    Bilateral Linear Lichen Planus Pigmentosus Associated with Hepatitis C Virus Infection

    Lichen planus pigmentosus is a rare subtype of lichen planus. We report a first case of lichen planus pigmentosus with bilateral linear distribution associated with hepatitis C virus infection. The lesion was improved after sun avoidance and treatment of hepatitis C virus infection with a combination of interferon and ribavirin. This case stresses the importance of screening for hepatitis C virus infection as lichen planus pigmentosus can be an associated condition

    Five-Year Retrospective Review of Acute Generalized Exanthematous Pustulosis

    Background. Acute generalized exanthematous pustulosis (AGEP) is an acute pustular eruption characterized by widespread nonfollicular sterile pustules. The aim of this study is to characterize the etiology, clinical features, laboratory findings, management, and outcome of patients with AGEP in Asians. Patient/Methods. A retrospective analysis was performed on patient who presented with AGEP between August 2008 and November 2012 in a tertiary center in Thailand. Results. Nineteen patients with AGEP were included. AGEP was generally distributed in seventeen patients (89.5%) and localized in two (10.5%). Fever and neutrophilia occurred in 52.6% and 68.4%, respectively. Hepatitis was found up to 26.3%. The most common etiology was drugs (94.7%), comprising of antibiotics (73.6%), proton pump inhibitors (10.5%), nonsteroidal anti-inflammatory drugs (5.3%), and herbal medicine (5.3%). Beta-lactams were the most common causal drug, particularly carbapenems and cephalosporins. This is the first report of Andrographis paniculata as an offending agent for AGEP. We found no differences between various treatment regimens (topical corticosteroid, systemic corticosteroid, and supportive treatment) regarding the time from drug cessation to pustules resolution (P=0.171). Conclusions. We have highlighted the presentation of AGEP among Asians. We found high association with systemic drugs. Carbapenems were one of the leading culprit drugs.     Assessment on Hydrologic Response by Climate Change in the Chao Phraya River Basin, Thailand

    The Chao Phraya River in Thailand has been greatly affected by climate change and the occurrence of extreme flood events, hindering its economic development. This study assessed the hydrological responses of the Chao Phraya River basin under several climate sensitivity and greenhouse gas emission scenarios. The Soil and Water Assessment Tool (SWAT) model was applied to simulate the streamflow using meteorological and observed data over a nine-year period from 2003 to 2011. The SWAT model produced an acceptable performance for calibration and validation, yielding Nash-Sutcliffe efficiency (NSE) values greater than 0.5. Precipitation scenarios yielded streamflow variations that corresponded to the change of rainfall intensity and amount of rainfall, while scenarios with increased air temperatures predicted future water shortages. High CO2 concentration scenarios incorporated plant responses that led to a dramatic increase in streamflow. The greenhouse gas emission scenarios increased the streamflow variations to 6.8%, 41.9%, and 38.4% from the reference period (2003-2011). This study also provided a framework upon which the peak flow can be managed to control the nonpoint sources during wet season. We hope that the future climate scenarios presented in this study could provide predictive information for the river basin.close

    Free-space optics (FSO), or wireless optical communications, has received extensive research due to its promise of practically limitless bandwidths. However, FSO has challenges yet to be met for a cost effective realisation. This D.Phil thesis explores a solution using a ferro-electric liquid crystal spatial light modulator (FLC SLM) and binary phase holograms to significantly reduce the hardware complexity of an FSO system with auto-alignment and turbulence compensation. The theory of binary phase hologram is presented and extended to obtain a new algorithm that is suitable for a FLC SLM. The algorithm is able to be used in a demonstration system to broadcast data streams to multiple receivers, showing the capability of using FLC SLM to form any beam configuration. An FSO transmitter is then developed that uses retro-reflectors as markers for the receivers. The transmitter combines an imaging system with the FLC SLM as a reconfigurable beam steering system for acquiring the retro-reflector location. The FLC SLM is also used to reduce aberrations in the optics, resulting in a significant increase in the transmitted beam power density. The accuracy of the acquisition is measured to give a small steering error without the use of a closed loop controller. An optical turbulence simulator, using the principals of binary phase hologram, is constructed to simulate optical beam propagation in turbulent conditions. The simulator accurately produces aberrations that have the same statistics with the theoretical prediction. Analysis of the phase distortion due to turbulence is performed and a wavefront sensorless turbulence compensation method based on the FLC SLM gives significant reduction in calculated bit error rates. New scintillation index derivation for multiple optical beams is described and then used to demonstrate further decrease in bit error rates.</p

    Diffuse Dermal Angiomatosis: A Clue to the Diagnosis of Atherosclerotic Vascular Disease

    Diffuse dermal angiomatosis (DDA) is a benign, acquired, reactive vascular proliferation. DDA is clinically characterized by painful purpuric plaque with central ulceration. The histopathologic hallmark is diffuse proliferation of endothelial cells that are arranged interstitially between collagen bundles of the reticular dermis. DDA has been reported in association with peripheral atherosclerotic disease, arteriovenous fistula and heavy smoking. We report the case of a 49-year-old Asian male with DDA who presented with a painful stellate-shaped purpuric patch on the right thigh. Histopathologic examination showed proliferation of CD34-positive spindle cells in the dermis. Our patient underwent vascular bypass surgery along with tight control of cardiovascular risk factors, which yielded successful results
