Modeling Hemorrhagic Stroke and Vasospasm in the Rodent

In January 2010, I started work as an undergraduate research assistant at SUNY Upstate Medical University in Dr. Mary Lou Vallano’s laboratory in the Department of Neuroscience and Physiology. My research project was part of a collaborative effort with the laboratory of Dr. Eric Deshaies, a vascular neurosurgeon at Upstate. The goal was to establish a rodent model of subarachnoid hemorrhage and delayed vasospasm, and to test possible protective strategies. We used an adult rat model, in which two injections of autologous blood were given in the cisterna magna region of the brain. Analysis was done using a combination of histology, immunohistochemistry, and molecular biology to examine vasospasm in the basilar artery, and changes in the surrounding neural tissues. Based on Dr. Deshaies’ patient information and clinical work, we examined whether exercise preconditioning or the injection of stannous chloride would decrease the inflammatory response and the occurrence or intensity of vasospasm. Our results indicate that the model is working, and they point to the possible protective effect of stannous chloride

The Art of Managing Expectations: Vocational Rehabilitation Counselors as Mediators of Expectations between Clients who are Deaf and Potential Employers

Services for individuals who are deaf or hard of hearing (DHH) often include tailored strategies for successful employment placement. Using Oliver’s (1980) Expectation Confirmation Theory, this study utilized exploratory and confirmatory analyses of semi-structured interviews of 10 VR professionals to examine their view of the role expectations play in their assessment of important factors that lead to successful outcomes for DHH clients. Analysis revealed a complex, multilevel system in which VRCs operate as active mediators of the expectations of both clients who are DHH and their potential employers

Maternal Syphilis: Variations in Prenatal Screening, Treatment, and Diagnosis of Congenital Syphilis

Syphilis is a sexually transmitted infection that, if left untreated, can impact fetal development. In this systematic review of syphilis in pregnancy, we attempt to better understand worldwide discrepancies regarding its diagnosis and management. OVID MEDLINE and PubMed databases were searched for keywords and 74 relevant articles were identified. Twenty-nine articles were ultimately included in our review. In the literature spanning from 1944—2014, we identified several variations in maternal syphilis screening and treatment, as well as a spectrum of gestational outcomes. Even following the publication of universal guidelines by the World Health Organization, the Centers for Disease Control and Prevention, and scientific investigators, practice patterns have continued to vary. Greater adherence to these guidelines could improve the quality of research in this area and promote earlier detection and thus prevention of maternal and congenital syphilis

Trauma and the use of formal and informal resources in the deaf population: perspectives from mental health service providers

Using grounded theory analysis, the current study identifies the perspectives of therapists and counselors regarding the nature of informal and formal resources supporting the treatment of deaf individuals with trauma. Nineteen counselors and therapists were interviewed, and accessibility, formal support, informal networks, and gaps in resources were identified as salient themes. Subsequent analysis identified concerns with confidentiality as a strong theme. This study emphasizes the need for counselors and therapists to become mindful of utilizing resources to support the treatment of trauma while being concerned with confidentiality

Clinical and neurocognitive outcome in symptomatic isovaleric acidemia

<p>Abstract</p> <p>Background</p> <p>Despite its first description over 40 years ago, knowledge of the clinical course of isovaleric acidemia (IVA), a disorder predisposing to severe acidotic episodes during catabolic stress, is still anecdotal. We aimed to investigate the phenotypic presentation and factors determining the neurological and neurocognitive outcomes of patients diagnosed with IVA following clinical manifestation.</p> <p>Methods</p> <p>Retrospective data on 21 children and adults with symptomatic IVA diagnosed from 1976 to 1999 were analyzed for outcome determinants including age at diagnosis and number of catabolic episodes. Sixteen of 21 patients were evaluated cross-sectionally focusing on the neurological and neurocognitive status. Additionally, 155 cases of patients with IVA published in the international literature were reviewed and analyzed for outcome parameters including mortality.</p> <p>Results</p> <p>57% of study patients (12/21) were diagnosed within the first weeks of life and 43% (9/21) in childhood. An acute metabolic attack was the main cause of diagnostic work-up. 44% of investigated study patients (7/16) showed mild motor dysfunction and only 19% (3/16) had cognitive deficits. No other organ complications were found. The patients' intelligence quotient was not related to the number of catabolic episodes but was inversely related to age at diagnosis. In published cases, mortality was high (33%) if associated with neonatal diagnosis, following manifestation at an average age of 7 days.</p> <p>Conclusions</p> <p>Within the group of "classical" organic acidurias, IVA appears to be exceptional considering its milder neuropathologic implications. The potential to avoid neonatal mortality and to improve neurologic and cognitive outcome under early treatment reinforces IVA to be qualified for newborn screening.</p
