Monitoring Appropriate Monoclonal Antibodies Prescribing via Administrative Data: An Application to Psoriasis

: The Italian Medicines Agency (AIFA) and the Italian Regional Health Systems have implemented measures together with data collection and analysis to improve medicines' appropriate prescription. Administrative databases represent rich Real-World Evidence (RWE) sources that may be leveraged for research purposes. Thus, such heritage may allow for appropriate prescription studies to be carried out on complex pharmaceutical molecules, as the appropriateness of prescriptions is essential both for patients' treatment and to ensure healthcare systems' sustainability. This study analyzed the appropriate monoclonal antibodies (mAbs) prescribed in psoriasis treatment across Tuscany, Italy. Data were extracted from several large administrative databases collected by the Tuscan Regional Healthcare System through record linkages. The analysis showed that over 30% of the 2020 cohort of psoriatic patients could be regarded as potentially inappropriate treated, signaling that mAbs are often prescribed as first-line treatment contrary to guidelines. Variation was observed in the appropriate prescription of mAbs, across different types of mAbs and areas. The study revealed potential inappropriate prescription, and its geographic variation should raise awareness among managers about the appropriate use of resources. Despite limitations, this could represent a pilot for future studies to evaluate the appropriate prescription of mAbs in other clinic conditions and across time

Ultrasound Poroelastic Tissue Typing

Employing the poroelastic theory of acoustic waves in gels, the ultrasound (US) propagation in a gel medium filled by poroelastic spherical cells is studied. The equation of fast compressional wave, the phase velocity and the attenuation as a function of the elasticity, porosity and concentration of the cells into the gel matrix are investigated. The outcomes of the theory agree with the preliminary measurements done on PVA gel scaffolds inseminated by porcine liver cells at various concentrations. The feasibility of a non-invasive tech-nique for the health assessment of soft biological tissues steaming by the model is analyzed

The true cost of cardiovascular imaging: focusing on downstream, indirect, and environmental costs

Abstract To develop a more realistic assessment of costs, herein named “true” costs, the extra-cancer from medical radiation, environmental damage from imaging paraphernalia and radioactive wastes must be included as long-term costs from imaging examinations. It is urgent to define the “true” costs across imaging modalities as it interferes on physicians’ decision to request an exam and on research projects such as cost-effectiveness analysis. Cardiology is the specialty that most will benefit from the outcome as cardiovascular exams represent almost 30% of the total exams acquired annually worldwide

Glucose and Fatty Acid Metabolism in a 3 Tissue In-Vitro Model Challenged with Normo- and Hyperglycaemia

Nutrient balance in the human body is maintained through systemic signaling between different cells and tissues. Breaking down this circuitry to its most basic elements and reconstructing the metabolic network in-vitro provides a systematic method to gain a better understanding of how cross-talk between the organs contributes to the whole body metabolic profile and of the specific role of each different cell type. To this end, a 3-way connected culture of hepatocytes, adipose tissue and endothelial cells representing a simplified model of energetic substrate metabolism in the visceral region was developed. The 3-way culture was shown to maintain glucose and fatty acid homeostasis in-vitro. Subsequently it was challenged with insulin and high glucose concentrations to simulate hyperglycaemia. The aim was to study the capacity of the 3-way culture to maintain or restore normal circulating glucose concentrations in response to insulin and to investigate the effects these conditions on other metabolites involved in glucose and lipid metabolism. The results show that the system’s metabolic profile changes dramatically in the presence of high concentrations of glucose, and that these changes are modulated by the presence of insulin. Furthermore, we observed an increase in E-selectin levels in hyperglycaemic conditions and increased IL-6 concentrations in insulin-free-hyperglycaemic conditions, indicating, respectively, endothelial injury and proinflammatory stress in the challenged 3-way system

No associations between physical activity and immunogenicity in SARS-CoV-2 seropositive patients with autoimmune rheumatic diseases prior to and after vaccination.

To investigate the association between physical activity and immunogenicity among SARS-CoV-2 seropositive patients with autoimmune rheumatic diseases prior to and following a 2-dose schedule of CoronaVac (Sinovac inactivated vaccine). This was a prospective cohort study within an open-label, single-arm, phase 4 vaccination trial conducted in Sao Paulo, Brazil. In this substudy, only SARS-CoV-2 seropositive patients were included. Immunogenicity was assessed by seroconversion rates of total anti-SARS-CoV-2 S1/S2 immunoglobulin G (IgG), geometric mean titers of anti-S1/S2 IgG, frequency of positive neutralizing antibodies, and neutralizing activity before and after vaccination. Physical activity was assessed through a questionnaire. Model-based analyses were performed controlling for age (30 kg/m2), and use of prednisone, immunosuppressants, and biologics. A total of 180 seropositive autoimmune rheumatic disease patients were included. There was no association between physical activity and immunogenicity before and after vaccination. This study suggests that the positive association between physical activity and greater antibody responses seen in immunocompromised individuals following vaccination is overridden by previous SARS-CoV-2 infection, and does not extend to natural immunity

O Largo da Banana e a presença negra em São Paulo

The Largo da Banana relates to the history of the black population in São Paulo. Acknowledged as one of São Paulo’s “cradle” of samba, samba musicians are its main spokespersons. Largo da Banana used to be located near the former Barra Funda Railway Station. From the first decades to around half the Twentieth century, informal laborers in the railway logistics used to gather together in that space. Within a precarious and unsteady daily life, they used to play samba and tiririca. In the fifties, the municipality built the Pacaembu Viaduct in that area, aiming to extend the homonym avenue beyond the railway road. In the sixties, samba musician Geraldo Filme wrote two songs in which he paid homage to Largo da Banana and regrettedits disappearance after the viaduct’s inauguration. In these and other of his songs, the artist recorded his perceptions on the life conditions of São Paulo’s blacks and samba musicians, their sociabilities, and also the urban changes he witnessed. Considering that his artistic work contributes to enlighten aspects of social reality, I analyze some of his songs to interpret the sociabilities at Largo da Banana, as well as the urban intervention in that location.O Largo da Banana está associado à história da população negra em São Paulo. Reconhecido como um dos “berços” do samba paulista, é sobretudo através dos próprios sambistas que essa história pode hoje ser conhecida. Localizado junto à antiga estação da Barra Funda, desde as primeiras décadas até meados do século passado, aquele espaço concentrou trabalhadores informais vinculados às atividades da ferrovia. Em meio a um cotidiano precário e instável, essa população realizava rodas de samba e de tiririca. Nos anos 1950, foi construído naquele local o Viaduto Pacaembu, que prolongava a avenida homônima para além davia férrea. A partir da década de 1960, o sambista negro Geraldo Filme compôs duas canções em que homenageava o Largo da Banana e lamentava seu desaparecimento após a inauguração do viaduto. Nessas e em outras composições, o artista registrou suas percepções sobre as condições de vida da população negra e sambista na cidade, suas sociabilidades, bem como sobre as transformações urbanas que testemunhou.
