    Histological and ultrastructural aspects of the tongue in undernourished rats

    There are few published studies on the effects of protein-caloric undernourishment on the oral mucosa. OBJECTIVES: The objective of this study is to verify the histological and ultrastructural aspects of the tongue mucosa in protein-caloric undernourished adult rats. MATERIAL AND METHODS: A clinical experimental study was done in thirty Wistar rats, 15 controls and 15 with protein-caloric undernourishment. The last group received ration in small amounts, with a reduced casein content, during 45 days. Rats were weighed every 3 days, from the first (90 days of life) to the last day of a 45-day dietary period, when they were sacrificed. Plasma was used for protein electrophoresis and their tongues were prepared for light and scanning electron microscopy. Analysus of variance and Student s t-test were used for statistical analysis. RESULTS: A significant decrease in weight and in plasma proteins was found in protein-caloric undernourished rats compared to the control group. Histological findings revealed no differences between the two groups and there were no statistically significant differences in the filiform papilla count under the scanning electron microscopy. CONCLUSION: Protein-caloric undernourishment does not cause alterations in the tongue mucosa of adult rats.Poucos estudos sobre os efeitos da desnutrição protéico-calórica na mucosa oral são encontrados na literatura. OBJETIVOS: O objetivo deste trabalho é verificar os efeitos da desnutrição protéico-calórica na mucosa da língua de ratos adultos, envolvendo microscopia óptica e eletrônica de varredura. MATERIAIS E MÉTODOS: Um estudo experimental foi realizado em trinta ratos Wistar, 15 controles e 15 com desnutrição protéico-calórica. O último grupo recebeu ração em pequena quantidade, com menor conteúdo de caseína, durante 45 dias. Os ratos foram pesados a cada 3 dias, do primeiro (90 dias de vida) ao último dia (45 dias de dieta), quando foram sacrificados. Foi realizada eletroforese de proteínas plasmáticas e as línguas foram preparadas para microscopia óptica comum e eletrônica. Para análise estatística utilizou-se análise de variância e teste T de Student. RESULTADOS: Foi observada uma significante diminuição no peso e nas proteínas plasmáticas dos ratos com desnutrição protéico-calórica em relação ao grupo controle. A análise histológica não mostrou diferenças entre os dois grupos, e os resultados, com respeito à contagem das papilas filiformes da mucosa lingual pela microscopia eletrônica, não revelou diferenças estatisticamente significantes entre os grupos.     Troglitazone Impedes the Oligomerization of Sodium Taurocholate Cotransporting Polypeptide and Entry of Hepatitis B Virus Into Hepatocytes

    Current anti-hepatitis B virus (HBV) agents, which include nucleos(t)ide analogs and interferons, can significantly suppress HBV infection. However, there are limitations in the therapeutic efficacy of these agents, indicating the need to develop anti-HBV agents with different modes of action. In this study, through a functional cell-based chemical screening, we found that a thiazolidinedione, troglitazone, inhibits HBV infection independently of the compound's ligand activity for peroxisome proliferator-activated receptor γ (PPARγ). Analog analysis suggested chemical moiety required for the anti-HBV activity and identified ciglitazone as an analog having higher anti-HBV potency. Whereas, most of the reported HBV entry inhibitors target viral attachment to the cell surface, troglitazone blocked a process subsequent to viral attachment, i.e., internalization of HBV preS1 and its receptor, sodium taurocholate cotransporting polypeptide (NTCP). We also found that NTCP was markedly oligomerized in the presence of HBV preS1, but such NTCP oligomerization was abrogated by treatment with troglitazone, but not with pioglitazone, correlating with inhibition activity to viral internalization. Also, competitive peptides that blocked NTCP oligomerization impeded viral internalization and infection. This work represents the first report identifying small molecules and peptides that specifically inhibit the internalization of HBV. This study is also significant in proposing a possible role for NTCP oligomerization in viral entry, which will shed a light on a new aspect of the cellular mechanisms regulating HBV infection

    Cyclosporin A Associated Helicase-Like Protein Facilitates the Association of Hepatitis C Virus RNA Polymerase with Its Cellular Cyclophilin B

    BACKGROUND: Cyclosporin A (CsA) is well known as an immunosuppressive drug useful for allogeneic transplantation. It has been reported that CsA inhibits hepatitis C virus (HCV) genome replication, which indicates that cellular targets of CsA regulate the viral replication. However, the regulation mechanisms of HCV replication governed by CsA target proteins have not been fully understood. PRINCIPAL FINDINGS: Here we show a chemical biology approach that elucidates a novel mechanism of HCV replication. We developed a phage display screening to investigate compound-peptide interaction and identified a novel cellular target molecule of CsA. This protein, named CsA associated helicase-like protein (CAHL), possessed RNA-dependent ATPase activity that was negated by treatment with CsA. The downregulation of CAHL in the cells resulted in a decrease of HCV genome replication. CAHL formed a complex with HCV-derived RNA polymerase NS5B and host-derived cyclophilin B (CyPB), known as a cellular cofactor for HCV replication, to regulate NS5B-CyPB interaction. CONCLUSIONS: We found a cellular factor, CAHL, as CsA associated helicase-like protein, which would form trimer complex with CyPB and NS5B of HCV. The strategy using a chemical compound and identifying its target molecule by our phage display analysis is useful to reveal a novel mechanism underlying cellular and viral physiology

    COVID-19 presenting as persistent hiccups: a case report

    Hiccups are involuntary, spasmodic contractions of the diaphragm and intercostal muscles and can be classified as acute (< 48 h), persistent (48 h to a month) or intractable (> 1 month). A previously healthy 29-year-old man sought the Emergency Department with flu-like symptoms and a two-day history of persistent hiccups. His physical examination was otherwise unremarkable and vital signs were within normal limits. An unenhanced computed tomography scan of the chest showed small focal ground-glass opacities scattered throughout 25% of the lungs. A COVID-19 test was positive. Chlorpromazine was prescribed for the hiccups with improvement over 10 h. The patient was discharged home on the same day without hiccups and no other complications

    Inhibition of preS1-hepatocyte interaction by an array of recombinant human antibodies from naturally recovered individuals

    Neutralizing monoclonal antibodies are being found to be increasingly useful in viral infections. In hepatitis B infection, antibodies are proven to be useful for passive prophylaxis. The preS1 region (21–47a.a.) of HBV contains the viral hepatocyte-binding domain crucial for its attachment and infection of hepatocytes. Antibodies against this region are neutralizing and are best suited for immune-based neutralization of HBV, especially in view of their not recognizing decoy particles. Anti-preS1 (21–47a.a.) antibodies are present in serum of spontaneously recovered individuals. We generated a phage-displayed scFv library using circulating lymphocytes from these individuals and selected four preS1-peptide specific scFvs with markedly distinct sequences from this library. All the antibodies recognized the blood-derived and recombinant preS1 containing antigens. Each scFv showed a discrete binding signature, interacting with different amino acids within the preS1-peptide region. Ability to prevent binding of the preS1 protein (N-terminus 60a.a.) to HepG2 cells stably expressing hNTCP (HepG2-hNTCP-C4 cells), the HBV receptor on human hepatocytes was taken as a surrogate marker for neutralizing capacity. These antibodies inhibited preS1-hepatocyte interaction individually and even better in combination. Such a combination of potentially neutralizing recombinant antibodies with defined specificities could be used for preventing/managing HBV infections, including those by possible escape mutants

    Revisiting the guidelines for ending isolation for COVID-19 patients

    新型コロナウイルス感染者隔離を終了するのはいつが良い?. 京都大学プレスリリース. 2021-07-27.Reducing COVID-19 isolation times. 京都大学プレスリリース. 2021-07-27.Since the start of the COVID-19 pandemic, two mainstream guidelines for defining when to end the isolation of SARS-CoV-2-infected individuals have been in use: the one-size-fits-all approach (i.e. patients are isolated for a fixed number of days) and the personalized approach (i.e. based on repeated testing of isolated patients). We use a mathematical framework to model within-host viral dynamics and test different criteria for ending isolation. By considering a fixed time of 10 days since symptom onset as the criterion for ending isolation, we estimated that the risk of releasing an individual who is still infectious is low (0–6.6%). However, this policy entails lengthy unnecessary isolations (4.8–8.3 days). In contrast, by using a personalized strategy, similar low risks can be reached with shorter prolonged isolations. The obtained findings provide a scientific rationale for policies on ending the isolation of SARS-CoV-2-infected individuals

    Actinic cheilitis adjacent to squamous carcinoma of the lips as an indicator of prognosis

    Many studies have shown an association between actinic cheilitis and squamous carcinoma of the lips. AIM: The aim of the study was to observe the relation between actinic cheilitis and the prognosis of squamous carcinoma of the lips. MATERIALS AND METHODS This is a retrospective cross-sectional cohort study of squamous carcinoma of the lips. Histological sections of squamous carcinoma tumors done at the the Departament of Pathology of the São Paulo Federal University between 1993 and 2000 were reviewed for evidence of actinic cheilitis in the lip vermillion adjacent to the tumor. Patient reports were reviewed to find information about exposure to sun, metastases and relapses. The occurrence or absence of relapses and metastases was correlated with the presence or absence of actinic cheilitis in the lip vermillion. Data was analyzed by Fisher s Exact test. RESULTS: Of the 31 selected patients, most were caucasian, males and with lower lip involvement. Statistical analysis demonstrated independence between the occurrence of metastases and relapse and gender, skin color and site (lower or upper lips). There was dependence between actinic cheilitis and solar elastosis, and between the absence of actinic cheilitis and the occurrence of metastases. There was no dependence between the absence of actinic cheilitis and the occurrence of relapses. CONCLUSION: It may be concluded that tumors originating from actinic cheilitis have a better prognosis.Muitos estudos demonstram associação entre queilite actínica e carcinoma espinocelular do lábio. OBJETIVO: Verificar a relação da queilite actínica com o prognóstico dessa neoplasia. MATERIAIS E MÉTODOS: Elaborou-se um estudo de coorte retrospectivo com corte transversal em carcinoma espinocelular do lábio. Cortes histológicos desse tumor, levantados entre 1993-2000, nos arquivos do Departamento de Patologia/Universidade Federal de São Paulo (UNIFESP), foram revisados para evidenciar presença ou ausência de queilite actínica no vermelhão adjacente ao tumor. Os prontuários dos pacientes foram revisados à procura de informações sobre exposição solar, metástase e recidiva. A ocorrência ou ausência de recidiva e metástase foi correlacionada com a presença ou ausência de queilite actínica no vermelhão. Os dados obtidos foram analisados pelo teste exato de Fisher. RESULTADOS: Dos 31 pacientes selecionados predominou o sexo masculino, cor da pele branca e localização no lábio inferior. Constatou-se: independência entre a ocorrência de metástase e recidiva com sexo, cor dos pacientes e localizações no lábio superior ou inferior; dependência entre a presença de queilite actínica e elastose solar, dependência entre a ausência de queilite actínica e presença de metástase; independência entre a ausência de queilite actínica e presença de recidiva.     Squamous cell carcinoma of the lip: assessment of prognostic factors

    Among lip cancers, 90% to 95% of them affect the lower lip and squamous cell carcinoma is the most frequent type. The TNM classification synthesizes the clinical characteristics of the tumor that allows prognosis and makes possible the comparisons of the results. Three parameters have to be considered: size of the tumor (T), propagation to regional lymphatic ganglion (N) and metastasis (M); however, patterns starting from 2cm are established by the TNM classification. In the case of squamous cell carcinoma of the lips, 2cm lesions are considered extremely large. AIM: The objective of this study was to verify the correlation among epidemiologic, clinical, evolutionary and histopathological characteristics of squamous cell carcinoma of the lips, having as parameter lesions as small as 0.5cm. MATERIAL AND METHODS: In the period 1993-2000 in São Paulo, Brazil, a transversal retrospective study was performed with patients exhibiting squamous cell carcinoma of the lips. The characteristics of the tumor were investigated through the analysis of patients' medical charts, original reports of the histopathological exams and tumors' samples. The tumors were classified from 0.5 to 2.5cm. In addition, type, grade of histologic differentiation, the presence of desmoplasia, muscular, neural and vascular infiltration, and type of inflammatory infiltrate were investigated. RESULTS: The statistical analyses indicated that metastasis and recurrences do not depend on patients' gender and race. The independence of the tumor's localization, either in the upper or lower lip, and the incidence of metastasis and recurrence were demonstrated. A correlation between lesions as large as 0.5cm and the occurrence of metastasis and recurrence was verified. Furthermore, it was observed that the size of lesion determines the infiltration in other tissues. Lymphoplasmocytarian is the type of inflammatory infiltrate that was found in each and every lesion. On the other hand, in some of the lesions, the inflammatory infiltrate was associated with eosinophils without correlation to size of tumor. CONCLUSION: Smaller tumors than 2cm, from a histopathological and clinical point of view, may present a distinct evolution behavior. Most of the lesions are ulcerative, though the ulcerative-vegetative is the one that presents the most metastasis and recurrence. The ulcerative-vegetative and vegetative types are associated with the largest lesions. The size of the tumor is related, in a similar way, to grades II and III, in which higher rates of metastasis and recurrence were observed. In decreasing order of frequency, the tumor invades muscles, nerves and blood vessels, which can be related to the size of the lesion. Thus, in order to have metastasis, it is necessary to have infiltration of muscles. Yet, blood vessels can only be involved when there is concomitant infiltration of nerves. As a result, desmoplasia is directly related to size of the lesion as well as to occurrence of metastasis.Dentre os cânceres do lábio de 90% a 95% dos casos afetam o lábio inferior, sendo o carcinoma espinocelular o mais freqüente. A classificação TNM sintetiza as características clínicas do tumor, permitindo realizar um prognóstico e possibilitando comparações dos resultados. Relaciona três parâmetros: tamanho do tumor (T), propagação aos gânglios linfáticos regionais (N) e metástases à distância (M), mas estabelece padrões a partir de 2cm. Para o carcinoma espinocelular do lábio lesões com 2cm são extremamente grandes. OBJETIVO: O objetivo deste estudo é verificar a relação entre as características epidemiológicas, clínicas, evolutivas e histopatológicas do carcinoma espinocelular do lábio tendo como parâmetro lesões de tamanhos a partir de 0,5cm. CASUÍSTICA E MÉTODO: Foi elaborado um estudo retrospectivo transversal em pacientes com carcinoma espinocelular do lábio, no período 1993-2000, em São Paulo, Brasil. Estudou-se prontuários, laudos originais dos exames histopatológicos e lâminas de tumores de pacientes com carcinoma espinocelular do lábio. Os tumores foram classificados de 0.5 em 0.5cm, sendo verificado o tipo, o grau de diferenciação histológica, a presença de desmoplasia, as invasões muscular, neural e vascular, e o tipo de infiltrado inflamatório. RESULTADOS: A análise estatística mostrou que metástases e recidivas não dependem da cor de pele ou do sexo dos pacientes e que há independência entre a localização do tumor, no lábio superior ou inferior, e a incidência de metástases e recidiva. Houve correlação entre o tamanho da lesão a partir de 0,5cm e a ocorrência de metástases e recidiva. Verificou-se que o tamanho da lesão determina a invasão em outros tecidos. O infiltrado inflamatório verificado em todas as lesões era linfoplasmocitário e, em algumas, associado com eosinófilos sem relação com o tamanho do tumor. CONCLUSÃO: Tumores menores que 2cm podem apresentar comportamentos evolutivos distintos, sob o ponto de vista clínico e histopatológico. O tipo mais prevalente de lesão é o ulcerativo e o que mais metastatiza e recidiva é o úlcero-vegetante. Os tipos úlcero-vegetante e vegetante estão ligados a lesões de maior tamanho. O tamanho do tumor se relaciona, de forma semelhante, com os graus II e III, nos quais ocorrem os maiores índices de metástases e recidivas. O tumor invade em ordem decrescente de freqüência músculos, nervos e vasos sanguíneos, e esta pode ser prevista pelo tamanho da lesão. É necessária a invasão dos músculos para a ocorrência de metástases, sendo que os vasos sanguíneos somente podem estar implicados quando há invasão concomitante dos nervos.     An ACAT inhibitor suppresses SARS-CoV-2 replication and boosts antiviral T cell activity

    The severity of disease following infection with SARS-CoV-2 is determined by viral replication kinetics and host immunity, with early T cell responses and/or suppression of viraemia driving a favourable outcome. Recent studies uncovered a role for cholesterol metabolism in the SARS-CoV-2 life cycle and in T cell function. Here we show that blockade of the enzyme Acyl-CoA:cholesterol acyltransferase (ACAT) with Avasimibe inhibits SARS-CoV-2 pseudoparticle infection and disrupts the association of ACE2 and GM1 lipid rafts on the cell membrane, perturbing viral attachment. Imaging SARS-CoV-2 RNAs at the single cell level using a viral replicon model identifies the capacity of Avasimibe to limit the establishment of replication complexes required for RNA replication. Genetic studies to transiently silence or overexpress ACAT isoforms confirmed a role for ACAT in SARS-CoV-2 infection. Furthermore, Avasimibe boosts the expansion of functional SARS-CoV-2-specific T cells from the blood of patients sampled during the acute phase of infection. Thus, re-purposing of ACAT inhibitors provides a compelling therapeutic strategy for the treatment of COVID-19 to achieve both antiviral and immunomodulatory effects. Trial registration: NCT04318314
