Improvement of the Rotation Arch of the Posterior Interosseous Pedicle Flap Preserving Both Reverse Posterior and Anterior Interosseous Vascular Sources.
Abstract
PURPOSE:
The reverse posterior interosseous artery flap has several advantages, not sacrificing any major blood vessel, but its relatively short pedicle limits the use to cover defects up to the metacarpophalangeal joint. Our purpose is to demonstrate that the ligature of the anterior interosseous artery (AIA), proximal to the communicating branch with the posterior interosseous artery, leads to an improved flap rotation arch, preserving both vascular sources.
METHODS:
Sixteen fresh cadavers with latex perfusion were analyzed before and after our technique of elongation, and the so-obtained measures were standardized in "percentage of elongation of the pedicle." Eight patient with the loss of substance at the dorsal aspect of the hand have been treated with this technique, and results were evaluated in terms of flap survival and complication rates.
RESULTS:
The medium length of the pedicle in the normal flap was 10.8\u2009cm, and after the section of the AIA, the medium length of the pedicle was 13.6\u2009cm with a medium increase of 2.8\u2009cm. It means a medium increase of 24% of the length of the pedicle. In all patients treated, full coverage of the defect was obtained, and we did not experience major complications.
CONCLUSIONS:
This anatomical study supported by our clinical experience demonstrates that the use of the variant described above permits to reach more distal part of the hand without being afraid to stretch the pedicle because of the connection with the anastomotic arcades of the AIA at the wrist reducing the risk of ischemia of the flap

ASSESSING SPREADABILITY OF WARM SEASON SPECIES USED FOR TURFGRASS ON THREE TYPES OF SUBSTRATE IN THE MEDITERRANEAN ENVIRONMENT
The aim of the study was to examine the performance of turfgrass warm season species, including 6 varieties of Cynodon dactylon in comparison with Zoysia japonica and Paspalum vaginatum in three substrates with different soil textures (sandy loam, sandy clay loam, clay), to find the one or the ones that adapt best in the Mediterranean environment. The results showed the best perfomances in sandy loam substrate and for C.dactylon cv. Transcontinental, Capriola and La Paloma. Z. japonica, Zenith, had lower values
Gastrointestinal colonization by KPC-producing Klebsiella pneumoniae following hospital discharge: duration of carriage and risk factors for persistent carriage
AbstractThe natural history of KPC-producing Klebsiella pneumoniae (KPC KP) carriage is unknown. We aimed to examine the duration of KPC KP carriage following hospital discharge and to study the risk factors for persistent carriage. A cohort of 125 KPC KP carriers was followed monthly for between 3 and 6 months after discharge from an acute-care hospital. Rectal swabs and data were collected at baseline and at each visit. KPC KP was detected by culture and direct blaKPC PCR. Acquisition time was regarded as the earliest date of KPC KP isolation. Resolution of carriage was defined as a negative KPC KP test in at least two consecutive samples. Analyses were separated for recent (<4 months) (REC, 75 patients) and remote (≥4 months) (REM, 50 patients) acquisition groups. Risk factors for persistent carriage were examined by survival analyses for the REC group and by prevalence methods for the REM group. The mean age of patients was 67.5 years and 49.6% were male. Forty-six (61%) patients in the REC group and 14 (28%) in the REM group were persistent carriers (p < 0.001). A significant risk factor for persistent carriage identified in both the REC and REM groups was the presence of any catheter (p < 0.05). Unique risk factor groups included long-term care facility (LTCF) residence (p < 0.01) and a low functional status as measured by the Barthel’s index (p < 0.05) in the REC group and high Charlson’s score in the REM group (p < 0.05). Out of the entire 100 patients who had at least one negative sample, only 65 remained negative on subsequent cultures. In conclusion, persistent carriage of KPC KP is associated with catheter use and a low functional status; it is more common in patients with recent acquisition and is related to LTCF stay. A single negative KPC KP test is insufficient to exclude persistent carriage
Lithium-Metal Free Sulfur Battery Based on Waste Biomass Anode and Nano-Sized Li2S Cathode
The realization of a stable lithium-metal free (LiMF) sulfur battery based on amorphous carbon anode and lithium sulfide (Li2S) cathode is here reported. In particular, a biomass waste originating full-cell combining a carbonized brewer's spent grain (CBSG) biochar anode with a Li2S-graphene composite cathode (Li2S70Gr30) is proposed. This design is particularly attractive for applying a cost-effective, high performance, environment friendly, and safe anode material, as an alternative to standard graphite and metallic lithium in emerging battery technologies. The anodic and cathodic materials are characterized in terms of structure, morphology and composition through X-ray diffraction, scanning and transmission electron microscopy, X-ray photoelectron and Raman spectroscopies. Furthermore, an electrochemical characterization comprising galvanostatic cycling, rate capability and cyclic voltammetry tests were carried out both in half-cell and full-cell configurations. The systematic investigation reveals that unlike graphite, the biochar electrode displays good compatibility with the electrolyte typically employed in sulfur batteries. The CBSG/Li2S70Gr30 full-cell demonstrates an initial charge and discharge capacities of 726 and 537 mAh g−1, respectively, at 0.05C with a coulombic efficiency of 74%. Moreover, it discloses a reversible capacity of 330 mAh g−1 (0.1C) after over 300 cycles. Based on these achievements, the CBSG/Li2S70Gr30 battery system can be considered as a promising energy storage solution for electric vehicles (EVs), especially when taking into account its easy scalability to an industrial level. © 2022 The Authors. Developmental exposures to perfluorooctanesulfonic acid (PFOS) impact embryonic nutrition, pancreatic morphology, and adiposity in the zebrafish, \u3cem\u3eDanio rerio\u3c/em\u3e
Perfluorooctanesulfonic acid (PFOS) is a persistent environmental contaminant previously found in consumer surfactants and industrial fire-fighting foams. PFOS has been widely implicated in metabolic dysfunction across the lifespan, including diabetes and obesity. However, the contributions of the embryonic environment to metabolic disease remain uncharacterized. This study seeks to identify perturbations in embryonic metabolism, pancreas development, and adiposity due to developmental and subchronic PFOS exposures and their persistence into later larval and juvenile periods. Zebrafish embryos were exposed to 16 or 32 μM PFOS developmentally (1–5 days post fertilization; dpf) or subchronically (1–15 dpf). Embryonic fatty acid and macronutrient concentrations and expression of peroxisome proliferator-activated receptor (PPAR) isoforms were quantified in embryos. Pancreatic islet morphometry was assessed at 15 and 30 dpf, and adiposity and fish behavior were assessed at 15 dpf. Concentrations of lauric (C12:0) and myristic (C14:0) saturated fatty acids were increased by PFOS at 4 dpf, and PPAR gene expression was reduced. Incidence of aberrant islet morphologies, principal islet areas, and adiposity were increased in 15 dpf larvae and 30 dpf juvenile fish. Together, these data suggest that the embryonic period is a susceptible window of metabolic programming in response to PFOS exposures, and that these early exposures alone can have persisting effects later in the lifecourse
