    The prevalence of metabolic syndrome among active sportsmen/sportswomen and sedentary workers in the Kumasi metropolis

    This study sought to establish the prevalence of the metabolic syndrome (MetS) among active sportsmen/sportswomen and sedentary workers in the Kumasi Metropolis using the National Cholesterol Education Program Adult Treatment Panel III (NCEP ATP III), World Health Organization(WHO), and International Diabetes Federation (IDF) criteria. The study was conducted between March and April, 2010. One hundred and eighty six (186) participants were involved with eighty five (85) being active sportsmen/sportswomen as study control and one hundredand one (101) sedentary workers from the Kumasi metropolis as study subjects. The study participants were recruited from a population of young and adult individuals between the ages of 19-82 years. The percentage prevalence of MetS was 1.6%, 7.4% and 14.4% when the WHO,NCEP-ATP III and IDF criteria respectively were applied on the total population. Generally, the prevalence of MetS was significantly higher among the sedentary group i.e. 3.5%, 14.0% and 26.7% for WHO, NCEP-ATP III and IDF criteria respectively as compared to the active population i.e. 0.0%, 2.0% and 3.9% for WHO, NCEP-ATP III and IDF criteria respectively. The prevalence of the MetS varied greatly between the active and sedentary subjects depending on the criteria used. Central obesity appears to be the key determinant of the prevalence of the MetSin Ghana. Preventive actions such as exercise, active lifestyles and healthy eating habits have to be implemented to reduce the tendency to obesity and MetS in the Ghanaian populace

    Epidemic of hypertension in Ghana: a systematic review

    Background Hypertension is a major risk factor for many cardiovascular diseases in developing countries. A comprehensive review of the prevalence of hypertension provides crucial information for the evaluation and implementation of appropriate programmes. Methods The PubMed and Google Scholar databases were searched for published articles on the population-based prevalence of adult hypertension in Ghana between 1970 and August 2009, supplemented by a manual search of retrieved references. Fifteen unique population-based articles in non-pregnant humans were obtained. In addition, two relevant unpublished graduate student theses from one university department were identified after a search of its 1996-2008 theses. Results The age and sex composition of study populations, sampling strategy, measurement of blood pressure, definition of hypertension varied between studies. The prevalence of hypertension (BP ≥ 140/90 mmHg ± antihypertensive treatment) ranged from 19% to 48% between studies. Sex differences were generally minimal whereas urban populations tended to have higher prevalence than rural population in studies with mixed population types. Factors independently associated with hypertension included older age group, over-nutrition and alcohol consumption. Whereas there was a trend towards improved awareness, treatment and control between 1972 and 2005, less than one-third of hypertensive subjects were aware they had hypertension and less than one-tenth had their blood pressures controlled in most studies. Conclusion Hypertension is clearly an important public health problem in Ghana, even in the poorest rural communities. Emerging opportunities such as the national health insurance scheme, a new health policy emphasising health promotion and healthier lifestyles and effective treatment should help prevent and control hypertension

    Renal insufficiency in Ghanaian HIV infected patients: need for dose adjustment

    BACKGROUND: Antiretrovirals (ARVs) could lead to clinically significant nephrotoxicity and as such will require dose adjustments in the presence of renal insufficiency. OBJECTIVE: To explore renal function estimating equations as alternatives for glomerular filtration rate (GFR) measurement in a stable cohort of HIV-infected patients. METHOD: In estimating renal insufficiency in Ghanaian HIV-infected patients, GFR for 276 HAART-naïve patients and 166 patients on HAART was estimated with the Cockcroft-Gault, 4v-MDRD and CKD-EPI estimating equations. RESULTS: Females outnumbered males by 3 to 1 in the HAART-naïve group and 4 to 1 in subjects on HAART. The prevalence of renal insufficiency calculated with the Cockcroft-Gault, 4v-MDRD and CKD-EPI equations was 8.7%, 9.1% and 8.7% in HAART-naïve patients; 14.5%, 12.6% and 12.6% in patients on HAART; 7.7%, 11.5% and 11.5% in HAART-naïve males; 10.8%, 8.1% and 8.1% in males on HAART; 9.1%, 8.0% and 7.5% in HAART-naïve females and 15.5%, 14.0% and 14.0% in females on HAART. The CKD-EPI equation yielded lower bias when compared to the Cockcroft-Gault and 4v-MDRD equations. CONCLUSION: Renal insufficiency is not uncommon among HIV infected Ghanaian patients. A significant proportion (10 to 11%) will require ARV dose adjustment at the time of initiating therapy or sometime during on-going therapy

    Screening HIV-infected adults in Malawi for anaemia: impact on eligibility for antiretroviral therapy.

    Clinical staging determines antiretroviral therapy (ART) eligibility when CD4 count is not available. Haemoglobin (Hb) ≤8 g/dL is an indication for the treatment. We measured Hb in HIV-positive Malawian adults undergoing clinical assessment for ART eligibility and calculated the percentage of patients with CD4 ≤ 350 cells/μL deemed eligible for ART by clinical staging with and without Hb measurement, using the existing threshold and an alternative proposed after comparing Hb values to CD4 counts. Three hundred and thirty-eight patients had CD4 counts measured and 226 (67%) had CD4 ≤ 350 cells/μL. Thirty-six (16%) patients with low CD4 count were eligible for ART by clinical assessment alone, 48 (21%) when Hb was also measured with a threshold of ≤8 g/dL and 74 (34%) with a threshold of ≤10 g/dL. Measuring Hb alongside clinical assessment could increase the number of patients with CD4 ≤ 350 cells/μL starting ART by 33% using a threshold of Hb ≤ 8 g/dL or 114% with a threshold of ≤10g/dL
