    Intervenções no local de trabalho com aconselhamento para a promoção de saúde e estilo de vida saudável sobre a dor musculoesquelética em trabalhadores de escritório: protocolo de revisão sistemática

    Objective: To review the scientific literature on Randomized Controlled Trials (RCTs/Experimental Studies) and non-Randomized Controlled Trails (non-RCTs/Quasi-experimental Studies) that tested the efficacy of interventions in the workplace, with counseling to health promotion and healthy lifestyle on musculoskeletal pain in office workers. Methods: This protocol follows Preferred reporting items for systematic review and meta-analysis protocols (PRISMA-P) statement. Searches were performed in five indexed electronic databases: PubMed, Cochrane Central Register of Controlled Trails (CENTRAL), PEDro, BIREME and SciELO; in addition to searches in a database of grey literature: Google scholar. Descriptors indexed in Medical Subject Headings (MeSH) and Descritores em Ciências da Saúde (DeCS) were used. No limits were applied on any of the bases. The process of selecting the studies will be carried out in duplicate by two independent researchers and if necessary, with the help of a third to establish consensus. Duplicate articles will be excluded, titles and abstracts will be read and complete text reading of eligible studies will be performed, as well as manual searches in the reference lists of the selected articles; according to the Preferred Reporting Items for Systematic Reviews and Meta-Analyses: The PRISMA Statement. Results: RCTs and non-RCTs publish in English or Portuguese will be included. The risk of bias will be assessed using the PEDro scale. Discussion: This systematic review will present a current overview of the effects of counseling strategies for health promotion and healthy lifestyle directed to the workplace on musculoskeletal pain in office workers. This information can contribute to the scientific literature on the area of occupational health and assist in the implementation of health interventions related to musculoskeletal pain in office workers. Foram realizadas buscas em cinco bases de dados eletrônicas indexadas: PubMed, Cochrane Central Register of Controlled Trails (CENTRAL), PEDro, BIREME e SciELO; além de buscas em uma base de dados da literatura cinzenta: Google Scholar. Foram utilizados descritores indexados no Medical Subject Headings (MeSH) e no Descritores em Ciências da Saúde (DeCS). Não foram aplicados limites em nenhuma das bases. O processo de seleção dos estudos será realizado de forma duplicada por dois pesquisadores independentes e se necessário, com auxílio de um terceiro para estabelecer consenso. Serão excluídos os artigos duplicados, lidos títulos e resumos e realizada a leitura de texto completo dos estudos elegíveis, além de buscas manuais nas listas de referências dos artigos selecionados; de acordo com os Principais itens para relatar Revisões sistemáticas e Meta-análises: a recomendação PRISMA. Resultados: Serão incluídos ECR e ECC publicados em inglês ou português. A qualidade metodológica será avaliada através da escala PEDro. Discussão: Esta revisão sistemática apresentará uma síntese atual sobre os efeitos de estratégias de aconselhamentos para promoção de saúde e estilo de vida saudável direcionadas ao local de trabalho sobre a dor musculoesquelética em trabalhadores de escritório. Essas informações podem contribuir com a literatura científica relativa à área da Saúde do trabalhador e auxiliar na implementação de intervenções de saúde relacionadas à dor musculoesquelética em trabalhadores de escritório.  Registro PROSPERO número: CRD42021227775

    Phase III Trial of Adjuvant Capecitabine After Standard Neo-/Adjuvant Chemotherapy in Patients With Early Triple-Negative Breast Cancer (GEICAM/2003-11_CIBOMA/2004-01)

The GEICAM/2003-11_CIBOMA/2004-01 trial explored extended adjuvant capecitabine after completion of standard chemotherapy in patients with early TNBC. Eligible patients were those with operable, node-positive-or node negative with tumor 1 cm or greater-TNBC, with prior anthracycline- and/or taxane-containing chemotherapy. After central confirmation of TNBC status by immunohistochemistry, patients were randomly assigned to either capecitabine or observation. Stratification factors included institution, prior taxane-based therapy, involved axillary lymph nodes, and centrally determined phenotype (basal v nonbasal, according to cytokeratins 5/6 and/or epidermal growth factor receptor positivity by immunohistochemistry). The primary objective was to compare disease-free survival (DFS) between both arms. Eight hundred seventy-six patients were randomly assigned to capecitabine (n = 448) or observation (n = 428). Median age was 49 years, 55.9% were lymph node negative, 73.9% had a basal phenotype, and 67.5% received previous anthracyclines plus taxanes. Median length of follow-up was 7.3 years. DFS was not significantly prolonged with capecitabine versus observation [hazard ratio (HR), 0.82; 95% CI, 0.63 to 1.06; P =.136]. In a preplanned subgroup analysis, nonbasal patients seemed to derive benefit from the addition of capecitabine with a DFS HR of 0.53 versus 0.94 in those with basal phenotype (interaction test P =.0694) and an HR for overall survival of 0.42 versus 1.23 in basal phenotype (interaction test P =.0052). Tolerance of capecitabine was as expected, with 75.2% of patients completing the planned 8 cycles. This study failed to show a statistically significant increase in DFS by adding extended capecitabine to standard chemotherapy in patients with early TNBC. In a preplanned subset analysis, patients with nonbasal phenotype seemed to obtain benefit with capecitabine, although this will require additional validation

    Objetivo: avaliar o valor prognóstico do receptor de estrógeno e da expressão das proteínas p53 e c-erbB-2 no câncer de mama com axila negativa. Métodos: foi realizado estudo imuno-histoquímico em material incluido em parafina, do arquivo do Instituto de Pesquisas Cito-Oncológicas (IPCO) da Fundação Faculdade Federal de Ciências Médicas de Porto Alegre (FFFCMPA), de 50 casos de câncer de mama sem comprometimento dos linfonodos axilares, em mulheres menopausadas, tratadas na Irmandade da Santa Casa de Porto Alegre (ISCMPA) e no Hospital Santa Rita de Porto Alegre (HSR) de janeiro 1990 a dezembro de 1994. Para análise estatística foram utilizados os testes de c² com correção de Yates, teste exato de Fisher e curvas de sobrevida pelo método de Kaplan-Meier comparados pelo teste log rank. O seguimento médio das pacientes foi de 3,6 anos (3,1-4,5). Dos 50 casos, 14 apresentaram recidiva no período observado. Resultados: a média de idade foi 61 anos (variação de 46 a 78 anos). A mastectomia radical modificada (MRM) foi realizada em 35 pacientes (70%) e 15 (30%) foram submetidas a setorectomia com esvaziamento axilar e posterior radioterapia. Metade das pacientes que apresentaram recidiva apresentaram-na nos três primeiros anos após o diagnóstico. O tamanho médio do tumor foi 2,8 cm (1,98-3,13) e o tipo histológico mais freqüente foi o carcinoma ductal infiltrante de tipo histológico não-especial (92%), conforme a graduação histológica de Bloom e Richardson, sendo 3 grau I (6,6%), 35 grau II (76%) e 8 grau III (17,4%); nos tumores que recidivaram não houve nenhum grau I, 9 (25,7%) eram grau II e 3 (37,5%) eram grau III. Em relação ao prognóstico, a taxa de intervalo livre de doença foi menor quando da associação de tumor pouco diferenciado (grau III) com receptor de estrógeno negativo (p = 0,006), p53 positivo (p = 0,006) e c-erbB-2 positivo (p = 0,001). Conclusão: mulheres menopausadas com câncer de mama sem comprometimento dos linfonodos axilares tem pior prognóstico, em relação ao intervalo livre de doença, quando apresentam associação de tumor pouco diferenciado com RE negativo, p53 positivo e c-erbB-2 positivo.<br>Purpose: to evaluate the prognostic value of estrogen receptor and p53 and c-erbB-2 proteins in lymph node-negative breast cancer. The mean follow-up of the patients was 3.6 years (3.1-4.5). Of the 50 cases, 14 showed recurrence during the period of follow-up. Results: the mean age was 61 years (46-78). Modified radical mastectomy was performed in 35 patients (70%) and 15 (30%) were submitted to lumpectomy/axillary dissection and postoperative radiation therapy. Fifty percent of the patients who showed recurrence did it in the first three years after the diagnosis. The mean size of the tumor was 2.8 cm (1.98-3.13) and the most frequent histological type was invasive ductal carcinoma of no special type (92%), according to the Bloom and Richardson graduation, 3 being stage I (6.6%), 35 stage II (76%) and 8 stage III (17.4%). In the tumors with recurrence, there was no grade I, 9 stage II (25.7%) and 3 stage III (37.5%). In relation to the prognosis, the disease-free interval was less when there was association of a poorly differentiated tumor with negative estrogen receptor (p = 0.006), positive p53 (p = 0.006) and positive c-erbB-2 (p = 0.001). Conclusion: postmenopausal women with lymph node-negative breast cancer showed worse prognosis in relation to disease-free interval when they presented poorly differentiated tumor associated with negative estrogen receptor, positive p53 and positive c-erbB-2

    Adjuvant docetaxel, doxorubicin, and cyclophosphamide in node-positive breast cancer: 10-year follow-up of the phase 3 randomised BCIRG 001 trial

    We compared standard adjuvant anthracycline chemotherapy with anthracycline-taxane combination chemotherapy in women with operable node-positive breast cancer. Here we report the final, 10-year follow-up analysis of disease-free survival, overall survival, and long-term safety. BCIRG 001 was an open label, phase 3, multicentre trial in which 1491 patients aged 18-70 years with node-positive, early breast cancer and a Karnofsky score of 80% or more were randomly assigned to adjuvant treatment with docetaxel, doxorubicin, and cyclophosphamide (TAC) or fluorouracil, doxorubicin, and cyclophosphamide (FAC) every 3 weeks for six cycles. Randomisation was stratified according to institution and number of involved axillary lymph nodes per patient (one to three vs four or more). Disease-free survival was the primary endpoint and was defined as the interval between randomisation and breast cancer relapse, second primary cancer, or death, whichever occurred first. Efficacy analyses were based on the intention-to-treat principle. BCIRG 001 is registered with ClinicalTrials.gov, number NCT00688740. Enrolement took place between June 11, 1997 and June 3, 1999; 745 patients were assigned to receive TAC and 746 patients were assigned to receive FAC. After a median follow-up of 124 months (IQR 90-126), disease-free survival was 62% (95% CI 58-65) for patients in the TAC group and 55% (51-59) for patients in the FAC group (hazard ratio [HR] 0·80, 95% CI 0·68-0·93; log-rank p=0·0043). 10-year overall survival was 76% (95% CI 72-79) for patients in the TAC group and 69% (65-72) for patients in the FAC group (HR 0·74, 0·61-0·90; log-rank p=0·0020). TAC improved disease-free survival relative to FAC irrespective of nodal, hormone receptor, and HER2 status, although not all differences were significant in these subgroup analyses. Grade 3-4 heart failure occurred in 26 (3%) patients in the TAC group and 17 (2%) patients in the FAC group, and caused death in two patients in the TAC group and four patients in the FAC group. A substantial decrease in left ventricular ejection fraction (defined as a relative decrease from baseline of 20% or more) was seen in 58 (17%) patients who received TAC and 41 (15%) patients who received FAC. Six patients who received TAC developed leukaemia or myelodysplasia, as did three patients who received FAC. Our results provide evidence that the initial therapeutic outcomes seen at the 5-year follow-up with a docetaxel-containing adjuvant regimen are maintained at 10 years. However, a substantial percentage of patients had a decrease in left ventricular ejection fraction, probably caused by anthracycline therapy, which warrants further investigation. Sanofi
