    Health and Human Trafficking: The role of health care professionals from prevention to aftercare

    Until very recently, the global health care community has lagged behind in identifying human trafficking as a health care issue. Globally, there is a great gap health care for most survivors of human trafficking today. Health, or rather the lack of it, is a huge factor in addressing trafficking in persons at all stages of the process. Trafficking is a health care issue because addressing the physical and mental health of a survivor is central to the restoration of his or her well-being. Counter-trafficking work is a multi-disciplinary effort and health care professionals can participate in a variety of roles. One of the obvious ways is in providing trauma-informed, competent, and compassionate health care for survivors of human trafficking. Another way is in receiving training and building awareness for more effective identification of victims in the health care setting. Evidence-based medicine is the foundation of our practices today and yet there is little data that documents the health of trafficked people, from their pre-trafficked situation through the long-term after care process. A proper understanding through research of the health risks will help to inform and improve our interventions. Health care professionals may participate in preventive work through outreach to at-risk populations. Advocacy of victims to law enforcement, legal aid, the media and to the general public is also an important role. Dr. Welch will discuss the gaps in health care as well as some of the many ways in which health care professionals can have a profound impact on counter-trafficking efforts

    Retrospective correction of involuntary microscopic head movement using highly accelerated fat image navigators (3D FatNavs) at 7T

    Purpose: The goal of the present study was to use a three- dimensional (3D) gradient echo volume in combination with a fat-selective excitation as a 3D motion navigator (3D FatNav) for retrospective correction of microscopic head motion during high-resolution 3D structural scans of extended duration. The fat excitation leads to a 3D image that is itself sparse, allowing high parallel imaging acceleration factors – with the additional advantage of a minimal disturbance of the water signal used for the host sequence. Methods: A 3D FatNav was inserted into two structural proto- cols: an inversion-prepared gradient echo at 0.33 0.33 1.00 mm resolution and a turbo spin echo at 600 mm isotropic resolution. Results: Motion estimation was possible with high precision, allowing retrospective motion correction to yield clear improvements in image quality, especially in the conspicuity of very small blood vessels. Conclusion: The highly accelerated 3D FatNav allowed motion correction with noticeable improvements in image quality, even for head motion which was small compared with the voxel dimensions of the host sequence

    Honeaite, a new gold-thallium-telluride from the Eastern Goldfields, Yilgarn Craton, Western Australia

    Honeaite, ideal formula Au3TlTe2, is a new mineral from the late Archaean Karonie gold deposit, Eastern Goldfields province, Western Australia. Honeaite is found with native gold, tellurobismuthite, petzite, hessite, calaverite, melonite, mattagamite, frohbergite, altaite, pyrrhotite and molybdenite. These minerals are concentrated in microvughs and microfractures mainly within areas of prehnite alteration of amphibolite. The mineralisation appears to have been deposited under greenschistfacies conditions at lower temperatures than most gold deposits in the Eastern Goldfields. Single-crystal X-ray studies identified the structure of honeaite as orthorhombic, space group Pbcm, and unit cell parameters a = 8.9671(4) Å, b = 8.8758(4) Å, c = 7.8419(5) Å, V = 624.14(6) Å3 (Z = 4). The strongest reflections of the calculated powder X-ray diffraction pattern are [d in Å (I rel)(hkl)]: 2.938(100)(022), 2.905 (39,8)(322, 411), 2.989 (31)(300), 2.833 (23)(310), 1.853 (17)(332). Electron-microprobe analysis (EDS mode) gave (wt%) Au 56.33, Tl 19.68, Te 24.30, total 100.31, leading to an empirical formula (based on 2 Te apfu) of Au3.00Tl1.01Te2.00. Honeaite is black with a metallic lustre and no observed cleavage. The calculated density is 11.18 g/cm3. In reflected plane-polarized light it is slightly bluish grey. Between crossed polars it is weakly anisotropic with dark brown to dark blue rotation tints. Reflectance values in air and in oil are given. Honeaite is named after the late Russell M.     Efficiency of spinal anesthesia versus general anesthesia for lumbar spinal surgery: a retrospective analysis of 544 patients.

    BACKGROUND: Previous studies have shown varying results in selected outcomes when directly comparing spinal anesthesia to general in lumbar surgery. Some studies have shown reduced surgical time, postoperative pain, time in the postanesthesia care unit (PACU), incidence of urinary retention, postoperative nausea, and more favorable cost-effectiveness with spinal anesthesia. Despite these results, the current literature has also shown contradictory results in between-group comparisons. MATERIALS AND METHODS: A retrospective analysis was performed by querying the electronic medical record database for surgeries performed by a single surgeon between 2007 and 2011 using procedural codes 63030 for diskectomy and 63047 for laminectomy: 544 lumbar laminectomy and diskectomy surgeries were identified, with 183 undergoing general anesthesia and 361 undergoing spinal anesthesia (SA). Linear and multivariate regression analyses were performed to identify differences in blood loss, operative time, time from entering the operating room (OR) until incision, time from bandage placement to exiting the OR, total anesthesia time, PACU time, and total hospital stay. Secondary outcomes of interest included incidence of postoperative spinal hematoma and death, incidence of paraparesis, plegia, post-dural puncture headache, and paresthesia, among the SA patients. RESULTS: SA was associated with significantly lower operative time, blood loss, total anesthesia time, time from entering the OR until incision, time from bandage placement until exiting the OR, and total duration of hospital stay, but a longer stay in the PACU. The SA group experienced one spinal hematoma, which was evacuated without any long-term neurological deficits, and neither group experienced a death. The SA group had no episodes of paraparesis or plegia, post-dural puncture headaches, or episodes of persistent postoperative paresthesia or weakness. CONCLUSION: SA is effective for use in patients undergoing elective lumbar laminectomy and/or diskectomy spinal surgery, and was shown to be the more expedient anesthetic choice in the perioperative setting

    Uptake of silver by jarosite and natrojarosite family compounds at 22 °C, 97 °C and 140 °C

Jarosite family minerals are known to incorporate silver (Ag), but the extent to which this occurs, and at what temperature range, is not well constrained. To address this knowledge gap, jarosite compounds with the A site filled with K, Na, Ag and H3O were synthesised at 22 °C, 97 °C and 140 °C to simulate low-, moderate- and high-temperature environments, respectively. The compounds were characterised by XRD, SEM, chemical analysis and Raman spectroscopy. All of the synthesised compounds took up Ag. In general, higher temperatures of synthesis increased alkali and Ag occupancy of the A site of the products. Silver contents increased with the increasing concentration of Ag in the starting solutions at all temperatures. The order of preference for occupancy of the A site in the synthesised solids is K > Na > H3O > Ag at all temperatures, which is consistent with the reported order of ΔGf of −3309 kJ/mol, −3270 kJ/mol, −3247 kJ/mol and −2948 kJ/mol for jarosite, natrojarosite, hydroniumjarosite and argentojarosite, respectively. The results of this study show that Ag can be incorporated in jarosite and natrojarosite at low-to-high temperatures, and therefore, jarosite family minerals can be important stores of Ag in in natural and engineered environments.Birkbeck, University of Londo

    Prevalence of Cardiovascular Disease in Patients With Potentially Curable Malignancies: A National Registry Dataset Analysis.

    BACKGROUND: Although a common challenge for patients and clinicians, there is little population-level evidence on the prevalence of cardiovascular disease (CVD) in individuals diagnosed with potentially curable cancer. OBJECTIVES: We investigated CVD rates in patients with common potentially curable malignancies and evaluated the associations between patient and disease characteristics and CVD prevalence. METHODS: The study included cancer registry patients diagnosed in England with stage I to III breast cancer, stage I to III colon or rectal cancer, stage I to III prostate cancer, stage I to IIIA non-small-cell lung cancer, stage I to IV diffuse large B-cell lymphoma, and stage I to IV Hodgkin lymphoma from 2013 to 2018. Linked hospital records and national CVD databases were used to identify CVD. The rates of CVD were investigated according to tumor type, and associations between patient and disease characteristics and CVD prevalence were determined. RESULTS: Among the 634,240 patients included, 102,834 (16.2%) had prior CVD. Men, older patients, and those living in deprived areas had higher CVD rates. Prevalence was highest for non-small-cell lung cancer (36.1%) and lowest for breast cancer (7.7%). After adjustment for age, sex, the income domain of the Index of Multiple Deprivation, and Charlson comorbidity index, CVD remained higher in other tumor types compared to breast cancer patients. CONCLUSIONS: There is a significant overlap between cancer and CVD burden. It is essential to consider CVD when evaluating national and international treatment patterns and cancer outcomes
