    Multimorbidity: constellations of conditions across subgroups of midlife and older individuals, and related Medicare expenditures

    Introduction: The Department of Health and Human Services’ 2010 Strategic Framework on Multiple Chronic Conditions called for the identification of common constellations of conditions in older adults. Objectives: To analyze patterns of conditions constituting multimorbidity (CCMM) and expenditures in a US representative sample of midlife and older adults (50–64 and ≥65 years of age, respectively). Design: A cross-sectional study of the 2010 Health and Retirement Study (HRS; n=17,912). The following measures were used: (1) count and combinations of CCMM, including (i) chronic conditions (hypertension, arthritis, heart disease, lung disease, stroke, diabetes, cancer, and psychiatric conditions), (ii) functional limitations (upper body limitations, lower body limitations, strength limitations, limitations in activities of daily living, and limitations in instrumental activities of daily living), and (iii) geriatric syndromes (cognitive impairment, depressive symptoms, incontinence, visual impairment, hearing impairment, severe pain, and dizziness); and (2) annualized 2011 Medicare expenditures for HRS participants who were Medicare fee-for-service beneficiaries (n=5,677). Medicaid beneficiaries were also identified based on their self-reported insurance status. Results: No large representations of participants within specific CCMM categories were observed; however, functional limitations and geriatric syndromes were prominently present with higher CCMM counts. Among fee-for-service Medicare beneficiaries aged 50–64 years, 26.7% of the participants presented with ≥10 CCMM, but incurred 48% of the expenditure. In those aged ≥65 years, these percentages were 16.9% and 34.4%, respectively. Conclusion: Functional limitations and geriatric syndromes considerably add to the MM burden in midlife and older adults. This burden is much higher than previously reported

    Quantification of hepatic steatosis with 3-T MR imaging: Validation in ob/ob mice

    Purpose: To validate quantitative imaging techniques used to detect and measure steatosis with magnetic resonance (MR) imaging in an ob/ob mouse model of hepatic steatosis. Materials and Methods: The internal research animal and resource center approved this study. Twenty-eight male ob/ob mice in progressively increasing age groups underwent imaging and were subsequently sacrificed. Six ob /+ mice served as control animals. Fat fraction imaging was performed with a chemical shift-based water-fat separation method. The following three methods of conventional fat quantification were compared with imaging: lipid extraction and qualitative and quantitative histologic analysis. Fat fraction images were reconstructed with single- and multiple-peak spectral models of fat and with and without correction for T2* effects. Fat fraction measurements obtained with the different reconstruction methods were compared with the three methods of fat quantification, and linear regression analysis and two-sided and two-sample t tests were performed. Results:Lipid extraction and qualitative and quantitative histologic analysis were highly correlated with the results of fat fraction imaging (r2 = 0.92, 0.87, 0.82, respectively). No significant differences were found between imaging measurements and lipid extraction (P = .06) or quantitative histologic (P = .07) measurements when multiple peaks of fat and T2* correction were included in image reconstruction. Reconstructions in which T2* correction, accurate spectral modeling, or both were excluded yielded lower agreement when compared with the results yielded by other techniques. Imaging measurements correlated particularly well with histologic grades in mice with low fat fractions (intercept, -1.0% ± 1.2 [standard deviation ]). Conclusion: MR imaging can be used to accurately quantify fat in vivo in an animal model of hepatic steatosis and may serve as a quantitative biomarker of hepatic steatosis. © RSNA, 2010

    Complex multimorbidity and health outcomes in older adult cancer survivors

    Objective: To characterize complex multimorbidity among cancer survivors and evaluate the association between cancer survivorship, time since cancer diagnosis, and self-reported fair/poor health, self-rated worse health in 2 years, and 2-year mortality. Methods: We used the 2010–2012 Health and Retirement Study. Cancer survivors were individuals who reported a (nonskin) cancer diagnosis 2 years or more before the interview. We defined complex multimorbidity as the co-occurrence of chronic conditions, functional limitations, and/or geriatric syndromes. In addition to descriptive analyses, we used logistic regression to evaluate the independent association between cancer survivor status and health outcomes. We also examined whether cancer survivorship differed by the number of years since diagnosis. Results: Among 15,808 older adults (age ≥50 years), 11.8% were cancer survivors. Compared with cancer-free individuals, a greater percentage of cancer survivors had complex multimorbidity: co-occurring chronic conditions, functional limitations, and geriatric syndromes. Cancer survivorship was significantly associated with self-reported fair/poor health, self-rated worse health in 2 years, and 2-year mortality. These effects declined with the number of years since diagnosis for fair/ poor health and mortality but not for self-rated worse health. Conclusion: Cancer survivor status is independently associated with more complex multimorbidity, and with worse health outcomes. These effects attenuate with time, except for patient perception of being in worse health

    The optical counterpart to gamma-ray burst GRB970228 observed using the Hubble Space Telescope

    Although more than 2,000 astronomical gamma-ray bursts (GRBs) have been detected, and numerous models proposed to explain their occurrence, they have remained enigmatic owing to the lack of an obvious counterpart at other wavelengths. The recent ground-based detection of a transient source in the vicinity of GRB 970228 may therefore have provided a breakthrough. The optical counterpart appears to be embedded in an extended source which, if a galaxy as has been suggested, would lend weight to those models that place GRBs at cosmological distances. Here we report the observations using the Hubble Space Telescope of the transient counterpart and extended source 26 and 39 days after the initial gamma-ray outburst. We find that the counterpart has faded since the initial detection (and continues to fade), but the extended source exhibits no significant change in brightness between the two dates of observations reported here. The size and apparent constancy between the two epochs of HST observations imply that it is extragalactic, but its faintness makes a definitive statement about its nature difficult. Nevertheless, the decay profile of the transient source is consistent with a popular impulsive-fireball model, which assumes a merger between two neutron stars in a distant galaxy.Comment: 11 pages + 2 figures.     Linguistic Factors Affecting Moraic Duration in Spontaneous Japanese

    Japanese is often referred to as a mora-timed language (Ladefoged 1975): the mora has been described as the psychological prosodic unit in the spoken language, and it is the metric unit of traditional poetry (Bloch 1950). However, it is clear that mo- rae are not strictly isochronous units (Beckman 1982). Thus, experimental studies have focused on detecting compensation effects that make average mora durations more equal through the modulation of the inherent duration of the segments involved (Han 1962; Port, Al-Ani, Maeda 1980; Homma 1981; Hoequist 1983a; 1983b; Warner, Arai 2001). Kawahara (2017) used the Corpus of Spontaneous Japanese to verify whether the dura- tional compensation effect within a /CV/ mora occurs in natural speech, in addition to read speech in the lab. He observed a statistically significant compensation effect of /CV/ morae, in which vowel duration tends to vary in response to the duration of the preced- ing consonant. However, as the same author has pointed out, the compensation is not absolute because there are several linguistic factors that potentially affect segments’ duration profiles. This study will support the idea that moraic isochrony does not occur in spontaneous Japanese by presenting empirical data on how linguistic factors can considerably affect variation in the average duration of morae

    Why Are Regulations Changed? A Parcel Analysis of Upzoning in Los Angeles

    Planners, officials, and neighborhood groups often debate zoning changes, yet there is little empirical evidence explaining why zoning and other land use regulations are changed. I use logistic regression models to examine density-enabling rezoning (“upzoning”) in Los Angeles. I find that upzoning occurs where there are development opportunities combined with limited political resistance. Upzoning is most likely on well-located parcels zoned for low-intensity, nonresidential uses. Meanwhile, homeowners—and particularly homeowners with access to valuable amenities—are associated with regulatory stasis. I conclude by recommending strategies for addressing homeowners’ concerns about higher density housing

    Whole Earth Telescope Observations of the Helium Interacting Binary PG 1346+082 (CR Bootis)

    We present our analysis of 240 hr of white-light, high-speed photometry of the dwarf nova-like helium variable PG 1346+082 (CR Boo). We identify two frequencies in the low-state power spectrum, at 679.670 ± 0.004 μHz and 669.887 ± 0.008 μHz. The 679.670 μHz variation is coherent over at least a 2 week time span, the first demonstration of a phase-coherent photometric variation in any dwarf nova-like interacting binary white dwarf system. The high-state power spectrum contains a complex fundamental with a frequency similar, but not identical, to the low-state spectrum, and a series of harmonics not detected in low state. We also uncover an unexpected dependence of the high-frequency power\u27s amplitude and frequency structure on overall system magnitude. We discuss these findings in light of the general AM CVn system model, particularly the implications of the high-order harmonics on future studies of disk structure, mass transfer, and disk viscosity

    Stabilization of mid-sized silicon nanoparticles by functionalization with acrylic acid

    We present an enhanced method to form stable dispersions of medium-sized silicon nanoparticles for solar cell applications by thermally induced grafting of acrylic acid to the nanoparticle surface. In order to confirm their covalent attachment on the silicon nanoparticles and to assess the quality of the functionalization, X-ray photoelectron spectroscopy and diffuse reflectance infrared Fourier spectroscopy measurements were carried out. The stability of the dispersion was elucidated by dynamic light scattering and Zeta-potential measurements, showing no sign of degradation for months
