    Frequent use of paracetamol and risk of allergic disease among women in an Ethiopian population

    Introduction The hypothesis that paracetamol might increase the risk of asthma and other allergic diseases have gained support from a range of independent studies. However, in studies based in developed countries, the possibility that paracetamol and asthma are associated through aspirin avoidance is difficult to exclude. Objectives To explore this hypothesis among women in a developing country, where we have previously reported aspirin avoidance to be rare. Methods In 2005/6 a population based cohort of 1065 pregnant women was established in Butajira, Ethiopia and baseline demographic data collected. At 3 years post birth, an interview-based questionnaire administered to 945 (94%) of these women collected data on asthma, eczema, and hay fever in the past 12 month, frequency of paracetamol use and potential confounders. Allergen skin tests to Dermatophagoides pteronyssinus and cockroach were also performed. The independent effects of paracetamol use on allergic outcomes were determined using multiple logistic regression analysis. Findings The prevalence of asthma, eczema and hay fever was 1.7%, 0.9% and 3.8% respectively; of any one of these conditions 5.5%, and of allergen sensitization 7.8%. Paracetamol use in the past month was reported by 29%, and associations of borderline significance were seen for eczema (adjusted OR (95% CI) = 8.51 (1.68 to 43.19) for 1–3 tablets and 2.19 (0.36 to 13.38) for ≥4 tablets, compared to no tablets in the past month; overall p = 0.055) and for ‘any allergic condition’ (adjusted OR (95% CI) = 2.73 (1.22 to 6.11) for 1–3 tablets and 1.35 (0.67 to 2.70) for ≥4 tablets compared to 0 in the past month; overall p = 0.071). Conclusions This study provides further cross-sectional evidence that paracetamol use increases the risk of allergic disease

    Bringing the Nature Futures Framework to life: creating a set of illustrative narratives of nature futures

    To halt further destruction of the biosphere, most people and societies around the globe need to transform their relationships with nature. The internationally agreed vision under the Convention of Biological Diversity—Living in harmony with nature—is that “By 2050, biodiversity is valued, conserved, restored and wisely used, maintaining ecosystem services, sustaining a healthy planet and delivering benefts essential for all people”. In this context, there are a variety of debates between alternative perspectives on how to achieve this vision. Yet, scenarios and models that are able to explore these debates in the context of “living in harmony with nature” have not been widely developed. To address this gap, the Nature Futures Framework has been developed to catalyse the development of new scenarios and models that embrace a plurality of perspectives on desirable futures for nature and people. In this paper, members of the IPBES task force on scenarios and models provide an example of how the Nature Futures Framework can be implemented for the development of illustrative narratives representing a diversity of desirable nature futures: information that can be used to assess and develop scenarios and models whilst acknowledging the underpinning value perspectives on nature. Here, the term illustrative refects the multiple ways in which desired nature futures can be captured by these narratives. In addition, to explore the interdependence between narratives, and therefore their potential to be translated into scenarios and models, the six narratives developed here were assessed around three areas of the transformative change debate, specifcally, (1) land sparing vs. land sharing, (2) Half Earth vs. Whole Earth conservation, and (3) green growth vs. post-growth economic development. The paper concludes with an assessment of how the Nature Futures Framework could be used to assist in developing and articulating transformative pathways towards desirable nature futures

    REPEATABILITY AND VALIDITY OF IUATLD RESPIRATORY QUESTIONNAIRE RESPONSES AS A MEASURE OF ASTHMA IN ETHIOPIAN POPULATION

    Objective: To assessed the repeatability and validity of the IUATLD respiratory symptoms questionnaire in relation to exercise-induced bronchoconstriction or bronchodilator responses in a community in southern Ethiopia.Design: A case-control study.Setting: Rural and small town setting in southern Ethiopia.Subjects: 247 adults and children who previously reported wheeze in the past year, and 174 who did not.Interventions: Administered IUATLD bronchial symptoms questionnaire; standardised free-running exercise test or (for those with airfl ow obstruction) assessment of bronchodilator response to inhaled salbutamol.Results: Kappa values for four-week repeatability for the wheeze and asthma questions were 0.61 (95 % CI 0.52 to 0.70) and 0.75 (0.63 to 0.87), respectively. Of the 58 people who reported wheeze in 2003 and in April 2006, only fi ve had a positive exercise test or bronchodilator challenge (Positive Predictive Value (PPV) 0.09, 95% CI 0.01 to 0.22). Of 12 who reported asthma in 2003 and April 2006, three had a positive result to either to exercise test or bronchodilator challenge test (PPV 0.25, 95% CI 0.05 to 0.50).Conclusion: Our fi ndings suggest that self-reported wheeze and asthma have good short-term repeatability, but do not closely refl ect exercise-induced bronchospasm or bronchodilator responsiveness. The validity of questionnaire methods of studying asthma epidemiology in developing countries needs further investigation
