM identified regions of varied contact stresses were small and likely would have been diluted through averaging or region-splitting using traditional analysis methods. The combined application of SSM and SPM provides a method to generalize and statistically-compare subject-specific mechanics and joint morphology

    “3D Bioprinting for Reconstructive Surgery: Principles, Applications and Challenges”

    Despite the increasing laboratory research in the growing field of 3D bioprinting, there are few reports of successful translation into surgical practice. This review outlines the principles of 3D bioprinting including software and hardware processes, biocompatible technological platforms and suitable bioinks. The advantages of 3D bioprinting over traditional tissue engineering techniques in assembling cells, biomaterials and biomolecules in a spatially controlled manner to reproduce native tissue macro-, micro- and nanoarchitectures are discussed, together with an overview of current progress in bioprinting tissue types relevant for plastic and reconstructive surgery. If successful, this platform technology has the potential to biomanufacture autologous tissue for reconstruction, obviating the need for donor sites or immunosuppression. The biological, technological and regulatory challenges are highlighted, with strategies to overcome these challenges by using an integrated approach from the fields of engineering, biomaterial science, cell biology and reconstructive microsurgery

    Genotype x environment interactions in eggplant for fruit phenolic acid content

    Eggplant fruit are a rich source of phenolic acids that influence fruit culinary quality and antioxidant content. We evaluated the influence of production environments and stability of diverse genotypes across environments for eggplant fruit phenolic acid content. Ten Solanum melongena accessions consisting of five F-1 hybrid cultivars, three open-pollinated cultivars and two land race accessions, plus one S. macrocarpon and one S. aethiopicum accession, were grown at two locations under greenhouse and open field environments. Twenty phenolic acid conjugates were identified in fruit flesh and assigned to six classes that included hydroxycinnamic acid amides, caffeoylquinic acid esters, hydroxycinnamoylquinic acid esters, malonylcaffeoylquinic acid esters, di-hydroxycinnamoylquinic acid esters, and other hydroxycinnamic acid conjugates. There were significant differences among accessions for total phenolic acid conjugate content and for all six classes. There were no significant differences detected among the environments for any of the variables. However, the environment x accession interaction was highly significant for all phenolic acid classes. Broad-sense heritability estimates for all six phenolic acid classes were high, ranging from 0.64 to 0.96. Stability analysis demonstrated widespread instability for phenolic acid content across environments. Stability of the predominant caffeoylquinic acid esters class positively influenced stability of total phenolic acid content for some but not all genotypes.     The relationship between childhood body weight and dental caries experience:an umbrella systematic review protocol

    Abstract Background Obesity and dental caries are global public health problems which can impact in childhood and throughout the life course. In simple terms, childhood dental caries and body weight are linked via the common risk factor of diet. An association between dental caries and obesity has been described in a number of studies and reviews. However, similarly, a relationship has also been noted between low body weight and caries experience in children. This protocol will provide the framework for an umbrella review to address the following question: Does the available evidence support a relationship between dental caries experience and body weight in the child population? Methods This review protocol outlines the process to carry out an umbrella systematic review which will synthesise previous reviews of childhood dental caries experience and body weight. An umbrella review methodology will be used to examine the methodological and reporting quality of existing reviews. Discussion The final umbrella review aims to aggregate the available evidence in order to provide a summary for policymakers and to inform healthcare interventions.     Brief Report: Is Impaired Classification of Subtle Facial Expressions in Children with Autism Spectrum Disorders Related to Atypical Emotion Category Boundaries?

    Impairments in recognizing subtle facial expressions, in individuals with autism spectrum disorder (ASD), may relate to difficulties in constructing prototypes of these expressions. Eighteen children with predominantly intellectual low-functioning ASD (LFA, IQ <80) and two control groups (mental and chronological age matched), were assessed for their ability to classify emotional faces, of high, medium and low intensities, as happy or angry. For anger, the LFA group made more errors for lower intensity expressions than the control groups, classifications did not differ for happiness. This is the first study to find that the LFA group made more across-valence errors than controls. These data are consistent with atypical facial expression processing in ASD being associated with differences in the structure of emotion categories

    Individual exposures to drinking water trihalomethanes, low birth weight and small for gestational age risk: a prospective Kaunas cohort study

    <p>Abstract</p> <p>Background</p> <p>Evidence for an association between exposure during pregnancy to trihalomethanes (THMs) in drinking water and impaired fetal growth is still inconsistent and inconclusive, in particular, for various exposure routes. We examined the relationship of individual exposures to THMs in drinking water on low birth weight (LBW), small for gestational age (SGA), and birth weight (BW) in singleton births.</p> <p>Methods</p> <p>We conducted a cohort study of 4,161 pregnant women in Kaunas (Lithuania), using individual information on drinking water, ingestion, showering and bathing, and uptake factors of THMs in blood, to estimate an internal dose of THM. We used regression analysis to evaluate the relationship between internal THM dose and birth outcomes, adjusting for family status, education, smoking, alcohol consumption, body mass index, blood pressure, ethnic group, previous preterm, infant gender, and birth year.</p> <p>Results</p> <p>The estimated internal dose of THMs ranged from 0.0025 to 2.40 mg/d. We found dose-response relationships for the entire pregnancy and trimester-specific THM and chloroform internal dose and risk for LBW and a reduction in BW. The adjusted odds ratio for third tertile vs. first tertile chloroform internal dose of entire pregnancy was 2.17, 95% CI 1.19-3.98 for LBW; the OR per every 0.1 μg/d increase in chloroform internal dose was 1.10, 95% CI 1.01-1.19. Chloroform internal dose was associated with a slightly increased risk of SGA (OR 1.19, 95% CI 0.87-1.63 and OR 1.22, 95% CI 0.89-1.68, respectively, for second and third tertile of third trimester); the risk increased by 4% per every 0.1 μg/d increase in chloroform internal dose (OR 1.04, 95% CI 1.00-1.09).</p> <p>Conclusions</p> <p>THM internal dose in pregnancy varies substantially across individuals, and depends on both water THM levels and water use habits. Increased internal dose may affect fetal growth.</p
