    Prognostic value of PDCD-1 and CTLA-4 in ovarian cancer patients

    Therapeutic effectiveness of treatments for ovarian cancer is not optimal. PDCD-1 and CTLA-4 offers the potential as a prognostic marker in addition to being a target for therapy. To assess the prognostic roles of PDCD-1 and CTLA-4 Gene in ovarian cancer, we utilized the Kaplan Meier plotter, a biomarker assessment tool with large quantities of data. The relationship between PDCD-1 and overall survival (OS) as well as CTLA-4 and OS were presented using Hazard Ratio, 95% CI and logrank P value. Then gene expression level was compared using H-Test and U test. The results were as follows: PDCD-1 and CTLA-4 gene expressions among 1582 ovarian cancer patients were shown with median gene expression value as the cut-off. Expression of PDCD-1 and CTLA-4 did not differ with regard to stages and P53 gene mutation. But the expression of CTLA-4 was higher in endometrioid than in serous cancer patients. Different grades of both PDCD-1 and CTLA-4 had different mean values. Higher expression of the PDCD-1 was not significantly correlated with better OS with HR 0.88 (95% CI: 0.77-1.01, P=0.061) but higher CTLA-4 was associated with better survival with HR 0.84 (95% CI: 0.73-0.96, P=0.0099) on the transcriptome level. In conclusion, lower expression of CTLA-4, but not PDCD-1 predicts worse survival

    Objective To evaluate pedicle screw fixation strength following augmentation and restoration with polymethylmethacrylate (PMMA). Methods Compact CD (CCD) screws of 6.0 mm diameter was implanted into the pedicle unilaterally. And a pilot hole was created using 3.5 mm drill contralaterally on eight fresh human lower lumbar vertebrae (L 3-5).Neither the screws nor the drillings penetrated the anterior cortex of the vertebral bodies. The screws were pulled out from the vertebrae at 5 mm/min. The screw track and the pilot hole were filled with 3-5 ml of PMMA. Then, the screws were implanted bilaterally, and the PMMA was allowed to harden for 1 hour. Morphology of PMMA around the screws was observed by section of the vertebrae along the screw. Results Pullout force of pedicle screw fixation in normal vertebrae was (825.5±254.7) N and (1 711.1±488.8)N following augmentation with PMMA. Restoration of a pullout pedicle screw fixation with PMMA was able to salvage pullout force to (1 832.4±497.4) N. The pullout forces following augmentation and restoration were significantly greater than that in normal vertebrae. Morphology of PMMA around the screws was related to the viscosity of PMMA during injection. No PMMA was found in vertebral canal or surface of pedicle. Conclusion Augmentation and restoration with PMMA offered a significantly increase of strength for pedicle screw fixation, and could be used clinically in case of failure of pedicle screw fixation. 目的 評價聚甲基丙烯酸甲酯(polymethylmethacrylate ,PMMA)強化和修復椎弓根螺釘固定的生物力學效果。方法8具成人新鮮腰椎骨L3~5,隨機選擇一側椎弓根放置直徑為6.0mm的CCD(compactCD)椎弓根螺釘 ,另一側以直徑為3.5mm的鉆頭導孔 ,均不穿透椎體前皮質。在材料試驗機上進行椎弓根螺釘拔出實驗 ,拔出速率為5mm/min。然后沿椎弓根孔道注入PMMA粉和水按體積1∶2混合的PMMA強化修復材料3~5ml,雙側椎弓根分別使用與前相同的CCD椎弓根螺釘 ,室溫下1h后再行前述拔出實驗。沿椎弓根螺釘長度方向鋸開標本 ,觀察PMMA在螺釘孔道中的分布范圍。結果CCD椎弓根螺釘在正常椎弓根內的最大拔出力為 (825.5±254.7)N ;在經強化后的正常椎弓根內和在修復失效的椎弓根內 ,CCD椎弓根螺釘最大拔出力分別為 (1711.1±488.8)N和 (1832.4±497.4)N ,高于正常椎弓根螺釘的最大拔出力 ,差異有顯著性意義(P<0.01)。PMMA在螺釘周圍的分布與其注入時的粘稠度有關 ,未見椎管內或椎弓根外有PMMA分布。結論沿椎弓根孔道注入PMMA可顯著增加椎弓根螺釘的拔出力 ,適用于螺釘松動和拔出的修復固

    T-Bet and Eomes Regulate the Balance between the Effector/Central Memory T Cells versus Memory Stem Like T Cells

    Memory T cells are composed of effector, central, and memory stem cells. Previous studies have implicated that both T-bet and Eomes are involved in the generation of effector and central memory CD8 T cells. The exact role of these transcription factors in shaping the memory T cell pool is not well understood, particularly with memory stem T cells. Here, we demonstrate that both T-bet or Eomes are required for elimination of established tumors by adoptively transferred CD8 T cells. We also examined the role of T-bet and Eomes in the generation of tumor-specific memory T cell subsets upon adoptive transfer. We showed that combined T-bet and Eomes deficiency resulted in a severe reduction in the number of effector/central memory T cells but an increase in the percentage of CD62LhighCD44low Sca-1+ T cells which were similar to the phenotype of memory stem T cells. Despite preserving large numbers of phenotypic memory stem T cells, the lack of both of T-bet and Eomes resulted in a profound defect in antitumor memory responses, suggesting T-bet and Eomes are crucial for the antitumor function of these memory T cells. Our study establishes that T-bet and Eomes cooperate to promote the phenotype of effector/central memory CD8 T cell versus that of memory stem like T cells. © 2013 Li et al

    Estimation of potential gains from bank mergers:a novel two-stage cost efficiency DEA model

    This paper develops a novel two-stage cost efficiency model to estimate and decompose the potential gains from Mergers and Acquisitions (M&As). In this model, a merged DMU is defined as a combination of two or more candidate DMUs. The merged DMU would surpass the traditional Production Possibility Set (PPS). In order to solve the problem, a Merger Production Possibility Set (PPSM) is constructed. The model minimizes the total cost of the merged DMU while maintaining its outputs at the current level, estimates the overall merger efficiency by comparing its minimal total cost with its actual cost. Moreover, the overall merger efficiency could be decomposed into technical efficiency, harmony efficiency and scale efficiency. We show that the model can be extended to a two-stage structure and these efficiencies can be decomposed to both sub-systems. To show the usefulness of the proposed approach, we applied it to a real dataset of top 20 most competitive Chinese City Commercial Banks (CCBs). We concluded that (1) There exist considerably potential gains for the proposed merged banks. (2) It is also shown that the main impact on potential merger gains are from technical and harmony efficiency. (3) As an interesting result we found that the scale effect works against the merger, indicating that it is not favorable for a full-scale merger

    BACKGROUND AND OBJECTIVE: Of three first-line anti-tuberculosis (anti-TB) drugs, isoniazid is most commonly associated with hepatotoxicity. Differences in INH-induced toxicity have been attributed to genetic variability at several loci, NAT2, CYP2E1, GSTM1and GSTT1, that code for drug-metabolizing enzymes. This study evaluated whether the polymorphisms in these enzymes were associated with an increased risk of anti-TB drug-induced hepatitis in patients and could potentially be used to identify patients at risk of liver injury. METHODS AND DESIGN: In a cross-sectional study, 2244 tuberculosis patients were assessed two months after the start of treatment. Anti-TB drug-induced liver injury (ATLI) was defined as an ALT, AST or bilirubin value more than twice the upper limit of normal. NAT2, CYP2E1, GSTM1 and GSTT1 genotypes were determined using the PCR/ligase detection reaction assays. RESULTS: 2244 patients were evaluated, there were 89 cases of ATLI, a prevalence of 4% 9 patients (0.4%) had ALT levels more than 5 times the upper limit of normal. The prevalence of ATLI was greater among men than women, and there was a weak association with NAT2*5 genotypes, with ATLI more common among patients with the NAT2*5*CT genotype. The sensitivity of the CT genotype for identifying patients with ATLI was 42% and the positive predictive value 5.9%. CT ATLI was more common among slow acetylators (prevalence ratio 2.0 (95% CI 0.95,4.20) )compared to rapid acetylators. There was no evidence that ATLI was associated with CYP2E1 RsaIc1/c1genotype, CYP2E1 RsaIc1/c2 or c2/c2 genotypes, or GSTM1/GSTT1 null genotypes. CONCLUSIONS: In Xinjiang Uyghur TB patients, liver injury was associated with the genetic variant NAT2*5, however the genetic markers studied are unlikely to be useful for screening patients due to the low sensitivity and low positive predictive values for identifying persons at risk of liver injury

    Designing ultrafine lamellar eutectic structure in bimodal titanium alloys by semi-solid sintering

    We report on a novel approach to design typical ultrafine lamellar eutectic structure in bimodal alloys fabricated by semi-solid sintering (SSS) of a eutectic mixture. In our work ultrafine lamellar eutectic structure was implemented by controlling the phase composition of eutectic reaction and consequently by regulating the structure of eutectic reaction-induced liquid phase through varying component number. Microstructure analysis indicate that although all SSSed alloys have the same three phase constitutions of bcc beta-Ti(Fe Co) and fcc Ti-2(Co Fe) the morphology and distribution of the eutectic structure transforms from limited length and minor quantity to partial fine alternating bcc beta-Ti and bcc Ti(Fe Co) lamellae and further to typical complete ultrafine alternating continuous lamellae in the SSSed ternary Ti-Fe-Co quaternary Ti-Fe-Co-Nb and quinary Ti-Fe-Co-Nb-Al alloys. Interestingly the SSSed Ti-Fe-Co-Nb-Al alloy presents a novel bimodal microstructure of coarse fcc Ti-2(Co Fe) surrounded by an ultrafine lamellar eutectic matrix containing ultrafine bcc beta-Ti and bcc Ti(Fe Co) lamellae. This bimodal microstructure exhibits ultra-high yield strength of 2050 MPa with plasticity in compression of 19.7% which exceed published values of equivalent materials.     Facile Synthesis of Monodisperse CdS Nanocrystals via Microreaction

    CdS-based nanocrystals (NCs) have attracted extensive interest due to their potential application as key luminescent materials for blue and white LEDs. In this research, the continuous synthesis of monodisperse CdS NCs was demonstrated utilizing a capillary microreactor. The enhanced heat and mass transfer in the microreactor was useful to reduce the reaction temperature and residence time to synthesize monodisperse CdS NCs. The superior stability of the microreactor and its continuous operation allowed the investigation of synthesis parameters with high efficiency. Reaction temperature was found to be a key parameter for balancing the reactivity of CdS precursors, while residence time was shown to be an important factor that governs the size and size distribution of the CdS NCs. Furthermore, variation of OA concentration was demonstrated to be a facile tuning mechanism for controlling the size of the CdS NCs. The variation of the volume percentage of OA from 10.5 to 51.2% and the variation of the residence time from 17 to 136 s facilitated the synthesis of monodisperse CdS NCs in the size range of 3.0–5.4 nm, and the NCs produced photoluminescent emissions in the range of 391–463 nm

    The Population Impact of a Large School-Based Influenza Vaccination Campaign

    The optimal vaccination strategy to mitigate the impact of influenza epidemics is unclear. In 2005, a countywide school-based influenza vaccination campaign was launched in Knox County, Tennessee (population 385,899). Approximately 41% and 48% of eligible county children aged 5-17 years were immunized with live attenuated influenza vaccine before the 2005-2006 and 2006-2007 influenza seasons, respectively. We sought to determine the population impact of this campaign.Laboratory-confirmed influenza data defined influenza seasons. We calculated the incidence of medically attended acute respiratory illness attributable to influenza in Knox and Knox-surrounding counties (concurrent controls) during consecutive seasons (5 precampaign and 2 campaign seasons) using negative binomial regression and rate difference methods. Age-stratified analyses compared the incidence of emergency department (ED) visits and hospitalizations attributable to influenza.During precampaign seasons, estimated ED visit rates attributable to influenza were 12.39 (95% CI: 10.34-14.44) per 1000 Knox children aged 5-17 years and similar in Knox-surrounding counties. During the campaign seasons, annual Knox influenza-associated ED visit rates declined relative to rates in Knox-surrounding counties: rate ratios 0.55 (95% CI: 0.27-0.83) and 0.70 (95% CI: 0.56-0.84) for the first and second campaign seasons, respectively. Overall, there were about 35% or 4.86 per 1000 fewer influenza-associated ED visits among Knox County children aged 5-17 years attributable to the campaign. No significant declines in Knox compared to surrounding counties were detected for influenza associated ED visits in children aged <5 years, all adults combined or selected adult age subgroups, although power for these analyses was limited. Alternate rate-difference analyses yielded consistent results.Vaccination of approximately 45% of Knox school-aged children with influenza vaccine was associated with a 35% annual reduction (4.86 per 1000) in ED visit rates attributable to influenza. Higher vaccination coverage and/or larger studies would be needed to determine whether similar interventions have indirect benefits in other age groups
