    Entropic Barriers, Frustration and Order: Basic Ingredients in Protein Folding

    We solve a model that takes into account entropic barriers, frustration, and the organization of a protein-like molecule. For a chain of size MM, there is an effective folding transition to an ordered structure. Without frustration, this state is reached in a time that scales as MΞ»M^{\lambda}, with λ≃3\lambda\simeq 3. This scaling is limited by the amount of frustration which leads to the dynamical selectivity of proteins: foldable proteins are limited to ∼300\sim 300 monomers; and they are stable in {\it one} range of temperatures, independent of size and structure.     A possible mechanism for cold denaturation of proteins at high pressure

    We study cold denaturation of proteins at high pressures. Using multicanonical Monte Carlo simulations of a model protein in a water bath, we investigate the effect of water density fluctuations on protein stability. We find that above the pressure where water freezes to the dense ice phase (β‰ˆ2\approx2 kbar), the mechanism for cold denaturation with decreasing temperature is the loss of local low-density water structure.     When modeling an extensive class of technical systems, the mathematical apparatus of queuing systems (QMS) is widely used. An example of such a system is the computer network, where computer applications are generated and executed. Applications are generated usually not regularly, but by accident, forming the so-called random order of applications (requirements). Service requests, it also continues some random time. One of the central issues in the organization of mass-service systems is the elucidation of the regularities that subordinate the moments when system requirements for service are submitted.The article explores the flow of events in technical systems of various purposes. On the basis of the fact that under the Poisson character of the flow mathematical modeling of the systems is greatly simplified, the problem of obtaining a simple criterion for determining the degree of approximation of the flow of events to a Poisson one is posed. Pearson’s criterion, regression, correlation and parametric criteria were investigated. A criterion based on the calculation of the waiting function was obtained again.     Radiological assessment of the Belarusian nuclear power plant site in the pre-operational period

    Field studies on the pre-operational period of a Belarusian NPP have allowed us to determine the β€œbackground” level of gamma-emitting radionuclides in individual components of the environment. The results of measuring the dose rate at the NPP construction site are from 0.048 to 0.089 ΞΌSv/h. External radiation in the surveyed area is at 96% due to 40K, 226Ra and 232Th. The radionuclides in the surface soil layer are: 40K - from 530 to 700 Bq/kg; 226Ra - from 30 to 55 Bq/kg; and 232Th - from 17 to 35 Bq/kg; 137Cs from 2 to 13 Bq/kg. The dose rate in the floodplain of the Viliya River from 0.033 to 0.082 ΞΌSv/h. The activity concentrations of the radionuclides in the surface soil layer of the floodplain of the Viliya River are: 40K - from 390 to 690 Bq/kg; 226Ra - from 33 to 50 Bq/kg; 232Th - from 15 to 50 Bq/kg; 137Cs - from 3 to 12 Bq/kg. The activity concentration of carbon-14 and tritium in the dominant vegetation species were determined to be: from 74.4 to 111.5 pMC and less than lower range limit, respectively.     The first experience of using the technique of two-stage resection of the liver (split in situ) in patients with extensive metastasis has been presented.     Background. Ovarian malignancies constitute the 8th most common cancer in women with regard to morbidity and the 6th most common with regard to mortality. Survival of patients with ovarian cancer depends on the type of surgery and the size of the residual tumour. An Β«aggressiveΒ» surgical approach is currently adopted in the treatment of ovarian malignancies, which is aimed at removing all visible tumour lesions and subsequent regional chemotherapy. However, outcomes of treatment remain unsatisfactory, which indicates a need for a search for new, alternative treatment methods. Intraperitoneal hyperthermic chemoperfusion is one of such methods being introduced into clinical practice.Purpose. To evaluate the effectiveness of hyperthermic chemoperfusion in complex therapy regimens for patients with advanced ovarian cancer after optimal interval surgery.Materials and methods. A retrospective, single-centre, non-randomized study was conducted at the State Budgetary Institution of Health Care Chelyabinsk Regional Clinical Centre of Oncology and Nuclear Medicine from January 2014 to February 2019. The study material included data from an observation of 61 patients diagnosed with ovarian cancer stage IIIA-C. Tumours were staged according to the FIGO / TNM classification (2009). Prior to initiation of therapy, all patients underwent an examination that included PET / CT and BRCA mutation testing. At the first stage, all subjects received 3 cycles of neoadjuvant combination chemotherapy (NACCT) according to the following regimen: paclitaxel 175 mg/m2 and carboplatin AUC 5-6 every 3 weeks.At the second stage, all patients had cytoreductive surgery and then were divided into 3 groups. Study enrollment was sequential: Group 1 consisted of 15 subjects (24.6%) who were treated with hyperthermic intraperitoneal chemoperfusion (HIPEC) with paclitaxel 100 mg/m2, Group 2 included 20 patients (32.8%) receiving HIPEC with cisplatin 100 mg/m2, and Group 3 was composed of 26 control patients (42.6 %) who had no HIPEC. Progression-free survival, i. e., the time from the date of diagnosis to disease progression as established using objective methods, was used as the study endpoint.Study results. Treatment outcomes after first-line chemotherapy with interval surgery were analyzed. According to the RESIST 1.1 1 criteria, in Group 1 16 patients (45.7 %) had complete response, 12 subjects (34.2 %) had partial response, 4 patients (11.4%) had disease stabilization, while disease progression was observed in 3 cases (8.5%). In Group 2, complete response, partial response, disease stabilization, and progression were observed in 13 (50%), 7 (26.9%), 3 (11.5%), and 3 (11.5%) cases, respectively.ΠΠΊΡ‚ΡƒΠ°Π»ΡŒΠ½ΠΎΡΡ‚ΡŒ.     A new lattice protein model with a four-helix bundle ground state is analyzed by a parameter-space Monte Carlo histogram technique to evaluate the effects of an extensive variety of model potentials on folding thermodynamics. Cooperative helical formation and contact energies based on a 5-letter alphabet are found to be insufficient to satisfy calorimetric and other experimental criteria for two-state folding.     Laser osteoperforation for the treatment of acute osteomyelitis. 10 years experience

    Results of the treatment of acute osteomyelitis in 237 patients in the two groups were studied. In the main (144 children) used the method of laser osteoperforation by diode laser with a wavelength of 0.8; 1.641.9 microns. In the comparison group (93 children) were carried out mechanical osteperfora-tion an electric drill. Laser osteoperforation performed percutaneously with perforations both medul-lary canal walls. The affected metaepiphysis and the adjacent part of the diaphysis was formed from 8 to 20 through-holes. When extramedullary phase in addition of small incisions lanced and drained purulent streaks. The proposed method reduced the incidence of complications in children with CSOs from 16.1% to 7.0%. Sustained remission after surgery with the use of laser technology was observed in 96.3% of children, and after conventional surgery in 91.4%. Going into the chronic stage of the disease in the study group of children was observed in 2 times less than after traditional operations.     Risk Factors of Portal Vein Thrombosis in Patients with Different Child-Pugh Classes Liver Cirrhosis

    Aim: to evaluate the frequency of portal vein thrombosis (PVT) and build predictive models of the development of PVT for patients with liver cirrhosis (LC) of A and B/C classes by Child-Pugh.Materials and methods. Research design is a case-control. The Case group included 130 patients with newly diagnosed PVT not caused by invasive hepatocellular carcinoma (HCC); 29 patients were assigned to class A, 101 patients were assigned to class B/C. From the database of cirrhotic patients without PVT 60 Controls for class A and 205 for B/C were selected using sratified randomization by sex, age and etiology of cirrhosis. The Mann-Whitney U-test and Pearson's chi-squared test were used to compare the groups. Odds ratios (OR) and 95 % confidence intervals (95 % CI) were calculated. Logistic regression models are constructed with the separation of the sample into training and test (0.7; 0.3). The operational characteristics of the models were calculated on the test sample; ROC analysis was carried out, the area under the ROC curve (AUC) was calculated.Results. The overall frequency of PVT was 4.1 % (95 % CI 2.7-5.8 %) in class A and 10.4 % (95 % CI 8.5-12.5 %) class B/C. Patients with class A and B/C PVT differed from the corresponding controls by more severe portal hypertension: the frequency of bleeding / number of interventions on varices compared with the control were 41/45 % vs. 7/8 % (p < 0.001) for class A and 25.7/30.7 % vs. 16.1/16.1 % (p < 0.05) for class B/C, ascites frequency was 24 % vs. 8 % (p < 0.05) for class A and 89.1 % vs. 68.3 % (p < 0.001) for class B/C. The cutoff by the portal vein diameter was the same for both classes β€” 13.4 mm; the spleen length was similar and amounted 17.5 mm for class A, 17.1 mm for class B/C. Patients with PVT differed from the corresponding controls by neutrophil-to-lymphocyte ratio: class A 2.33 (1.82; 3.61) vs. 1.76 (1.37; 2.20), p < 0.01, class B/C 2.49 (1.93; 3.34) vs. 2.15 (1.49; 3.26), p < 0.05. Patients of class B/C had a higher incidence of newly diagnosed malignant tumors - 23.8% (primarily HCC that does not invade the portal vein), compared with control and cases of class A - 6.3 % and 3 % (p < 0.05), respectively. The best model for class A included variceal bleeding, ascites, portal vein diameter, absolute number of neutrophils, for class B β€” ascites, spleen length, portal vein diameter, malignant tumors / local factors; sensitivity, specificity, accuracy and AUC were 79.3 %, 90 %, 86.5 %, 0.897 and 73.3 %, 68.3 %, 69.9 %, 0.789, respectively.Conclusion.
