    Publication Activities of Pedagogical Universities: Quantitative and Qualitative Indicators

    The article aims to study the effectiveness of teachers’ research activities at pedagogical universities. This indicator is measured by scientific publications that promote scientific research results and the dissemination of scientific flows that affect productivity and effectiveness of scientific activities at universities. The main methods of research are the quantitative analysis of the characteristics of teachers’ publication activity at pedagogical universities, the analysis of bibliometric data and their classification, statistical and comparative analysis. Within the framework of the study, a literature review is presented. The article highlights the results of the of the teachers’ publication activity analysis over the past five years and trends in the development of publication activity. The authors have also provided an analysis of the quality and the level of journals in which authors affiliated with pedagogical universities are published. In addition, the shortage of peer-reviewed journals for educational research publication is revealed. The conclusions are made about the specifics of promoting research of pedagogical universities: orientation of university teachers to publish research results in peer-reviewed journals; formation of collaborations with the leading Russian and foreign scientists or research groups for joint research. The authors also propose measures to eliminate difficulties in the teachers’ publication activities: training in the basics of legal literacy for intellectual activity results protection and promotion, as well as the formation of a support system for scientific and pedagogical staff for publishing research results in peer-reviewed journals

    Prescribing frequency and adherence to statins in outpatients with type 2 diabetes mellitus and comorbid cardiovascular diseases

    BACKGROUND: Due to the high rate of growth in the incidence and burden of cardiovascular complications, type 2 diabetes mellitus (T2DM) is a significant medical problem in the world. Even in the absence of cardiovascular disease (CVD), patients with T2DM are classified as high and very high risk. In addition to glycemic control, an extremely important aspect of managing this group of patients is prevention of cardiovascular complications. T2DM and hyperlipidemia determines the target group for statins. At the same time, little is known about the frequency of administration of this class of drugs among people with T2DM.AIM: To study prescribing frequency and adherence to statins in outpatients with T2DM and comorbid cardiovascular diseases.METHODS: 156 patients with type 2 diabetes (87.2% — women, average age — 65.2 years) were examined as part of an outpatient appointment with an endocrinologist at the city polyclinic ofTomsk. We used a standard questionnaire compiled on the basis of adapted international methods, including information on cardiac pathology, medications, income level, and Morisky-Green test. Anthropometric parameters, fasting plasma glucose, glycated hemoglobin, lipid spectrum parameters were measured. Methods of parametric and nonparametric statistics were used for comparisons.RESULTS: Statins were prescribed to 45.0% of the surveyed, and 47.0% of them were constantly taking statins. In 41 and 39% of cases, statins were prescribed by an endocrinologist and a cardiologist, respectively. Those taking statins were characterized by a more severe functional class of angina pectoris (p=0.03), a higher prior myocardial infarction rate (p=0.01). For other concomitant diseases, and also indicators of carbohydrate metabolism, differences between the groups were not revealed. One third of patients were adherent (3–4 points), 2/3 were not adherent to treatment (0–2 points), respectively. Patients with incomes between 1 and 2 cost of living took statins more often than the rest (p=0.021).CONCLUSION: An insufficient frequency of prescription and adherence to statin therapy in patients with T2DM was revealed. In most cases, statins were prescribed by an endocrinologist or cardiologist. Functional class of angina pectoris, prior myocardial infarction and moderate income were associated with more frequent use of statins. To increase the coverage of patients with T2DM with statin treatment, more attention needs to be paid to the issues of CVD prevention from both medical professionals and patients

    Objective: to analyze the nosological spectrum, demographic, clinical and laboratory characteristics of diseases with a significant enlargementof major salivary (SG) / lacrimal glands, and/or accessory organs of the eye and paranasal sinuses lesions in rheumatological practice.Patients and methods. This work includes 73 patients who underwent a complex clinical and laboratory, imaging, pathomorphological and histomolecular examination, which was necessary to establish a nosological diagnosis. In all cases, the diagnosis was confirmed pathomorphologically.Results and discussion. Sjogren's syndrome (SjS) was diagnosed in 30 (41%) patients (14 of them developed lymphoproliferative disorder, LPD, as a complication), granulomatosis with polyangiitis (GPA) – in 12 (16.4%), IgG4-related disease (IgG4-RD) – in 10 (13.7%), sarcoidosis – in 6 (8.2%), non Langerhans cell histiocytosis – in 2 (2.7%), AL-amyloidosis – in 1 (1.4%), Warthin's tumor – in 1 (1.4%), chronic atrophic rhinitis – in 1 (1.4%), infectious lesions – in 3 (4.1%) (HIV-associated – in 2, dirofilariasis – in 1), idiopathic inflammatory pseudotumor – in 6 (8.2%). In 1 (1.4%) patient, the diagnosis could not be established.A massive increase of major SG was observed in 46 patients, more often (in 28 cases) with SjS with LPD or without it, with IgG4-RD (in 7) and sarcoidosis (in 6). Orbital lesions were observed in 18 patients: in 7 with IgG4-RD, in 5 with idiopathic inflammatory pseudotumor, in 2 with sarcoidosis, in 2 with GPA, and in 1 each with non Langerhans cell histiocytosis and dirofilariasis. Nasal lesions in the form of chronic rhinosinusitis with or without nasal septum perforation, were found in 18 patients, 12 of whom suffered GPA and 6 – IgG4-RD.Two algorithms, that can facilitate the choice of additional studies and the direction of diagnostic search have been proposed for practicing rheumatologists.Conclusion. Taking into account the possible similarity of clinical manifestations of the diseases with the formation of mass-like tissue, the differential diagnosis should be based on pathomorphological study.Цель исследования – анализ нозологического спектра, а также демографичеcких и клинико-лабораторных особенностей заболеваний, протекающих со значительным увеличением больших слюнных (СЖ) / слезных желез, поражением придаточного аппарата глаза и параназальных синусов в ревматологической практике.Пациенты и методы. В настоящую работу включено 73 пациента, которым проведено комплексное клинико-лабораторное, визуализационное, патоморфологическое и гистомолекулярное исследование, необходимое для установления нозологического диагноза. Во всех случаях диагноз подтвержден патоморфологически.Результаты и обсуждение. Болезнь Шёгрена (БШ) диагностирована у 30 (41%) пациентов (у 14 из них возникло осложнение в виде лимфопролиферативного заболевания, ЛПЗ), гранулематоз с полиангиитом (ГПА) – у 12 (16,4%), IgG4-связанное заболевание (JgG4-СЗ) – у 10 (13,7%), саркоидоз – у 6 (8,2%), нелангергансоклеточный гистиоцитоз – у 2 (2,7%), AL-амилоидоз – у 1 (1,4%), опухоль Уортина – у 1 (1,4%), хронический атрофический ринит – у 1 (1,4%), инфекционные поражения – у 3 (4,1%; ВИЧ-ассоциированное – у 2, дирофиляриоз – у 1), идиопатический воспалительный псевдотумор – у 6 (8,2%). У 1 (1,4%) больной диагноз установить не удалось.Массивное увеличение больших СЖ имелось у 46 пациентов, чаще (в 28 случаях) при БШ с ЛПЗ или без него, при IgG4-CЗ (в 7) и саркоидозе (в 6). Поражение орбит отмечалось у 18 пациентов: у 7 – с IgG4-CЗ, у 5 – с идиопатическим воспалительным псевдотумором, у 2 – с саркоидозом, еще у 2 – с ГПА и по 1 – с нелангергансоклеточным гистиоцитозом и дирофиляриозом. Поражение носа в виде хронического риносинусита выявлено у 18 пациентов с перфорацией носовой перегородки или без нее, 12 из которых страдали ГПА и 6 – IgG4-CЗ.На основании анализа нозологий, проявляющихся увеличением больших СЖ, органов орбит и поражением ЛОР-органов, предложено два алгоритма, которые могут облегчить выбор дополнительных исследований и направление диагностического поиска для практикующих ревматологов.Заключение. Учитывая возможную схожесть клинических проявлений заболеваний, протекающих с формированием «плюс»-ткани, основой их дифференциальной диагностики должно быть патоморфологическое исследование

    Listeria monocytogenes meningoencephalitis against the background of the new coronavirus infection: a clinical case

    Background: Among the bacteria that affect the central nervous system, Listeria monocytogenes (facultative intracellular bacterium) is one of the most lethal to humans and animals. Listeriosis affects domestic and farm animals (pigs, small and large cattle, horses, rabbits, less often cats and dogs), as well as domestic and ornamental birds (geese, chickens, ducks, turkeys, pigeons, parrots and canaries). L. monocytogenes can be detected in fish and seafood (shrimp). The source of L. monocytogenes infection are animals in which the disease may manifest itself or occur in erased and asymptomatic forms followed by the transition to a long-term carriage. This pathogen is found throughout the world in foodstuffs, and most cases of infection occur through the ingestion of contaminated food. Particularly susceptible to the disease are embryos, newborns, the elderly and individuals with immunodeficiencies and chronic diseases. L. monocytogenes can cause intracranial hemorrhage, meningitis, meningoencephalitis, and rhombencephalitis. Clinical case description: This paper presents our own clinical observation of the development of severe listeriosis meningoencephalitis in a 47 year-old patient against the background of the new coronavirus infection (COVID-19). We describe the details of the clinical presentation, the treatment and the favorable outcome in our patient. Conclusion: Invasive listeriosis is a rare disease. The knowledge about the clinical manifestations of this disease is needed not only for epidemiologists and infectious disease specialists, but also for physicians of other specialties. Untimely diagnosis and inadequate antibacterial therapy are dangerous leading to severe somatic and neurological complications with a lethal outcome or disability both in children and adult persons

    FEATURES OF THE FORMATION OF PRODUCTIVITY AND ADAPTIVE REACTIONS IN LEGUMINOUS CROP VARIETIES WITH RECESSIVE ALLELES OF GENES

    Researches were carried out on varieties, hybrids, isogenic lines and mutants of peas of various use developed from the beginning of the 1980s up to 2013. Experimental material was investigated under field conditions in the collection, hybridization and breeding nurseries as well as in the process of competitive variety testing in Orel Province. It is shown that one or several recessive alleles of the genes introduced into the genotypes of new varieties of grain, fodder and vegetable peas, blue lupin and soybean can influence the changees in a great number of morphological and physiological parameters determining plant growth, development and also characteristics in the system of yield formation. Reduction of leaf area in leafless pea varieties and lupin varietitts with limited branching is connected with the weakening of root system growth, which is caused by trophic interaction between these organs
