    Smokers’ beliefs about the tobacco control potential of “a gene for smoking”: A focus group study

    BACKGROUND: Several genetic variations associated with nicotine dependence and lung cancer exist. Translating this knowledge into tobacco control policy relies on smokers’ perceptions of the implications of the research. This study explored smokers’ beliefs about the tobacco control uses for research examining genomics, smoking, and addiction. METHOD: Smokers (N = 85) participated in one of thirteen focus groups and one interview, stratified by race (eight black, six white) and education (seven < Bachelor’s degree, seven ≥ Bachelor’s degree). Data were analyzed by two independent coders using standard analysis and validation techniques. RESULTS: Nearly all groups suggested using genetic information for youth-oriented tobacco prevention education. Beliefs about the effectiveness of such actions varied. Many participants believed that providing smokers personalized genetic testing results or informing them about the existence of a gene would not motivate people to quit. All smokers emphasized the need for improved smoking cessation treatment options. Using genomics research to develop gene therapies and personalized drug treatments were also mentioned, yet perceptions of such treatments were mixed. Whereas some participants viewed the possibility positively, others expressed concern about cost and access. Participants who were skeptical of the effectiveness of using genetic information for tobacco control noted that the harms of tobacco use are widely known and genetic information does not add much of a deterrent. CONCLUSION: Participants generated several possible tobacco control uses for genomics research findings. Our findings suggest that tobacco control experts should consult with smokers prior to implementing tobacco control measures. The potential public health benefits of genetics and genomics research related to tobacco use cannot be realized until communication strategies that are most likely to encourage and support tobacco avoidance decisions, and minimize mistrust and backlash, are identified

    Community Peer-Led Exercise Groups: Reasons for Success

    Purpose: This paper considers reasons for the successful maintenance of community based, falls-prevention programs. While the physical achievement of such programs has been demonstrated through randomized trials, other features influential in ongoing membership have received less attention. This study examined the sustainability of a specific model of a community-based program in a New Zealand city: SAYGO, the strength and balance classes for older adults lead by older volunteer leaders recruited from local communities. Method: A qualitative, descriptive approach was used and first-hand knowledge of the experiences of those involved in the groups gathered. Data collection methods included individual interviews of two group organizers and seven focus groups: six with the members of the exercise groups (57 participants) and one with the peer leaders from these same groups (6 participants). Results: Three major themes emerged from the interviews. Two were related to the outcomes of the groups (ie. physical and social benefits). The third was concerned with the support needs of the groups to ensure their on-going maintenance. The aspect that most invigorated the participants was the social value of the group. Conclusions: A major feature that contributed to the sustainability of the peer led exercise groups was the positive social connectedness created by the modeling of a caring culture by the peer-leaders. This caring culture involves support and inclusion of every member and acting as a resource and confidant for individual issues. Because group leaders are similar in age and physical problems, it is expected that they will empathize with participants, and because of their community and agency links, it is expected that they will be able to act as a resource for information on issues related to the participants, therefore, stand between the formal and informal domains and are perceived to have knowledge and connections in both. This, we suggest, is a major, previously unconsidered feature in the sustainability of these groups

    Uniformity Dressed as Diversity? Reorienting female associate professors

    The Norwegian Research Council has, via the research programme Balanse Programmet, funded research as well as practice-oriented initiatives for increasing the representation of women in research leadership positions (Lund, 2020). In this chapter we explore the experience of female associate professors with practice-oriented equality promoting initiatives, specifically that of preliminary evaluations, at one Norwegian higher education institution. We investigate whether, how and with what consequences preliminary evaluations speed up female associate professors’ processes of becoming full professors. To make sense of the effects of this initiative, we use Sara Ahmed’s (2004, 2006) affective economy and Dorothy Smith’s (2005) concept of ‘ruling relations’ to explicate the affects and discourses that these preliminary evaluations hook into, activate, (re)produce and resist. We argue that these reveal a paradox because while they increase the amount of female research leaders they do so at the cost of diverse ways of doing and living academic work.publishedVersio

    In another time and place: The Handmaiden as an adaptation

    This article considers the South Korean auteur director Park Chan-wook’s latest film The Handmaiden, which is the film adaptation of British writer Sarah Waters’s third novel Fingersmith. Transporting the story of love and deception from Victorian England to 1930s Korea under Japanese colonial rule, the film offers a compelling case of transnational or cross-cultural adaptation. In the process of cultural relocation, the film gives prominence to the ethnic identities and hierarchies in colonial Korea, and in recounting the unfolding lesbian love story between a petty-thief-disguised-as-maid and a noble lady, the film provides a spectacular, visual ‘translation’ of the novel’s approach to the story of same-sex desire. Despite all the changes the film makes to the original novel, the author Waters claims that the film is ‘faithful’ to her work. Taking her comments as a framework, the article explores the ways in which the film carries over the transgressive allure of the original story, while addressing the issues of history and identity in another time and place

    Promoting children\u27s social and emotional wellbeing in childcare centers within low socioeconomic areas: strategies, facilitators and challenges

    Althought childcare centres have a vital role to play in the social and emotional development of children, the strategies used to promote children&rsquo;s wellbeing in such settings are not well researched. This study aimed to identify the strategies, facilitators and key challenges for promoting children&rsquo;s social and emotional wellbeing as reported by childcare directors and workers during semi-structured interviews. They reported mainly informal strategies with few formalised policies, curricula or strategies. Staff reported frequent difficulties communicating with parents and/or children due to many families speaking little or no English. Lack of staff training and inadequate resources for activities were other key challenges they identified. Perceived facilitators included staff having strong relationships with each other and sharing a common philosophy, as well as having an open door policy for parents. Systematic development of skills to promote children&rsquo;s social and emotional wellbeing could help leverage childcare staff&rsquo;s potential to promote children&rsquo;s wellbeing during a crucial stage of child development

    UK guideline for the use of HIV post-exposure prophylaxis 2021

    We present the updated British Association for Sexual Health and HIV (BASHH) guidelines for post-exposure prophylaxis (PEP) to HIV following sexual exposures, occupational exposures and other nonoccupational exposures in the community. This serves as an update to the 2015 BASHH guideline on PEP following sexual exposures and the 2008 Expert Advisory Group on AIDS guidelines on HIV PEP. We aim to provide evidence-based guidance on best clinical practice in the provision, monitoring and support of PEP for the prevention of HIV acquisition following sexual, occupational and other nonoccupational exposures in the community. The guideline covers when to prescribe PEP, what antiretroviral agents to use and how to manage PEP. This includes (i) evidence of PEP efficacy; (ii) evidence relating to individual-level efficacy of antiretroviral therapy to prevent the sexual transmission of HIV; (iii) data on the detectable (transmissible) prevalence of HIV in specific populations; (iv) risk of HIV transmission following different types of sexual and occupational exposure; (v) baseline risk assessment; (vi) drug regimens and dosing schedules; (vii) monitoring PEP; (viii) baseline and follow-up blood-borne virus testing; (ix) the role of PEP within broader HIV prevention strategies, for example, HIV pre-exposure prophylaxis (PrEP). The guideline also covers special scenarios such as PEP in pregnancy, breastfeeding and chronic hepatitis B virus infection, and when PEP should be considered in people using HIV PrEP. The guidelines are aimed at clinical professionals directly involved in PEP provision and other stakeholders in the field. A proforma to assist PEP consultations is included. A public consultation process was undertaken prior to finalizing the recommendations
