    The relationship between the piriformis muscle, low back pain, lower limb injuries and motor control training among elite football players

    Objectives: Australian Football League (AFL) players have a high incidence of back injuries. Motor control training to increase lumbopelvic neuromuscular control has been effective in reducing low back pain (LBP) and lower limb injuries in elite athletes. Control of pelvic and femoral alignment during functional activity involves the piriformis muscle. This study investigated (a) the effect of motor control training on piriformis muscle size in AFL players, with and without LBP, during the playing season, and (b) whether there is a relationship between lower limb injury and piriformis muscle size. Design: Stepped-wedge intervention. Methods: 46 AFL players participated in a motor control training programme consisting of two 30. min sessions per week over 7-8 weeks, delivered across the season as a randomised 3 group single-blinded stepped-wedge design. Assessment of piriformis muscle cross-sectional area (CSA) involved magnetic resonance imaging (MRI) at 3 time points during the season. Assessment of LBP consisted of player interview and physical examination. Injury data were obtained from club records. Results: An interaction effect for Time, Intervention Group and LBP group (F=3.7, p=0.03) was found. Piriformis muscle CSA showed significant increases between Times 1 and 2 (F=4.24, p=0.046), and Times 2 and 3 (F=8.59, p=0.006). Players with a smaller increase in piriformis muscle CSA across the season had higher odds of sustaining an injury (OR. =1.08). Conclusions: Piriformis muscle size increases across the season in elite AFL players and is affected by the presence of LBP and lower limb injury. Motor control training positively affects piriformis muscle size in players with LBP

    Application of Spatial Data Modeling and Geographical Information Systems (GIS) for Identification of Potential Siting Options for Various Electrical Generation Sources

    Oak Ridge National Laboratory (ORNL) initiated an internal National Electric Generation Siting Study, which is an ongoing multiphase study addressing several key questions related to our national electrical energy supply. This effort has led to the development of a tool, OR-SAGE (Oak Ridge Siting Analysis for power Generation Expansion), to support siting evaluations. The objective in developing OR-SAGE was to use industry-accepted approaches and/or develop appropriate criteria for screening sites and employ an array of Geographic Information Systems (GIS) data sources at ORNL to identify candidate areas for a power generation technology application. The initial phase of the study examined nuclear power generation. These early nuclear phase results were shared with staff from the Electric Power Research Institute (EPRI), which formed the genesis and support for an expansion of the work to several other power generation forms, including advanced coal with carbon capture and storage (CCS), solar, and compressed air energy storage (CAES). Wind generation was not included in this scope of work for EPRI. The OR-SAGE tool is essentially a dynamic visualization database. The results shown in this report represent a single static set of results using a specific set of input parameters. In this case, the GIS input parameters were optimized to support an economic study conducted by EPRI. A single set of individual results should not be construed as an ultimate energy solution, since US energy policy is very complex. However, the strength of the OR-SAGE tool is that numerous alternative scenarios can be quickly generated to provide additional insight into electrical generation or other GIS-based applications. The screening process divides the contiguous United States into 100 x 100 m (1-hectare) squares (cells), applying successive power generation-appropriate site selection and evaluation criteria (SSEC) to each cell. There are just under 700 million cells representing the contiguous United States. If a cell meets the requirements of each criterion, the cell is deemed a candidate area for siting a specific power generation form relative to a reference plant for that power type. Some SSEC parameters preclude siting a power plant because of an environmental, regulatory, or land-use constraint. Other SSEC assist in identifying less favorable areas, such as proximity to hazardous operations. All of the selected SSEC tend to recommend against sites. The focus of the ORNL electrical generation source siting study is on identifying candidate areas from which potential sites might be selected, stopping short of performing any detailed site evaluations or comparisons. This approach is designed to quickly screen for and characterize candidate areas. Critical assumptions supporting this work include the supply of cooling water to thermoelectric power generation; a methodology to provide an adequate siting footprint for typical power plant applications; a methodology to estimate thermoelectric plant capacity while accounting for available cooling water; and a methodology to account for future ({approx}2035) siting limitations as population increases and demands on freshwater sources change. OR-SAGE algorithms were built to account for these critical assumptions. Stream flow is the primary thermoelectric plant cooling source evaluated in this study. All cooling was assumed to be provided by a closed-cycle cooling (CCC) system requiring makeup water to account for evaporation and blowdown. Limited evaluations of shoreline cooling and the use of municipal processed water (gray) cooling were performed. Using a representative set of SSEC as input to the OR-SAGE tool and employing the accompanying critical assumptions, independent results for the various power generation sources studied were calculated

    Genomic mining of prokaryotic repressors for orthogonal logic gates

    Genetic circuits perform computational operations based on interactions between freely diffusing molecules within a cell. When transcription factors are combined to build a circuit, unintended interactions can disrupt its function. Here, we apply 'part mining' to build a library of 73 TetR-family repressors gleaned from prokaryotic genomes. The operators of a subset were determined using an in vitro method, and this information was used to build synthetic promoters. The promoters and repressors were screened for cross-reactions. Of these, 16 were identified that both strongly repress their cognate promoter (5- to 207-fold) and exhibit minimal interactions with other promoters. Each repressor-promoter pair was converted to a NOT gate and characterized. Used as a set of 16 NOT/NOR gates, there are >10[superscript 54] circuits that could be built by changing the pattern of input and output promoters. This represents a large set of compatible gates that can be used to construct user-defined circuits.United States.     Non-Adherence in Patients on Peritoneal Dialysis: A Systematic Review

    Background: It has been increasingly recognized that non-adherence is an important factor that determines the outcome of peritoneal dialysis (PD) therapy. There is therefore a need to establish the levels of non-adherence to different aspects of the PD regimen (dialysis procedures, medications, and dietary/fluid restrictions). Methods: A systematic review of peer-reviewed literature was performed in PubMed, PsycINFO and CINAHL databases using PRISMA guidelines in May 2013. Publications on non-adherence in PD were selected by two reviewers independently according to predefined inclusion and exclusion criteria. Relevant data on patient characteristics, measures, rates and factors associated with non-adherence were extracted. The quality of studies was also evaluated independently by two reviewers according to a revised version of the Effective Public Health Practice Project assessment tool. Results: The search retrieved 204 studies, of which a total of 25 studies met inclusion criteria. Reported rates of nonadherence varied across studies: 2.6 1353% for dialysis exchanges, 3.9 1385% for medication, and 14.4 1367% for diet/fluid restrictions. Methodological differences in measurement and definition of non-adherence underlie the observed variation. Factors associated with non-adherence that showed a degree of consistency were mostly socio-demographical, such as age, employment status, ethnicity, sex, and time period on PD treatment. Conclusion: Non-adherence to different dimensions of the dialysis regimen appears to be prevalent in PD patients. There is a need for further, high-quality research to explore these factors in more detail, with the aim of informing intervention designs to facilitate adherence in this patient populatio

    Factors contributing to posttraumatic growth and its buffering effect in adult chidren of cancer patients undergoing treatment

    This study examined relationships among demographic, clinical, and psychosocial variables in adult children of cancer patients. Two hundred and fourteen participants completed measures of posttraumatic growth (PTG), distress, posttraumatic stress disorder (PTSD) symptoms, social support, and family functioning. Significant gender differences in all PTG dimensions were found, as well as associations among PTG, gender, parental dependency, distress, PTSD, and family functioning. Social support was not a mediator in the relationship between gender and PTG. Gender, education, disease duration, dependency, distress, and family flexibility predicted PTG. Finally, PTG had amoderating effect in the relationship between distress and PTSD/social support.     Meta-analysis of genome-wide association studies of HDL cholesterol response to statins

    BACKGROUND: In addition to lowering low density lipoprotein cholesterol (LDL-C), statin therapy also raises high density lipoprotein cholesterol (HDL-C) levels. Inter-individual variation in HDL-C response to statins may be partially explained by genetic variation.METHODS AND RESULTS: We performed a meta-analysis of genome-wide association studies (GWAS) to identify variants with an effect on statin-induced high density lipoprotein cholesterol (HDL-C) changes. The 123 most promising signals with p&lt;1×10(-4) from the 16 769 statin-treated participants in the first analysis stage were followed up in an independent group of 10 951 statin-treated individuals, providing a total sample size of 27 720 individuals. The only associations of genome-wide significance (p&lt;5×10(-8)) were between minor alleles at the CETP locus and greater HDL-C response to statin treatment.CONCLUSIONS: Based on results from this study that included a relatively large sample size, we suggest that CETP may be the only detectable locus with common genetic variants that influence HDL-C response to statins substantially in individuals of European descent. Although CETP is known to be associated with HDL-C, we provide evidence that this pharmacogenetic effect is independent of its association with baseline HDL-C levels.</p
