    Hepatitis C Infection Among Injecting Drug Users in England and Wales (1992–2006): There and Back Again?

    Changes in hepatitis C virus (HCV) prevalence from 1992 to 2006 were examined by using 24,311 records from unlinked anonymous surveillance of injecting drug users in England and Wales. Bayesian logistic regression was used to estimate annual prevalence, accounting for changing recruitment patterns (age, gender, injecting duration, geographic region, interactions) and the sensitivity and specificity of different oral fluid testing devices. After controlling for these differences, the authors found that the adjusted HCV prevalence decreased from 70% (95% credible interval: 62, 78) in 1992 to 47% (95% credible interval: 43, 51) in 1998 before rising again to 53% (95% credible interval: 48, 58) in 2006. Women injecting drug users had a higher HCV risk than did men (odds ratio = 1.50, 95% credible interval: 1.31, 1.73). Two regions (London and North West) had a markedly higher HCV prevalence than did the rest of England and Wales. Among individuals who had injected for less than 1 year, the adjusted HCV prevalence in 2006 was higher than that in 1992 (28% vs. 19%, respectively). HCV infection can be prevented. The public health challenge in England and Wales is to increase action in order to regain a downward trend in HCV risk and the benefit that has been lost since 1998

    Prosocial interactions among preschoolers frequentlyinvolve the use of objects. Naturalistic studies are then necessary toreveal the variety of interactive patterns as well as indicate somepossible relationships between special kind of objects and behavioralcategories. The present study was devoted to: (a) classify the observedprosocial interactions exhibited by five year olds into a behavioralsystem, and (b) analyse the relationships between behavioral patternsand sex, activities and different type of objects. Besides those found inthe literature, new categories were here considered as prosocial.Object circulation was more frequent than cooperative simultaneoususage of objects. Similar patterns were observed for boys and girls,both having preferences for social exchanges with same sex partners.Important relationships were also found between prosocial behaviorsand specific objects and activities.  A utilização do objeto como mediador nas interações prósociaisentre crianças pré-escolares exige a realização de pesquisasnaturalísticas capazes de revelar os diferentes padrões interativos esugerir possíveis relações entre tipo de objeto e categorias comportamentais.Os dados obtidos no presente estudo permitiram a elaboraçãode um sistema de categorias, sendo analisadas as relações existentesentre estas e variáveis como sexo, atividade e natureza do objeto envolvidono episódio. Além da ampliação do rol de categorias consideradascomo pró-sociais, verificou-se maior tendência à transferência deobjetos do que a seu uso partilhado pelas crianças, assim como padrõescomportamentais semelhantes para meninos e meninas, ambosapresentando preferência por companheiros do mesmo sexo.     Prognostic Role of Gene Mutations in Chronic Myelomonocytic Leukemia Patients Treated With Hypomethylating Agents

    Somatic mutations contribute to the heterogeneous prognosis of chronic myelomonocytic leukemia (CMML). Hypomethylating agents (HMAs) are active in CMML, but analyses of small series failed to identify mutations predicting response or survival. We analyzed a retrospective multi-center cohort of 174 CMML patients treated with a median of 7 cycles of azacitidine (n = 68) or decitabine (n = 106). Sequencing data before treatment initiation were available for all patients, from Sanger (n = 68) or next generation (n = 106) sequencing. Overall response rate (ORR) was 52%, including complete response (CR) in 28 patients (17%). In multivariate analysis, ASXL1 mutations predicted a lower ORR (Odds Ratio [OR] = 0.85, p = 0.037), whereas TET2mut/ASXL1wt genotype predicted a higher CR rate (OR = 1.18, p = 0.011) independently of clinical parameters. With a median follow-up of 36.7 months, overall survival (OS) was 23.0 months. In multivariate analysis, RUNX1mut (Hazard Ratio [HR] = 2.00, p = .011), CBLmut (HR = 1.90, p = 0.03) genotypes and higher WBC (log10(WBC) HR = 2.30, p = .005) independently predicted worse OS while the TET2mut/ASXL1wt predicted better OS (HR = 0.60, p = 0.05). CMML-specific scores CPSS and GFM had limited predictive power. Our results stress the need for robust biomarkers of HMA activity in CMML and for novel treatment strategies in patients with myeloproliferative features and RUNX1 mutations.
