    Plastic or glass: a new environmental assessment with a marine litter indicator for the comparison of pasteurized milk bottles

    AbstractPurposeToday plastic is the most used material for food packaging, but its incorrect disposal is creating environmental issues to oceans, soil and air. Someone believes that the solution is to ban plastic and substitute it with glass packaging. Is it the right choice? This study aims at comparing the environmental impact of bottles made of PET, R-PET, non-returnable glass and returnable glass in order to understand which is the most environmental friendly packaging solution.MethodsA literature analysis on the environmental impact of glass and PET bottles is carried out, taking into account their production, transport and disposal phase. Then, an environmental assessment of PET, R-PET, glass and returnable glass bottles, used to package 1 l of pasteurized milk, has been carried out using the life cycle assessment methodology and a new indicator. Inventory data were provided by an important milk processing and packaging factory located in Italy. Results were estimated using some relevant impact categories of the ReCiPe 2016 MidPoint (H) method, then a marine litter indicator (MLI) has been proposed in order to evaluate the polluting potential of milk bottles dispersed into the Mediterranean Sea.Results and discussionLCA results show that R-PET bottle gives the lowest contribution to global warming, stratospheric ozone depletion, terrestrial acidification, fossil resource scarcity, water consumption and human carcinogenic toxicity, followed by PET bottle, returnable glass bottle, and finally non-returnable glass bottle. Glass is the worst packaging option because of high energy demand in the bottle production and its weight and in the transport phase. Some improvements can be obtained with returnable glass, but even if we consider that a bottle could be reused eight times, results are not comparable to the PET or R-PET bottles used only once. However, according to the MLI, returnable glass bottles become the first option, because a lot of plastic bottles could potentially be dispersed into the sea.ConclusionsThe substitution of plastic with glass does not help to reduce the GWP and others LCI categories, while could contribute to reduce the marine litter: overall it is important to dispose correctly packaging materials, investing in recycling and reusing. In particular, great improvements can be obtained using bottles made with recycled materials, as R-PET. In conclusion, it is necessary to disadvantage waste dispersion, giving incentives to returnable packaging and raising people awareness of environmental problems

    Roaring to Softly Whispering: X-Ray Emission after ∟3.7 yr at the Location of the Transient AT2018cow and Implications for Accretion-powered Scenarios

    We present the first deep X-ray observations of luminous fast blue optical transient (LFBOT) AT 2018cow at ∼3.7 yr since discovery, together with the reanalysis of the observation at δt ∼ 220 days. X-ray emission is significantly detected at a location consistent with AT 2018cow. The very soft X-ray spectrum and sustained luminosity are distinct from the spectral and temporal behavior of the LFBOT in the first ∼100 days and would possibly signal the emergence of a new emission component, although a robust association with AT 2018cow can only be claimed at δt ∼ 220 days, while at δt ∼ 1350 days contamination of the host galaxy cannot be excluded. We interpret these findings in the context of the late-time panchromatic emission from AT 2018cow, which includes the detection of persistent, slowly fading UV emission with νLν ≈ 1039 erg s−1. Similar to previous works (and in analogy with arguments for ultraluminous X-ray sources), these late-time observations are consistent with thin disks around intermediate-mass black holes (with M• ≈ 103–104M☉) accreting at sub-Eddington rates. However, differently from previous studies, we find that smaller-mass black holes with M• ≈ 10–100 M☉ accreting at ≳the Eddington rate cannot be ruled out and provide a natural explanation for the inferred compact size (Rout ≈ 40 R☉) of the accretion disk years after the optical flare. Most importantly, irrespective of the accretor mass, our study lends support to the hypothesis that LFBOTs are accretion-powered phenomena and that, specifically, LFBOTs constitute electromagnetic manifestations of super-Eddington accreting systems that evolve to ≲Eddington over a ≈100-day timescale

    Protopine/Gemcitabine Combination Induces Cytotoxic or Cytoprotective Effects in Cell Type-Specific and Dose-Dependent Manner on Human Cancer and Normal Cells

    The natural alkaloid protopine (PRO) exhibits pharmacological properties including anticancer activity. We investigated the effects of PRO, alone and in combination with the chemotherapeutic gemcitabine (GEM), on human tumor cell lines and non-tumor human dermal fibroblasts (HDFs). We found that treatments with different PRO/GEM combinations were cytotoxic or cytoprotective, depending on concentration and cell type. PRO/GEM decreased viability in pancreatic cancer MIA PaCa-2 and PANC-1 cells, while it rescued the GEM-induced viability decline in HDFs and in tumor MCF-7 cells. Moreover, PRO/GEM decreased G1, S and G2/M phases, concomitantly with an increase of subG1 phase in MIA PaCa-2 and PANC-1 cells. Differently, PRO/GEM restored the normal progression of the cell cycle, altered by GEM, and decreased cell death in HDFs. PRO alone increased mitochondrial reactive oxygen species (ROS) in MIA PaCa-2, PANC-1 cells and HDFs, while PRO/GEM increased both intracellular and mitochondrial ROS in the three cell lines. These results indicate that specific combinations of PRO/GEM may be used to induce cytotoxic effects in pancreatic tumor MIA PaCa-2 and PANC-1 cells, but have cytoprotective or no effects in HDFs

    Turn-by-turn measurements of the energy spread at negative momentum compaction factor at KARA

    The Karlsruhe Research Accelerator (KARA), the storage ring at KIT, allows short electron bunch operation with positive as well as negative momentum compaction factor. For both cases, the beam dynamics are studied. Using a line array camera KALYPSO (KArlsruhe Linear arraY detector for MHz rePetition rate SpectrOscopy), based on TI-LGAD, the horizontal intensity distribution of the emitted visible part of the synchrotron radiation is measured at a 5-degree port of a bending magnet on a turn-by-turn time scale. As the measurement is located at a dispersive section, the dynamics of the energy spread can be studied by measuring the horizontal bunch profile. The MHz acquisition rate and the low-light sensitivity of the line camera allow measurements at low bunch currents and the investigation of the microbunching instability. This contribution presents the results of the bunch profile measurements performed at positive and negative momentum compaction factor

    Providing a nurse-led complex nursing INtervention FOcused on quality of life assessment on advanced cancer patients: The INFO-QoL pilot trial.

    PURPOSE Unmet needs for advanced-disease cancer patients are fatigue, pain, and emotional support. Little information is available about the feasibility of interventions focused on patient-reported outcome measurement developed according to the Medical Research Council (MRC) Framework in advanced-disease cancer patients. We aimed to pilot a nurse-led complex intervention focused on QoL assessment in advanced-disease cancer patients. METHODS The INFO-QoL study was based on an exploratory, nonequivalent comparison group, pre-test-post-test design. Study sites received either the INFO-QoL intervention or usual care. Adult advanced-disease cancer patients admitted to hospice inpatient units that gave their informed consent were included in the study. Subjects were 187 patients and their families and 19 healthcare professionals. We evaluated feasibility, acceptability, and patients' outcomes using the Integrated Palliative Care Outcome Scale. RESULTS Nineteen healthcare professionals were included. The mean competence score increased significantly over time (p < 0.001) and the mean usefulness score was high 8.63 (Âą1.36). In the post-test phase, 54 patients were allocated to the experimental unit and 36 in the comparison unit. Compared to the comparison unit, in the experimental unit anxiety (R2 = 0.07; 95% CI = -0.06; 0.19), family anxiety (R2 = 0.22; 95% CI = -0.03; 0.41), depression (R2 = 0.31; 95% CI = -0.05; 0.56) and sharing feelings (R2 = 0.09; 95% CI = -0.05; 0.23), were improved between pre-test and post-test phase. CONCLUSIONS The INFO-QoL was feasible and potentially improved psychological outcomes. Despite the high attrition rate, the INFO-QoL improved the quality and safety culture for patients in palliative care settings

    Clinical features and outcomes of elderly hospitalised patients with chronic obstructive pulmonary disease, heart failure or both

    Background and objective: Chronic obstructive pulmonary disease (COPD) and heart failure (HF) mutually increase the risk of being present in the same patient, especially if older. Whether or not this coexistence may be associated with a worse prognosis is debated. Therefore, employing data derived from the REPOSI register, we evaluated the clinical features and outcomes in a population of elderly patients admitted to internal medicine wards and having COPD, HF or COPD + HF. Methods: We measured socio-demographic and anthropometric characteristics, severity and prevalence of comorbidities, clinical and laboratory features during hospitalization, mood disorders, functional independence, drug prescriptions and discharge destination. The primary study outcome was the risk of death. Results: We considered 2,343 elderly hospitalized patients (median age 81 years), of whom 1,154 (49%) had COPD, 813 (35%) HF, and 376 (16%) COPD + HF. Patients with COPD + HF had different characteristics than those with COPD or HF, such as a higher prevalence of previous hospitalizations, comorbidities (especially chronic kidney disease), higher respiratory rate at admission and number of prescribed drugs. Patients with COPD + HF (hazard ratio HR 1.74, 95% confidence intervals CI 1.16-2.61) and patients with dementia (HR 1.75, 95% CI 1.06-2.90) had a higher risk of death at one year. The Kaplan-Meier curves showed a higher mortality risk in the group of patients with COPD + HF for all causes (p = 0.010), respiratory causes (p = 0.006), cardiovascular causes (p = 0.046) and respiratory plus cardiovascular causes (p = 0.009). Conclusion: In this real-life cohort of hospitalized elderly patients, the coexistence of COPD and HF significantly worsened prognosis at one year. This finding may help to better define the care needs of this population

    The “Diabetes Comorbidome”: A Different Way for Health Professionals to Approach the Comorbidity Burden of Diabetes

    (1) Background: The disease burden related to diabetes is increasing greatly, particularly in older subjects. A more comprehensive approach towards the assessment and management of diabetes’ comorbidities is necessary. The aim of this study was to implement our previous data identifying and representing the prevalence of the comorbidities, their association with mortality, and the strength of their relationship in hospitalized elderly patients with diabetes, developing, at the same time, a new graphic representation model of the comorbidome called “Diabetes Comorbidome”. (2) Methods: Data were collected from the RePoSi register. Comorbidities, socio-demographic data, severity and comorbidity indexes (Cumulative Illness rating Scale CIRS-SI and CIRS-CI), and functional status (Barthel Index), were recorded. Mortality rates were assessed in hospital and 3 and 12 months after discharge. (3) Results: Of the 4714 hospitalized elderly patients, 1378 had diabetes. The comorbidities distribution showed that arterial hypertension (57.1%), ischemic heart disease (31.4%), chronic renal failure (28.8%), atrial fibrillation (25.6%), and COPD (22.7%), were the more frequent in subjects with diabetes. The graphic comorbidome showed that the strongest predictors of death at in hospital and at the 3-month follow-up were dementia and cancer. At the 1-year follow-up, cancer was the first comorbidity independently associated with mortality. (4) Conclusions: The “Diabetes Comorbidome” represents the perfect instrument for determining the prevalence of comorbidities and the strength of their relationship with risk of death, as well as the need for an effective treatment for improving clinical outcomes

    Antidiabetic Drug Prescription Pattern in Hospitalized Older Patients with Diabetes

    Objective: To describe the prescription pattern of antidiabetic and cardiovascular drugs in a cohort of hospitalized older patients with diabetes. Methods: Patients with diabetes aged 65 years or older hospitalized in internal medicine and/or geriatric wards throughout Italy and enrolled in the REPOSI (REgistro POliterapuie SIMI—Società Italiana di Medicina Interna) registry from 2010 to 2019 and discharged alive were included. Results: Among 1703 patients with diabetes, 1433 (84.2%) were on treatment with at least one antidiabetic drug at hospital admission, mainly prescribed as monotherapy with insulin (28.3%) or metformin (19.2%). The proportion of treated patients decreased at discharge (N = 1309, 76.9%), with a significant reduction over time. Among those prescribed, the proportion of those with insulin alone increased over time (p = 0.0066), while the proportion of those prescribed sulfonylureas decreased (p &lt; 0.0001). Among patients receiving antidiabetic therapy at discharge, 1063 (81.2%) were also prescribed cardiovascular drugs, mainly with an antihypertensive drug alone or in combination (N = 777, 73.1%). Conclusion: The management of older patients with diabetes in a hospital setting is often sub-optimal, as shown by the increasing trend in insulin at discharge, even if an overall improvement has been highlighted by the prevalent decrease in sulfonylureas prescription

    The role of SPICA-like missions and the Origins Space Telescope in the quest for heavily obscured AGN and synergies with Athena

    arXiv:2106.08345v1SPICA Collaboration Team.In the black hole (BH)–galaxy co-evolution framework, most of the star formation (SF) and the BH accretion are expected to take place in highly obscured conditions. The large amount of gas and dust absorbs most of the UV-to-soft-X radiation and re-emits it at longer wavelengths, mostly in the IR. Thus, obscured active galactic nuclei (AGN) are very difficult to identify in optical or X-ray bands but shine bright in the IR. Moreover, X-ray background (XRB) synthesis models predict that a large fraction of the yet-unresolved XRB is due to the most obscured (Compton thick, CT: NH ≥ 1024 cm−2) of these AGN. In this work, we investigate the synergies between putative IR missions [using SPace Infrared telescope for Cosmology and Astrophysics (SPICA), proposed for European Space Agency (ESA)/M5 but withdrawn in 2020 October, and Origins Space Telescope, OST, as ‘templates’] and the X-ray mission Athena (Advanced Telescope for High ENergy Astrophysics), which should fly in early 2030s, in detecting and characterising AGN, with a particular focus on the most obscured ones. Using an XRB synthesis model, we estimated the number of AGN and the number of those which will be detected in the X-rays by Athena. For each AGN, we associated an optical-to-Far InfraRed (FIR) spectral energy distribution (SED) from observed AGN with both X-ray data and SED decomposition and used these SEDs to check if the AGN will be detected by SPICA-like or OST at IR wavelengths. We expect that, with the deepest Athena and SPICA-like (or OST) surveys, we will be able to photometrically detect in the IR more than 90% of all the AGN (down to L2−10keV ∼ 1042 erg s−1 and up to z ∼ 10) predicted by XRB synthesis modeling, and we will detect at least half of them in the X-rays. The spectroscopic capabilities of the OST can provide ≈51 000 and ≈3 400 AGN spectra with R = 300 at 25–588µm in the wide and deep surveys, respectively, the last one up to z ≈ 4. Athena will be extremely powerful in detecting and discerning moderate- and high-luminosity AGN, allowing us to properly select AGN even when the mid-IR torus emission is ‘hidden’ by the host galaxy contribution. We will constrain the intrinsic luminosity and the amount of obscuration for ∼20% of all the AGN (and ∼50% of those with L2−10keV > 3.2 × 1043 erg s−1) using the X-ray spectra provided by Athena WFI. We find that the most obscured and elusive CT-AGN will be exquisitely sampled by SPICA-like mission or OST and that Athena will allow a fine characterisation of the most luminous ones. This will provide a significant step forward in the process of placing stronger constraints on the yet-unresolved XRB and investigating the BH accretion rate evolution up to very high redshift (z ≥ 4).FP, CV, CG, LB, and LS acknowledge financial support by the Agenzia Spaziale Italiana (ASI) under the research contract 2018-31-HH.0.
