    The prison health system presents an opportunity to improve Aboriginal prisoners’ health and wellbeing, diagnose and treat health and mental health problems, and mitigate the effects of harmful behaviours. Executive summary The prison health system presents an opportunity to improve Aboriginal prisoners’ health and wellbeing, diagnose and treat health and mental health problems, and mitigate the effects of harmful behaviours. Improving prison health systems for Aboriginal people can also reduce high rates of postrelease hospitalisation and mortality experienced by Aboriginal prisoners and improve quality of life. Aboriginal prisoners experience higher rates of health and mental health problems than non-Aboriginal prisoners. The impact on prison health care is foreshadowed by consistent increases in the number of Aboriginal people imprisoned in Victoria each year. One in 33 Aboriginal males is imprisoned in Victoria at any one time, and the rate of overrepresentation is increasing for both Aboriginal men and women. More than 50% of Aboriginal people released from Victorian prisons return within two years, which places increasing importance on continuity of care. With large numbers of Aboriginal people moving in and out of the prison system, a strong relationship should exist between prison health services and prisoners’ community health and mental health provider. The 28 Aboriginal Community Controlled health Organisations (ACCHOs) and their auspiced organisations across Victoria are located within 55km of all Victorian prisons. ACCHOs are a critical extension of prison health care given Aboriginal prisoners access ACCHOs more frequently than mainstream services in the community. ACCHOs’ comprehensive support and engagement of Aboriginal people plays a big part in improving quality of life and improving poor health and mental health outcomes by providing a holistic, healing health service. The Victorian Aboriginal Community Controlled Health Organisation (VACCHO), with support from the Victorian Government Department of Justice, explored ways to improve continuity of care for Aboriginal people in Victorian prisons and identify ways to improve relationships and partnerships between ACCHOs and prison health services. ACCHOs, prison health services, and Koori support staff members from the Department of Justice were interviewed and their responses analysed for common themes. We found no relationship or partnership between ACCHOs and prison health services interviewed despite policy references requiring it within the Justice Health Policy and Quality Framework (attached to the prison health services contracts). Responses also indicated that prison health service systems were not meeting cultural safety policy standards. ACCHOs identified several areas in need of improvement to assist Aboriginal prisoner health including prisoner release planning and the transfer of health information. Given the low level of contact between ACCHOs and prison health services there were few working examples that could be shared. A list of recommendations based on interview responses, a literature review and exploration of non-Victorian models is presented as a first step in improving health and mental health outcomes for Aboriginal prisoners

    The ethical beliefs and behaviours of Victorian fitness professionals

    A survey based on those employed by Petitpas, Brewer, Rivera, and Van Raalte (1994), Pope, Tabachnick, and Keith-Spiegel (1987), Tabachnick, Keith-Spiegel, and Pope (1991), and Pope and Vetter (1992) was used to investigate the ethical beliefs and behaviours of Victorian fitness professionals. Although there is evidence that Victorian fitness professionals are knowledgeable about some general ethical principles, the results of this study suggest that there is some lack of consensus among Victorian fitness professionals about the ethical appropriateness of a number of complex issues relating to business practices, confidentiality, dual relationships, and personal and professional boundaries. The findings suggest there is a need to improve the professional and ethical education of fitness professionals and to develop comprehensive ethical principles and a code of conduct that is relevant to the individuals working in the Australian fitness profession

    Australia's insurance crisis and the inequitable treatment of self-employed midwives

    Based upon a review of articles published in Australia's major newspapers over the period January 2001 to December 2005, a case study approach has been used to investigate why, when compared with other small business operators, including medical specialists, Australian governments have appeared reluctant to protect the economic viability of the businesses of self-employed midwives. Theories of agenda setting and structuralism have been used to explore that inequity. What has emerged is a picture of the complex of factors that may have operated, and may be continuing to operate, to shape the policy agenda and thus prevent solutions to the insurance problems of self-employed midwives being found

    A generic model for the assessment of disease epidemiology: the computational basis of DisMod II

    Epidemiology as an empirical science has developed sophisticated methods to measure the causes and patterns of disease in populations. Nevertheless, for many diseases in many countries only partial data are available. When the partial data are insufficient, but data collection is not an option, it is possible to supplement the data by exploiting the causal relations between the various variables that describe a disease process. We present a simple generic disease model with incidence, one prevalent state, and case fatality and remission. We derive a set of equations that describes this disease process and allows calculation of the complete epidemiology of a disease given a minimum of three input variables. We give the example of asthma with age-specific prevalence, remission, and mortality as inputs. Outputs are incidence and case fatality, among others. The set of equations is embedded in a software package called 'DisMod II', which is made available to the public domain by the World Health Organization

    Process redesign for time-based emergency admission targets

    Purpose: Hospitals have used process redesign to increase the efficiency of the emergency department (ED) to cope with increasing demand. While there are published studies suggesting a positive outcome, recent reviews have reported that it is difficult to conclude that these approaches are effective as a result of substandard research methodology. The purpose of this paper is to explore the perceptions of hospital staff on the impact of a process redesign initiative on quality of care. Design/methodology/approach: A retrospective qualitative case study examining a Lean Six Sigma (LSS) initiative in a large metropolitan hospital from 2009 to 2010. Non-probability sampling identified interview subjects who, through their participation in the redesign initiative, had a detailed understanding of the implementation and outcomes of the initiative. Between April 2012 and January 2013 26 in-depth semi-structured interviews were conducted and analysed with thematic content analysis. Findings: There were four important findings. First, when asked to comment on the impact of the LSS implementation, without prompting the staff spoke of quality of care. Second, there was little agreement among the participants as to whether the project had been successful. Third, despite the recognition of the need for a coordinated effort across the hospital to improve ED access, the redesign process was not successful in reducing existing divides among clinicians and among managers and clinicians. Finally, staff expressed tension between production processes to move patients more quickly and their duty of care to their patients as individuals. Originality/value: One of the first studies to explore the impact of process redesign through in-depth interviews with participating staff, this study adds further evidence that organisation implementing process redesign must ensure the supporting management practices are in place

    Skin cancer has a large impact on our public hospitals but prevention programs continue to demonstrate strong economic credentials

    Objectives: While skin cancer is still the most common cancer in Australia, important information gaps remain. This paper addresses two gaps: i) the cost impact on public hospitals; and ii) an up-to-date assessment of economic credentials for prevention.Methods: A prevalence-based cost approach was undertaken in public hospitals in Victoria. Costs were estimated for inpatient admissions, using State service statistics, and outpatient services based on attendance at three hospitals in 2012–13. Cost-effectiveness for prevention was estimated from ‘observed vs expected’ analysis, together with program expenditure data.Results: Combining inpatient and outpatient costs, total annual costs for Victoria were 48millionto48 million to 56 million. The SunSmart program is estimated to have prevented more than 43,000 skin cancers between 1988 and 2010, a net cost saving of 92million.Skincancertreatmentinpublichospitals(92 million. Skin cancer treatment in public hospitals (9.20∼10.39perhead/year)was30timescurrentpublicfundinginskincancerprevention(10.39 per head/year) was 30-times current public funding in skin cancer prevention (0.37 per head/year).Conclusions: At about $50 million per year for hospitals in Victoria alone, the cost burden of a largely preventable disease is substantial. Skin cancer prevention remains highly cost-effective, yet underfunded.Implications for public health: Increased funding for skin cancer prevention must be kept high on the public health agenda. Hospitals would also benefit from being able to redirect resources to non-preventable conditions

    In what ways does the mandatory nature of Victoria's municipal public health planning framework impact on the planning process and outcomes?

    BACKGROUND: Systems for planning are a critical component of the infrastructure for public health. Both in Australia and internationally there is growing interest in how planning processes might best be strengthened to improve health outcomes for communities. In Australia the delivery of public health varies across states, and mandated municipal public health planning is being introduced or considered in a number of jurisdictions. In 1988 the Victorian State government enacted legislation that made it mandatory for each local government to produce a Municipal Public Health Plan, offering us a 20-year experience to consider. RESULTS: In-depth interviews were undertaken with those involved in public health planning at the local government level, as part of a larger study on local public health infrastructure and capacity. From these interviews four significant themes emerge. Firstly, there is general agreement that the Victorian framework of mandatory public health planning has led to improvements in systems for planning. However, there is some debate about the degree of that improvement. Secondly, there is considerable variation in the way in which councils approach planning and the priority they attach to the process. Thirdly, there is concern that the focus is on producing a plan rather than on implementing the plan. Finally, some tension over priorities is evident. Those responsible for developing Municipal Public Health Plans express frustration over the difficulty of having issues they believe are important addressed through the MPHP process. CONCLUSION: There are criticisms of Victoria's system for public health planning at the local government level. Some of these issues may be specific to the arrangement in Victoria, others are problems encountered in public health planning generally. In Victoria where the delivery structure for public health is diverse, a system of mandatory planning has created a minimum standard. The implementation of the framework was slow and factors in the broader political environment had a significant impact. Work done in recent years to support the process appears to have led to improvements. There are lessons for other states as they embark upon mandated public health plans

    From little things, big things grow: a local approach to system-wide maternity services reform in the absence of definitive evidence

    For nearly two decades calls have been made to expand the role of midwives within maternity services in Australia. Although some progress has been made, it has been slow and, at system-wide level, limited. There are many barriers that prevent the expansion of midwifery-led services in Australia including funding arrangements for midwifery care, a lack of political will and resistance from powerful medical interest groups. The ongoing debate that exists about the evidence for the safety of midwifery-led care, particularly for the intrapartum phase, is likely to be an important reason why policy-makers are reluctant to implement system-wide reforms of maternity services

    Factors associated with length of stay and the risk of readmission in an acute psychiatric inpatient facility: a retrospective study

    OBJECTIVE: This study was to investigate factors influencing the length of stay and predictors for the risk of readmission at an acute psychiatric inpatient unit. METHOD: Two comparative studies were embedded in a retrospective cross-sectional clinical file audit. A randomly selected 226 episodes of admissions including 178 patients during a twelve-month period were reviewed. A total of 286 variables were collected and analysed. A case control study was employed in the study of length of stay. A retrospective cohort study was used to investigate the predictors for the risk of readmission. RESULTS: Logistic regression analyses showed that 10 variables were associated with length of stay. Seclusion during the index admission, accommodation problems and living in an area lacking community services predicted longer stay. During the follow-up period 82 patients (46%) were readmitted. Cox regression analyses showed 9 variables were related to the risk of readmission. Six of these variables increased the risk of readmission, including history of previous frequent admission, risk to others at the time of the index admission and alcohol intoxication. More active and assertive treatment in the community post-discharge decreased the risk of readmission. CONCLUSIONS: Length of stay is multifactorially determined. Behavioural manifestations of illness and lack of social support structures predicted prolonged length of stay. Good clinical practice did not necessarily translate to a shorter length of stay. Therefore, length of stay is predictable, but not readily modifiable within the clinical domain. Good clinical practice within the community following discharge likely reduces the risk of readmission. Quality of inpatient care does not influence the risk of readmission, which therefore raises a question about the validity of using the rate of readmission as an outcome measure of psychiatric inpatient care
