    In the course of experiments in 210 white mongrel male rats, endured 5-min clinical death from acute blood loss the mechanisms of energy, carbohydrate, lipid metabolism disturbance in postresuscitated brain tissue have been studied. It has been established that primary and secondary hypoxia, metabolic acidosis, excessive lipid peroxidation process activity, endotoxemia, neuron overload by Ca ions appeared to be the principle pathogenetic factors of the abovementioned disturbances. Pathogenetic block-scheme of early postresuscitation metabolic disturbances in brain was presented.     The investigation was dedicated to the exploration of reamberin influence on oxidative stress and metabolic disturbances in 64 patients with abdominal sepsis. The authors used reamberin for correction of oxidative processes and metabolic disturbances for treatment of the patients with abdominal sepsis. The authors registered that reamberin improved the oxygen-transport function of the blood and metabolic processes during the abdominal sepsis via the hybernation condition β€” a form of adaptive reaction of organs and systems during the endotoxemia for creation of more effective environment for functioning of main organs and the whole organism. The authors show that the main pathogenetic mechanisms for multiorgan dysfunction are metabolic disturbances and energy deficiency in organs and systems with subsequent tissue hypoxia

    Impact of General Anesthesia and Antioxidants on Cognitive, Static and Locomotor Functions During Laparoscopic Cholecystectomy

    Aim of the study:Β to reduce cognitive impairment during laparoscopic cholecystectomy by perioperative administration of drugs with antihypoxic and antioxidant effects under the control of stabilography.Β  Β Materials and methods.Β We studied the effect of general anesthesia and antioxidants on cognitive, static and locomotor functions during laparoscopic cholecystectomy. We studied 90 patients with acute calculous cholecystitis randomized into three experimental groups (n=30, each group). The control group was composed of 24 healthy individuals. Group 1 patients received no antioxidants, group 2 patients received the combination antioxidant drug (sodium fumarate+sodium chloride+potassium chloride+magnesium chloride) in the post-operative period, and group 3 patients received methylethylpyridinol. The patient groups were comparable in sex, age and type of inflammatory and destructive process in the gallbladder. Surgical intervention was performed under endotracheal anesthesia. Premedication with atropine and promedol was given, and anesthesia induction was carried out with propofol, fentanyl and suxamethonium. Sevoflurane, fentanyl andcisatracurium were employed to maintain anesthesia, analgesia and myorelaxation, respectively. The patients were examined before surgery, 24 and 48 hours after surgery. In the groups of patients who received antioxidant therapy, blood sampling for hematological and biochemical examinations was performed 30 min after the administration of antioxidants. Stabilographic studies and MoCA test (Montreal Scale) were performed before antioxidant administration prior to surgery and after surgery, on days 2 and 3.Β  Β Results.Β Neuropsychological testing revealed postoperative cognitive dysfunction on standard therapy which included impaired attention and concentration, executive function, memory, speech, visual constructional skills, abstract thinking, counting, and orientation (21 points on the MoCA scale versus 28–30 points for normal). We found that the pathogenetic factors of cognitive dysfunction included insufficient antioxidant protection, decreased TNF-Ξ± and elevated interleukin-18 levels along with an increased level of C-reactive protein in plasma, which manifested as activation of free-radical oxidation processes and reduced antioxidant system and performance of nonspecific resistance. Perioperative use of the combination antioxidant drug and methylethylpyridinol antioxidants reduced the frequency and severity of postoperative cognitive impairment in patients after laparoscopic cholecystectomy.Β  Β Conclusion.Β The most important pathogenetic factors of cognitive dysfunction after laparoscopic cholecystectomy include activation of free-radical oxidation, reduction of antioxidant defense system performance and lack of nonspecific resistance factors. Adding the combination antioxidant drug or methylethylpyridinol to the standard therapy reduces the intensity of radical oxygen species generation, maintains the antioxidant potential, activates production and secretion of nonspecific resistance factors, preventing the development and reducing the severity of cognitive disorders in the perioperative period. Neuropsychological testing and stabilographic examination allow identifying the risk of cognitive disorders in patients after laparoscopic cholecystectomy and provide a rationale for the use of antioxidant therapy for their prevention

    ESTIMATION OF PREVALENCE OF ADVERSE GENOTYPES AND POLYMORPHIC ALLELES OF GENES-CANDIDATES OF ARTERIAL HYPERTENSION IN CHILDREN WITH ENDEMIC GOITER IN ZABAYKALSKY KRAI

    This study is devoted to study the prevalence of genotypes and polymorphic alleles of genes-candidates of arterial hypertension in children with endemic goiter (EZ) in the Zabaykalsky Krai. To identify the frequency of occurrence of adverse genotypes and alleles polymorphisms Gly16Arg ADRB2, Gln27Glu ADRB2 and Thr174Met gene AGT, as factors of risk of development of arterial hypertension, were examined 194 children aged 7-11 years with a diagnosis of endemic goiter. The frequency of mutant allele 16Gly genotype Gly16Arg associated with increased agonist-induced suppression of beta-2 adrenergic receptors has not been above (63,2 %) than in the group of healthy children (60,3 %) (p(j2) > 0.05). The frequency of allele Arg16 defining a greater degree of vasodilating effect, was 39.7 % and 36.8 %, respectively (p(x2) > 0.05). Thefrequency characteristics of alleles genotype Gln27Glu in group I were the following: wild type 27Glu 41 %, the mutant allele Gln27 59 %, in the control group 42,2 % (p(Ο‡2) > 0.05) and 57.8 % (pfy2) > 0.05) respectively. The frequency of wild-type Thr174 genotype Thr174Met accounted for 85.3 % in group I and 86.2 % in the control group (p(x2) > 0.05) and the frequency of occurrence ofthe mutated allele 174Met 14.7 % in the group of children with diffuse nontoxic goiter and 13.8 % in healthy children, respectively (pCx2) > 0.05). Significant differences in the distribution of genotypes and alleles between groups are not identified, and the frequency characteristics were comparable with the prevalence in the population and the results of other authors. The distribution of genotypes and frequency of alleles of a gene Gly16Arg and Gln27Glu ADRB2, Thr174Metgene AGT in both groups corresponded to the expected according to the hardy-Weinberg equilibrium in the population sample. The results obtained in the course of molecular-genetic study led to the conclusion that polymorphisms Gly16Arg ADRB2, Gln27Glu ADRB2 and Thr174Met gene AGT are not factors of risk of development of arterial hypertension in children with endemic goiter

    The prevalence of genotypes and alleles of polymorphic marker S1 /S2 Apolipoprotein C3 in healthy adolescent Caucasians (Russians) living in Eastern Siberia was studied. Material for research was the DNA extracted from the venous blood; genotype was determined by PCR-RFLP for each sample. In the studied sample in 88.9 % of cases genotype S1/ S1 was detected, genotype S1/S2 was found in 8.6 %, S2/S2 - in 2.5 %. The share of S2 allele was 6.8 %, which is not significantly different from the previously studied Caucasoid populations

    Objective: to evaluate the clinical efficacy of hypoxen and nutrients used in the combination therapy of patients operated for bleeding gastroduodenal ulcer. Subjects and methods. Seventy-four patients were examined and treated. All the patients were divided into 3 groups: 1) standard therapy + hypoxen in a daily dose of 1 g for a week; 2) standard therapy + hypox-en and nutrients; 3) standard therapy only. Hypoxen and nutrients were administered on postoperative day 1 via an enter-al feeding tube. Systemic hemodynamic and hemostatic parameters were determined using MICAR-RHEO hardware-software rheographic unit APG2-01 analyzer, respectively. Results. It was established that the hyper- and eukinetic hemodynamic types were predominant before surgery and the eukinetic type was prevalent in the early postoperative period. Hypoxen and nutrients were observed to positively affect central hemodynamic parameters, such as blood pressure, cardiac output, circulating blood volume, and heart rate. Hypercoagulation changes (shorter blood clotting time in a study group and elevated serum fibrinogen levels in a comparison group) on postoperative day 10 suggest that the hemostatic parameters should be monitored and corrected as soon as possible.
