    Patient-centred pharmaceutical design to improve acceptability of medicines : similarities and differences in paediatric and geriatric populations

Children and older adults differ in many aspects from the other age subsets of population and require particular considerations in medication acceptability. This review highlights the similarities and differences in these two age groups in relation to factors affecting acceptability of medicines. New and conventional formulations of medicines are considered regarding their appropriateness for use in children and older people. Aspects of a formulation that impact acceptability in these patient groups are discussed, including, for example, taste/smell/viscosity of a liquid and size/shape of a tablet. A better understanding of the acceptability of existing formulations highlights opportunities for the development of new and more acceptable medicines and facilitates safe and effective prescribing for the young and older populationsPeer reviewedFinal Published versio

    Hitomi Constraints on the 3.5 keV Line in the Perseus Galaxy Cluster

    High-resolution X-ray spectroscopy with Hitomi was expected to resolve the origin of the faint unidentified E 3.5 ≈ keV emission line reported in several low-resolution studies of various massive systems, such as galaxies and clusters, including the Perseus cluster. We have analyzed the Hitomi first-light observation of the Perseus cluster. The emission line expected for Perseus based on the XMM-Newton signal from the large cluster sample under the dark matter decay scenario is too faint to be detectable in the Hitomi data. However, the previously reported 3.5 keV flux from Perseus was anomalously high compared to the sample-based prediction. We find no unidentified line at the reported high flux level. Taking into account the XMM measurement uncertainties for this region, the inconsistency with Hitomi is at a 99% significance for a broad dark matter line and at 99.7% for a narrow line from the gas. We do not find anomalously high fluxes of the nearby faint K line or the Ar satellite line that were proposed as explanations for the earlier 3.5 keV detections. We do find a hint of a broad excess near the energies of high-n transitions of S xvi E 3.44 ≃ keV rest-frame)—a possible signature of charge exchange in the molecular nebula and another proposed explanation for the unidentified line. While its energy is consistent with XMM pn detections, it is unlikely to explain the MOS signal. A confirmation of this interesting feature has to wait for a more sensitive observation with a future calorimeter experiment.United States. National Aeronautics and Space Administration.     Age-mediated changes in the gastrointestinal tract

    Physiological functions of the two extreme ends of the age spectrum, children (<18 y old) and older adults (aged 65 y and over), differ from healthy young adults. This consequently affects the pharmacokinetic profiles of administered drugs, which, in turn, impacts upon clinical practice and drug therapy. The gastrointestinal milieu acts as a distinct and vital organ regulating the dissolution, absorption and metabolism of orally ingested drugs. Age-mediated alteration in the physiology and function of the gut can reshape the pharmacokinetics of certain drugs. However, our understanding of this topic is limited. This article references the gut physiology of healthy adults to capture the available evidence in the literature on the extent and nature of the changes in childhood and older age. The gut, as an organ, is examined with regards to the effect of age on luminal fluid, microbiota, transit and motility, and the intestinal mucosa. Whilst drastic developmental changes were observed in certain aspects of the gastrointestinal environment, the examination reveals significant gaps in our knowledge in the physiology and function of the developing or ageing gut. The revelation of the unknown paves the way towards a better characterization of the human gastrointestinal tract for optimized drug therapy in children and older adults

Assembly annotation indicates that the isolate has a chromosomal bla OXA-436 carbapenemase and a plasmid-borne bla NDM-1 gene.     Superficieibacter electus gen. nov., sp. nov., an extended-spectrum β-lactamase possessing member of the enterobacteriaceae family, isolated from Intensive Care Unit surfaces

    <p>Two Gram-negative bacilli strains, designated BP-1(T) and BP-2, were recovered from two different Intensive Care Unit surfaces during a longitudinal survey in Pakistan. Both strains were unidentified using the bioMerieux VITEK MS IVD v2.3.3 and Bruker BioTyper MALDI-TOF mass spectrometry platforms. To more precisely determine the taxonomic identity of BP-1(T) and BP-2, we employed a biochemical and phylogenomic approach. The 16S rRNA gene sequence of strain BP-1(T) had the highest identity to Citrobacter farmeri CDC 2991-81(T) (98.63%) Citrobacter amalonaticus CECT 863(T) (98.56%), Citrobacter sedlakii NBRC 105722(T) (97.74%) and Citrobacter rodentium NBRC 105723(T) (97.74%). The biochemical utilization scheme of BP-1(T) using the Analytic Profile Index for Enterobacteriaceae (API20E) indicated its enzymatic functions are unique within the Enterobacteriaceae but most closely resemble Kluyvera spp., Enterobacter cloacae and Citrobacter koseri/farmeri. Phylogenomic analysis of the shared genes between BP-1(T), BP-2 and type strains from Kluyvera, Citrobacter, Escherichia, Salmonella, Kosakonia, Siccibacter and Shigella indicate that BP-1(T) and BP-2 isolates form a distinct branch from these genera. Average Nucleotide Identity analysis indicates that BP-1(T) and BP-2 are the same species. The biochemical and phylogenomic analysis indicate strains BP-1(T) and BP-2 represent a novel species from a new genus within the Enterobacteriaceae family, for which the name Superficieibacter electus gen. nov., sp. nov., is proposed. The type strain is BP-1(T) (= ATCC BAA-2937, = NBRC 113412).</p

    Polarization Relaxation Induced by Depolarization Field in Ultrathin Ferroelectric BaTiO3_3 Capacitors

    Time-dependent polarization relaxation behaviors induced by a depolarization field EdE_{d} were investigated on high-quality ultrathin SrRuO3_{3}/BaTiO3_{3}/SrRuO3_{3} capacitors. The EdE_d values were determined experimentally from an applied external field to stop the net polarization relaxation. These values agree with those from the electrostatic calculations, demonstrating that a large EdE_{d} inside the ultrathin ferroelectric layer could cause severe polarization relaxation. For numerous ferroelectric devices of capacitor configuration, this effect will set a stricter size limit than the critical thickness issue

    Dispersion of clusters of nanoscale silica particles using batch rotor-stators

    Nanoparticle powders added into a liquid medium form structures which are much larger than the primary particle size (aggregates and agglomerates)-typically of the order of 10’s of microns. An important process step is therefore the deagglomeration of these clusters to achieve as fine a dispersion as possible. This paper reports the findings of a study on the dispersion of hydrophilic fumed silica nanoparticle clusters, Aerosil 200 V, in water using two batch rotor-stators: MICCRA D-9 and VMI. The MICCRA D-9 head consists of a set of teeth for the stator and another for the rotor, whereas the VMI has a stator with slots and a rotor which consists of a 4-bladed impeller attached to an outer set of teeth. The dispersion process, studied at different power input values and over a range of concentrations (1, 5, 10 wt.%), was monitored through the evolution of PSD. Erosion was found to be the dominant breakage mechanism irrespective of operating conditions or rotor-stator type. The smallest attainable size was also found to be independent of the power input or the design of the rotor-stator. Break up kinetics increased upon the increase of power input, and this also depended on the rotor-stator design. With MICCRA D-9 which has smaller openings on both the stator and rotor, the break up rate was faster. Increasing the particle concentration decreased break up kinetics. It could also be shown that operating at high concentrations can still be beneficial as the break up rate is higher when assessed on the basis of specific power input per mass of solids
