Heart failure patients from hospital settings in Poland: Population characteristics and treatment patterns, a multicenter retrospective study
Background: Despite physicians’ increasing knowledge regarding heart failure (HF),
a significant percentage of patients still do not receive adequate treatment. The aim of this
multicentre, retrospective descriptive study was to reveal the pharmacotherapy patterns in HF
patients hospitalized in cardiology (CARD) and internal medicine (INT) wards in Poland.
Methods: Included into the study were 800 consecutive patients who were admitted to the
hospitals: 350 patients from 7 CARD wards and 450 patients from 9 INT wards.
Results: The average age in the study group was about 70 years (youngest participant under
40 and oldest at 95 years of age). Decompensation of HF or acute HF were the most frequent
causes of hospitalization (in both groups > 50%). The main etiology was coronary artery
disease, either alone or together with hypertension (from about 60% in INT patients to about
78% in CARD patients, p = NS). With regard to pharmacotherapy, angiotensin converting
enzyme inhibitors were used in 81% of cases (77% CARD and 83% INT; p = 0.05); out of this
group, doses were at optimal or larger in 39.3% of patients (38% CARD patients and 39.4%
INT patients). Beta-blockers were administered in 31.4% and 19.1% of patients from the
CARD and INT groups respectively (p < 0.0001), but optimal dosing was negligible.
Conclusions: Compared with an assessment conducted several years ago, the past education
initiatives have significantly improved the quality of standard-based HF treatment. However,
suboptimal dosing and the use of drugs that do not improve prognosis remain an unresolved

Chorzy z niewydolnością serca leczeni w warunkach szpitalnych w Polsce: charakterystyka oraz metody leczenia, retrospektywna analiza wieloośrodkowa
Wstęp: Pomimo wzrastającej wiedzy lekarzy dotyczącej niewydolności serca (HF) znaczący
odsetek chorych nadal nie jest właściwie leczony. Celem niniejszego wieloośrodkowego, retrospektywnego
badania opisowego była ocena metod farmakoterapii stosowanych u chorych
z HF, hospitalizowanych na oddziałach kardiologicznych oraz chorób wewnętrznych w Polsce.
Metody: Do badania włączono kolejnych 800 chorych przyjętych do szpitali: 350 pacjentów
z 7 oddziałów kardiologicznych i 450 pacjentów z 9 oddziałów chorób wewnętrznych.
Wyniki: Średnia wieku w opisywanej grupie wynosiła około 70 lat (najmłodszy chory miał
mniej niż 40 lat, najstarszy > 95 lat). Najczęstszymi przyczynami hospitalizacji były dekompensacja
HF oraz ostra HF (w obu grupach > 50%). Najczęstszą etiologię stanowiła choroba
wieńcowa, zarówno izolowana, jak i współwystępująca z nadciśnieniem tętniczym (od ok. 60%
w grupie pacjentów z oddziałów chorób wewnętrznych do ok. 78% chorych z oddziałów kardiologicznych,
p = NS). W ramach farmakoterapii inhibitory receptora angiotensyny stosowano
u 81% chorych (77% osób z oddziałów kardiologicznych i 83% pacjentów z oddziałów chorób
wewnętrznych, p = 0,05); wśród tych pacjentów stosowane dawki leków były zgodne z zaleceniami
lub większe u 39,3% chorych (38% osób z oddziałów kardiologicznych i 39,4% pacjent
ów z oddziałów chorób wewnętrznych). Beta-adrenolityki stosowano u 31,4% i 19,1% osób,
odpowiednio na oddziałach kardiologicznych i wewnętrznych (p < 0,0001), jednak nie przestrzegano
zalecanych dawek. Wnioski: W porównaniu z oceną sprzed kilku lat zaobserwowano znaczącą poprawę jakości
leczenia chorych z HF zgodnie ze standardami, prawdopodobnie w następstwie przeprowadzenia
szkoleń edukacyjnych dla lekarzy. Problemem pozostaje jednak stosowanie zbyt małych
dawek leków oraz preparatów, które nie wpływają na rokowanie

Efficient calculation of two-dimensional periodic and waveguide acoustic Green’s functions
New representations and efficient calculation methods are derived for the problem of propagation from an infinite regularly spaced array of coherent line sources above a homogeneous impedance plane, and for the Green's function for sound propagation in the canyon formed by two infinitely high, parallel rigid or sound soft walls and an impedance ground surface. The infinite sum of source contributions is replaced by a finite sum and the remainder is expressed as a Laplace-type integral. A pole subtraction technique is used to remove poles in the integrand which lie near the path of integration, obtaining a smooth integrand, more suitable for numerical integration, and a specific numerical integration method is proposed. Numerical experiments show highly accurate results across the frequency spectrum for a range of ground surface types. It is expected that the methods proposed will prove useful in boundary element modeling of noise propagation in canyon streets and in ducts, and for problems of scattering by periodic surfaces
Sovremennoe eolovoe rel'efoobrazovanie v stepach i lesostepah ugo vostocnoj Sibiri
Na podstawie danych dotyczących powtarzalności burz pyłowych i zmian wartości modułu eolicznej migracji materii przedstawiono skalę rozwoju procesów eolicznych w semiarydalnych obszarach równin śródgórskich i przedgórskich w południowej części Syberii. Stwierdzono, że intensywność wiatrowego przemieszczania materiału na południu Wschodniej Syberii wzrasta od północnych lasostepów do stepów i dalej – do pustynniejących stepów, zgodnie ze wzrostem suchości i kontynen-talizmu klimatu. Stwierdzono też wzrost roli deflacji we współczesnych procesach rzeźbotwórczych, a także poszerzenie granic morfogenezy eolicznej kosztem obszarów lasostepowych. Wydzielono 6 stopni prawdopodobnego pustynnienia eolicznego obszarów półsuchych (subarydalnych): 1 – bardzo słabe, 2 – słabe, 3 – umiarkowane, 4 – duże, 5 – bardzo du-że, 6 – wyjątkowo duże. Przeprowadzono klasyfikację stepów i lasostepów południowej części Wschodniej Syberii pod względem stopnia ryzyka ekologiczno-geomorfologicznego rozwoju procesów eolicznych
Noise level spread in the vicinity of a crossroads
In the paper, the PROP10 program is used to present the time-average sound level on façades of buildings in the vicinity of a crossroads. The propagation model within an urban system forming canyon streets includes the wave interaction with obstacles, multi-reflections from building walls and single and double diffraction at their wedges. For the current purpose the program is provided with a road model in which the vehicle route is divided into sub-segments with lengths and linear densities of emitted energy that depend on the traffic organization in the vicinity of the crossroads. The lengths of the sub-segments, where vehicles wait and are forced to a subsequent start and stop, depend on the flow rates on lanes, the equivalent vehicle length, the red and green light times and the speed of leaving a crossroads. The linear energy density at these sub-segments depends on a parameter characterizing the energy emitted during the starting of a vehicle. The time-average sound level is analyzed as a function of the enumerated parameters
Possible Improvement of Acoustical Climate. Part I: Measurements and Theoretical Description
In the paper, the simulation PROP5 program is used to predict the sound level in proximity of a road with defined surroundings. The simulation involves road geometry (number of lanes and their positions) and traffic structure (vehicle flow rates and their average speeds), with equivalent omnidirectional point sources representing vehicles. In Part I of the paper, the agreement between measurement and simulation results is tested to verify the accuracy degree of the applied models of a road, as a noise source and propagation throughout surrounding space. In Part II, using the pre-tested simulation program, the possibility of acoustic climate improvement has been analyzed
Possible Improvement of Acoustical Climate. Part II: Possible Solutions
In the paper, the simulation PROP5 program with the road model defined as a noise source and road surroundings model, is used to predict the efficiency of noise protecting means for the chosen building. The appropriate models of verified accuracy have been chosen by comparison of the simulation results with field measurements (Walerian et al., 2010). Here, using the pre-tested simulation program, the possibility of acoustic climate improvement has been analyzed in the ranges of practical variations of the input parameters. The road parameters: its geometry (number of lanes and their positions) and traffic structure over lanes (vehicle flow rates and their average speeds) have been taken under consideration as changeable parameters, that could be corrected to obtain acoustical climate improvement. Moreover, an acoustical screen designing has been considered. The screen efficiency has been evaluated under conditions defined by the input parameters of the road and its surroundings
