    An Altered Derivatives Marketplace: Clearing Swaps Under Dodd-Frank

    Though over a year has passed, the impact of the Dodd Frank Act remains unclear. This Note examines the provisions of the Act that relate to swap transactions within the context of pre-reform and postreform markets. In order to reduce the uncertainties inherent in unregulated swap transactions, the Act employs a comprehensive framework, which includes mandatory clearing through derivative clearing organizations, extensive reporting requirements, margin requirements, and position limits. This Note argues that, in doing so, the Dodd Frank Act addresses the fundamental failures of pre-reform derivative markets. However, the importance of the role for derivative clearing organizations under this framework creates a risk that these organizations will become systemically significant, mirroring problems with under-capitalized and over-exposed financial institutions in the downturn

    Use of full-dose contrast-enhanced CT for extrahepatic staging using Gallium-68-DOTATATE PET/CT in patients with neuroendocrine tumors

    PURPOSEStudies have demonstrated that positron emission tomography/computed tomography (PET/CT) with Gallium-68 (68Ga)-labeled somatostatin analogues are effective at detecting metastatic disease in neuroendocrine tumors (NET), especially extrahepatic metastases. However, PET in combination with full-dose contrast-enhanced CT (ceCT) exposes patients to higher radiation (~25 mSv). The use of non-contrast-enhanced low-dose CT (ldCT) can reduce radiation to about 10 mSv and may avoid contrast-induced side effects. This study seeks to determine whether ceCT could be omitted from NET assessments.METHODSWe retrospectively compared the performance of PET/ldCT versus PET/ceCT in 54 patients (26 male, 28 female) who had undergone a 68Ga-DOTATATE PET/CT. The selection criteria were as follows: available ldCT and ceCT, histologically confirmed NET, and follow-up of at least 6 months (median, 12.6 months; range, 6.1–23.2 months). The PET/ldCT and PET/ceCT images were analyzed separately. We reviewed metastases in the lungs, bones, and lymph nodes. The results were compared with the reference standard (clinical follow-up data).RESULTSThe PET/ceCT scans detected 139 true-positive bone lesions compared with 140 lesions detected by the PET/ldCT scans, 106 true-positive lymph node metastases (PET/ceCT) compared with 90 metastases detected by the PET/ldCT scans, and 26 true-positive lung lesions (PET/ceCT) compared with 6 lesions detected by the PET/ldCT scans. The overall lesion-based sensitivity for full-dose PET/ceCT was 97%, specificity 86%, negative predictive value (NPV) 93%, and positive predictive value (PPV) 93%. The overall lesion-based sensitivity for PET/ldCT was 85%, specificity 73%, NPV 72%, and PPV 85%.CONCLUSIONThis study presents the first evidence that ceCT should not be omitted from extrahepatic staging using 68Ga-DOTATATE PET/CT in patients with NET. ceCT alone can be used as a follow-up to reduce radiation exposure when the patient has already undergone PET/ceCT and suffers from non-DOTATATE-avid NET

We have explored a wide range of linear electronic densities in the weak (strong) coupling regimes that appear when the ratio of spin-orbit to confining energy is small (large). These results are used to obtain the conductance of the wire.     Objective: The objective of this study was to study recurrence in patients with differentiated thyroid carcinoma who after initial therapy consisting of total thyroidectomy and I-131 ablation, were cured defined as a negative TSH-stimulated Tg-levels and a negative I-131 whole body scan (WBS) at the first follow-up after ablation. Methods: Retrospective data for differentiated thyroid carcinoma patients from three university hospitals were pooled. Out of 1993 patients, 526 cured patients were included. All patients received at least one more TSH-stimulated WBS and Tg-measurement within 5 years after initial treatment. Results: 12 patients (2.1%) developed a recurrence after an average interval of 35 months (range: 12-59 months) following administration I-131 ablation. Overall disease-free survival according to the method of Kaplan-Meier was 96.6%. There was no difference in disease-free survival between high- and low-risk patients (p=0.61). Recurrence was first discovered by Tg-measurement during levothyroxin therapy in 7 patients, and by TSH-stimulated Tg-measurement in 5 patients. I-131 WBS did not contribute to the detection of recurrences. Multivariate analysis showed that age TNM-stage (p=0.015) and histology (p=0.032) were independent predictors of disease-free survival. Conclusion: Recurrence is a rare event in patients with DTC who received total thyroidectomy with subsequent I-131 ablation, and who had a negative first follow-up TSH-stimulated I-131 WBS and negative concurrent Tg. In the study population there were no recurrences after more than 5 years of follow-up.Ziel: Das Ziel dieser Studie war es, die Rezidivrate zu untersuchen bei Patienten mit einem differenziertem Schilddrüsen-Karzinom (DTC), die nach der ersten Behandlung, bestehend aus totaler Thyreoidektomie und I-131 Ablation, geheilt wurden. Heilung wurde definiert als eine negative TSH-stimulierte Tg-Messung und eine negative I-131 Ganzkörperszintigrafie (GKS) im ersten Follow-up nach der Ablation. Methoden: Retrospektive Daten für Patienten mit einem differenziertem Schilddrüsen-Karzinom aus drei Universitätskliniken wurden gemeinsam analysiert. 526 von 1993 Patienten wurden geheilt. Alle Patienten erhielten mindestens eine weitere TSH-stimulierte GKS und TG-Messung innerhalb von 5 Jahren nach der ersten Behandlung. Ergebnisse: 12 Patienten (2,1%) entwickelten ein Rezidiv nach einer durchschnittlichen Zeitdauer von 35 Monaten (Bereich: 12-59 Monate) nach der I-131-Ablation. Das rezidiv-freie Überleben berechnet mit der Methode von Kaplan-Meier lag bei 96,6%. Es gab keinen Unterschied im rezidiv-freien Überleben zwischen Hoch- und Niedrig-Risiko-Patienten (p = 0,61). Ein Rezidiv wurde zum ersten Mal entdeckt mittels Tg-Messung während Thyreosuppressiver Levothyroxin-Einnahme bei 7 Patienten, und mittels TSH-stimulierter Tg-Messung bei 5 Patienten. Die I-131-GKS führte nicht zur Erkennung von Rezidiven. Multivariate Analysen zeigten, dass TNM-Stadium (p = 0,015) und Histologie (p = 0,032) unabhängige Prädiktoren für das Rezidiv-freies Überleben waren. Fazit: Ein Rezidiv ist ein seltenes Ereignis bei Patienten mit DTC, die nach totaler Thyreoidektomie mit anschließender I-131-Ablation einen negativen ersten Follow-up bestehend aus TSH-stimulierter I-131 GKS und gleichzeitiger TG-Messung hatten. In der untersuchten Patientengruppe ergaben sich keine Rezidive nach mehr als 5 Jahren Nachsorge

    Controversies in Radioiodine Treatment of Low- and Intermediate-risk Thyroid Cancer

    In differentiated thyroid cancer, radioiodine therapy (RIT) is usually carried out after thyroidectomy. Although the potent beneficial effects of radioiodine are undisputed in high-risk patients, much controversy remains surrounding many aspects of RIT in low- and intermediate-risk patients. Other than the indication for postoperative RIT, controversies also include, among others, the intent of RIT and the choice of activity for RIT or the mode of thyroid stimulating hormone stimulation. Furthermore, there is even controversy on the definition of what constitutes low- or intermediate-risk patients. Here the various issues will be discussed and an overview of the different points of view in a number of more prominent national and international guidelines and current literature is presented
