The incidence of all stroke and stroke subtype in the United Kingdom, 1985 to 2008: a systematic review
<p>Abstract</p> <p>Background</p> <p>There is considerable geographic variation in stroke mortality around the United Kingdom (UK). Whether this is due to geographical differences in incidence or case-fatality is unclear. We conducted a systematic review of high-quality studies documenting the incidence of any stroke and stroke subtypes, between 1985 and 2008 in the UK. We aimed to study geographic and temporal trends in relation to equivalent mortality trends.</p> <p>Methods</p> <p>MEDLINE and EMBASE were searched, reference lists inspected and authors of included papers were contacted. All rates were standardised to the European Standard Population for those over 45, and between 45 and 74 years. Stroke mortality rates for the included areas were then calculated to produce rate ratios of stroke mortality to incidence for each location.</p> <p>Results</p> <p>Five papers were included in this review. Geographic variation was narrow but incidence appeared to largely mirror mortality rates for all stroke. For men over 45, incidence (and confidence intervals) per 100,000 ranged from 124 (109-141) in South London, to 185 (164-208) in Scotland. For men, premature (45-74 years) stroke incidence per 100,000 ranged from 79 (67-94) in the North West, to 112 (95-132) in Scotland. Stroke subtype data was more geographically restricted, but did suggest there is no sizeable variation in incidence by subtype around the country. Only one paper, based in South London, had data on temporal trends. This showed that there has been a decline in stroke incidence since the mid 1990 s. This could not be compared to any other locations in this review.</p> <p>Conclusions</p> <p>Geographic variations in stroke incidence appear to mirror variations in mortality rates. This suggests policies to reduce inequalities in stroke mortality should be directed at risk factor profiles rather than treatment after a first incident event. More high quality stroke incidence data from around the UK are needed before this can be confirmed.</p

Carbon stable isotope analysis of cereal remains as a way to reconstruct water availability: preliminary results
Reconstructing past water availability, both as rainfall and irrigation, is important to answer questions about the way society reacts to climate and its changes and the role of irrigation in the development of social complexity. Carbon stable isotope analysis of archaeobotanical remains is a potentially valuable method for reconstructing water availability. To further define the relationship between water availability and plant carbon isotope composition and to set up baseline values for the Southern Levant, grains of experimentally grown barley and sorghum were studied. The cereal crops were grown at three stations under five different irrigation regimes in Jordan. Results indicate that a positive but weak relationship exists between irrigation regime and total water input of barley grains, but no relationship was found for sorghum. The relationship for barley is site-specific and inter-annual variation was present at Deir ‘Alla, but not at Ramtha and Khirbet as-Samra
Sex differences in in-hospital mortality following a first acute myocardial infarction: Symptomatology, delayed presentation, and hospital setting
Background: Women generally wait longer than men prior to seeking treatment for acute myocardial infarction (AMI). They are more likely to present with atypical symptoms, and are less likely to be admitted to coronary or intensive care units (CCU or ICU) compared to similarly-aged males. Women are more likely to die during hospital admission. Sex differences in the associations of delayed arrival, admitting ward, and mortality have not been thoroughly investigated. Methods: Focusing on presenting symptoms and time of presentation since symptom onset, we evaluated sex differences in in-hospital mortality following a first AMI in 4859 men and women presenting to three emergency departments (ED) from December 2008 to February 2014. Sex-specific risk of mortality associated with admission to either CCU/ICU or medical wards was calculated after adjusting for age, socioeconomic status, triage-assigned urgency of presentation, blood pressure, heart rate, presenting symptoms, timing of presentation since symptom onset, and treatment in the ED. Sex-specific age-adjusted attributable risks were calculated.Results: Compared to males, females waited longer before seeking treatment, presented more often with atypical symptoms, and were less likely to be admitted to CCU or ICU. Age-adjusted mortality in CCU/ICU or medical wards was higher among females (3.1 and 4.9 % respectively in CCU/ICU and medical wards in females compared to 2.6 and 3.2 % in males). However, after adjusting for variation in presenting symptoms, delayed arrival and other risk factors, risk of death was similar between males and females if they were admitted to CCU or ICU. This was in contrast to those admitted to medical wards. Females admitted to medical wards were 89 % more likely to die than their male counterparts. Arriving in the ED within 60 min of onset of symptoms was not associated with in-hospital mortality. Among males, 2.2 % of in-hospital mortality was attributed to being admitted to medical wards rather than CCU or ICU, while for females this age-adjusted attributable risk was 4.1 %. Conclusions: Our study stresses the need to reappraise decision making in patient selection for admission to specialised care units, whilst raising awareness of possible sex-related bias in management of patients diagnosed with an AMI
State and irrigation: archeological and textual evidence of water management in late Bronze Age China
Ancient China remains an important case to investigate the relationship between statecraft development and ‘total power.’ While important economic and social developments were achieved in the late Neolithic, it was not until the late Bronze Age (first millennium BC) that state-run irrigation systems began to be built. Construction of large-scale irrigation projects, along with walls and defensive facilities, became vital to regional states who were frequently involved in chaotic warfare and desperate to increase food production to feed the growing population. Some of the irrigation infrastructures were brought into light by recent archeological surveys. We scrutinize fast accumulating archeological evidence and review rich historical accounts on late Bronze Age irrigation systems. While the credibility of historical documents is often questioned, with a robust integration with archeological data, they provide important information to understand functions and maintenance of the irrigation projects. We investigate structure and organization of large-scale irrigation systems built and run by states and their importance to understanding dynamic trajectories to social power in late Bronze Age China. Cleverly designed based on local environmental and hydrological conditions, these projects fundamentally changed water management and farming patterns, with dramatic ecological consequences in different states. Special bureaucratic divisions were created and laws were made to further enhance the functioning of these large-scale irrigation systems. We argue that they significantly increased productivity by converting previously unoccupied land into fertile ground and pushed population threshold to a new level. A hypothesis should be tested in further archeological research

Water and landscape: Ancient Maya settlement decisions
In this chapter we present the major transitions in Maya history from the Late Preclassic through Terminal Classic periods in the Maya Lowlands, focusing first on major settlement and subsistence systems, followed by major social and environmental costs. We particularly focus on how the Maya built and relied on increasingly complex water and agricultural systems to adapt in the humid tropics where everything in life was rainfall dependent. The seasonality of rainfall required innovative strategies to contain water throughout the long dry season in the face of growing population and socio-political complexity. Drastic changes occurred when several prolonged droughts struck the Maya area, resulting in them exploring new areas and subsistence practices. The chapter concludes with a few thoughts on how understanding ancient Maya water and land use is relevant for today's issues regarding sustainable land and water use
